Abstract:
Früher benutzte, straff geführte, sogenannte «constrained» Scharnierprothesen hatten eine hohe Frühlockerungsrate, da die Hebelkräfte des Ellbogengelenkes direkt auf die Prothesen-Knochen-Verbindung übertragen wurden. Die später eingeführten Oberflächenprothesen sind nicht gekoppelt. Bei diesen Modellen ist das Gelenk von den periartikulären Weichteilen geführt, welche somit einen Teil der Hebelkräfte absorbieren. Somit ist die Belastung auf die Prothese geringer. Die Oberflächenprothesen haben mehrheitlich eine tiefe Lockerungsrate, erfordern aber intakte Kondylen und Seitenbänder am Ellbogen. Deshalb ist deren Anwendungsbereich beschränkt und symptomatische postoperative Instabilitäten sind bekannte Komplikationen. Eine heute häufig benützte Prothese ist die Coonrad-Morrey Prothese. Es ist eine locker geführte, sogenannte «semiconstrained» Scharnierprothese. Auch bei diesem Modell wird ein Teil der Hebelkräfte des Ellbogens von den periartikulären Weichteilen absorbiert. Die Lockerungsrate ist in der Praxis kein Problem mehr, und ist mit 4% zehn Jahre nach Implantation (rheumatoide Arthritis) ähnlich wie bei Hüft- oder Knieprothesen. Dieses Implantat wird außerdem durch einen kleinen Bügel an der Humerusschaftkortikalis stabilisiert. Dadurch ist diese Prothese auch bei defekten Seitenbändern oder zerstörten Kondylen stabil. Die Coonrad-Morrey Prothese kann deshalb bei praktisch jeder Indikation eingesetzt werden, sogar bei ausgeprägter Zerstörung des Gelenkes und auch bei vollständig fehlendem distalen Humerus. Die Langzeitresultate bei der rheumatoiden Arthritis sind positiv mit 96% guten und sehr guten Resultaten. Die Wiederherstellung der Funktion ist ebenfalls erfolgreich mit einer durchschnittlichen Flexion von 131°, einem Extensionsdefizit von 28° sowie einer freien Pronation und Supination. Die Komplikationsrate beträgt etwa 10%, und ist somit relativ tief. Auch die Resultate der posttraumatischen Arthrose sind insgesamt positiv mit 83% zufriedenstellenden Resultaten. Die meisten dieser Patienten erfahren eine Besserung durch den Ellbogengelenkersatz, aber eine Schmerzbefreiung wird nur in etwa drei Viertel der Patienten erreicht. Die Komplikationsrate ist mit 30% relativ hoch, und weist darauf hin, dass die Ellbogentotalprothese weder schwere Belastung noch übermäßige sportliche Tätigkeit toleriert.

Abstract:
Surgeons and patients are demanding improved replacements for dysfunctional ossicular bones. Replacement of the entire ossicular chain and positioning the prosthesis between the stapes footplate and the tympanic membrane are of particular challenges. Many materials have been used since the first ossicular replacement; at the present time, the two most widely used materials are titanium and hydroxyapatite. Studies have indicated little difference between these two materials. A variety of novel additions have been added to total ossicular replacement prostheses to overcome limitations associated with conventional prostheses. Testing of novel prosthesis materials and designs over long time intervals is needed in order to validate and expand their usage.
