Full textPsychopathic personality traits are associated to multiple couple difficulties (Savard, Sabourin, & Lussier, 2006, 2011) that can also have negative effects on adult romantic attachment (Savard, Brassard, Lussier, & Sabourin, 2015). Despite high levels of relationship distress, insecure attachment and dysfunctional personality traits in couples seeking treatment (Daspe, Sabourin, Péloquin, Lussier, & Wright, 2013; Lussier, Wright, & Sabourin, 2008; Mikulincer & Shaver, 2007), the joint contribution of psychopathic traits and attachment insecurities to couple distress has only been explored in community couples so far. The aim of the study is to examine the dyadic association between primary and secondary psychopathy and attachment related anxiety and avoidance in couples seeking treatment. The sample consisted of 183 couples, who completed French versions of the Self-Reported Psychopathy Scale (LSRP; Levenson, Kiehl, & Fitzpatrick, 1995; Savard, Lussier, & Sabourin, 2014) and the Experiences in Close Relationships Questionnaire (ECR; Brennan, Clark & Shaver, 1998; Lafontaine & Lussier, 2003). Actor-partner interdependence model analyses revealed that in both partners, primary and secondary psychopathy was associated with attachment anxiety and avoidance (actor effects). Partner effects between primary psychopathy and attachment anxiety were also found in both men and women, whereas only men’s secondary psychopathy was related to women’s attachment anxiety. Our findings complement those reported in the community and demonstrate the importance to consider attachment insecurities when working with couples who are presenting psychopathic personality traits.

Full textLa présente thèse a pour objectif d'examiner le caractère dynamique des rapports entre l'agression sexuelle à l'enfance (ASE) et les répercussions sexuelles et conjugales à l'âge adulte. Plus spécifiquement, la première étude examine la validité d'un modèle médiationnel de l'association entre l'ASE, les séquelles sexuelles (c.-à-d., compulsion sexuelle et évitement sexuel) et l'ajustement conjugal. Les analyses acheminatoires menées à l'aide d'un échantillon de 686 adultes en relation de couple révèlent une association positive entre l'ASE, la compulsion sexuelle et l'évitement sexuel qui sont reliés, à leur tour, à une plus faible satisfaction conjugale. La deuxième étude tente de reproduire ce modèle médiationnel en modifiant l'indicateur du fonctionnement conjugal. Ainsi, elle a pour objectif d'examiner le lien entre l'ASE, la compulsion sexuelle et les relations sexuelles extradyadiques. Les résultats d'analyses d'équations structurelles effectuées auprès de 669 adultes en relation de couple font état d'une association positive entre la sévérité de l'ASE et la compulsion sexuelle qui augmente ensuite la probabilité de s'engager dans des relations sexuelles extradyadiques. Afin de mieux comprendre l'interdépendance entre le fonctionnement sexuel et conjugal des survivants d'ASE, la troisième étude examine le rôle du contexte relationnel sous-jacent aux répercussions sexuelles. L'objectif principal est d'analyser l'invariance des associations entre l'ASE, la compulsion sexuelle et l'évitement sexuel selon le statut conjugal. Les résultats d'analyses acheminatoires réalisées auprès de 1033 adultes démontrent que la sévérité de l'ASE est associée à la compulsion sexuelle chez les individus non-mariés tandis que la sévérité de l'ASE est associée à l'évitement sexuel chez les individus mariés. Ces trois études confirment l'absence de différences de genre quant aux répercussions sexuelles et conjugales ce qui soutient l'hypothèse de similarité entre les hommes et les femmes. Cette série d'études démontre également l'importance de deux patrons de réponses sexuelles, la compulsion sexuelle et l'évitement sexuel, dans le développement de difficultés conjugales tout en soulignant l'émergence de l'évitement sexuel suite à l'ASE précisément au sein des relations de couple où l'engagement et l'intimité sont élevés. Ainsi, la thèse souligne l'importance de tenir compte de la dynamique entre les facteurs relationnels et sexuels pour comprendre les liens complexes qui expliquent les difficultés autant sexuelles que conjugales chez les survivants d'ASE.

Full textIntroduction: This thesis is the result of reflexions and clinical experiences from our work of pregnancy in women suffering from lifetime eating disorders (ED). Among different methods of assisted reprodutive technology (ART), we chose to study a special population: women seeking GnRH pulsatile administration treatment. Main hypothesis: there is an important rate of women with a lifetime ED seeking GnRH pulsatile administration treatment . Secondary hypothesis: maternal ED have influences on the desire for a child, pregnancy and early mother-child interactions. Main objective: to evaluate pregnant women with a lifetime ED after seeking GnRH pulsatile administration treatment, from the 4th month of pregnancy until the 8th week of postpartum. Secondary objectives: to obtain scientific data about women with lifetime ED seeking GnRH pulsatile administration treatment ; to better inform the multidisplinary care of these patients in the ART services ; to obtain scientific data about the experience of pregnancy in women with a lifetime ED and about the presence and severity of ED during pregnancy and postpartum ; to estimate the prevalence of postpartum depression in these women. Design: A descriptive comparative three-group design in which collected data were compared - one of women with lifetime ED who became pregnant after GnRH pulsatile administration treatment, one of women without lifetime ED who became pregnant after using other ART methods , one of women without lifetime ED who had a spontaneous pregnancy. Evaluation criteria: EDE to evaluate ED during the pre- and post-natal period ; clinical semi-structured interviews to evaluate how the pregnancy is experienced ; IRMAG to evaluate maternal representations ; HAD to evaluate anxiety and depression during the pre and postnatal period ; STAI to evaluate anxiety ; EPDS to evaluate risk of postpartum depression. This study is observational and prospective: during the prenatal period - 3 evaluations were conducted, between 16 and 20 weeks of amenorrhea (SA), between 24 and 28 SA and between 28 and 32 SA ; during the postnatal period - 1 evaluation was conducted around the 8th week. Conclusion: Eating disorders and GnRH pulsatile administration can be considered as a real public health issue, requiring strategies to address prevention, identification and multidisciplinary care for these women during pregnancy and postpartum. An eating disorder screening tool should be included in the initial intake, because these patients may be at a higher risk for negative maternal and fetal outcomes than non eating disorder patients

Full textDo pregnancy and birth systematically turn a woman into a mother? What does turn a woman into a mother? What is to be pregnant in the eyes of the unconscious? How does psychoanalysis comprehend the perinatal moment, the becoming mother? To the never-ending interrogation surrounding the maternal and feminine, where Freud sees maternity as the accomplishment of feminity, Lacan differentiates the register of mother's satisfactions from the one of women's desire. By promoting this gap between the mother and the woman, he breaks with the Freudian idea of idealization of the mother by the woman, and renews considerably the clinic of maternity. This dissertation on the perinatal moment puts at its heart the primordial role of the symbolic fonction of the Father, and the rectifications in the relationship to the object which the phallic treatment of the child-object imposes. From fetishistic maternai jouissances to the consideration of lack, castration or the nothing in the feminine ideal, this study offers a psychopathologic and psychoanalytic reading of the unconscious dimensions of the clinic destinies of the becoming mother. Through the role of pregnancy in psychosis, through the unconscious motives of perinatal depression, through incoercible pregnancy vomiting, it thus brings to light the multiple unconscious arrangements that each woman is constrained to, in the complex work that constitutes the becoming mother

Full textRegarding the dream model elaborated by Freud, this thesis considers the latest figurations of sexuality in the psychoanalytical field as well as in our so-called post-modern culture, with its characteristic discourse on the politics of gender and sexuality. In the analytic field, the unconscious meaning of infantile sexuality is being lost as it is conflated with genitality and then opposed to narcissism. Such a sexological behaviourism reinforces the splitting of sex and gender at the heart of transsexual thought, one which excludes animism. Because of their striking embodiment of sexual difference, so-called transsexuals disturb visual perception and destabilize pre-established ideas of gender, obliging us to emancipate ourselves from thinking in dualistic terms. Thus, we can renew the transgressive nature of auto-erotic sexuality and its dream figures. This is the trans-sexuality that characterizes all psychopathological formations

Full textSexual abuse is a tremendous threat to both physical and psychological integrity of human beings. This prevalent trauma induces several biopsychosocial consequences that affect the various life domains of the victim. Sexual assault victims (SAV) are seven times more at risk of developing posttraumatic stress disorder (PTSD) than someone without a history of sexual abuse. Daytime posttraumatic symptoms arise, and SAV find themselves in a state of hypervigilance, accompanied by intrusive thoughts, potential dissociative states and avoidance behaviors. Although less studied than those experienced during the day, nighttime posttraumatic symptoms (e.g. recurring nightmares, insomnia) would maintain the SAV in PTSD and persist beyond the usual treatments targeting PTSD. Recurring nightmares are even linked to a higher risk of suicidality. Given that sexual trauma often occurs in the evening or at bedtime, SAV are more likely to suffer (77%) from nighttime symptoms than other posttraumatic populations. Several treatments addressing directly nighttime posttraumatic symptoms have emerged over the past three decades. The aim of the present project is to systematically review the effectiveness of these treatments targeting nighttime symptoms (nightmares and sleep disturbances) in SAV suffering from PTSD. This review includes an exhaustive and diversified selection of studies as well as an assessment of the methodological quality of the 22 primary studies that comprise it. Coding and data extraction were performed by three independent judges (kappa = 0.93). Once primary data were gathered, summary tables were rendered, analyzed and interpreted to highlight relevant information on clinical samples, methodological designs and interventions, for both research and clinical practice. Despite the heterogeneity within and between the primary studies, imagery rehearsal therapy (psychological intervention) and prazosin (pharmacological intervention) appear to be the most recurrent and proven effective interventions to treat nighttime PTSD symptoms (such as a significant reduction of nightmares and insomnia symptoms) and to noticeably increase the sleep quality and overall quality of life in SAV. This systematic review highlights the need for further study of the subject, and more importantly the use of complex methodological designs (e.g. larger and more representative clinical samples of SAV, more RCTs, more valid and objective sleep measures). Future research should include quantitative data analysis (as is the case with meta-analysis) for a better understanding of the effectiveness of these treatments and interventions.

Full textTo date, the studies have demonstrated some link between victimization and sexual behaviour problems(SBP) among youth. However, empirical findings about youth engaging in sexually abusive behaviours have focused primilary on the role and importance of sexual abuse experiences and SBPs in response to this form of victimization. As a result, a limited number of studies have considered the possibility that these youth have been exposed to polyvictimization. To overcome this important limitation, this study was articulated under three sub-objectives. Thus, this study aims (1) to better contextualize the experience of sexual abuse of youths with SBPs (2) to better understand the developmental trajectories of victimization of these youths, as well as (3) to better understand the influence of sexual abuse and polyvictimization on the continuity of SBPs. The current study is based on the retrospective and longitudinal analysis of data collected since 2002 at the Centre intégré universitaire de santé et de services sociaux (CIUSSS) de la Capitale- Nationale (Lussier, Chouinard-Thivièrge, McCuish, Nadeau & Lacerte, 2018). In order to meet the objectives of this study, all reports to Child protective service in Quebec between the ages of 0 and 17 were examined among a sample (n = 1034) of children who presented SBPs before the age of 18. The results of the comparative analysis revealed the prevalence as well as the influence of sexual victimization and polyvictimization experienced during childhood and adolescence among children with SBPs. The implications of this study in terms of research, theory and practice, are also discussed. The study highlights the complexity of the traumatic life context in which SBPs manifested by these youth take place.

Full textThis research proposes to look at the psychodynamic interactions at play in the transference movements that emerge when a teacher encounters the reality of a pupil with behavioural disorders. From the incomprehensible to the unbearable, from the difficulty to the suffering –the troubled pupil intrudes into the teacher's subjective experience and generates « identity breakdowns », hinting at a certain psychic rigidity in one's professional position. The « extrapolation » of the concepts of transference and counter-transference from the field of psychoanalysis outlines the contours of a counter-transference specific to the teacher in a troubled teaching relationship. As a desired subject, he mobilises counter-transferential attitudes, observable in their effects, falling under either an already-existing responsiveness or one of varying intensity in terms of disharmony in the articulation of his desires, and constructed and prescribed representations characterising his interpretative filters. Understanding the economics of the psychodynamics of the teacher with regard to professional distress, in a clinical approach with a psychoanalytical orientation, constitutes a foundation to consider rethinking the interpretation of his subjective experience in favour of a Psychic Postural Plasticity of the Teacher

Full textAs part of developmental psychopathology approaches, the aim of the thesis is tobroaden the knowledge of the mechanisms in the inmates’ violent externalised auto-agressive and hetero-agressive behaviour. Our concern is to find the mental, social and environmental impact of life events experienced in different life periods on these violent externalised troubles. From the reconstruction of life charts, the life course has been retraced and the developmental trajectories have been studied by focusing on the suicidal behaviour in prison and the transgressive sexual act. In a first study, life charts of inmates with a history of suicide attempt in prison, built with relatives and caregivers interviews, were compared to life charts of inmates without suicide attempts. The findings enabled to develop a predictive model of the suicidal behaviour proving the importance of mood disorders before the detention and behaviour disorders during the detention. In a second study, four sex offenders profiles created from life trajectories can be observed and distinguished according to the occurrence of events in four major domains of risk factors. The results of the two empiritical studies allow to discuss the theoretical and clinical perspectives as part of developmental psychopathology approaches in the understanding of violent externalised troubles and their care in prison

Full textKallmann syndrome (KS) combines hypogonadotropic hypogonadism and anosmia. Anosmia is related to the absence or hypoplasia of the olfactory bulbs and tracts. Hypogonadism is due to gonadotropin-releasing hormone (GnRH) deficiency, which presumably results from a failure of the embryonic migration of neuroendocrine GnRH cells from the olfactory epithelium to the forebrain. In the first part, we foxind that mutations in PROKR2 and PROK2, encoding prokineticin receptor-2 and prokineticin-2, are présent in 9% of patients with KS. These mutations are mainly missense mutations resulting in loss of fonction. In addition, we discussed of the digenic or oligogenic mode of inheritance of PROKR2 and others genes involved in KS. Furthermore, we showed that PROKR2 and FGFR1 are co-expressed in same neurons, suggesting cross-talk between the FGF/FGFR1 and PROK2/PROKR2 signaling pathway. In the second part of the thesis, we observed that neuroendocrine cells synthetizing GnRHl migration defect is common in four arhinencephalie fetuses, including on fetus with KS. Based on these results we defined an olfacto-genital fetopathological sequence in human, that is to say different genetic developmental diseases resulting in abnormal olfactory development are responsible for a defect in migration of neuroendocrine cells synthetizing GnRHl

Full textThe advent of Highly Active Antiretroviral Therapy (HAART) in 1996 considerably changed life perspectives of people living with HIV (PLWHs). However, concerns have been raised that these improvements may increase the opportunities for relapse to risk behaviors. This thesis proposes to study the determinants of risky behaviors among PLWHs in France, and to analyze to what extent HAART has modified or not these behaviors. This thesis uses data of two surveys: - MANIF 2000, cohort of 467 patients HIV-infected through injection drug use, initiated in 1995. - The ANRS-EN12-VESPA, conducted in 2003 among a sample of nearly 3000 individuals, representative of people living with HIV followed in hospital. Empirical results show that none of the HIV-clinical outcomes appears as significantly determinant of risky behaviors, whatever the sexual orientation and the type of partner considered. However, HAART related side effects appear as significantly associated with a higher frequency of unsafe sex among injecting drug users engaged in occasional relationships. Poor mental quality of life is also a capital determinant of unsafe sex with occasional partners among gay men. Within heterosexual men and women, analyses point out the impact of social vulnerability on risky behaviors. Among sero-nonconcordant couples, there is a significant relationship between risky behaviors and concealment of HIV-status. Finally, the last analysis shows the impact of HIV-experience on sexual difficulties perceived by PLWHs

Full textInfertility represents a major public health problem and concerns 10 to 15% of couples in the general population. A male factor is responsible for the infertility of the couple in about half of all cases. In approximately 30% of them, the etiology remains unexplained.The first working axis concerned the molecular study of a cohort of infertile patients (nonobstructiveazoospermia/ cryptozoospermia and disorder of the sex development or DSD) for whom analyses of standard karyotype and/or microdeletions of AZF regions were not able to explain the phenotype. The impact of copy number variations of DNA (CNVs) detected by comparative genomic hybridization (CGH-array) is poorly documented. A custom design 400K micoarray, genome-wide and enriched on a wide panel of 445 genes linked with infertility and DSD has been achieved. This array allowed the identification of 171 CNVs of interest.These results underline the potential of this design for diagnosis of male infertility. The second objective of this work was the in vitro modelisation of male infertility in a context of genetic abnormality. For that purpose, human induced pluripotent stem cells (hiPSCs) were generated from erythroblasts by means of not integrative Sendaï virus, in two patients carrying genetic abnormalities (complex chromosomal rearrangement and 46,XX-SRY negative karyotype associated with AMH gene mutation). Secondly, functionality of hiPSCs generated was tested by germ cells in vitro differentiation. Primordial germ cell (PGC) stage was successfully obtained. Cells expressed key PGC markers such as SOX17. The perspectives of this work will be to continuethe germinal differentiation towards more mature stages and so to be able studying the meiotic process in a context of genetic abnormality

Full textPTSD is classically associated with verbal memory impairments and specific brain alterations. However, few studies have investigated the population of female victims of civilian traumas. During three studies using magnetic resonance imaging, we investigated the condition of the brain correlates of episodic and working memory in patients with sexual abuse-related PTSD. Our work has highlighted an important matching between the brain regions underlying strong verbal memories encoding and those that are usually affected by PTSD. Moreover, we demonstrated the absence of volumetric shrinking of these regions in our sample. Finally, we noticed a potentially compensatory alteration of functional activation pattern in this population during verbal working memory processing of trauma-related and unrelated items. These results suggest a gender-dependent specificity of PTSD neurocognitive mechanisms and further support the interest of using cognitive remediation in its treatment

Full textBetween 25,000 and 30,000 people are co-infected with HIV and hepatitis C virus (HCV) in France, with major clinical repercussions. Major therapeutic advances have been made in recent years thanks to the development of direct-acting antivirals for HCV treatment, with very good efficacy and safety profiles. In the same time, while the number of co-infected patients has globally decreased among HIV-infected patients, new groups at risk of HCV transmission have emerged in this population, such as men who have sex with men (MSM). Co-infected patients’ day-to-day experience with the disease remains poorly documented. In this work, we analyze the perceptions (quality of life, functional impact of fatigue) and behaviors (sexual risk behaviors, addictive behaviors) of co-infected patients and their relationships with clinical parameters, and we try to determine to what extent such data can be used to improve the management of co-infection and to better target HCV prevention actions

Full textThe concept of "psychosocial risks at work" gathers several work-related suffering risks. These risks are, at the same time, related to the generating acts of pain and to the expressions of suchpain on employees’ health. Despite its formulation, this concept refers to events affecting both mental and physical health of workers. Such affecting events share the same social ground. Indeed, psychosocial risks are mainly caused by the companies’ organization themselves, management methods or harmful relationships. Even though the expression of psychosocial risks was formerly developed out of the legal environment, it is now generally used in law to deal with issues of health and safety at work. However, the comprehensive understanding of such risks is not yet obvious from a legal perspective. These are complicated to define and delimit, tinged with subjectivity and unlikely to be understood in light of the employment law history. However, law is increasingly taking into account the different dimensions of the workers’ individuality. Also, if the psychosocial risks fail to be recognized, so far, as an independent and entire legal concept, both laws (domestic and international) and case law are more and more referring to it. Therefore, measures and legal solutions applicable to the working relationships in private companies enable to prevent such risks, punish or compensate them. As such, a wide range of people, whether related or not to the company, are able to duly face these factors

Full textDespite the central role they play in sexuality, sexual desire and sexual behaviour remain poorly understood in committed couples. The paucity of empirical results in this area contributes to the distance between research and clinical practice, and negatively impacts the quality of care offered to the many couples seeking help for sexual desire issues – the most common complaint in sex therapy. The objective of the three articles in this doctoral research was to help address this gap by studying sexual desire within a relational perspective, working from the assumption that each partner’s wellbeing was influenced by that of the other partner, and that this influence was mediated in part by the couples’ sexual behaviour. Results from this research suggest that positive interactions during sex, such as self- and other-approach motives and genital and affective behaviours, are associated with greater sexual satisfaction and intimacy (study 1). In contrast, sexual difficulties such as low sexual desire are associated with restrictions in sexual behaviour and sexual dissatisfaction (study 2), and asynchronies between partners such as sexual desire discrepancy are associated with sexual distress (study 3). Furthermore, the third study begins to establish a direction and span to these associations, by suggesting that issues with sexual desire discrepancy are predictive of sexual distress from one day to the next, and that these daily associations are mirrored by more distal associations spanning a year or more. These results are consistent with recent research on emotional regulation in sexuality, and more specifically, with proposals that sexual desire plays a regulatory role in the couple, mediated by variations in sexual behaviour. It is hoped that, beyond these conceptual contributions, this research will be of use to clinicians. In particular, these results support the use of therapies that focus on everyday interactions between partners to help couples struggling with sexual desire issues.
