Full textHuman toxocarosis is a worldwide helminthic zoonosis caused by infection of Toxocara spp. second stage larvae (L2) and other nematodes of Ascaridida order. It may trigger, among several clinical syndromes, visceral toxocarosis or visceral larva migrans (VLM). As humans are paratenic hosts, the parasite is unable to achieve the adult stage, making diagnosis highly dependent of a combination of symptoms supported by positive serology and epidemiological history. The reference method for toxocarosis immunodiagnosis is based on commercial micro- ELISA kit using T. canis L2 excretory/secretory antigens (E/S Ag) in the detection of IgG anti-Toxocara spp. antibodies in human serum. Despite its high sensitivity, the test presents some disadvantages, namely, the occurrence of cross-reactions with antibodies generated by other helminth infections, as well as the cost of the kits. The aim of this study was to obtain a sensitive, specific and low cost serological method, easily applicable for routine VML diagnosis. Considering that, similarly to other parasites, the lipid component of T. canis L2 larvae antigens may be responsible for crossreactions with other helminths, we produced a delipidized antigen (AgDs) from T. canis L2 larvae. The sensitivity and specificity of a micro-ELISA test using AgDs for IgG and IgG4 anti- Toxocara antibody detection was evaluated in the immunodiagnosis of VLM, as compared with the reference commercial kit ELISA-IgG® (Bioactiva Diagnostica), used in the laboratory of Medical Helminthology of the Instituto de Higiene e Medicina Tropical. In total, 142 sera from suspected VLM individuals, 25 from individuals with other helminthiasis and 6 from non-infected controls were tested. It was observed a significant level of agreement (Kappa=0,168; p-value<0,05). between AgDs and ELISA-IgG® kit, only for detection of anti-Toxocara spp. IgG antibodies. The sensitivity of AgDs micro-ELISA was lower than the ELISA-IgG® Kit, 42.9% and 22.6% in the detection of IgG and IgG4 antibodies, respectively. The specificity was lower than the ELISA-IgG® kit in the detection of IgG antibodies (72% and 76%, respectively), however it was higher for IgG4 (80%). The lower number of false positive results with this isotype compared with IgG results corroborates previous studies about the usefulness of IgG4 in the immunodiagnostic of this and other helminthiasis. VLM infection was predominant in males (60.7%), but significant statistical differences between genders were only found with the ELISA-IgG® kit (χ2, p≈0,008 <0,05). Concerning age groups, infection was predominant in subjects younger than 10 years old (in all micro-ELISA tests), but the differences were not statistically significant comparing to the higher age groups. In this study, the ELISA-IgG® kit specificity was lower than that described by the manufacturer, registering cross-reactions with sera from individuals with filariasis, schistosomiasis and fascioliasis, emphasizing some limitations of the methods applied in lab routine, as well as the need for further research, especially targeting antigenic preparations with greater specificity.
