Full textThis study evaluates the French public action against alcoholism: constantly reinforced during 50 years, its health results have been in stagnation since 1995. To evaluate, research into scientific theories, into the genesis and into public action analyses, European comparisons are completed. The study confirms the adoption slowness of preventive behaviours. European alcohol or tobacco mortalities are converging on asymptotes, which are different from zero: the major determinants of the mortalities evolutions over a generation are the initial mortalities and the consumption evolutions. Political factors are secondary: law escorts the manners and customs. The public action means have very decreasing efficiencies. Then, it is convenient to mobilise the concerned social groups and also the innovators in individual health management, to promote the observability of the prevention advantages : in the long run, the remainder of the population notes them and adjusts its behaviours

Full textFacing the rise of non-communicable diseases, physical activity and sedentary behavior are a major health issue. Evaluating the synergies of movement behavior dimensions in order to establish its link with health emerges is a key challenge. The development of accelerometry has revolutionized the understanding of these links, traditionally studied using declarative data, associated with well-established biases. The classical accelerometry approach allows continuous measurement over long periods in free living conditions but encounters limitations inherent in signal processing and in the non-linear relationship between accelerometry and energy expenditure to characterize human movement in a satisfactory way. My first objective was to clarify the limits of the current approach and to contribute to the improvement of the phenotyping of movement behavior through new analytic methods relying on the automatic activity recognition of postures and activities. In the second part of my thesis, I applied these new approaches to the study of the Fulani of Senegal, a population in epidemiological transition. My work has clarified the limitations of traditional approaches to accelerometry and the value of activities recognition through automatic learning algorithms to overcome the difficulties encountered. For the first time, they show the contribution of this approach to the detailed characterization of a population's physical activity and sedentary behaviors, in relation to its environment. It should contribute in a useful way to the development of future recommendations that are more appropriate for the general population

Full textSi cette ville abrite des phénomènes de santé spatialement différenciés, deux types de mécanismes peuvent l'expliquer. Le premier renvoie aux disparités socio-spatiales : les caractéristiques comme l'âge, le niveau d'éducation et le niveau de vie qui sont autant de facteurs de risque, ne sont en effet pas réparties de façon aléatoire à Vientiane. Néanmoins, il serait approximatif d'interpréter les disparités spatiales de santé comme le reflet des disparités socio-spatiales alors que celles-ci restent de faible ampleur à Vientiane en ce début de XXIème siècle. De fait, un second type de mécanisme participe activement à l'inégale répartition des problèmes de santé dans cette ville : les habitants, au-delà de leurs caractéristiques propres, sont d'autant plus touchés par le diabète, l'hypertension, le surpoids et l'obésité et d'autant moins touchés par l'anémie, la maigreur et le retard de croissance qu'ils résident dans un village plus urbanisé. La santé des habitants de Vientiane est étroitement liée au niveau d'urbanisation de leur lieu de résidence parce que celui-ci influence leur mode de vie, c'est-à-dire leurs comportements (alimentaires, de soin, etc.) à travers les équipements qu'il présente et les normes sociales qu'il véhicule.
Bien qu'avérée, l'influence du lieu de résidence sur la santé ne doit pas être considérée comme opérant de façon identique sur tous les habitants de Vientiane : cette influence varie selon leur pratique et leur maîtrise de l'espace (c'est-à-dire leur capital spatial). Cette recherche souligne donc l'influence du lieu de résidence sur la santé des individus sans toutefois céder à un quelconque déterminisme géographique.

Full textIt is now well established that nutritional factors are the main determinants of the emergence of chronic diseases. In Morocco, there is currently no data to support this evidence, although this country is experiencing an epidemiological transition. The aim of the thesis was to study the relationship between nutritional factors and overweight or obesity in a representative sample of Moroccan adults aged 18 years and above. This sample was obtained by a simple random technique after stratification by sex and origin. In total, 2896 subjects have participated in this cross-sectional survey. Obesity and overweight were very frequent (13.5% and 29.9% respectively) with large socioeconomic disparities. To better describe the behavior of Moroccan food, most likely related to the emergence of this phenomenon, several complementary approaches were used. First, adherence to Mediterranean diet was measured by a score constructed using a confirmatory approach. Then, the identification of major patterns of consumption observed spontaneously by exploratory methods (Principal Component Analysis). Mediterranean diet was not the commonly diet followed by the entire Moroccan population. In addition, four major patterns were identified and describe better the nutritional transition in Morocco whose specificity is the maintenance of some traditional foods such as cereals and vegetables with some Westernized foods newly introduced such as dairy products. These profiles were associated with sociodemographic characteristics of individuals. A significant association was found between obesity and westernized dietary patterns but not with the Mediterranean diet which was associated with overweight

Full textAn epidemiological transition is observed in tropical areas with the growing of cardiovascular atherosclerosis diseases. The objective of this thesis was to contribute to a better knowledge of the epidemiology of cardiovascular risk factors in tropical area, particularly in Benin. An analysis of prevalence of cardiovascular risk factors in general population was led from Benin national STEPS survey. A systematic review of cardiovascular prospective cohort studies in sub-Saharan Africa (SSA) was conducted. Methodology of TAHES (Tanvè health study) cohort was described; a pilot study was implemented. These data show high frequencies of several cardiovascular risk factors in Benin; differences between urban and rural areas were observed. They underline the rarity of cardiovascular cohort studies in ASS. Besides, they support the hypothesis of cardiovascular cohort feasibility in general population in Benin
