This study proposes a reading of the financial statements of co-operatives operating within the Florentine metropolitan area, with a view to underscoring business aspects of a quantitative and monetary nature. The entrepreneurial nature of the co-operatives is taken as read, focusing as with any other corporate entity on assets and economic results as indicators of success. The financial statements were collected from co-operatives with their registered offices in the provinces of Florence, Prato and Pistoia for the financial years 2004-2007, and the accounts were then aggregated, reclassified and analysed. The observations on the resulting data are then broken down by province and by type of co-operative (A or B).

The case of a 63-year-old woman with severe confabulation and reality confusion after rupture of an aneurysm of the anterior communicating artery is described. It shows how pervasive and disruptive the disorder can be. The patient falsely reported recent activities and fiercely enacted obligations she had actually held 20 years previously. Imagined activities were reported as true experiences. In contrast, she did not confabulate in response to questions of general knowledge or about her remote past—a first indication of there being different forms of confabulation. She returned to reality after 17 months, although she remained severely amnesic.

Hepatitis B virus (HBV) is the most common cause of chronic viral hepatitis and hepatocellular carcinoma in the world. Worldwide, more than 250 million people are chronically infected with HBV, causing nearly 780,000 deaths each year, and mother-to-child transmission (MTCT) accounts for more than one-third of chronic HBV infections. Universal vaccination in neonates is the most effective strategy for eliminating infections worldwide. Maternal antiviral treatment during the antepartum/postpartum period for mothers with high HBV viral loads is effective in preventing HBV MTCT. Full immunization coverage is currently the only way to reach the goal of eradicating HBV infection. Operational research and, in some resource-limited settings, international funding may be essential to bring the vaccine where neonates and infants need it, including remote locations where home births are common. Continued improvements in the coverage and timeliness of HBV vaccination and education of clinicians about its importance are needed.

Hepatitis B virus (HBV) has been causally linked to a variety of renal diseases, the most common being glomerular diseases and systemic autoimmune disease. Membranous nephropathy (MN) is the commonest HBV-associated glomerulonephritis (HBV-GN), followed by membranoproliferative glomerulonephritis (MPGN), mesangial proliferative glomerulonephritis, immunoglobulin (Ig)-A nephropathy, and focal segmental glomerulosclerosis (FSGS). Polyarteritis nodosa is a rare manifestation. The incidence of HBV-associated renal diseases seems to be decreasing with the introduction of vaccination programmes.HBV-MN is the most frequent cause of nephrotic syndrome in children in countries with high endemicity of HBV infection. The clinical course is usually benign in children with high rates of spontaneous remission rates and low risk of progression to renal failure. The prognosis is worse in adults. Of the systemic autoimmune disorders associated with HBV infection that involve the kidneys, the strongest link has been found with polyarteritis nodosa (PAN), a lesion that causes arteritis of medium-sized renal vessels. HBV-associated PAN (HBV-PAN) usually manifests in the first year after infection, and is clinically indistinguishable from classic PAN.Diagnosis of HBV-GN or -PAN is based on the clinical picture, histological findings, evidence of viral replication in serum and/or liver and detection of HBV antigens or DNA in the tissue. Besides deposition of immune complexes, other mechanisms such as virus-induced cytopathic damage have been proposed to explain the pathogenesis.HBV-GN and HBV-PAN appear to respond to antiviral treatment. Both show remission after HBeAg seroconversion. The available studies predominantly employed first-generation agents like interferon alpha and lamivudine, which showed suppression of viral replication and clinical remission of HBV-associated renal disease. Immunosuppressive therapy appears to be inevitable for the control of severe HBV-PAN and could be helpful in addition to antiviral treatment for cases of HBV-GN not responding clinically to antiviral treatment.

Abstract:
Charles Skinner Hallpike was born in 1900 in Muree, a small hill station and health resort in the northernmost part of India (now a part of Pakistan). He was baptized at the Church of St. James in Delhi. This was one of the three places of worship—a Hindu temple, a mosque, and a Christian church—built by Hallpike’s great-grandfather, James Skinner. Hallpike began his medical training at Guy’s Hospital in 1919. After graduating from medical school, he was appointed House Surgeon to T. B. Layton in the Ear, Nose and Throat Department at Guy’s Hospital. He took his Membership of the Royal College of Physicians of London and followed with his Fellowship of the Royal College of Surgeons. Hallpike had a reputation of being rather unapproachable, but he was comfortable with his clinical peers and had a protective attitude to the people who worked for him.

Joe B. Hall shares memories that stretch across his ninety years. He tells of his youth in Cynthiana, Kentucky, where his love for family, the outdoors, fishing, sports, work, and Kentucky all started. He describes what is was like to be a student at the University of Kentucky in 1947, and a member of the celebrated coach Adolph Rupp’s Wildcats during the Fabulous Five period. Those famous five players made his chances of playing for Kentucky slim, so as a sophomore, he transferred to Sewanee, where he did play basketball well and acquired a great friend in his coach Lon Varnell, who took him and other players on a summer tour to Europe to play basketball. Choosing not to return to Sewanee, Joe B. took a job as a salesman, married Katharine Dennis, and decided his goal in life was to be a college basketball coach. After he completed his bachelor’s degree, he acquired experience coaching first at a high school, then at two colleges, and earned his master’s degree. Throughout that time, Coach Rupp kept in contact with Joe B. When Coach Rupp asked him to return to UK to work as his first assistant, he happily accepted. Coach Rupp and Joe B. respected each other, and Joe understood that colorful character as well anyone could. Yet later, when Coach Rupp resisted the university’s mandatory retirement law and refused to announce his successor, the turmoil in the basketball program surprised and saddened Joe B. Joe B. accepted the challenge of becoming head coach in 1972. He frankly discusses his failures as well as his successes. Exciting are his accounts of the two games in the 1974-1975 season the Wildcats played against Bobby Knight’s Indiana and the game against John Wooden’s Bruins for the NCAA in 1975. He also discusses the mysterious manner in which the Wildcats lost to Georgetown, and the pure exhilaration he and his players felt winning the NCAA championship. The book includes a chapter on the Wildcat Lodge, and another on the humorous antics of some of his players. Serious health problems caused Joe B. to retire early, and he tells us about the other interesting work he did after coaching. His favorite retirement job was the radio talk show he shared with Coach Denny Crum for ten years.

The non-Hodgkin lymphomas (NHL) are a heterogeneous group of over forty lymphoid neoplasms that have undergone a major redefinition over the last twenty-five years, in part due to advances in immunology and genetics as well as implementation of the WHO classification system. NHLs are considered clonal tumors of B-cells, T-cells, or natural killer (NK) cells arrested at various stages of differentiation, regardless of whether they present in the blood (lymphoid leukemia) or lymphoid tissues (lymphoma). In the United States, the age-standardized NHL incidence rate (per 100,000) doubled from 1973 (10.2) to 2004 (21.4) and then stabilized, while five-year relative survival rates improved from 42% in 1973 to 70% in 2004. Established risk factors for NHL or specific NHL subtypes include infectious agents (HTLV-1, HIV, EBV, HHV8, HCV, H. pylori), immune dysregulation (primary immunodeficiency, transplantation, autoimmunity, and immunosuppressive drugs), family history of lymphoma, and common genetic variants identified by genome-wide association studies.

Abstract Unverricht-Lundborg disease (ULD; EPM1) is an inherited neurodegenerative disorder characterized by onset at 6–15 years, stimulus-sensitive, action-activated myoclonus, epilepsy, and progressive neurological deterioration. It is caused by biallelic pathogenic variants in the CSTB gene, encoding a cystatin B. The most common of these is an unstable expansion of a dodecamer repeat element in the promoter region of the gene, leading to marked downregulation of CSTB expression. Total loss of CSTB is associated with severe neonatal-onset encephalopathy. A cystatin B–deficient mouse models the EPM1 disease relatively well. Myoclonic seizures, preceded by microglial activation, develop at one month of age and are followed by progressive gray and white matter degeneration and motor problems. CSTB is an inhibitor of cysteine proteases of the cathepsin family showing both nuclear and cytoplasmic localization with partial co-localization with lysosomal markers. CSTB function has been linked to protecting neurons from oxidative damage through an oxidative stress-responsive cystatin B-cathepsin B signaling pathway. In the nucleus CSTB has been shown to participate in regulation of cell cycle, and histone H3 tail proteolytic cleavage. Downstream effects of CSTB dysfunction have also been implicated in apopotosis, microglial dysfunction, inflammation, neurogenesis and synapse physiology. Despite the progress made, the exact disease mechanisms in EPM1 remain elusive. This chapter discusses the clinical features of EPM1 and recent advances in understanding its pathophysiology.

Systemic lupus erythematosus (SLE) is a systemic autoimmune disease with various clinical manifestations. The hallmark of SLE is the presence of antibodies against nuclear constituents, such as double-stranded (ds)DNA, histones, and nucleosomes. Local deposition of antinuclear antibodies in complex with nuclear autoantigens induces serious inflammatory conditions that can affect several tissues and organs, including the kidney.The levels of antinucleosome and anti-dsDNA antibodies seem to correlate with glomerulonephritis and these autoantibodies can often be detected years before the patient is diagnosed with SLE. Apoptotic debris is present in the extracellular matrix and circulation of patients with SLE due to an aberrant process of apoptosis and/or insufficient clearance of apoptotic cells and apoptotic debris. The non-cleared apoptotic debris in patients with SLE may lead to activation of both the innate (myeloid and plasmacytoid dendritic cells) and adaptive (T and B cells) immune system. In addition to the activation by apoptotic debris and immune complexes, the immune system in SLE may be deregulated at the level of (a) presentation of self-peptides by antigen-presenting cells, (b) selection processes for both B and T cells, and (c) regulatory processes of B- and T-cell responses. Lupus nephritis may be classified in different classes based on histological findings in renal biopsies. The chromatin-containing immune complexes deposit in the capillary filter, most likely due to the interaction of chromatin with the polysaccharide heparan sulphate. A decreased renal expression of the endonuclease DNaseI further contributes to the glomerular persistence of chromatin and the development of glomerulonephritis.Current treatment of lupus nephritis is not specific and aims to reduce the inflammatory response with general immunosuppressive therapies. However, research has revealed novel potential therapeutic candidates at the level of dendritic cells, B cells, and T cells.

This chapter reviews the book The University of Oxford. A History (2016), by Laurence W. B. Brockliss. The book is divided into four parts: the ‘Catholic University’ (c.1100–1534), the ‘Anglican University’ (1534–1845), the ‘Imperial University’ (1845–1945), and the ‘World University’ (1945–2015). Brockliss’s account broadly endorses the idea that the 400 years or so between the high middle ages and the Victorian era of reform were a time of decline for Oxford, reaching the lowest point under the Hanoverians. Yet this interpretation has been seriously questioned by others, including Lucy Sutherland and Leslie Mitchell. The book tackles a range of topics related to Oxford, including its undergraduate curriculum, student population, research, funding, and institutional obstacles to reform.

Se trata también del primer informe regional sobre la eliminación de la transmisión maternoinfantil y durante la primera infancia de la hepatitis B y la enfermedad de Chagas congénita. Los resultados principales son los siguientes: El acceso de las embarazadas a la atención prenatal y del parto es alto en la Región de las Américas. El tamizaje de la infección por el VIH y la sífilis en las embarazadas sigue siendo alto, pero se han logrado pocos avances para salvar las brechas; por otra parte, el tamizaje de la enfermedad de Chagas en las embarazadas es muy variable, dado que oscila entre 7% y 55% en los pocos países que presentan informes al respecto. El tratamiento de la infección por el VIH y la sífilis en las embarazadas seropositivas sigue en aumento. La vacunación contra la hepatitis B se ha estabilizado en 87% de los menores de 1 año que completan su tercera dosis, aunque continúa en aumento la adopción de políticas de administración de una dosis al nacer de la vacuna contra el virus de la hepatitis B a todos los recién nacidos. Durante mucho tiempo se ha observado una disminución continua de la transmisión maternoinfantil del VIH, pero comienza a estabilizarse. Los casos de sífilis congénita están en aumento. Se considera que la transmisión maternoinfantil causa más de 20% de los casos nuevos de enfermedad de Chagas. This document reports the progress made in the Americas towards the EMTCT of HIV and syphilis between 2010 and 2017. It is the first Regional report regarding elimination of mother-to-child and early childhood transmission of hepatitis B and congenital Chagas disease. The main findings are as follows: Access to prenatal and delivery care for pregnant women is high in the Americas. Screening of pregnant women for HIV and syphilis remains high but little progress has been made in closing the gaps; meanwhile, screening of pregnant women for Chagas disease varies widely, ranging from 7% to 55% among the few reporting countries. HIV and syphilis treatment of seropositive pregnant women continues to increase. Vaccination for hepatitis B has stabilized at 87% of children under one year old who completed their third dose, and policies for universal timely hepatitis B vaccine birth dose are increasingly being adopted. MTCT of HIV experienced longstanding reductions but has begun to stabilize. Congenital syphilis cases are on the rise. Mother-to-child transmission is estimated to cause over 20% of new cases of Chagas disease.

The lung is a relatively new site for the application of ultrasound. Beyond the more established assessment of pleural effusion, this organ has been traditionally considered off limits for sonographic investigation, since air is a well-known foe of the ultrasound beam. However, in recent years it has been shown that this apparent physical limitation can be overcome when the air content decreases, as happens in a diseased pulmonary parenchyma. The most useful lung ultrasound sign for cardiologists is the presence of B-lines, the sonographic hallmark of pulmonary interstitial syndrome, including interstitial pulmonary oedema. Bilateral multiple B-lines are present in patients with pulmonary congestion and may help assess and semiquantify the extent of extravascular lung water in patients with heart failure. This sign is low cost, easy to perform, can be repeated at bedside, and does not employ ionizing radiation. Lung ultrasound is also useful for detecting other pulmonary conditions such as pneumothorax, and lung consolidations such as pneumonia or pulmonary infarction.

This chapter reviews the history of treaty-making with the Indigenous peoples of Canada. After an initial period of roughly equal relationships, colonial authorities increasingly used treaties as a domestic law concept aimed at securing control over Indigenous land. The practice was continued after Confederation, but there appears to be a major misunderstanding as to the terms of those treaties, in particular as to the purported extinguishment of Aboriginal title. After a 50-year hiatus, treaty-making resumed in 1975 with the signing of ‘land claims agreements’ in most of the Canadian north. These agreements not only provide for the sharing of land, they also contain detailed provisions with respect to co-management of natural resources and, in some cases, self-government. Canadian law now affords statutory and constitutional protection to treaty rights, and courts are prepared to take into account extrinsic and oral evidence in interpreting treaties.

This chapter traces the evolution of natural resources jurisdictional litigation from Confederation in 1867 until the present. Issues of resource ownership, legislative control, and environmental protection are all explored. In the years immediately following Confederation, the principal focus was establishing resource ownership for all provinces. Once that was established through the Natural Resources Transfer Agreements, legislative jurisdiction over resources became the issue. The caselaw from this period resulted in the resource amendment, which was the only change to the federal-provincial division of powers constitutionally entrenched by the Constitution Act, 1982. Issues of ownership and legislative jurisdiction were settled just in time for the courts to turn their focus to environmental protection. The author demonstrates the way in which litigation and negotiation through the years has led to the current conflict between environmental and resource protection, and the potential profits available in the fossil fuel sector, with special emphasis on pipelines.

The Exile in the Maghreb entails the first attempt at describing the historical reality of the legal and social condition of the Jews in the Muslim countries of North Africa (principally Algeria and Morocco) over a thousand year period from the Middle Ages (997 C.E.) to the French colonization (1830 Algeria/1912 Morocco.). The Exile is not a formal history but a chronological anthology of documents drawn from literary (section A) and archival sources (section B), many of which are published for the first time. In section A, Arabic and Hebrew chronicles, Muslim legal, and theological texts are followed by the accounts culled from European travelers—captives, diplomats, doctors, clerics, and adventurers. Each document is introduced and annotated in such a way as to bring out its importance. The second section (B) reflects the diplomatic activity deployed by humanitarian organizations in favour of North African Jewry. Spanning the 19th and early 20th centuries, these are mainly drawn from the archives of the Alliance Israélite Universelle (Paris) and the Anglo-Jewish Association (London). The documents are richly elucidated with illustrations taken from the international press. The book presents a new and illuminating insight into the status of Jews under the Crescent. The Jews of North Africa were the only minority under Islam, in this region and their history reflects Judaism's exclusive encounter with Islam.

Sjögren's syndrome is a chronic, immune-mediated, condition of unknown aetiology characterized by focal lymphocytic infiltration of exocrine glands associated with dry mouth and eyes. It occurs in its own right (primary Sjögren's syndrome, pSS), or as a late feature of other rheumatic diseases such as rheumatoid arthritis, systemic lupus erythematosus or scleroderma (secondary Sjögren's syndrome). There is a strong female bias. pSS typically affects women in their middle years with an estimated prevalence of 0.1–0.6%. 75% of patients have anti-Ro and/or anti-La antibodies, often with raised immunoglobulin levels (hypergammaglobulinaemia). In patients without these antibodies the diagnosis can be confirmed by salivary gland biopsy. Treatment is generally symptomatic using artificial tears, saliva replacements/stimulants and good dental hygiene. Three-quarters of patients with pSS report significant fatigue with a negative impact on quality of life. This can be the most disabling symptom. Approximately 20% of patients develop systemic features including persistent salivary gland swelling, cutaneous vasculitis, peripheral neuropathy, interstitial lung disease, autoimmune cytopenias or renal tubular acidosis. Hydroxychloroquine and corticosteroids are the most widely used therapies for systemic features. There is a 44fold increased risk of mucosa-associated lymphoid tissue (MALT) B-cell lymphoma in pSS, typically affecting the salivary glands. On account of abnormalities in the B-cell system in pSS there is current interest in the use of anti-B-cell directed monoclonal antibodies to treat pSS and a number of clinical trials are in progress. This approach is already successfully in use for treating MALT lymphoma in pSS.
