Abstract:
Aim of study. Considering the high spreading of tinnitus in population, the purpose of our research was to study the possibility and expediency of using hypnosuggestive therapy in treatment of tinnitus. Material and methods. The study included 55 people with tinnitus complaints. We divided three groups of patients depending on the severity of the manifestations, audiometry results, Tinnitus Handicap Inventory (THI) score according to this scale of severity, results of GAD-7 test and the PHQ-9 (Patient Health Questionnaire -9). Results. In the first group of patients, according to the tinnitus severity scale the score was up to 19 points and was considered as insignificant and mild. The score of the GAD-7 questionnaire was less than 8 points, PHQ-9 - up to 5 points. In the second group the severity of tinnitus was moderate and severe and counted more than 38 points. The GAD-7 questionnaire score was less than 8 points, PHQ-9 - up to 5 points. In the third group, 12 people complained of chronic tinnitus, sleep disturbances, nervousness, and reduced work capacity. The score of tinnitus severity in patients of this group was more than 58 points and was assessed as severe and catastrophic. According to the results of the GAD-7 and PHQ-9 questionnaires, all 12 people were diagnosed with anxiety disorders and depression, two parients even had suicidal thoughts. In the third group of patients with +++ hypnotability, after ten sessions of individual hypno-suggestive therapy, it was possible to reduce the severity of tinnitus to the third moderate degree in 7 people. 4 patients reported about complete regress of tinnitus. Conclusions. Tinnitus is a common problem and can negatively affect work, family and social life. It requires the individual approach and involvement of specialists of various profiles in the diagnostic process. The diagnostic algorithm for patients with tinnitus should include questionnaires to estimate the psychosomatic profile. The use of hypno-suggestive therapy can significantly reduce the severity of tinnitus and improve the quality of lives of patients.

Background: Habit cough is one of the causes of chronic cough in children. Many cases do not reach the psychiatrists due to lack of specific clinical features and diagnostic tests. We present a case of 13 year old boy with habit cough who was successfully treated by suggestion therapy and lignocaine based mouth gargle as a distracter. This case highlights that suggestion therapy can be an important treatment option to this fairly common condition.

Abstract:
Sixty-eight patients suffering from generalized anxiety disorder were allocated to one of four groups to study how hypnotizability (high versus low) and type of suggestion (direct versus indirect) were related to treatment outcome. Patients were assessed on three occasions (Pre-treatment; Post-treatment; and three month Follow-up) using seven anxiety measures and one depression inventory. The Creative Imagination Scale (CIS) was used to assess susceptibility to hypnotic responding. Twenty Ss dropped out prematurely due to both practical and treatment related issues. Interestingly, dropouts were also found to have been significantly more depressed than completers. Patients were given six treatment sessions of hypnotherapy at weekly intervals while the author remained blind to CIS scores. Therapy focused on accessing S's unconscious abilities and resources for symptom resolution, using either direct or indirect suggestion. Patients were provided with self-hypnosis tapes for anxiety reduction in between appointments. Results indicated that completers significantly improved on all dependent measures, but few differences emerged between the four groups. Interestingly, highly hypnotizable patients who received indirect suggestion relapsed slightly on all measures by the Follow-up period, having improved from Pre- to Post-treatment. Discussion is offered on the possible implications these results have for the integrity of the independent variables, concluding that both hypnotizability and type of suggestion have a clinically significant interactive effect. Direct suggestion is recommended for highly hypnotizable generalized anxiety disorder clients, while both direct and indirect suggestion can be used with low hypnotizables, as long as all are couched within permissive language. Limitations of the present research are discussed, along with recommendations for clinical practice and future research

South Africa's water resources are, in global terms, scarce and extremely limited (DWAF, 2004). Groundwater is a valuable source of potable water in South Africa. It was found that most of the health facilities in the Limpopo Province depend on groundwater as sole source of potable water. Groundwater quality is to a great extent influenced by the dominant land use in the vicinity of an aquifer. It is therefore important to carefully manage possible pollution sources of anthropogenic origin. This may be seen as pro–active water quality management that may result in significant saving on water treatment. This aim of this study is to obtain a regional view of the water quality situation at clinics and other health facilities in the Limpopo Province. It was found that the general water quality at health facilities in the Province is questionable. It is of concern to note that 56% of health facilities use water that is unacceptable for human consumption. Water quality may be managed by introducing appropriate treatment options to treat the water to ideal water quality standards. This dissertation explores some treatment options used at health facilities in the Province. The efficiency of these treatment systems is also investigated.<br>Thesis (M.Sc. Abstract:
This dissertation investigates the effects of instruction on the acquisition of pragmatic competence in the classroom setting, and specifically in the English as a Foreign Language (EFL) classroom. In particular, we examine the effects of instruction on learners' pragmatic development of a specific speech act, that of suggestions, with the goal of expanding the range of learning targets addressed in pragmatic interventional studies (Kasper and Rose, 2002). Apart from dealing with instructional effects, we also take into account the need to investigate various types of instruction in order to ascertain their effectiveness (Kasper and Rose, 2002). In this way, we analyse two different treatments (i.e. explicit and implicit) that have been operationalised on the basis of the principles underlying the paradigms of Focus on FormS and Focus on Form (Long, 1991, 1996, 1998; Doughty and Williams, 1998; Long and Robinson, 1998; Doughty, 2001). The aim of comparing both teaching approaches is to determine whether the two treatments are equally effective in developing learners' pragmatic competence regarding their production, awareness and confidence when judging the appropriateness of suggestions in different situations. Additionally, we are also concerned with issues related to research methodology and, bearing in mind findings from studies that observed task effects between oral and written production tasks (Houck and Gass, 1996; Sasaki, 1998; Safont, 2001), we have compared our learners' performance when making suggestions in two different tasks: an oral production task (i.e. phone messages) and a written production task (i.e. email).<br/>The results of the investigation indicate that, in comparison to the control group, both the explicit and implicit groups improved their pragmatic competence regarding their production, awareness and confidence when judging the appropriateness of suggestions in different situations. Moreover, comparing the explicit and implicit groups' performance in the post-test no statistical differences were observed between the two instructional treatments, which illustrates the effects of both explicit and implicit instruction to develop learners' pragmatic competence in the EFL classroom. The findings also show that the production task in which learners are engaged influences their use of suggestions, since there were task effects between the two production tasks employed in our study (i.e. phone messages and emails). In conclusion, the results obtained in this dissertation expand the scope of enquiry in the field of interlanguage pragmatics as well as outline practical pedagogical implications and open several lines of investigation to be examined in future research.

Abstract:
Panic disorder (PD) is a common psychiatric illness with highly stereotyped symptoms including a sense of shortness of breath or feelings of suffocation. PD is characterized by spontaneous and recurrent panic attacks (PAs) that consist of incapacitating periods of acute-onset respiratory, cardiovascular, gastrointestinal, autonomic and cognitive symptoms. According to the DSM-5, recurrent panic attacks in PD are categorized as being either spontaneous (unexpected) or cued (expected). Accumulating evidence suggests that spontaneous PAs may be provoked by interoceptive sensory triggers caused by fluctuations in the internal homeostatic milieu. An important internal homeostatic trigger for the genesis of PAs, supported by an emerging body of work, is acid–base imbalance and associated pH chemosensory mechanisms. Largely founded on panic provocation studies with agents promoting homeostatic pH imbalance (e.g. sodium lactate or CO2) and related to the false suffocation alarm theory the role of acid–base and chemosensory systems in panic provides strong scientific insights on the genesis of PAs. Heightened sensitivity to carbon dioxide (CO2) is an established biological correlate of PD. Indeed inhaled CO2 triggers PAs in most individuals with PD but only a minority of unaffected controls. More recently the acid sensing ion channel-1 (ASIC1) has been proposed as a candidate gene responsible for these phenomena. Indeed this channel is expressed in central nervous system and in particular in amygdala, brainstem, structures involved in chemosensory detection and breathing. According to twin studies, shared genetic determinants appear to be the major underlying cause of the developmental of adult PD and of altered sensitivity to CO2. Moreover, in addition to genetic determinants, environmental risk factors affect the liability to these traits, and several life events that influence the susceptibility to PD also predict heightened CO2 reactivity. There is evidence that genetic and environmental determinants interact to influence human responses to CO2 suggesting also epigenetic mechanisms to underlie the development of PD. Valuable animal models for PD are lacking due to the difficulty to measure “panic” in animals. Indeed models of PD used in pre-clinical research measure the defensive behaviors showed by the animals in response to a real aversive stimulus and not spontaneously and/or in the absence of real dangerous situation, as in PD. Not being able to interview the animal, about its symptoms, such as fear of dying or going crazy as in human PD patients, CO2 hypersensitivity, observed in patients with panic and their unaffected relatives, represents a valid endophenotype to model this disorder in animals. PD is a chronic disorder with variable course and treatments available until now are not specific and are often only modestly efficacious. Typical pharmacologic treatments are antidepressants (SSRI) or anxiolytics (benzodiazepines). An alternative strategy is psychotherapy (cognitive behavioral therapy) and often patients are treated with a combination of psycho- and pharmaco-therapies. In some cases therapies (e.g., benzodiazepines) may be associated with treatment-specific side effects or risks such as sedation or the risk of dependence or tolerance. For these reasons is important to find a therapy specific for PD, possibly with the fewest side effects. The aims of my PhD Thesis were: 1) to validate the RCF protocol in mice as a useful manipulation procedure affecting individual emotionality, different from the classical maternal separation (Handling), usually applied in rodents, to evaluate the effects of an early adverse environment. I evaluated the short and long-term effects of these early manipulations on several behavioral, molecular and physiological parameters (mother-pups interaction; stress response; emotionality; CO2 panic-related response; gluco- and mineral-corticoid receptors mRNA expression; etc.) 2) to analyze possible molecular mechanisms underlying the panic-related CO2 hypersensitivity showed by RCF animals and evaluate different pharmacological treatments (chloridiazepoxide, chlorogenic acid and amiloride) able to recover the normal respiratory response to hypercapnia, on the basis of molecular suggestions 3) to verify the cognitive capability of RCF animals trough learning tests (such as active avoidance test and novel recognition test) and investigate the capability of exposure to 6% CO2 on animals’ behavioral conditioning, in RCF and Control subjects. Indeed, humans with PD show behavioral conditioning to panic attacks and develop PA also in absence of unconditioned stimulus. 4) to investigate whether the CO2 hypersensitivity showed by RCF animals was a transgenerational transmissible trait. First of all, results reported in this study suggest that the behavioral and physiological phenotypes observed during development and adulthood depend on characteristics and timings of early adversities capable of activate different biological processes. Reasonably, the response of the animal to the early manipulations is different and aimed at maximizing individual fitness: the early environment could exert its programming role during this developmental plastic period, through specific epigenetic modifications. Short, even if repeated, separations from the mother (Handling protocol) induce habituation to a relatively low stressing environment, enhancing the capability of the subject to face new stressful situations. By contrast, the disruption of the infant attachment bond (RCF protocol) is associated to a modification in the respiratory response to high CO2 in breathing air, an endophenotype these animals share with PD patients. The disruption of infant-mother bond in RCF animals suggested by the enhanced separation anxiety at 8 days age supports the relation between SAD and PD already suggested in literature. In addition the CO2 reactivity showed by these animals represents a useful tool to study PD in pre-clinical research. Molecular alterations found in RCF animals (experiment 2a) supported the involvement of acid-base balance dysregulation in development of CO2 hypersensitivity. Indeed RCF animals showed a higher expression in ASIC1 gene that codifies for acid sensing ion channels. These channels are sensitive to lower levels of pH being able to detect changes in CO2 concentration in the body and adjust the respiratory function to receive enough O2 not to compromise biological processes. Molecular investigations in addition revealed alterations in GABAergic transmission in RCF animals supporting the idea of an involvement of this neurotransmitter in the development of PD. RCF animals showed an increased expression of Dbi which is an inhibitor of GABAergic transmission. These molecular findings have provided indications suggesting that a possible rescue treatment for PD patients should consist in reducing CO2 hypersensitivity. Lowering of this increased respiratory response to modest increase in CO2 could reduce the negative feeling associated to condition, reducing the conditioning potentiality that favor the development of panic disorder, after repeated panic attacks. The use of benzodiazepine such as chlordiazepoxide was able to restore the normal respiratory response to CO2 as well, giving pharmacological validation to RCF model. However, benzodiazepines have several contraindications, especially for chronic treatments and their sedative effect should also be taken into consideration. Even if I only present few data on the effects of chlorogenic acid and amiloride on RCF animals, I think these results are very interesting and need further and deeper evaluation. Both these compounds interacted with the pH sensitive channels (asics) and their administration was able to restore the respiratory response observed in control animals. It is well known, that panic attacks are able to condition behaviors of PD patients. They indeed tend to avoid situations and places similar to those where a panic attack previously occurred. Similarly RCF animals showed, in experiment 3, behavioral conditioning to the situation previously paired with CO2 (tone exposure). It should be now explored whether RCF animals generalize the conditioned fear, suggesting how an initial panic attack can evolve into panic disorder in humans. Finally RCF model demonstrated a transgenerational transmission of the respiratory endophenotype (experiment 4) supporting the hypothesis of gene-enviroment interplay role to predisposition to panic disorder (Spatola et al., 2011). The epigenetic mechanisms responsible for this trans-generational transmission are under investigation as well as possible strategies to prevent this phenomenon. In conclusion, the Repeated Cross-Fostering protocol seems a valid mouse model of Panic Disorder in humans: RCF mice show typical features of this disorder such as separation anxiety during childhood, CO2 hypersensitivity and CO2 conditioned and avoidance behaviors. Acid sensing ion channels are interesting molecular markers which can be used as new targets for pharmacological treatments and can help to explain hyper-responsiveness to CO2 in PD patients as well.

Spirituality in the discipline of nursing has gained popularity over the past two decades. National and provincial nursing associations and colleges expect nurses to be educated in providing spiritual health assessments and interventions in order to provide holistic nursing care. There is a paucity of research in the nursing literature on the meaning of spirituality from the perspectives of individuals who experience chronic mental illness, specifically schizophrenia. Spirituality remains an elusive construct, challenging psychiatric nurse educators, researchers and practitioners in the development of nursing curricula to guide psychiatric nursing practice. The focus of this research study was to develop a substantive theory about the experience of spirituality among individuals living with schizophrenia using grounded theory methodology in the tradition of Glaser and Strauss (1967). Forty semi-structured interviews and four focus groups were conducted with 20 participants who self-reported to be diagnosed with schizophrenia. Findings: The substantive theory "spirituality as connection" indicates that spirituality for persons living with schizophrenia involves a dialectical process in which one strives to be connected to one's spiritual self (body-mind-spirit), significant others (family, friends, G o d / Higher Power, health care professionals), community (others living with a mental illness, others who are well, a faith community, the community in which participants lived), and nature, while at the same time experiencing situations and incidents that promote disconnection from these sub-themes. Strategies used by participants to achieve connection included: taking prescribed atypical anti-psychotic medications, maintaining their health and a healthy lifestyle, use of prayer / meditation, caring for self and others, and engaging in creative activities that added meaning to their life experiences. Among the 17 factors contributing to connection, exemplars are: reconnecting with one's spirit through prayer and meditation, attending drop-in centres for persons living with a mental illness, and walking / hiking in nature. Outcomes include feeling peaceful, love, contentment, being accepted and nurtured by others. Among the 14 factors contributing to disconnection, exemplars are: the effect of the illness on relationships with other people, the stigma of being in a psychiatric ward, being unemployed, and taking typical antipsychotic medications. Outcomes include feeling powerlessness, isolation, rejection and alienation.

His questions address the received assumptions that shape political and ethical debate and the suggestion that their terms of reference require a radical shake-up. Kirby’s counter-intuitive treatment of familiar and accepted ways of thinking pays special attention to the nature/culture division and its myriad reconfigurations (body versus mind; primitive, or first, versus complex, or second; illiteracy versus literacy). She interrogates the routine and almost automatic logic that segregates what is deemed abstract and ideational from the pragmatic gravitas and political urgency that we tend to secure in empirical, “on the ground” evidence. For Kirby, this notion of material evidence and the weight of its truth claims, together with the corollary belief that the ideational and abstract are entirely other to physical and material reality, promote an insidious political agenda that sustains misogyny, racism, and ecological degradation as inevitable. By underlining the implicated ecologies of life whose dynamic cross-overs and impurities are also manifest in our thought structures, we are challenged to work with/in a sense of corruption that is irreducible and not simply negative.

Abstract For metastatic esophagogastric cancer (EGC), treatments aim to extend survival time, manage symptoms, and enhance the quality of life . However, determining the best treatments for patients with EGC is challenging due to patients’ variability. Personalised treatments supported by predictive models enable tailoring treatment process to individuals. Even so, traditional predictive models often neglect the interaction between treatments, limiting their utility in comprehensive planning. State-of-the-art Predictive Process Monitoring shows promising results in predicting the outcome of the treatment process but often lacks transparency. This paper investigates the potential of supporting healthcare experts in personalising the EGC treatment process, using eXplainable Predictive Process Monitoring methods. A real-world case study among 7,090 patients identifies expert needs for helpful explanations and discusses the capabilities and limitations of existing methods, suggesting future research directions. Our findings demonstrate high-quality explanations with strong fidelity, providing insights validated by expert knowledge. While the resulting explanations are not always actionable, experts acknowledged their value for exploratory analysis.

AbstractRising health care costs is a challenge for all health care systems, and new and expensive cancer drugs is an important contributor to this. Many countries – like Norway – have therefore established priority setting institutions and systems for drug appraisals where equal treatment, neutrality and transparency are key values. Despite this, controversy surrounding drug reimbursement decisions are persistent.The development of personalised cancer medicine is seen by many as a potential solution to difficult priority setting decisions, by tailoring the right drug to the right patient at the right time. We, however, see personalised oncology and medicine in general not only as a solution, but also as a potential contributor high costs and to persisting controversy. We will argue that attempts to improve and strengthen the priority setting system – without accepting that a wider perspective on science and society is required – is likely to fuel even more controversy.In contrast, our suggestion takes a different approach building on post-normal science. From a co-production perspective, scientific, technological and societal developments are causally entangled into each other. Alongside refining priority setting principles, one can and ought to raise normative questions about the trajectory of personalised cancer medicine and of how to create a well-functioning public sphere. How can we imagine a well-functioning system of technological development and health care priority setting? Which changes in research policy and funding could support such a system? And which properties could biomarkers have in order to help society manage the health gap?

Abstract The corrosion behavior of pure titanium UNS R50250 and UNS R50400 were investigated and compared with electrochemical behavior of Ti-0.2Pd and Ti-0.3Mo-0.8Ni alloys (UNS R52400 and UNS R53400 respectively). Surface treatments, especially anodic oxidations, were conducted in order to enhance the corrosion resistance of pure titanium up to the corrosion resistance of titanium alloys one. Samples were then tested with potentiodynamic analyses in chloride and fluoride containing solutions, in order to find critical pitting potentials for each condition and treatment. The results show that, despite all anodization treatments increased titanium corrosion resistance to different extents, no treatment was able to increase it up to UNS R52400 level. A suggestion on the best anodization procedure to increase corrosion resistance while maintaining ease of treatment is given based on potentiodynamic test results.

Abstract During fabrication of stainless steel, the surface of stainless steel wrought products undergoes major changes, including mechanical (eg. gouges, grinding marks) iron contamination, effects due to welding, and general surface contamination. Various surface treatments, both mechanical and chemical, are performed in order to improve the appearance and/or improve the corrosion resistance of the stainless steel. A literature review has been made to compare mechanical to chemical treatments, and to see if any particular chemical treatment is more effective. Chemical surface treatments seem to be always better than mechanical treatments. Various pickling acids seem to give basically similar results. Surface finish also plays a role. Much of the laboratory testing is likely of questionable value in practice. Some suggestions for further study are included.

New chemicals being brought to the market place, for scale and corrosion control, are likely to be influenced by developing legislation and regulations. Studies of thermal Polyaspartate, a 'green inhibitor', have found it to be an excellent Squeeze control additive for barium and calcium sulphate inhibition. Adsorption and coreflood studies demonstrate the good surface active properties of this thermal Polyaspartate that leads to long squeeze lifetimes. Good calcium compatibility would indicate use in high TDS brines for both sulphate and carbonate control. Corrosion studies demonstrate efficient corrosion control in carbon dioxide systems suggesting that this particular Polyaspartate is a good building block for more efficient formulated products.

Abstract Wire-bound packaging crates are used by the US Army to transport materials. Because these crates may be exposed to harsh environments, they are dip-treated with a wood preservative (biocide treatment). For many years, zinc-naphthenate was the most commonly used preservative for these packaging crates and few corrosion problems with the wires were observed. Recently, copper based alternatives to zinc naphthenate have been used, and corrosion problems have been observed. Here, we present the results of laboratory corrosion testing of 10 different wood treatments to see which preservatives have the potential to cause corrosion problems. The laboratory test was designed to mimic the production process; wires from packaging crates were attached to wood and dipped in the wood preservative and stored in a polyethylene bag for either 2 or 8 weeks to simulate how the crates are stored in the warehouse. The amount of corrosion was examined both visually and gravimetrically. Zinc naphthenate and Preservative A were the least corrosive preservatives, and there were no statistical differences between these two preservatives. The remaining treatments had at least 5 times more corrosion than these two preservatives. Three of the 10 treatments showed a linear increase in the amount of corrosion with time, suggesting that they were still corroding as rapidly at week 8 as they were at week 2 and present a high risk of corrosion. From these results, Preservative A (a copper ethanolamine complex containing citric acid) appears to be the least corrosive alternative to zinc naphthenate.

Abstract A low pH excursion can cause severe damage to an open recirculating cooling system which can cause a plant to shut down. With properplanning, the probability of having a pH excursion can be reduced but not eliminated entirely. Assuming that an accidental excursion has a finite probability of occurring, proper planning can minimize the effects of low pH. Once control of pH has been regained after a low pH excursion, a program of repassivation is necessary to minimize corrosion of active metal surfaces. Depending upon the severity of an excursion, repassivation using an all-organic corrosion inhibitor may be more difficult and time consuming than with other in- hibitors. This paper provides suggestions on how to reduce the probability of having a low pH excursion, details plans on how to regain control of the pH, and presents step-by-step instructions to reestablish the water treatment program when an all-organic corrosion inhibitor is being used.

Abstract To maximize the mechanical properties of Ni-base superalloys, solution heat treatment is essential to sufficiently homogenize the dendritic segregations formed during solidification. To investigate the homogenization behavior during solution heat treatment, a Ni-base single crystal superalloy, TMS-238, was heat treated under various conditions; temperatures ranging from 1573 to 1613 K for times ranging from 2 to 100 h. After solution heat treatment, the average concentrations of Re, an element that exhibits the highest degree of segregation, in dendrite core and inter-dendritic regions were analyzed. From these results, apparent diffusion constants, Dapp, were determined based on a proposed homogenization model. Obtained Dapp values were significantly smaller than the diffusion constant of Re in Ni, strongly suggesting that the apparent diffusion coefficients should be obtained experimentally when using the target alloy.

Slime and water samples from the failure site were screened for corrosion causing bacteria. Pseudomonas sp. and Bacillus sp. were the most common aerobic bacteria isolated whilst sulfate reducing bacteria (SRB) were the major anaerobic bacteria. Compared to sterile controls, metal coupons exposed to Pseudomonas sp and Bacillus sp. showed initiation of severe pitting corrosion suggesting the possibility of MIC. As the appearance of pits was different in both the tested strains, it was thought that the mechanisms of corrosion are different.

We investigate the impact of non-invasive anemia screening on treatment adherence in El Salvador. Anemia is a common childhood condition in low-income countries, due mainly to iron-deficiency, which if left untreated during critical development periods can have lasting health consequences. While effective treatments exist, adherence to treatment is often challenging, since the symptoms of mild to moderate anemia such as lack of energy or paleness can be easily overlooked. We test whether making anemia risks salient through screening during child-well visits can improve treatment adherence, using a cluster-randomized trial in high-poverty areas in El Salvador. Preliminary results indicate that screening enhances treatment adherence and there is suggestive evidence of a reduction of anemia among children 6 to 23 months old. Our results highlight the potential of screening in making anemia risks salient and motivating caregivers to follow treatment plans.

Abstract:
This brief summarizes the Cochrane Review, “Baclofen for Alcohol Use Disorder,” by R. Agabio, R. Saulle, S. Rösner, and S. Minozzi (https://doi.org/10.1002/14651858.CD012557.pub3). The Cochrane Review examines evidence for benefits and harms of baclofen on achieving and maintaining abstinence or reducing alcohol consumption in people with alcohol use disorder (AUD) compared to placebo, no treatment, or any other pharmacological relapse prevention treatment. Review authors conclude that baclofen likely reduces the risk of relapse to any drinking and increases the percentage of abstinent days, mainly among participants who completed alcohol detoxification. Overall, the review suggests that baclofen may help people with AUD in maintaining abstinence. Evidence from a recent review by the RTI–University of North Carolina Evidence-based Practice Center on pharmacotherapy for the treatment of AUD also supports the use of Food and Drug Administration–approved medications like naltrexone and acamprosate. Treatment decisions should consider medication effectiveness, ease of use, side effects, and patient preferences. The review and original commentary acknowledge the limitations of the evidence, suggesting that future research should prioritize direct comparisons between medications, studies in primary care settings, and investigation of treatment effectiveness in diverse populations.

Abstract:
Parkinson's disease (PD) is characterized by divergent clinical symptoms and prognosis, suggesting the presence of distinct subtypes. Identifying these subtypes is crucial for understanding the underlying pathophysiology, predicting disease progression, and developing personalized treatments. In this study, we propose a connectivity-based subtyping approach, which measures each patient's deviation from the reference structural covariance networks established in healthy controls. Using data from the Parkinson's Progression Markers Initiative, we identified two distinct subtypes of de novo PD patients: 248 patients with typical cortical-striato-thalamic dysfunctions and 41 patients showing weakened dorsal raphe nucleus (DRN)-to-cortical/striatal projections. The proposed subtyping approach demonstrated high stability in terms of random sampling of healthy or diseased population and longitudinal prediction at follow-up visits, outperforming the traditional motor phenotypes. Compared to the typical PD, patients with the DRN-predominant subtype were characterized by less server motor symptoms at baseline and distinct imaging biomarkers, including larger striatal volumes, higher concentration of cerebrospinal fluid amyloid-β and amyloid-β/t(p)-tau ratio. Subtype-specific associations and drug effects were identified that the DRN subtype exhibited more pronounced medication effects on motor symptoms, potentially regulated by DRN serotonergic modulation through striatal dopaminergic neurons. The DRN serotonergic inputs also regulated non-motor symptoms, the aggregation of CSF biomarkers and the conversion to more severe disease states. Our findings suggest that the DRN-predominant subtype represents a unique clinical and biological phenotype of PD characterized by an enhanced response to anti-parkinsonian treatment, more favorable prognosis and slower progression of dopamine depletion. This study may contribute to clinical practice of precision medicine, early invention and individualized treatments in PD and other neurodegenerative diseases.

Future emissions regulations for the natural gas industry are imminent and will be, in some cases, very severe.� One of many possible technologies that may be applied to meet these regulations is an exhaust gas after treatment called Selective Catalytic Reduction or SCR. Application of SCR to 2 stroke cycle internal combustion engines offers a unique set of challenges unlike any other in existing SCR systems. This paper reviews these various challenges in detail, enumerates a functional specification for a �typical� compressor station, evaluates a selection of possible SCR system candidates� and offers some suggestions for a successful application. Due to the complexity of select issues, further investigation may be needed.

Freshwaters can experience growths of toxin-producing cyanobacteria or harmful algal blooms (HABs). HAB-producing cyanobacteria can develop akinetes, which are thick-enveloped quiescent cells akin to seeds in vascular plants or quiescent colonies that overwinter in sediment. Overwintering cells produce viable “seed beds” for HAB resurgences and preventative treatments may diminish HAB intensity. The purpose of this literature review was to identify (1) environmental factors triggering germination and growth of overwintering cells, (2) sampling, identification, and enumeration methods, and (3) feasibility of preventative algaecide treatments. Conditions triggering akinete germination (light ≥0.5 μmol m-2s-1, temperature 22-27℃) differ from conditions triggering overwintering Microcystis growth (temperature 15-30℃, nutrients, mixing). Corers or dredges are used to collect surficial (0-2 cm) sediment layers containing overwintering cells. Identification and enumeration via microscopy are aided by dilution, sieving, or density separation of sediment. Grow-out studies simulate environmental conditions triggering cell growth and provide evidence of overwintering cell viability. Lines of evidence supporting algaecide efficacy for preventative treatments include (1) field studies demonstrating scalability and efficacy of algaecides against benthic algae, (2) data suggesting similar sensitivities of overwintering and planktonic Microcystis cells to a peroxide algaecide, and (3) a mesocosm study demonstrating a decrease in HAB severity following preventative treatments. This review informs data needs, monitoring techniques, and potential efficacy of algaecides for preventative treatments of overwintering cells.

This document uses responses from a questionnaire to examine whether tax policy is a barrier to information technology (IT) services imports in Latin America and the Caribbean. The responses show that existing policies do not create a level playing field in the upfront tax burdens paid on transactions, although whether domestic or imported IT services face lower burdens varies by country. The responses also underline that the issue of taxation of services is primarily taken up in double taxation treaties rather than preferential trade agreements, suggesting that trade agreements do not ensure equitable treatment between domestic and imported services as effectively for services as for goods.

We conduct a randomized controlled trial in Mexico City to determine willingness to pay (WTP) for SMS air quality alerts and to study the effects of air quality alerts, reminders, and a reusable N95 mask on air pollution information and avoidance behavior. At baseline, we elicit WTP for the alerts service after revealing whether the household will receive an N95 mask and participant compensation, but before revealing whether they will receive alert or reminder services. While we observe no significant impact of mask provision on WTP, higher compensation increases WTP, suggesting a possible cash-on-hand constraint. The perception of high pollution days prior to the survey is positively correlated with WTP, but the presence of actual high pollution days is not correlated with WTP. Follow-up survey data demonstrate that the alerts treatment increases reporting of receiving air pollution information via SMS, a high pollution day in the past week, and staying indoors on the most recent perceived high pollution day. However, we observe no significant effect on the ability to correctly identify which specific days had high pollution. Similarly, households that received an N95 mask are more likely to report utilizing a mask with filter in the past two weeks, but we observe no effect on using a filter mask on the specific days with high particulate matter. Although we nd that air quality alerts increased the salience of air quality and avoidance behavior, these results illustrate the difficulty that information treatments face in overcoming perceptions to effectively reduce exposure to air pollution.

Abstract:
The objective of this paper is to better understand peer popularity during adolescence and detect discrimination. The paper subsequently explores how student's characteristics affect their popularity. It is found that physically attractive students and with high academic performance are highly ranked by their peers, but the former effect is only significant in mixed schools, suggesting that is primarily driven by mating. Other traits, such as skin color, nationality and parental socioeconomic background do not affect peer popularity, although ethnic origin and parental education are statistically significant in some specifications. The findings are informative about discrimination in the school system. In particular, it appears that the unequal treatment based on race, wealth and nationality found in other social environments in Argentina is not observed among adolescents attending school.

This paper estimates the impact of a large temporary poverty relief program, Uruguay's PANES on birth outcomes. Using program administrative data and longitudinal vital statistics, a significant and precisely estimated reduction in the fraction of low-weight newborns (less than 2,500 g. ) on the order of 10 to 20 percent was found to be a result of treatment. The cash (and in-kind) transfer components of the program were considered to drive the results, suggesting that unrestricted social assistance has the potential to positively affect birth outcomes, most likely through improved nutrition. Assuming that all the effect of the program was through the transfer, an elasticity of low birthweight with respect to welfare transfers on the order of around 0. 30 can be inferred.

Purpose: The aim of this study was to assess the ocular morbidity among the patients post SARS-CoV-2 infection. Methods: This is a prospective observational study conducted on patients post SARS-Co-V-2 infection, at a dedicated tertiary COVID-19 hospital in South India from June,2020 to August, 2021.A thorough history regarding the onset, duration, progression, nature of symptoms and its associated factors, medication history, treatment history were elicited and documented. Ocular examination was performed by an ophthalmologist with proper care. Further investigations including imaging were sought for, depending on clinical indications. A Results: total of 60 patients were examined. Of them, 41 (68.3%) were males and 19 (31.7%) were females, the age group ranged from 12 years to 62 years. On clinical examination, 45 (75%) developed ocular manifestations, of which 22 (48.8%) had features suggestive of Bilateral viral conjunctivitis solely,20(44.4%) had features suggestive of viral retinitis and 2(4.4%) had combined features of conjunctivitis and viral retinitis. 1 (2.2%) patient had developed Mucor mycosis. In our study, Viral retinitis was most common ocula Conclusion: r morbidity among the covid patients.This pandemic has changed our lives in many ways and various ocular morbidities were seen too, of which few cases recovered with timely diagnosis and management.
