This textbook provides an important tool to cover major aspects of anesthesia care in non–operating room anesthesia (NORA) locations. It outlines perioperative concerns for the most commonly performed procedures in NORA settings. An overview of various anesthesia delivery techniques and tools required to optimize the patient before endoscopy, cardiac, and neuroradiology procedures are provided. The text also covers specialized situations, including a pediatric update on anesthesia/sedation strategies for dental procedures, electroconvulsive therapy, cosmetic procedures, ophthalmologic surgery, procedures in the emergency department, and infertility treatment. Abstract:
Computer-assisted interventions (CAI) on basic number skills have been studied over the last 70 years. The technical development from large, room-sized, mainframe computers to handhelds is revising the school pedagogy in a similar fashion to what school books once did. Computers provide a tool for delivering instruction, but still the contents of CAI have followed on the ruling pedagogical trends of the time. The basic models of repetitive practice to gain arithmetic fluency and problem solving–oriented discovery learning can be found from CAI on numerical skills. The increasing knowledge about educational neuroscience has not changed these models, but turned the attention to the details in the types of numerical stimuli used in training. During these 70 years, there has been a prolific increase in the amount and quality of studies on CAI, but a declining trend in the effectiveness. The charm of novelty is transforming to a daily tool for learning.

Cancer continues to be a leading cause of mortality worldwide and ranks second among the pediatric population. Radiation therapy (XRT) is a potent tool used by oncologists to manage this disease. Although treatment is typically painless, the need for immobilization has resulted in anesthesiologists frequently being consulted to provide service to these patients during their treatment. In addition to the challenges of providing anesthesia in a non–operating room environment, the provider must tailor an appropriate anesthetic for the simulation or XRT treatment sessions. Complications are rare but may be exacerbated by the distance between the patient and the anesthesia provider. Finally, the anesthesiologist must be prepared to discuss issues related to multiple anesthetic exposures with the patient and caregivers.

Abstract:
This book seeks to revitalize the humanitarian mission of the international law governing armed conflict, which is being frustrated due to states’ actual practice. In order to achieve its two aims—creating an environment in which full abidance by the law becomes an attainable norm, thus facilitating the second and more important aim of reducing human suffering—it calls for the acknowledgment of realpolitik considerations that dictate states’ and militaries’ behavior. This requires recognition of the core interests of law-abiding states, fighting in their own self-defense—those that, from their militaries’ professional perspective, are essential in order to exercise their defense. Internalizing the importance of existential security interests, when drawing the contours of the law, should not automatically come at the expense of the core values of the humanitarian agenda—for example, the distinction rule. Rather, it allows more room for the humanitarian arena. The suggested tool to allow for such an improved dialogue is the standards and principles of military professionalism. Militaries function in a professional manner; they respect their respective doctrines, operational principles, fighting techniques, and values. Their performances are not random or incidental. The suggested paradigm surfaces and leverages the constraining elements hidden in military professionalism. It suggests a new paradigm in balancing the principles of military necessity and humanity, it deals with the legality of a preemptive strike and the leveraging of military strategy as a constraining tool, and it offers a normative framework for introducing deterrence within the current contours of the law.

Abstract:
Late antiquity saw a proliferation of Christian texts dwelling on the emotions and physical sensations of dying—not as a heroic martyr in a public square or a judge’s court but as an individual, at home in a bed or in a private room. In sermons, letters, and ascetic traditions, late ancient Christians imagined the last minutes of life and the events that followed death in elaborate detail. This book traces how, in late ancient Christianity, death came to be thought of as a moment of reckoning: a physical ordeal whose pain is followed by an immediate judgment of one’s actions by angels and demons and, after that, fitting punishment. This emphasis on the experience of death ushered in a new ethical sensibility among Christians, in which one’s death was to be imagined frequently and anticipated in detail. This was initially meant as a tool for individuals: preachers counted on the fact that becoming aware of a judgment arriving at the end of one’s life tends to sharpen one’s scruples. But, as this book argues, the change in Christian sensibility toward death did not just affect individuals. Death imagined as the moment of reckoning created a fund of images and ideas within late ancient Christian culture about just what constituted a human being and how variances in human morality should be treated. This had significant effects on the Christian adoption of power in late antiquity, especially in the case of power’s heaviest baggage: the capacity to authorize violence against others.
