Abstract:
Chapter 8 discusses the promotion of women's psychosomatic health by prevention or early treatment of cancer and obesity. Health providers have to consider the biological, psychological, social, and cultural factors that alter psychosomatic interactions to generate these health conditions. Primary/secondary prevention need more emphasis than tertiary prevention or treatment. The transition of normal cervical epithelium to cervical-intraepithelial neoplasia (CIN), and the progression of CIN 2/3 to cancer is preventable. Two-thirds of patients with CIN have HPV infection. Cervical screening allows astute clinical decision-making as CIN could revert back to normal epithelium. Colposcopically-directed early treatment of CIN 2/3 is a secondary preventive measure. Cervical screening has reduced cervical cancer in the West but organised screening is unavailable in low-middle income countries where cervical cancer is common. Sociocultural practices promote unsafe sex, such as when minors in these countries acquire HPV infection through marriage to an older infected male or when women/adolescents are war victims. Inebriated party-goers may acquire HPV infection through unsafe sex. HPV vaccines protect against 70% of carcinogenic HPV strains only. Serious adverse effects after vaccination are uncommon. Barrier contraception prevents HPV, and other sexually transmitted diseases. Obesity increases the risk of endometrial cancer. Type-1 endometrial cancer relates to obesity and starts at a younger age, unlike type-2. Obesity also affects fertility. Transgenerational changes in the fetus of the obese gravida can promote obese offspring. Bariatric surgery for obesity is however expensive, with a potential for complications. WHO directives thus advise on prevention of obesity, and the overweight habitus. Primary prevention of obesity through lifestyle changes should start in childhood.

Democracy aid has grown considerably since the end of the Cold War. In the late 1980s, less than US$1 billion a year went to democracy assistance; by 2015, the estimated total was more than $10 billion. Despite this overwhelming commitment to spreading democracy abroad, the results have been mixed, and in some cases, this aid has in fact undermined the longer-term prospects for democratic development. What factors account for these different outcomes? Why are democracy promotion efforts far more successful in some cases as opposed to others? Promoting Democracy answers these questions while also providing an often overlooked perspective - the perspective of those most directly affected by the impact of this assistance. By examining two primary conflicttopeace transition cases- the Palestinian territories and El Salvador- and drawing from over 150 interviews with grassroots activists, political leaders, heads of NGOs, and directors of donor agencies, Manal A. Jamal investigates how democracy assistance shaped the re-constitution of political and civic life. She examines these developments at a more macro, general level in terms of democratic outcomes and then at the level of civil society by tracing transformations in one social movement sector--the women’s sector--in each case. She argues that ultimately the pervading political settlements determined the different outcomes, and that democracy assistance mediated these processes. The book then expands the temporal and geographic aperture of the study by examining developments in the Palestinian territories following Ḥamas’ 2006 election victory, and then by investigating the impact of political settlements and the mediating role of democracy assistance in Iraq and South Africa during the start of their political transitions. Jamal challenges more simple accounts that rely on NGO professionalization to explain civil society outcomes and illustrates how pervading political settlements that govern political relations in these contexts ultimately determined the different outcomes. By providing a systematic analysis of how democracy assistance impacts civil society and broader democratic outcomes, she provides new ways of understanding the relationship between foreign aid and domestic political contexts and resolves key debates about the limits of democracy promotion in non-inclusive political contexts.

Successful transitioning involves input from both paediatric and adult multidisciplinary teams with overlap between the two services and tailored to the needs of each individual. This includes varying the duration of the preparatory phase and the timing of transition and eventual transfer based on the chronological and developmental age, maturity, medical stability, and psychosocial issues of each individual patient. Buddy systems and peer support may aid smooth transitioning for some, with the promotion of patients attending a clinic with similar age groups, which can include the formation of a young adult clinic. Effective collaboration between children’s and adult services is required to obtain successful transition of adolescent patients. Moving from one paediatric hospital to different adult specialists in different hospitals requires excellent communication between various teams. Some staff members from each adult nephrology unit should specialize in adolescent medicine and construct their own transitional care pathways to ensure effective communication and collaboration with appropriate paediatric units and facilitate continuity of care with ongoing educational and social programmes.

Abstract:
This chapter examines the United States' liberal democratic internationalism during the period 1989–2008, with particular emphasis on the evolution of American democracy promotion from what Reinhold Niebuhr called its “fortunate vagueness” to a hard ideology. It begins with a discussion of the neo-Wilsonian ideology that appeared in the “long decade” of the 1990s—a way of thinking characterized by both voluntarism and a pseudoscientific certitude that was absent in liberalism of the earlier periods. In particular, it considers the emergence of a “hard liberal internationalist ideology” that was comparable to Marxism–Leninism. It also explores the rise of democratic globalism as progressive imperialism, focusing on democratic peace theory and democratic transition theory. The chapter argues that American democracy promotion had become a pretext for “just war,” as evidenced by the invasion of Iraq in March 2003.
