Full textThis study assesses firstly, the relations between psychopathy and personality disorders (DSM Axis II) and secondly with the risk for recidivism. A particular interest is related to the Bordeline disorder considering his implication in the treatment (some borderline characteristics would be of a better forecast). The results show a link exists between psychopathy and the presence of Narcissistic, Antisocial, Borderline and Paranoid disorders. Contrary to expected, the Borderline disorder does not reduce the level of psychopathy (importance of the impulsive behaviours). These disorders are also associated with the risk for recidivism and the number of violent offences (but not with the type of offences violent versus non violent). The results suggest that impulses, antisocial, narcissistic and paranoid characteristics are essential to find. The importance of clinical and empirical assessment in research but also in clinical practice for diagnostic and prognostic precision is underlined

Full textAddictions, acting-out and eating disorders are known to be comorbid with borderline personality disorder. Their high frequencies mean that they support involvements and functions in the way that borderline personality operates. Borderline subjects are led to acting-out urgently by mood disorder, depression, emotional instability and incapacity to regulate them. In borderline personality, defense mechanisms are diversified between nevrosis and psychosis. The major dysfunction seems to set on fail of self-containing and porosity of the borders of the self. Identity disorder and narcissistic failures are the main issues in borderline personality disorder which lead to recreate pathological relationship to the primary object with later relationship. Dependent relationship and fear of abandonment manage borderline subjects and their symptomatology with addictions and acting-out. This research aim to examine different dimensions: interpersonal dependency, attachment, alexithymia, depression, hopelessness feeling, anaclitism, dependent and avoidant personality disorders according to addictions with and without drugs, and suicide attempts and non suicidal self-injury

Full textOur thesis explores the underlying mechanisms of thought and language disorders commonly observed in patients suffering from bipolar disorder, but also to a lesser degree, in people with hypomanic personality traits. To explore the underlying semantic processes, we conducted four studies with a semantic ambiguity resolution task coupled with event-related potentials recording and fMRI. Our aim was to identify biomarkers for a better comprehension of clinical or subclinical symptoms associated with bipolar disorder.Through our studies, we have shown difficulties to handle semantic inhibition in the two explored populations with, at an electrophysiological level, a reduced N400 amplitude during semantic inhibition process, correlated to Social Vitality score (hypomanic personality traits) in non-clinic population, and to hypomania score in bipolar patients. In the fMRI studies, activations of the superior frontal gyrus and of the inferior parietal lobule were correlated to hypomanic traits and were also found in bipolar patients when compared to the control group. Supplementary right fronto-temporal activations were observed in bipolar patients. Our results imply an impairment of the semantic inhibition process already present in people with bipolar disorders and people with hypomanic traits. These results combined with the existing literature suggest that N400 modulations and fronto-temporo-parietal activations found might be transnosographic biomarkers of thought and language disorders, in bipolar spectrum disorders but also in other disorders like schizophrenia spectrum disorders

Full textThe objective of this work is to evaluate the quality of insight and the characteristics of impulsivity in patients with borderline personality disorder. We assessed cognitive and clinical insight (BCIS, SUMD), impulsivity (UPPS) and symptomatology (BPQ). Our studies 1 and 2 assessed the impulsivity level of borderline personality disorder and the impact of the positive and negative urgency dimensions on symptomatology. Our studies 4 and 5 assessed the quality of clinical and cognitive insight and its impact on impulsivity and symptomatology. In study 5 specifically we linked the metacognition dimension to specific mechanisms and confirmed the presence of good insight level in borderline patients as well as a high level of metacognition. Our study 3 examined the difference between impulsivity and aggression in the field of personality disorders. Finally, our 6th study examines the dynamics of positive urgency in relation to insight. Our following studies explore the dynamics of impulsivity, highlighting the important role of negative but also positive urgency. This work made it possible to complete theoretical knowledge, to highlight the importance of positive emotions in the impulsive processes of TPBs, to confirm the presence of good quality insight and to explore new avenues for therapies for this disorder

Full textIntroduction: People living with a cluster B personality disorder (PD) experience several functional difficulties in their everyday activities. These individuals also experience marked disruptions in their identity that affect their social participation and quality of life. The notion that engagement in occupations shapes one's identity is widely recognized in occupational therapy. Despite the clinical importance of these issues, there has been no empirical validation for this clientele. Objective: The purpose of this study was to explore how occupations and their meanings in the eyes of people living with group B PD shape their identity. Methods: This exploratory study used a mixed methods design. Its qualitative component is based on an interpretative descriptive design while the quantitative component is descriptive. Ten adults, aged between 18 and 35 years old, living with a cluster B PD shared narratives on occupations that are important for them and how their occupational experiences shape their vision of themselves. As a complement, the Engagement in meaningful activities survey (EMAS) and the Meaningful activity wants and needs assessment (MAWNA) were administered, to optimize the triangulation of the data. Results: Participants often engage in a variety of occupations that reflect who they are although they point out that they need more. Four cases illustrate the different contributions of occupation to participants' identity: 1) the importance of occupations as a source of social affiliation that influences identity construction; 2) the contribution of past and anticipated occupations; 3) the idealized contribution of a sole occupation and 4) the contribution of virtual occupations. Conclusion: These results highlight the importance of the context in which occupations take place and of subjective experiences to better understand how occupations contribute to the identity of people living with a cluster B PD.

Full textAbstract: Introduction: Borderline Personality Disorder (BPD) has a major impact on the daily lives of people affected by it. A rigorous and reliable assessment of the day-to-day functioning of people with BPD is essential to adequately guide rehabilitation services. Objectives: The main objectives of this project are to: 1) to design a tool for measuring the functioning of people with BPD; 2) validate this new measurement tool by verifying its internal consistency, test-retest reliability and convergent construct validity with tools measuring functioning. Methods: The development of the dimensions of the questionnaire and the measurement scale was done through a literature review and consultations with 42 experts (persons with BPD, clinicians and researchers) through various methods (focus groups, Delphi survey). A qualitative pre-test was carried out with eight persons with BPD in order to verify the intelligibility of the questionnaire. For test-retest reliability, the new questionnaire was administered twice, with a two-week interval, to 36 adults with BPD. The convergent construct validity was obtained by comparing the scores from the 36 participants with those obtained from the World Health Organization Disability Assessment Schedule (WHODAS 2.0) and the Global Functioning Assessment Scale (GAF), a tool completed by a psychiatrist. Results: The FAB self-administered questionnaire consists of 38 items divided into four areas: basic day-to-day activities, community-based activities, dimensions of the social environment, and internal dimensions of the person. Each item is assessed according to two rating scales: 1) the person's mode of functioning and its impact on health and well-being; 2) the degree of difficulty perceived. The FAB’s internal consistency was high (Cronbach’s alpha of 0.88 for scale 1 and 0.82 for scale 2). The FAB demonstrated very good to excellent test-retest reliability (ICC=0.92, 95% CI: 0.85-0.96). Both scales were moderately correlated with the GAF (r=0.48, p=0.003 and r=0.41, p=0.013) and strongly correlated with the WHODAS 2.0 (r=-0.70, p<0.001 for both scales). Conclusion: The FAB is a promising new questionnaire addressing functional dimensions affected by BPD. It can serve as a trigger for more in-depth discussion between persons with BPD and clinicians to identify targets for change in their daily functioning.

Full textThis thesis addresses current concerns about a specific grief psychopathology, the Prolonged Grief Disorder (PGD) affecting the elderly. After developing an inventory of knowledge about grief, specifically relating to PGD, (chapter 1) and a theoretical model of Grief (chapter 2) to identify the psychological and psychopathological characteristics of the elderly people experiencing grief over the death of someone, (chapter 3), the importance of continuing the study of this Disorder was realized (chapter 4). To this end, a preliminary study was conducted which enabled the validation of the Inventory of Complicated Grief-Revised (Prigerson and al., 2004). This validation enabled the development of a threshold score which could be used as diagnosis criteria for PGD (chapter 5). In light of the suggested theoretical models regarding grief and its outcomes, a second study was conducted to test a model that explains personality effects on the development of PGD (chapter 6). The approach would not be comprehensive or complete if, on a topic like grief, a qualitative method was not adopted to understand the essence of the expression of PGD through the feelings of an elderly person. A third and final study enabled the identification of a pathological personality profile, by using MMPI-2, and the establishment of the qualitative method of confirmation of PGD, through case studies (chapter 7). The overall goal of these studies is to contribute to increase awareness and understanding of PGD as a psychopathological disorder

Full textThe subject of our work was to study the influence of type of attachment and maltreatment on personality disorders in a population of patients addicted to drugs who receive replacement therapy. Our sample consisted of 140 patients with a diagnosis of toxic drug addiction substitu-tion treatment, including 70 women who represent the experimental group and 70 men who represent the control group. To evaluate the traumatic events in childhood, we used the CASRS (Child Abuse Self Report Scale), for assessing personality disorder we used MCMI (Millon Clinical Multiaxial Inventory) and finally the ASQ (Attachment Scale questionnaire) to meas-ure the type of attachment. Our results show that there are significant differences between males and females. A higher occurrence of abuse during childhood, demonstrates insecure at-tachment, more comorbid personality disorder and a socio-familial especially among women, which may have etiological significance. These epidemiological differences have an impact on the severity of substance abuse (more importantly the emotional and physical abuse). There-fore, these results encourage us to rethink our strategies of care for more specific approaches while handling such patients

Full textDisorganization of speech is commonly observed in the manic phase of bipolar disorder as incomprehensible language, disjointed or illogical ideas. Recent studies had explored the cognitive processes that underlie these disturbances of language and formal thought disorders in bipolar trouble. These studies appear to be in favor of semantic abnormalities. To determine the nature of these disturbances, we carried out studies which focus on two specific processes: the automatic spreading activation and the semantic inhibition. To assess these processes, two lexical decision tasks based on semantic priming were built. These tasks are proposed to 17 euthymic bipolar patients as well as to a group of 61 people from the general population for whom hypomanic personality traits and affective temperaments are evaluated. The results show lower efficacy for both processes in bipolar disorder. These disturbances are associated with the disturbances of formal thought disorders. In the general population, the results show that only the dysfunction of the automatic spreading activation is associated with hyperthymic and irritable temperaments. This association is not found for semantic inhibition, suggesting the intervention of compensatory cognitive strategies when the processes are controlled. Our results, combined with those of the literature, seem to be in favor of a dimensional approach to bipolar disorder and underline the interest of investigating cognition in the attenuated forms of the disorder

Full textAutobiographical memory plays a core role in the self’s construction. It is through memories of experienced events that the identity notion is constructed and that life goals and problem-solving strategies are elaborated. Investigate autobiographical memory offers an overview of how individuals forge and perceive themselves through the prism of life story. In this thesis we have chosen to examine the autobiographical memory of men with antisocial personality disorder, having committed crimes and offences and inpatients in forensic hospital. Study their autobiographical memories and more specifically the characteristics of their self-defining memories allowed us to highlight deficits in their retrieval’s abilities. Their difficulties in structuring and in meaning making from their past events as well as the emotion of anger associated with their recall seem to indicate the influence of their traumatic history in their symptomatology. Through semi-qualitative, behavioral and physiological measures, we emphasized the importance of the integrative dimension in the identity process. Hence, this thesis, by its contribution of knowledge on antisocial personality disorder, opens new clinical and scientific perspectives

Full textThe research presented originates in a double set of problems : on the one hand revelation of cannabis addiction and on the other hand the link between cannabis addiction and borderline personality adaptation ("character neurosis" and "character psychosis"). With this aim in mind, we hypothesized that self esteem, behavioral involvement, activity system and representation of the rules and expectations of society define the type of personality adjustment of cannabis addict ("character neurosis" et "character psychosis"). The first phase of our research consisted of characterization of the sample. Our population consisted of 70 cannabis addict selected according to precise inclusion criteria (the cannabis dependency questionnaire, the Addiction severity index and criteria of bordreline personnahty according to the DSM IV). The second phase investigated our general hypothesis based on self-esteem, behavioral involvement, activity system and representation of the rules and expectations of society. In order to test our hypothesis and its correlations, we used the following evaluation tools : the Self esteem inventory, Hand test, the Telic dominance scale and the semi-structured interview mode. Self esteem, activity system and representation of the rules and expectations of society are in fact discriminant components of the cannabis addicts in our sample. Behavioral involvement was weakly significant in differentiating cannabis addicts according to personality adjustment. In conclusion, the personality adjustment of the cannabis addict, has an influence on cannabis consumption. Cannabis addiction is controlled if personality adjustment is of "character neurosis" type, but on the other hand addiction will be reinforced if the adjustment is of "character psychosis" type

Full textFor several years, research has suggested that executive functions (EF), as well as some temperament dimensions and personality traits involved in the regulation of both emotion and behaviors, are differentially linked to symptoms of attention deficit hyperactivity disorder (ADHD). The main aim of this study is to examine the relationships between EF, temperament dimensions, personality traits, and ADHD symptoms in a clinical and a nonclinical sample of adolescents. 118 adolescents (75 ADHD, 68% males) aged 12 to 16 and their parents and teachers underwent a comprehensive diagnostic assessment including ADHD clinical interview, ADHD rating scales and a measure of intellectual ability. Performance-based tests, EF behavior rating scales, and temperament and personality measures were also included. Study I showed that ADHD adolescents perform worse than controls on cognitive performance-based EF measures and behavioral EF ratings. The predominantly inattentive (ADHD-I) and combined (ADHD-C) presentations did not differ in terms of task performance. The performance-based tests and EF rating scales distinguished well between ADHD and control groups. Regression analysis showed that neuropsychological tests were not very good predictors of behavior rating scale outcomes in ADHD and control groups. Study II indicated that the degree of concordance between informants (parents, teachers and self-reports) in the behavioral EF ratings ranges between poor and medium for the ADHD group, and between moderate and good for controls. The Bland and Altman graphical method, which allows better visualization of the extent of agreement between informants showed that informants agreed on the presence of difficulties in the EF rating scales, but disagreed regarding their frequency and intensity in ADHD adolescents. Study III revealed significant differences between ADHD and control groups on cognitive EF and temperamental dimensions. Compared with controls, the ADHD group scored lower on measures comprising EF of working memory, planning and inhibition and Effortful control dimension, and higher on Surgency and Negative affectivity dimensions. These two types of measure differentiated well between ADHD and control groups. Mediation analysis showed that effortful control had a mediating role on the relations between cognitive EF and inattentive symptoms. Study IV found that the ADHD group scored lower on measures of cognitive EF (working memory, planning and inhibition) than controls, as well as on the personality traits of Conscientiousness, Openness and Agreeableness, and scored higher than controls on Neuroticism. These two types of measure differentiated well between ADHD and control groups. Mediation analysis showed that cognitive EF explained the relationship between both Conscientiousness and Openness personality traits and inattentive symptoms. In summary, the results highlight complex relationships between cognitive and behavioral aspects of EF, temperamental dimensions, personality traits and ADHD. Differential relationships between EF deficits, low levels of effortful control, Conscientiousness and Openness, and the expression of ADHD symptoms during adolescence are also found. These complex relationships illustrate the value of an accurate diagnosis and a comprehensive approach to the treatment of ADHD adolescents.

NP can be defined as the ability of the nervous system to respond to intrinsic or extrinsic stimuli by reorganizing its structure, function and connections and can be described from a translational perspective at many levels, from molecular to cellular to systems to behaviour. Psychiatric disorders are characterized by a high degree of heterogeneity (pathophysiological, etiologic and clinical levels) which can be conceived as an advantage when examined from the perspective of the NP. In fact, psychiatric disorders can be used as models to study the different aspects of the NP.Individual-dependent plasticity−The lack of susceptibility genes related to several psychiatric disorders may be due to tendency to look for genetic effects on disease rather than genetic effects on vulnerability to environmental causes of disease. In fact, \\\"vulnerability genes\\\" may function more like \\\"plasticity genes\\\", resulting in greater susceptibility of individuals to both positive (e.g., environmental support and enrichment) and negative (e.g., childhood maltreatment) facets of environmental experiences. This concept has been proposed in an integrative model of a frequent psychiatric disorder : the borderline personality disorder (BPD). Age-dependent plasticity−There are qualitatively and quantitatively different changes in the brain in re¬sponse to what appears to be the same experience at dif¬ferent ages. This aspect of NP has been studied by using the \\\"Borderline Personality or Complex Posttraumatic Stress Disorder controversy\\\" with a genetic association study, with independent replication, on genes associated with the physiological response to stress in the hypothalamic–pituitary–adrenal (HPA) axis. Symptom-dependent plasticity−Adaptative neuroplastic modifications in the structure and function of the human brain in response to environmental demands have been showed in numerous situations in healthy controls. Interestingly, NP have also been associated to adaptation to pathology in several psychiatric disorders. A recent example has been provided by the study of visual hallucinations in schizophrenia patients. Hallucinations can be defined as perceptions in the absence of external stimuli. In schizophrenia, hallucinations constitute the most typical and disabling symptoms of the disorder and may manifest in all sensory modalities. Several MRI findings support the hypothesis that distinct patterns of connectivity, particularly within networks involving the hippocampal complex (HC), could be associated with different hallucinatory modalities. The aim of our study was to investigate HC connectivity as a function of the hallucinatory modality, i.e., auditory or visual in two carefully selected subgroups of schizophrenia patients with only auditory hallucinations (AH) or with audio-visual hallucinations (A+VH). Hippocampal volume and shape analysis showed localized hippocampal hypertrophy in the A+VH group. These structural findings indicate that plastic changes are associated with hallucinations. Indeed, the hippocampus is capable of plastic deformation, and the present findings are consistent with experience-dependent shape modifications of the hippocampus that involve mechanical tension along the axon.This PhD thesis highlights several arguments that the NP perspective provide new insights to the pathophysiology, to improve and emphasize therapeutic innovation of psychiatric disorders

Full textThis research aims to explore the intrapsychic world of mothers and infants through the study of early mother-infant interactions of borderline mothers with their three-month-old infants. Newly acquired motherhodd and birth are opportunities for psychic reorganisation. The advent of motherhood is a developmental narcissistic upheaval, a "crisis" that weakens the fragilities of the already unascertained identity of borderline mothers. The newborn infant is eager for social and emotional communication. Its needs require mandatory care, implying necessary changes in the cargiver's establishment of relationships. The dysphoric impulsive borderline mothers alternate between the fear of being abandonned and the search for immediate and complete closeness, denial of void and loss, and bitter uncontrolable anger. These mothers are in constant fear of being overwhelmed by feelings triggered by their inffants' emotional needs. In an effort to keep their own emotions form overriding them, they establish patterens of intrusive repetitive, non-contingent interactions. Only direct instructions such as that of the Still Face Paradigm are able to interrupt the flow of maternal stimulations. We speculate that depressive mood could be reconsidered as a newly -discovered inclination towards what might become the start of self-reflection. This creates a means for the boderline to reunite with some of her own emotional chilhood experiences, therefore facilitating her empathic movements towards her infant. The infant in turn, will be able to show more positive affect and less autonomic behaviors and distancing allowing more room for efficient personal self-regulation

Full textThe Five Factor Model of personality which has a very good cross-cultural and trans-situational factorial stability, made it possible to undertake scientific studies on the relationships between personality and performance or counter-productive behaviors at work. The principal aims of this study consist in examining 1) the effects of the social desirability on the factor structure and the scores of the NEO PI R in the situation of recruitment, 2) the predictability of the NEO-PI-R for performance and counter-productive behaviors at work, and 3) the interpretation of the personality disorders by means of the NEO-PI-R and its applicability in the prediction of the counter-performance. We undertook our research with 21,349 job applicants in a French public transportation company. Our results show that the factor structure of the NEO-PI-R was not affected by the social desirability, nevertheless its scores were affected in the situation of recruitment. The applicants presented themselves more "Conscientious" and less "Neurotic" than the individuals of normative sample. The scores of the personality disorders which are calculated by means of the NEO-PI-R have correct sensitivities in spite of the dissymmetry of the distributions. As for the relationship between personality and performance, the dimension "Conscientiousness" predicts positively success in training and negatively counter-productive behaviors, the dimension "Neuroticism" predicts positively bus drivers’ accidents. The examination of the predictability of performance with the facets and the personality disorders offers additional information

Full textThe subject of our work was to study the influence of type of attachment and maltreatment on personality disorders in a population of patients addicted to drugs who receive replacement therapy. Our sample consisted of 140 patients with a diagnosis of toxic drug addiction substitu-tion treatment, including 70 women who represent the experimental group and 70 men who represent the control group. To evaluate the traumatic events in childhood, we used the CASRS (Child Abuse Self Report Scale), for assessing personality disorder we used MCMI (Millon Clinical Multiaxial Inventory) and finally the ASQ (Attachment Scale questionnaire) to meas-ure the type of attachment. Our results show that there are significant differences between males and females. A higher occurrence of abuse during childhood, demonstrates insecure at-tachment, more comorbid personality disorder and a socio-familial especially among women, which may have etiological significance. These epidemiological differences have an impact on the severity of substance abuse (more importantly the emotional and physical abuse). There-fore, these results encourage us to rethink our strategies of care for more specific approaches while handling such patients
