The criminal trials of twenty-seven women processed for the crimes of abortion and infanticide in the state of Puebla, Mexico during the nineteenth century reveal both community and state perspectives about contemporary notions of gender, motherhood, and honor. Medical management is a relatively safer option than the conventional surgical method in the first-trimester miscarriage. The chapter included studies which allocated women in the pharmacological intervention for spontaneous miscarriage in the first trimester and determined the effectiveness, safety, and side-effect. Authors have included study trials of women with first-trimester miscarriage and conducted a review generating both direct and mixed evidence on the effectiveness and side effects of medical management. Out of 2246 studies found in the databases, 32 were included in the systematic review comprising of 56116 patients undergoing first-trimester miscarriage with 54,890 undergoing medical termination of pregnancy, and 7 studies were included in the meta-analysis. Results showed that the odds of success in medical intervention were low when compared with surgical intervention. Medical management of patients with symptoms of early miscarriage is safer and more effective compared to expectant and well tolerated compared to surgical management.

Abstract While pregnant people are among those most in need of access to effective treatment and preventive drugs and vaccines, evidence to inform use of pharmaceuticals in pregnancy is sorely lacking. Recent years have seen growing recognition of the urgent need to increase responsible research in pregnancy. This chapter outlines the state of play on key issues regarding such research. First, it describes evidence gaps, their systemic causes, and their human and moral costs. Next, it addresses conditions under which pregnant people may ethically be included in studies, highlighting issues of allowable risk, consent, and abortion. Finally, it turns to the important issue of fair inclusion of pregnant participants in research, at the levels of both agenda-setting structures and individual trials. Together these considerations point to the collective responsibility of the global research community to identify and pursue research that meets the needs and honors the interests of pregnant people.

Abstract Kelsey sent questionnaires to state health boards to determine the extent of birth defects. Merrell had not followed up with physicians, nor had many physicians kept records on patients; “clinical trials” were simply free samples. Some soldiers’ wives in Germany were affected. FDA inspectors contacted some uncooperative physicians. Most thalidomide baby lawsuits were settled by Richardson-Merrell, although the company put up a strong defence. Lawyers strongly pressured the working-class single mother of Peggy McCarrick to give up her suit. Kelsey was deposed by Richardson-Merrell counsel. When Sherri Chessen, an Arizona children’s TV personality, tried to get an abortion for her thalidomide baby, it made national news, and she became the flashpoint for anti-choice and pro-choice debates. Forced to go to Sweden to have the procedure, Chessen was vilified by male reporters as flighty while she went public to warn other women. Her experience laid the groundwork for the 1973 Roe v. Wade decision.
