Abstract:
Aim of study. Considering the high spreading of tinnitus in population, the purpose of our research was to study the possibility and expediency of using hypnosuggestive therapy in treatment of tinnitus. Material and methods. The study included 55 people with tinnitus complaints. We divided three groups of patients depending on the severity of the manifestations, audiometry results, Tinnitus Handicap Inventory (THI) score according to this scale of severity, results of GAD-7 test and the PHQ-9 (Patient Health Questionnaire -9). Results. In the first group of patients, according to the tinnitus severity scale the score was up to 19 points and was considered as insignificant and mild. The score of the GAD-7 questionnaire was less than 8 points, PHQ-9 - up to 5 points. In the second group the severity of tinnitus was moderate and severe and counted more than 38 points. The GAD-7 questionnaire score was less than 8 points, PHQ-9 - up to 5 points. In the third group, 12 people complained of chronic tinnitus, sleep disturbances, nervousness, and reduced work capacity. The score of tinnitus severity in patients of this group was more than 58 points and was assessed as severe and catastrophic. According to the results of the GAD-7 and PHQ-9 questionnaires, all 12 people were diagnosed with anxiety disorders and depression, two parients even had suicidal thoughts. In the third group of patients with +++ hypnotability, after ten sessions of individual hypno-suggestive therapy, it was possible to reduce the severity of tinnitus to the third moderate degree in 7 people. 4 patients reported about complete regress of tinnitus. Conclusions. Tinnitus is a common problem and can negatively affect work, family and social life. It requires the individual approach and involvement of specialists of various profiles in the diagnostic process. The diagnostic algorithm for patients with tinnitus should include questionnaires to estimate the psychosomatic profile. The use of hypno-suggestive therapy can significantly reduce the severity of tinnitus and improve the quality of lives of patients.

Background: Habit cough is one of the causes of chronic cough in children. Many cases do not reach the psychiatrists due to lack of specific clinical features and diagnostic tests. We present a case of 13 year old boy with habit cough who was successfully treated by suggestion therapy and lignocaine based mouth gargle as a distracter. This case highlights that suggestion therapy can be an important treatment option to this fairly common condition.

Background: It remains unclear as to whether verbal suggestions and expectancies can influence the perception of post-needling soreness. The aim of this study was to analyze the effects of verbal suggestions on post-needling soreness after dry needling of the trapezius muscle. Methods: This study is a randomized controlled trial including healthy subjects randomly assigned to one of three groups receiving different verbal suggestions about the effects of dry needling and the occurrence of post needling soreness (positive, negative, or neutral). Then, dry needling on a latent trigger point of the upper trapezius muscle was performed and the following outcomes were measured immediately after, 24, 48, and 72 h, and one week after the intervention: post-needling soreness intensity, pressure pain threshold (PPT), temporal summation (TS) and conditioned pain modulation (CPM). Results: Seventy-three consecutive participants were screened and 42 participants (12 men and 30 women, aged: 24 ± 8 years old) were eligible and finished the study protocol. The results showed that verbal suggestion did not influence the perception of post-needling soreness, since there were no differences between groups (p &lt; 0.05) on the intensity of post-needling soreness or tenderness over a one-week follow-up. Moreover, verbal suggestion did not associate with changes in sensorimotor variables of TS and CPM. Conclusions: The induction of different types of expectations through verbal suggestion does not influence the perception of acute pain perceived during the performance of a deep dry needling technique and post-needling pain or soreness after deep dry needling on a latent upper trapezius myofascial trigger point (MTrP).

Abstract:
The potential of psychedelic drugs in the treatment of mental health problems is increasingly being recognized. However, relatively little thrust has been given to the suggestion that individuals without any mental health problems may benefit from using psychedelic drugs, and that they may have a right to do so. This review considers contemporary research into the use of psychedelic drugs in healthy individuals, including neurobiological and subjective effects. In line with findings suggesting positive effects in the treatment of mental health problems, such research highlights the potential of psychedelic drugs for the enhancement of wellbeing even in healthy individuals. The relatively low risk associated with usage does not appear to align with stringent drug laws that impose heavy penalties for their use. Some policy implications, and suggestions for future research, are considered.

Abstract:
Adoptive immunotherapy with interleukin-2 (IL-2) and lymphokine-activated killer (LAK) cells has been effective in treating some advanced malignancies in animals and humans. One complication of this treatment is a reversible, oliguric, acute renal failure, which has been ascribed to renal hypoperfusion and resultant prerenal azotemia. We serially studied renal function in 10 patients receiving high-dose regimens of recombinant interleukin-2 (rIL-2) to attempt to delineate further the nature of the renal dysfunction caused by IL-2 treatment. Renal plasma flow was computed from iodine 131 (I-131 Hippuran; Mediphysics, Paramus, NJ) orthoiodohippurate, excretion curves, and glomerular filtration rate (GFR) was determined by creatinine clearance. Studies done prior to and on day 4 of treatment showed that GFR fell in nine of 10 patients, with a mean decrease of 43% +/- 8%, and renal plasma flow fell in five of the 10 patients with a mean decrease of 5% +/- 10%. The average pretherapy filtration fraction was calculated to be 23% +/- 1% and after 4 days of treatment, decreased to a mean value of 15 +/- 2%. The BUN to creatinine ratio also declined in all patients. These findings collectively suggest that IL-2 nephrotoxicity may result from an intrarenal defect in addition to the previously described prerenal azotemia. Additionally, radionuclide studies of renal function are a reliable and reproducible noninvasive method of assessing these changes in renal function.

Aspects of Canada’s health regulatory system are currently being reviewed. This is timely, as the regulation and definition of drugs, foods, and natural health products (NHPs) is in need of revision to facilitate greater transparency and less ambiguity. A number of studies have illustrated the importance of a nutritious diet to prevent and manage chronic disease. Therefore, legislation surrounding food health claims needs to be adjusted so that it is more informative for disease prevention and, in some cases, treatment. Canada is modernizing the regulation of self-care products, under which NHPs, including probiotic products, are listed. With the growing appreciation for the role that microbes play in human health and the recognition that many foods, including those containing probiotic organisms, can prevent or mitigate disease, this provides an opportunity to reassess regulatory categories.

In the late nineteenth century, German-speaking physicians and psychiatrists intensely debated the benefits and risks of treatment by hypnotic suggestion. While practitioners of the method sought to provide convincing evidence for its therapeutic efficacy in many medical conditions, especially nervous disorders, critics pointed to dangerous side effects, including the triggering of hysterical attacks or deterioration of nervous symptoms. Other critics claimed that patients merely simulated hypnotic phenomena in order to appease their therapist. A widespread concern was the potential for abuses of hypnosis, either by giving criminal suggestions or in the form of sexual assaults on hypnotized patients. Official inquiries by the Prussian Minister for Religious, Educational and Medical Affairs in 1902 and 1906 indicated that relatively few doctors practised hypnotherapy, whereas the method was increasingly used by lay healers. Although the Ministry found no evidence for serious harm caused by hypnotic treatments, whether performed by doctors or by lay healers, many German doctors seem to have regarded hypnotic suggestion therapy as a problematic method and abstained from using it.

The facts show that corruption among Government Officials is considered an acceptable behavior, even though the impact is tremendous. If there are officials who reject or are anti-corruption, they will be removed or will be eliminated by themselves. Meanwhile, internal and external controls are not effective in overcoming it. There must be innovation in efforts to eradicate corruption in Indonesia. The findings of this study indicate that government officials in their work environment will be very effective if involved in efforts to prevent and eradicate acts of corruption. This is manifested in 2 forms of effective management,namely:(1)Apparatus Complaint Management and(2)Suggestion Box Management. Apparatus Complaint Management has the character of conveying corrupt behavior or actions that occur in the apparatus work environment. Suggestion Box Management is an opportunity provided for all officials to convey their ideas and input to prevent and eradicate acts of corruption in the workplace.

Advances in observational and computational assets have led to revolutions in the range and quality of results in many science and engineering settings. However, those advances have led to needs for new research in treating model errors and assessing their impacts. We consider two settings. The first involves physically-based statistical models that are sufficiently manageable to allow incorporation of a stochastic “model error process”. In the second case we consider large-scale models in which incorporation of a model error process and updating its distribution is impractical. Our suggestion is to treat dimension-reduced model output as if it is observational data, with a data model that incorporates a bias component to represent the impacts of model error. We believe that our suggestions are valuable quantitative, yet relatively simple, ways to extract useful information from models while including adjustment for model error. These ideas are illustrated and assessed using an application inspired by a classical oceanographic problem.

The maxillary canines rank second after the third molars in terms of the frequency of impaction. The permanent canine teeth play a fundamental role in a functional occlusion and a balanced smile. This study aimed to review the literature and introduce clinical guidelines for surgical canine exposure techniques. The choice of treatment is influenced by several factors such as canine location, severity of impaction, patient’s age, and other considerations. There are three treatment options, namely observation, exposure and extraction. Based on the location of the impacted canine, the amount of keratinized gingiva and distance from the gingival margin, a flow diagram is introduced for decision-making to choose an appropriate surgical technique.

Plant diseases are some of the most relevant topics affecting crop yield productivity because they lead to economic losses for farmers and impact food availability. To address such a concern, we developed a system that utilizes deep learning techniques, particularly Convolutional Neural Networks (CNNs), to identify diseases by analyzing images. Detection is just one aspect of our project. In addition, we prepared responses that provide actionable steps for managing the illness in question. The suggested approach is to design the tool as user-friendly and intuitive for farmers. Additionally, users have the ability to inquire further about the illness and its management. Although this past research already demonstrated the efficacy of CNNs for such problems, our approach integrates an additional layer. It leverages a potent AI model to suggest relevant, real-time treatment options tailored to the situation. This model serves to issue practical recommendations comparable to those given by an expert. This system provides farmers with a sophisticated yet simple method for mitigating risks to their crops and enhancing their decision-making by merging straightforward and useful instructions with the automated identification of plant diseases. There were two methods of using &lt;i&gt;Seunggi-tang&lt;/i&gt;: the Ha treatment with usual volume and the Hwa treatment, with reduced volume. &lt;i&gt;Seunggi-tang&lt;/i&gt; especially &lt;i&gt;Soseunggi&lt;/i&gt; and &lt;i&gt;Jowiseunggi-tang&lt;/i&gt;, were used not only as the Ha treatment but also as the Hwa treatment for several diseases such as gastrointestinal diseases with Bokman or Chogyeonghudang.Conclusions: This study reveals that &lt;i&gt;Seunggi-Tang&lt;/i&gt; could be used Ha or Hwa treatment and Hwa treatment could be utilized in various clinical situation. Further study is needed.

Background: A habitual cough, persisting after the cause is gone, was described in a 1694 medical book. Successful treatment of this disorder known as habit cough was reported in 1966 by the “art of suggestion”. The purpose of this article is to provide the current basis for diagnosis and treatment of the Habit Cough Syndrome. Method: The epidemiology and clinical course of habit cough were reviewed; original data were obtained from three sources. Results: Unique clinical presentation was the basis for diagnosis of habit cough. Diagnosis was made 140 times with increasing frequency over 20 years at the University of Iowa clinic and 55 times over 6 years at a London clinic. Suggestion therapy provided more frequent cessation of cough than just reassurance. A Mayo Clinic archive of chronic involuntary cough found 16 of 60 still coughing 5.9 years after initial evaluation. Ninety-one parents of children with habit cough and 20 adults reported cessation of coughing from viewing a publicly available video of successful suggestion therapy. Conclusions: Habit cough is recognizable from the clinical presentation. It is effectively treated in most children by suggestion therapy in clinics, by remote video conferencing, and by proxy from viewing a video of effective suggestion therapy.

The relationship between kidneys and anticoagulation is complex, especially after introduction of the direct oral anticoagulants (DOAC). It is recently growing evidence of an anticoagulant-related nephropathy (ARN), a form of acute kidney injury caused by excessive anticoagulation. The pathogenesis of kidney damage in this setting is multifactorial, and nowadays, there is no established treatment. We describe a case of ARN, admitted to our Nephrology Unit with a strong suspicion of ANCA-associated vasculitis due to gross haematuria and haemoptysis; the patient was being given dabigatran. Renal biopsy excluded ANCA-associated vasculitis and diagnosed a red blood cell cast nephropathy superimposed to an underlying IgA nephropathy. Several mechanisms are possibly responsible for kidney injury in ARN: tubular obstruction, cytotoxicity of heme-containing molecules and free iron, and activation of proinflammatory/profibrotic cytokines. Therefore, the patient was given a multilevel strategy of treatment. A combination of reversal of coagulopathy (i.e., withdrawal of dabigatran and infusion of its specific antidote) along with administration of fluids, sodium bicarbonate, steroids, and mannitol resulted in conservative management of AKI and fast recovery of renal function. This observation could suggest a prospective study aiming to find the best therapy of ARN.

Abstract:
The review presents current data on heart disease in systemic amyloidosis. The article discusses variants of amyloi-dosis with common heart disease: AL-, ATTRwt- and ATTRv amyloidoses. Clinical signs of amyloid cardiomyopathy and extracardiac manifestations that allow one to suggest amyloidosis are described. Methods for diagnosing amyloid cardiomyopathy are presented. Echocardiographic diagnostic characters include increased thickness of the left ventricular wall, atrioventricular valve, right ventricular wall, thickening of the interatrial septum, and also diastolic dysfunction, and decreased global longi-tudinal strain with relative preservation of apical longitudinal strain. Magnetic resonance imaging reveals signs specific for amyloidosis: increased extracellular volume and diffuse suben-docardial or transmural delayed enhancement after gadolinium administration. Scintigraphy using technetium-99m labeled compounds can confirm the diagnosis of ATTR cardiomyopathy. The diag-nosis of ATTR cardiomyopathy is only possible in combination with blood and urine tests to exclude the presence of a monoclonal protein. Screening for monoclonal protein includes three laboratory tests: determination of free light chains in blood serum, electrophoresis of serum protein and urine protein with immunofixation. Indications for myocardial biopsy and the role of extracardiac tissue biopsy are discussed. Endomyocardial biopsy should be performed: a) in case of strong clinical suggestion of cardiac amyloidosis in a patient with monoclonal pro-tein; b) in case of strong clinical suggestion of cardiac amyloidosis, despite negative or equivocal scintigraphy result; c) if cardiac scintigraphy is not available. The possibilities of pathogenetic therapy for ATTR- and AL amyloidosis, aimed at stopping or slowing amyloid deposi-tion in the heart, and symptomatic treatment are discussed.
