Full textBackground: Nurses act on ethical and moral values to be able to give their patient ethical care. The foundation of the ethical care that nurses provide is based on their empathic and sympathetic abilities. The ethical ability can negatively be affected by stress. The definition of stress is a wide concept with different meanings. One specific type of stress that nurses encounters is the moral distress which is hard to explain and difficult to handle. The moral distress emerges when nurses are unable to act according to their moral judgment. Moral distress is emotional challenging and may affect patient care. Aim: To illustrate nurses’ experiences of moral distress. Method: The study is a literature review containing ten articles of qualitative design. The articles were analyzed according to Friberg’s method. In which results relevant to the aim were categorized and subsequently referred to themes and sub-themes. Results: The analysis resulted in four major themes: Experienced emotions of moral distress, Situations where feelings of moral distress is experienced, Encounters that’s makes nurses vulnerable and Handle the emotions of moral distress. The latter major theme included two sub-themes: The importance of dialog and Personal abilities and experiences. Discussion: The results of the literature review have been discussed in relation to Travelbees nursing theory and perception of the nurse and other relevant studies. The importance of mutual ethical guide lines between nurses and doctors is discussed and the need of ethical guidance and support in difficult care situations. The meaning of dialog and communication to make moral distress manageable for nurses is discussed.

Full textIntroduction: Breast cancer is the most common malignant disease among women in Sweden. Research has shown that the children of women with breast cancer are affected by the mother's disease due to the fact that the relationship between mother and child is special. The mother is traditionally responsible for the care of the children and when she falls ill the children experience anxiety about the future and their well-being is affected. The responsibility of the nurse is to promote health and prevent illness. The nurse should work from a holistic perspective ensuring that family and relatives are involved in order for all to cope better with the new situation. Aim of the study: The purpose of this literature review is to describe the experience of mothers between 19-65 years old with breast cancer while having 0-30-year old children. Furthermore, the purpose is also to describe the methodological aspect of data collection for the included articles of the study. Method: A descriptive literature review. Systematic searches on well-known databases using search terms was applied. Subject headings were used. 11 articles were included in the study. The data analysis was done gradually and the data was examined both individually and within the group before consensus was reached and the data categorized under themes. Findings: This study shows that the experience of breast cancer for mothers between 19-65 years old having children of 0-30 years is influenced by the fact that they are mothers. Results showed that mothers tried to maintain normality and to not let the breast cancer diagnosis affect their families, specifically their children, more than necessary. Mothers prioritized their children's needs before their own needs. The results showed that this is because of the mother´s conception of the children as being the woman’s and mother’s responsibility. The included articles in this study collected their data through different types of interview techniques and questionnaires. Conclusion: In order to provide good care for the mother affected by breast cancer and her family, it is important for the nurse to highlight not only the patient as a woman, but also as a mother and thus considering her entire lifeworld.

Full textThe purpose of the study was to describe the abused children´s experience of everyday life and encounters in hospital care. The method used was qualitative narrative analysis. Five autobiographic pathografies were analyzed. Results: in the analysis emerged five themes: ”the dysfunctional family”, ”lie as a strategy”, ”the lonely struggle of the child”, ”to be invisible in care situations” and ”from the abyss to sanctuary”. The discussion was based on a synthesis of the Human-to-Human Relationship Model by Travelbee and the findings, i e the five themes. The conclusion made it clear that the abused children´s experiences of everyday life and encounters in hospital care mean, that the child entirely must give up its rights to - not only mutual, but also simply human - meetings in the home and in hospital care, that the child then becomes entirely cut off from other people, at home, in hospital care, in school - abandoned to inhuman suffering with meanings of pain, shame, guilt, betrayal, that the child, despite the total defeat, had an inner force, a wish for a better future, which is made concrete through the child's sanctuary, the hospital, where the child gather courage, trust and hope.

Full textBackground: World health organisation estimates that 200 million women are living with genital mutilation around the world. Female genital mutilation is a traditional procedure that causes extensive complications for the woman's body and mind and is a violation of human rights. The basic trained nurse has the main responsibility of the nursing care and should provide the woman with good patient-centered care. Therefore, knowledge about the different aspects of genital mutilation is necessary. Aim: To explore genital mutilated women’s experiences of the encounter with healthcare regarding approach, knowledge and support. Method: A literature review based on 9 qualitative scientific articles from the Pubmed and CINAHL databases. Result: The women’s experience culminated nine subcategories; “Positive experiences of the encounter with healthcare”, “Negative attitudes from the healthcare professionals”, “Theexperience of being different and being a subject of study”, “The healthcare professionals’ awareness regarding gender mutilation”, “The healthcare professionals lack of knowledge”, “Information regarding possibilities and risks”, “The language- a resource or an obstacle?”, “Customized support” and “Lack of support”. The results of this study demonstrated legitimate arguments for the necessity of broader understanding and training for health care professionals in order to improve the management of this patient group. Conclusion: Women with female genital mutilation experienced comfort when healthcare professionals took their needs into account and handled the encounter well. Regardless, the negative experiences were most prominent. These were based on healthcare professionals’ lack of cultural awareness and knowledge about genital mutilation as well as difficulties in communication. Due to the increased migration from countries where female genital mutilation is practised this patient group could be expected to increase in European healthcare, therefore more research and education within this field is needed in order to improve the care for genital mutilated women.

Full textBackground: There is a heteronorm in society and research shows that LGBTQI-persons are exposed to violence, discrimination and prejudice in society. Research also shows that there is a lack of knowledge about the health of LGBTQI-persons in health care. LGBTQI-persons have poorer health compared to the rest of the population, both in terms of mental and somatic health. The work in health care must be based on a set of values that describes that all people have equal value and person-centered care is advocated. Aim: The aim was to describe LGBTQI- person's experiences of healthcare. Method: A literature review has been conducted to answer the aim. Eleven qualitative scientific articles retrieved from the databases CINAHL Complete and PubMed have been critically reviewed according to Fribergs method. Results: The results are presented under three themes, Norms and normativity, Healthcare provider’s attitudes and Healthcare provider’s level of knowledge. Conclusion: The literature review shows that LGBTQI-persons feel that the attitude of the health care providers is decisive for how the encounter with healthcare is experienced. Negative experiences of healthcare in the form of poor treatment or inadequate care or fear of a negative treatment have in some cases led to healthcare being avoided, which can lead to poorer health. An inclusive attitude can bridge any lack of knowledge and various normative assumptions and thus lead to a more positive experience of encountering healthcare.

Full textBackground: Sedation of mechanically ventilated patients is often necessary for comfort and for implementing the best possible treatment. The trends in sedation have gone from deep to light sedation. Previous research indicates that this is beneficial physiologically for patients and is perceived as positive by both patients and relatives. However, there is little research on how this new approach is experienced by nurses and how it affects their work situation. Purpose: To describe nurses' experiences of caring for lightly sedated patients during mechanical ventilation. Method: The study was conducted with a qualitative design and data was collected through five interviews with semi-structured questions. The data was analysed with inductive content analysis, resulting in the fomulation of three main categories and eight sub-categories. Result: The nurses' experiences of caring for lightly sedated patients during mechanical ventilation was described in three main categories: An experience of interplay, an experience of being insufficient and an experience of the importance of team work. Conclusion: Light sedation contributes to an opportunity to establish a relationship with the patient through an interplay of communication, participation and information. But caring for lightly sedated patients was experienced as challenging and difficult for nurses, leading to feelings of inadequacy and stress. It takes knowledge, experience and a multidisciplinary teamwork to successfully take advanage of the positive effects of reduced sedation depth. Therefore it is important to educate critical care staff about the benefits of light sedation and about how they should communicate with the patients

Full textBackground: E-Health is rapid development in healthcare and reduced the distance between nurses and patients and have made healthcare more accessible. E-Health can be seen as a complement to a traditional healthcare meeting and can offer the nurse a direct contact with patients. One of the goals of e-Health is to make the patient more involved in his care, the nurse has an important role in communicating with patients. Aim: To illuminate nurses' experiences of communication with the patient via e-Health. Method: The chosen method of the study was a literature review. Ten qualitative articles were selected in the databases PubMed and CINAHL Compleate which responded to the aim. The articles were analyzed and thematized. Results: The result was presented in two themes; The meaning of digital technology in communication between nurse and patient and Nurse`s experience of communication through e-Health. Nurse`s express their concern about the loss of interpersonal relationships related to lack of communication skills as well as a difficult digital technology. Discussion: The result was discussed based on Travelbees theory of interpersonal relations. E-Health were discussed through a society perspective an illuminated the role of nurse in e-Health. The physical meeting has been replaced by meetings at a distance and the nurse hade to develop communicative skills to create an interpersonal relationship with the patient via e-Health. The nurse lacks support in the use of digital technology, which is perceived as a barrier to communication as it complicates the creation of relationship.

O presente estudo tem como foco principal a compreensão dos significados expressos pelas mulheres submetidas a mastectomia. Por meio do cuidado à mulher, pode-se estabelecer uma interação pessoa-pessoa para encontrar os significados na sua experiência, a partir do sentido encontrado na doença e no sofrimento. Portanto, o objetivo deste trabalho é identificar e compreender os significados da experiência vivida pelas mulheres mastectomizadas, durante o processo de diagnóstico e tratamento de câncer, com base nos conceitos de Joyce Travelbee. O estudo é descritivo-exploratório, com abordagem qualitativa, e foi desenvolvido em um Abrigo Assistencial, na cidade de Santa Maria - RS. A população constituiu-se de mulheres mastectomizadas que estavam em tratamento radioterápico e quimioterápico. A partir dos resultados obtidos neste estudo, foram explicitadas e discutidas outras questões sobre o cuidado, além daquelas que me levaram a iniciar este estudo, contribuindo para uma prática mais humanística. Este aspecto foi observado, pela magnitude da sensibilidade das mulheres e, avaliando os significados expressos, percebi que estão relacionados tanto ao momento em que ocorre a experiência, como a fatores relacionados às expectativas e possibilidades que a mulher tem quanto à sua vida futura, com seus familiares e com seu mundo sociocultural.

Full textBackground. Threats and violence in emergency rooms are a growing problem both nationally and internationally. The most vulnerable are nurses, as previously described in current research. Studies have also described the factors that increase the risk that patients will behave violently. Relatives return in studies that map the etiology of threats and violence in emergency care but have never been studied in detail. Purpose. The purpose of the study was to describe the significance of threats and violence from family members for the patient care at an emergency room. The study also attempted to answer three specific questions related to this purpose, which focused on the impact of the family's threats and violence on caregiving, the interpersonal relationship and what strategies the nurse utilize in these situations. Method. The study was conducted with a qualitative approach. Ten nurses in five different emergency departments in the two southernmost regions in Sweden were interviewed with a semi-structured method during a four week period during the spring term of 2020. The emergency clinics were selected to represent both small and large departments as well as both rural and urban environments. Data was analyzed through content analysis according to Graneheim and Lundman´s method, both manifest and latent content were searched. Results. The result indicates that threatening and violent relatives have a direct impact on patient care. Relatives' threats and violence lead to a loss of empathy for the patient being cared for and the principles of nursing being violated. The result also points to a large loss of time in connection with the nurses being forced to deal with the relatives' threats and violence, time that would otherwise have been spent on caring for the patient. Re-prioritization and fast track of the patient being cared for are established as a result of the threats and violence of the relatives, which leads to medical priorities being left behind. The nurses are also forced to work out a variety of strategies to deal with the threats and violence of relatives in an attempt to reach the patient. One latent theme emerged Conclusion. Threatening and violent relatives in the emergency department lead to a significant deterioration of all aspects of the patient's care. Further research is needed to identify the extent of the problem.

Malmo University: Faculty of Health and Society, School of Health Sciences, 2012.Glioma is a serious malignancy where the possibility of recovery is limited. The disease has a profound impact on both the patient and family in everyday life. Information and support are nursing interventions that may affect the patient and families cope with their situation and improve their wellbeing. The aim of this systematic literature review was to describe the needs for information and support that patients with glioma and their caregivers are experiencing. The method was a systematic literature review in which medical and nursing databases were searched. The results were based on thirteen processed qualitative studies. Two major categories and seven sub-themes emerged after the analysis. The results showed that information and support measures were demanded by both patients and families throughout the course of the disease. The needs differed during the disease trajectory and both patients and families expressed a need for individual adaptation of these measures. Information and support operations at the onset and at diagnosis and surgery were valuable. Support to cope with existential issues and role of changes were also seen as important. Conclusions: The right information and the right support interventions contribute to patient and relatives are able to maintain hope and find meaning in the emotionally stressful situation.Keywords: Family, Glioma, Information, Patient, Support, Travelbee

Full textAbstract Background: Mental ill-health among children and adolescents are increasing and are a growing public health problem. The schoolnurses primary task is to promote physical health and prevent mental ill-health. For this work, the school is an important arena. Aim: The aim of the study was to illuminate school-nurses´experiences of prevention and early detection of mental ill-health among schoolchildren in grades F-9. Method: Qualitative method was used and ten semi-structured interviews were conducted. The interviews were recorded and transcribed verbatim. The interviews were analyzed using qualitative content analysis and categories and subcategories were created. Results: The analysis resulted in two categories and nine subcategories. The categories are; Good conditions for the school nurse´s work with mental health which includes the subcategories The meaning of the dialogue, The importance of creating confidence and good relations, To observe early signs, Effective collaboration facilitates and To obtain and offer support and Barriers to the school nurse´s work with mental health which includes the subcategories Lack of time and resources, Lack of collaboration, Lack of knowledge and experience and Lack of support. Conclusion: The school nurse is at work with mental ill-health a vital resource. Working with mental ill-health places great demands on the schoolnurse´s knowledge and experience and the ability to adapt to the child´s individual needs. Much indicates that Joyce Travelbees theory of interpersonal aspects of nursing can be implemented in the schoolnurse´s work with mental ill-health. Full textBackground: In today's society, the word addiction is often stigmatized and has a negative tone. The use of illegal drugs increases in society, which means a greater burden on healthcare. According to the Health Care Act the aim is to promote good health and provide equal treatment for the entire population. The care should be given with respect for the equal worth of all people and the dignity of the individual. Studies show that patients with substance abuse experienced a stigmatized behaviour from healthcare professionals. Aim: The aim of this literature review was to describe the healthcare professionals' experiences of caring for patients with substance abuse. Method: This literature review was made by studying ten scientific articles. The articles have been analysed according to Friberg's method of analysis where the authors found similarities and differences of experiences, which were reported in various main themes and sub themes. Results: Based on the analysis, two main themes were created: Negative experiences and Positive experiences. Negative experiences were divided into four sub themes: Education and knowledge, Fear and lack of confidence, Feeling insufficient and Stigmatization. Discussion: The main findings of the result have been discussed in relation to the background of the literature review and Joyce Travelbee's Human-To-Human Relationship Model as a reference frame.

Full textBackground: 185 million people around the world were estimated to abuse drugs in the end of the late 1990´s. Drug abuse can cause acute deaths or organ injuries and infections, such as hiv and viral hepatitis. Data from the Institution of Public Health and the Swedish National Board of Health and Welfare show that drug abusers experience difficulties receiving care and that care given to these patients is unsatisfactory. Aim: The purpose of this literature overview was to examine nurses´ perceptions of patients who abuse drugs within health care. Method: A literature overview based on nine quantitative and one qualitative study was carried out. Articles were searched for in the databases PubMed, Cinahl and PsycInfo. The quality of the studies was estimated by using a review protocol. As a theoretic frame of reference Trvelbee´s nursing theory was used. Results: Five groups of nurses´ perceptions of drug users were identified. They were perceived to be problematic, spineless, bearers of certain qualities, difficult to nurse and entitled to care. One group of nurses´ change of perceptions was also identified. Conclusion: There is a risk that nurses´ negative perceptions of drug abusers lead to unmet care needs for these patients.

Full textPregnancy loss and early miscarriage are common adverse outcome in pregnancy. A more rare diagnosis is ectopic pregnancy that can be very dramatic and life-threatening. Nurses from different areas in hospitals meet women in need of care after such a loss. The purpose of this literature review was to find out what the research show about women’s emotional experience of pregnancy loss and from a nursing view find out how the women experienced the care they received. To reach the purpose, a literature review was done according to Friberg (2006). The results showed three different themes; Emotional experiences, How the women coped with the emotional experiences and How the women reacted to the care given. The women experienced the pregnancy loss differently but they had one thing in common: The way they handled the situation depended on how the health professional treated them. If the caregivers were sympathetic, compassionate and gave time to the women they coped a lot better than if the caregivers were insensi-tive, callous and showed a matter of fact attitude. The result is discussed in rela-tion to Travelbees nursing philosophy. The women need to be provided with targeted care. The care must be based on every woman’s individual needs. These patients often spend a very short time at the hospital with short time for treatment witch leads the nurses to a challenge of how to get the time to treat every woman individually. This would lead to a better understanding for the women’s needs

Full textBackground: It´ about 2,5 million Swedes who seek emergency treatment per year (Socialstyrelsen, 2015). This leads to high pressure at the emergency department. Resources are often scarce and contributes to increased waiting times. Media shows a negative image of care and organizational problems often appears. Looking at patients' experiences of the situation at the emergency department, creates conditions for improving the healthcare situation at emergency department. Aim: To describe peoples experience of being treated at the emergency department by using Travelbees nursing theory. Method: A deductive literature review was conducted and nine articles were analyzed by Travelbees nursing theory. Results: Travelbees concepts suffering, meaning, human relation, human as an individual and communication creates theme in the result. Patients experiencing that communication between them and the healthcare professionals is of great importance. This contact makes the patient feel confirmed. If the patient feel that they are not confirmed because of lacking communication or human contact it leads to a feeling of dissatisfaction. Conclusion: Healthcare professional’s treatment affects the patients experience in the emergency department and helps them to cope with the situation. Continuous contact with health professionals is of importance for the patient and makes them feel confirmed. The nurse must be aware of which expectations the patient has when they seek emergency care and be able to give professional care.

Full textEach year 50 000 new cases of cancer in Sweden is discovered, which means that more than 100 people is diagnosed every day. Getting diagnosed with cancer is often a shock to the victim. Social support is of great importance for how the crisis will be handled by the patient. The aim of the literature review was to identify cancer patients' experiences of participation in various forms of support and self-help groups that can be led or initiated by a nurse. The method is a literature review conducted in ten qualitative articles. As a theoretical framework Travelbees nursing theory has been used. The results showed that support groups or self-help groups gave the experience of a feeling of fellowship and belonging among the participants, to get and give support to each other, stronger self-confidence, to feel hopeful and negative experiences of participation.

Full textThe purpuse of this study is to elucidate the relationship between nutrition and pressure ulcer by seeking the answers to the following three questions: Does nutri-tion have any implication on the appearance and healing of pressure ulcers? What nutrients are needed to prevent appearance and promote healing of pressure ul-cers? Is there a connection between malnutrition and/or eating-difficulties and pressure ulcers? The method for this literature review consisted of a systematic searches in databases and searching through references. Ten scientific quantitative research articles in this subject have been analysed and concluded. The result is presented in four themes: The significance of nutrition, the significance of energy intake, the significance of vitamins and minerals and sub findings. The result of this analysis shows a connection between improved nutritional status and both lower incidence and improved healing of pressure ulcers.

Full textThe concept of the traveller has changed considerably in Irish society over the last fifteen years. Traditionally, the traveller was either the emigrant, forced to leave Ireland to better himself, or the gypsy, marginalized since the time of the Famine (1846). Since the advent of the Celtic Tiger (1994), and the wealth and prosperity it brought, the new traveller is the immigrant or refugee who comes to Ireland because of the opportunities it offers. I also believe that the definition can be broadened to the Irish people themselves, who can learn to travel within their own space by embracing the multitude of cultures, ethnicities, languages and religions that are now a part of the society. In fact, the very definition of 'Irishness' is shifting and morphing away from the majority Catholic Celtic nation, to an exciting blend of otherness. We must become travellers by leaving our preconceived notions of 'the other' behind, and travelling out of our own personal narrative into the stories of others. In an attempt to show this shift in 'Irishness', and the difficulties it brings, this novel has been structured to echo the great Irish literary work Ulysses by James Joyce. While I am not so ludicrous as to compare my effort to Joyce's, I do not believe that any creative work deserves 'holy grail' status. Therefore, while my aims are far more minor than Joyce's, I felt that shadowing Ulysses would be a perfect way to contemporise the issues that still lie at the heart of the Irish psyche. Just as Joyce set his book on one day, June 16th 1904, The Traveller is set on June 16th 2004 (Centenary Year). In the same way as Joyce used 'The Odyssey' as a foundation for a reflection on the Irish society of 1904, The Traveller loosely recreates Ulysses in contemporary Dublin. Like Joyce, I see this as a way of creating a thread of continuity between the actions of a small group of people in a particular place and the wider historical context, as well as showing the archetypal nature of human relationships. The Traveller does not adhere slavishly to the plotline of Ulysses, but builds upon it, creating additional characters and transferring the original themes to a modern context. Where appropriate, I have echoed the style of certain chapters in Joyce's original, ego the soliloquy format for the last chapter. Where Joyce used the central character of Bloom, a Jewish advertising salesman of Hungarian extraction, my central character is Omar, a Muslim journalist of Egyptian descent. They both represent the outsider in their own time, and suffer from being born into a tradition/religion that is alien and threatening to the society in which they live. Neither is a hero in the Homeric sense, but ends up being one through ordinary humanity. In The Traveller, the character of Omar is key to exposing the difficulties faced by 'the stranger' , even one who is half Irish and has grown up in that society. His journey through one day indirectly illuminates the key themes of politics, religion, marginalisation and love, as do the characters of Kinch, Flora, and BláithÃn.

Full textDenna licentiatuppsats behandlar upplevd tillgänglighet, det vill säga resenärsperspektivet på tillgänglighet, som ett viktigt komplement till konventionell (objektiv) tillgänglighet inom hållbart resande. Uppsatsen introducerar upplevd tillgänglighet och dess nytta och plats i forskningen samt implikationer för planering och utvärdering av hållbara transporter. Vidare behandlas påverkansfaktorer för upplevd tillgänglighet samt relaterade individuella utfall som subjektivt välbefinnande och social exkludering/inkludering utifrån teori och empiri. Uppsatsen innefattar två empiriska studier. Studie I utvecklar och testar ett psykometriskt mätinstrument för upplevd tillgänglighet med data från tre enkät-tillfällen (n= 750) i Karlstad. Dataset 1 analyserades med explorativ faktoranalys för att få fram ett instrument som fångar upplevd tillgänglighet (PAC) och validerades därefter i två dataset. Studie II avsåg att undersöka vad som bidrar till upplevd tillgänglighet med fokus på kvalitetsattribut i resan (service quality), resenärens upplevda trygghet, samt ålder och resvana. Samma data som i studie I användes i en conditional process model för att undersöka sambanden, samt i en klusteranalys för att ytterligare undersöka skillnader i resultatet mellan olika grupper av resenärer. Resultatet visar att upplevd tillgänglighet kan ha betydelse som komplement till befintliga mätningar och utvärderingar av tillgänglighet genom att bidra med resenärsperspektivet. Ett antal påverkansfaktorer för upplevd tillgänglighet kan konstateras, däribland kvalitet samt resenärens upplevda känsla av trygghet. Betydelsen av kvalitet för tillgänglighet är densamma oavsett hur ofta man reser, däremot upplever de som reser ofta en signifikant högre tillgänglighet än de som reser sällan. Äldre resenärer och resenärer i 30-årsåldern påvisar signifikant lägre upplevd tillgänglighet.
This thesis proposes perceived accessibility as a complement to conventional accessibility in sustainable transport planning and evaluation. Perceived accessibility is defined as the possibilities and ease of engaging in preferred activities using different transport modes and captures the traveller experience and perspective of accessibility. The thesis introduces an instrument for measuring perceived accessibility (PAC) along with determinants for perceived accessibility, such as feelings of safety, age, and perceived quality. Related social outcomes such as social inclusion and subjective well-being are discussed, along with possible implications for transport planning and suggestions for future research.
