Full textThe present doctoral thesis aims to study whether it is possible to reduce the clinical heterogeneity of Borderline Personality Disorder (BPD) when grouping patients based on the symptomatological predominance of certain components of its psychopathology. The main hypothesis is that BPD represents a diagnostic category where groups of patients can be differentiated according to the predominant component. Three secondary hypotheses are proposed and tested in three already published exploratory studies, which make up the thesis. Results of these studies show that BPD patients with a comorbid Attention Deficit-Hyperactivity Disorder (ADHD) exhibit higher scores in impulsivity and a higher presence of impulsive spectrum clinic, compared to those patients without the comorbidity; BPD symptoms fit better to a model of three components (behavioral dysregulation, affective dysregulation, disturbed relatedness) than to a unidimensional model; and finally, the predominance of one of these components defines groups of BPD patients with higher intra-group clinical homogeneity and differentiable between them. Given these results, BPD can be considered a disorder composed of three components of its psychopathology. This conclusion does not invalidate BPD category and could be useful to explain its conceptualization complexity. In this sense, the predominance of a particular psychopathological component allows defining clinically homogeneous subgroups of patients. Each subgroup shows a specific profile of comorbid disorders, which have common symptoms with the BPD phenotype that characterizes the group. The consideration of BPD as a disorder consisting of three components of its psychopathology allows proposing different research lines that can answer the previous conflicting results regarding the etiopathogenesis, diagnosis and treatment of BPD.

Full textBorderline Personality Disorder (BPD) is a diagnostic that was initially used in descriptions from classical psychiatrists, referring to unstable personalities. Since the 80s, BPD appears in main diagnostic classifications as a polymorphous diagnostic that is still found in present classifications (DSM-5). The objective of this thesis is to study the neurobiological bases of the disorder in a sample of stable patients with BPD, matched by sex, IQ and age with a sample of healthy controls. The thesis includes three articles that evaluate: 1) structural changes with structural Magnetic Resonance Imaging (MRI), 2) functional changes with functional MRI (fMRI) acquired while performing a cognitive paradigm (the n-back task), and 3) changes based on resting state fMRI and diffusion MRI images. Results revealed structural alterations including volumetric reductions in gray matter in the prefrontal cortex, as well as a tendency to enlargement of the amygdala and hippocampus. Task related functional findings point to widespread deficits in deactivation involving, among other areas, the anterior node of the Default Mode Network (DMN). Finally, the combined resting state and diffusion MRI study showed converging abnormalities in genual and perigenual structures as well as alterations in the connectivity of the fronto-cortico-limbic circuit.

Full textEl trastorno límite de la personalidad (TLP) es un trastorno mental severo, crónico y complejo que ha sido objeto de estudio desde hace muchos años. Concretamente, una de las áreas más interesantes del funcionamiento interpersonal en las personas con TLP son las relaciones sentimentales (RS) debido a su alto componente de intimidad y proximidad emocional. En este contexto general, la presente tesis doctoral tuvo como objetivo principal conocer las principales variables psicológicas relacionadas con el estatus de RS (estable mínimo de 12 meses vs. sin RS) que presentan las personas con TLP. Primeramente, se realizó una revisión teórica sobre los hallazgos científicos sobre el tema hasta la actualidad para definir las variables implicadas en el funcionamiento interpersonal de las RS en personas con TLP. Posteriormente, se realizó un estudio comparativo transversal entre las personas con TLP con una RS (n=23) y sin una RS estable (n=26). El análisis estadístico mostró diferencias significativas entre los grupos en diferentes variables psicopatológicas y relacionales (p.e., agresividad, salud psicológica, autoestima y episodios traumáticos en la infancia), presentando peor funcionalidad emocional y relacional el grupo TLP con una RS. Seguidamente, se comparó a las personas con TLP y a sus parejas, obteniendo diferencias significativas en el funcionamiento relacional (p.e., estilo de vinculación, estilo de comunicación, satisfacción emocional y dimensiones de la personalidad).En conjunto, los resultados obtenidos no confirman la hipótesis general del presente trabajo, ya que las personas con TLP con una RS presentan unas características clínicas e interpersonales más disfuncionales que las personas con TLP que no mantienen una RS estable. Dichos hallazgos fueron discutidos considerando los resultados de otros estudios previos sobre el tema.
Borderline Personality Disorder (BPD) is a severe, chronic and complex mental disorder that had been studied for many years. Specifically, one of the most interesting areas of interpersonal functioning in people with BPD is romantic relationships (RR) due to its high component of intimacy and emotional proximity. In this general context, the present doctoral thesis had as a main objective to know the main psychological variables related to the stable RR of people with BPD. First, a theoretical review was carried out on the scientific findings on the subject until now to define the variables involved in the interpersonal functioning of RR in people with BPD. Then, a cross-sectional comparative study was carried out among people with BPD with a RR (n=23) and without a stable RR (n=26). The statistical analysis showed significant differences between the groups in different psychopathological and relational variables (e.g., aggression, psychological health, self-esteem, childhood traumatic episodes). The BPD group with a RR showed a worse emotional state and high interpersonal dysfunction. Subsequently, people with BPD and their partners were compared obtaining significant differences in interpersonal functioning (e.g., attachment style, communication style, emotional satisfaction and personality dimensions). Overall, the results obtained do not confirm the general hypothesis of the present study, since people with BPD with a RR have more dysfunctional clinical and interpersonal characteristics than people who do not maintain a stable RR. These findings were discussed considering the results of other previous studies on the subject.

Full textThe main scope of this study lies in the validation of the Borderline Symptom List short version (BSL-23) questionnaire, in its Spanish population version, once known the original instrument psychometric sufficiency. In order to establish BSL-23 psychometric properties, it was administered to 240 subjects, previous statistics that evaluated validity and reliability of the scale were analyzed. The results were satisfactory, founding high internal correlations (94%) and temporal stability (73%) in a week interval, in a subsample of 74 subjects. For the evaluation of convergent validity the Spanish version of BSL-23 was correlated with different scales of depressing symptomatology, state-trait anxiety, impulsivity and hostility, obtaining high and middle results, respectively. In order to obtain the change sensibility measure after a therapeutic intervention in our study, with a subsample of patients within 12 weeks, the scale BSL-23 scores were correlated with the Borderline Personality Disorder global impression for patients scale (ICG-BPD-S).Data obtained were very significant (79%), indicating that BSL-23 shows a satis- factory change sensibility. After that validation, it has been achieved to reproduce the structure of an only original version factor and obtaining similar psychometric properties. As in the English version, the Spanish BSL-23 is a valid and reliable tool for the BPD severity evaluation and its change sensibility after therapeutic interventions.

Full textUn número importante de los síntomas del trastorno límite de la personalidad se manifiestan en el contexto interpersonal, concretamente, por dificultades en la cognición social. El propósito de este estudio es analizar la cognición social en mujeres con trastorno límite de la personalidad, comparado con un grupo control, y estudiar la relación con psicopatología asociada y maltrato en la infancia. La cognición social se evalúa con la Película para la Evaluación de la Cognición Social (MASC; Movie for the Assessment of Social Cognition, Dziobek et al., 2006), instrumento reciente con elevada validez ecológica y validada al español (Lahera et al., 2014). Método: Muestra compuesta por 79 mujeres, 47 diagnosticadas de trastorno límite de la personalidad y 32 constituyeron el grupo control. Se realizaron comparaciones entre pacientes y grupo control en relación con la cognición social. La asociación entre trastorno límite de la personalidad, psicopatología y maltrato se estudió a través de correlaciones, comparación de medias y regresión lineal. Conclusiones: Peor desempeño de la cognición social en las mujeres con trastorno límite de la personalidad comparado con grupo control. La severidad de la sintomatología límite se relacionó con peor funcionamiento de la cognición social general y con ausencia de mentalización. El área cognitiva y el malestar psíquico global se vincularon a fallos en cognición social (en general y en áreas cognitivas) y a errores por hipermentalización. Determinada psicopatología en el trastorno límite de la personalidad se relacionaba con dificultades en la cognición social. Maltrato global y abuso sexual se asociaron a mayor afectación de teoría de la mente cognitiva y negligencia física a errores por ausencia de mentalización. Finalmente, la alteración cognitiva en el trastorno límite de la personalidad predijo en un 26% las alteraciones en la cognición social correcta y el psicoticismo en un 20,8% los errores por hipermentalización.
A significant number of the symptoms of borderline personality disorder appear in the interpersonal context, specifically, as difficulties in social cognition. The purpose of this study is to analyse social cognition in women with borderline personality disorder, in comparison to a control group, and to study the relationship with associated psychopathology and child abuse. Social cognition was assessed with the Movie for the Assessment of Social Cognition (MASC, Dziobek et al., 2006), a recent instrument with high ecological validity and authenticated in Spanish (Lahera et al., 2014). Full textThe objective of this doctoral thesis, presented as compendium of articles, is to go into the knowledge of the comorbidity of substance related disorder (SUD) and borderline personality disorder (BPD), especially the presence of comorbidity with other psychiatric disorders, impulsivity as the common factor, and the presence of substance-induced psychotic symptoms in this population. The patients recruited for this study were provided from two different samples: ambulatory center of drug treatment, Section of Addictions and Dual Disorders (CAS) of Psychiatry Service of Universitari Vall d´Hebron Hospital (HUVH) and from the borderline personality disorder program at the same hospital. In the first study 937, sample of patients from CAS were described, especially borderline personality disorders comorbidity. SUD-BPD patients were characterized by female gender, a high prevalence of comorbidity with affective disorders (OR 2.59), anxiety disorders (OR1.90) or eating disorders (OR 4.29). Regarding Axis II, the comorbidity more prevalent in this group was the dependent personality disorder (OR 4.04), paranoid personality disorder (OR 3.70) and antisocial personality disorder (OR 3.46). The more prevalent psychoactive substance was the cocaine (OR 2.16) and the benzodiazepines (OR 1.90). The sample was characterized by an early age of onset consumption (OR 0.94) and dependence (OR 4.04) as an expression of the high severity of the addiction in SUDBPD patients. The second article of the present thesis was carried out with 156 patients from the BPD program of HUVH. Comorbidity of BPD with SUD was evaluated from subtypes of BDP, according to the presence of psychopathological components or diagnostic criteria grouped by confirmatory factorial analysis (CFA). Subtype of behavioral dysregulation was observed, with a more prevalence associated with the substance use, especially alcohol (χ2(2)=7,74, p=,021) and cocaine (χ2(2)=11,22, p=,004) and higher values of impulsivity in DIB-R scale (DIB-R: F(2)=6,99, p=,001), also more behavioral dysregulation and psychiatric admissions but not significantly. The last part of work, consisting in exploring the presence of induced psychotic symptoms (SIP) in SUD-BPD patients and the relationship with cognitive distortions, typical of BPD, evaluated with nine criteria of BPD DSM-5 with the structured clinical interview for DSM (SCID-II). A total of 62.5% of the patients registered SIP, with more prevalence of self-reference (50.8%) and auditory hallucinations (30.6%) especially in cocaine (OR 6.32) and cannabis (OR 4.09) users. The half of the sample evaluated score 3 in nine criteria of BPD DSM-5 (53.8%) but no relationship was found between SIP and criteria 9 (in 68.3% of the patients) being able to suppose the different mechanism of action between SIP and cognitive distortions of BPD. To conclude, SUD-BPD patients were female, presented high comorbidity with other psychiatric disorders and higher severity of the addiction (so worse prognosis), higher impulsivity but not more induced psychotic symptoms in those with cognitive distortions evaluated with nine criteria of BPD DSM-5. Future following studies with longer samples will allow to know deeper the common risk factors between SUD and BPD and to develop better prevention and treatment strategies.

Full textIntroducción: Se ha encontrado una fuerte asociación empírica entre el Trastorno Límite de la Personalidad (TLP), la desregulación emocional y dificultades en la mentalización. Sin embargo, existen pocas investigaciones empíricas, tanto en Chile como en el mundo, que asocien el TLP en adolescentes con estas variables. Objetivos: Describir las características de la mentalización y la regulación emocional en adolescente chilenos entre 15 a 18 años con TLP, y compararlos con adolescentes sin ningún diagnóstico psiquiátrico. Método: En un muestreo no-probabilístico intencionado, se entrevistó a 7 sujetos con TLP y 7 adolescentes sin TLP. Se les aplicó el Mini International Neuropsychiatric Interview para Niños y Adolescentes (MINI-Kid), y el SCID-II para discriminar patología psiquiátrica y TLP. Además, se aplicó la versión española de la Película para Evaluar Cognición Social (MASC-SP) y la Escala de Dificultades en la Regulación Emocional (DERS-E) para medir mentalización y regulación emocional. Resultados: Se observaron mayores dificultades en la mentalización y regulación emocional en el grupo con TLP en comparación al grupo sin TLP. Además, se observó que la hipermentalización es el error más frecuente en ambos grupo, con una mayor tendencia en el grupo con TLP. Conclusiones: Los resultados, a pesar de las limitaciones, se condicen con otros estudios y marcan un positivo precedente para el estudio de los TLP en adolescentes en Chile

Full textCurrent hypotheses on the etiology of Borderline Personality Disorder (BPD) propound that both genetic and environmental factors, as well as their interaction, may be involved in its development. Among environmental factors childhood trauma stand out. Among genetic factors, however, research has been focused on monoaminergic systems and has not yet provided consistent results. Changing the focus to genes of the hypothalamus-pituitary-adrenal (HPA) axis and other systems of the organism’s stress response may be fruitful, since dysfunctions in these systems have been described in both subjects with BPD and subjects with history of childhood trauma. Therefore, the main objective of this doctoral thesis is to identify associations between BPD and polymorphisms within genes of systems involved in the stress response (HPA axis and noradrenergic system), as well as to explore the possible modulation of these associations by childhood trauma. Secondary objectives are to evaluate whether a relationship exists between childhood trauma and BPD severity, and if it is modulated by temperamental traits, as well as to investigate whether childhood trauma and/or BPD severity are associated with an increased methylation of a HPA axis’ gene, the glucocorticoid receptor (GR), in subjects with BPD. The results of this study suggest an involvement of genes of these systems (FKBP5, CRHR1, COMT, DBH y SLC6A2) in BPD development, as well as its modulation by the history of childhood trauma. They also show an association between childhood emotional abuse and BPD severity, mostly in subjects with high neuroticism. Finally, they show that both childhood physical abuse and BPD severity are associated with an increased GR methylation in subjects with BPD. In sum, our results support an involvement of genetic and environmental factors in BPD’s etiology and severity, and suggest that genes of the stress response systems may be associated with risk of BPD under the modulation of childhood trauma. They open the door to further studying the role of these interactions in BPD and, if confirmed in independent samples, to the design of therapeutic or even preventive strategies focused on these factors.
