Abstract:
This study aimed at comparing two groups of patients with Addison's disease: A, including 180 patients described in 1991 and B, consisting of 138 patients registered since 1991. The incidence of coexisting autoimmune disorders was evaluated and etiological factors were analyzed. Immunological and imaging studies (computed tomography in group B) were performed. Adrenal autoantibodies were examined by an indirect immunofluorescence technique in group A, and by the assay measuring autoantibodies against steroid 21-hydroxylase in group B. Adrenal autoantibodies were revealed in 37% of patients examined by the immunofluorescence method and in 63% investigated by the modern technique. Tuberculosis was found in 52 patients in the group A and in two patients in the group B; metastatic infiltrations of the adrenals in CT were detected in 16 patients. Probable autoimmune Addison's disease was diagnosed in 125/180 patients (69%) in the group A and in 116/138 patients (84%) in the group B.

In the Faculty of Agriculture plant nursery at the University of Novi Sad, Serbia, Common hazel (<em>Corylus avellana</em> L.) cultivars were grafted on the Turkish hazel (<em>Corylus colurna</em> L.) rootstock. To produce highquality planting material, seeding success, growth and development of one-year-old seedling was followed. This paper presents results of Turkish hazel A<sub>1</sub>, B<sub>1</sub>, B<sub>4</sub>, B<sub>7</sub> and C<sub>3</sub> genotype nut germination from sowing performed in 2009. The results indicate high percentage of germinated nuts and a high plant survival rate. One-year-old seedlings measured during a vegetation rest period in 2010 and two-year-old seedlings measured in 2011 were better developed compared to the seedlings examined in the previous years, indicating that the application of the drip irrigation system in seedling development yields positive results.

Leaf demography, sclerophylly and ecophysiology were assessed on Banksia petiolaris (ground creeper, large vertical leaves) and B. baueri (shrub, smaller horizontal leaves) in the dry mediterranean scrub-heath of Western Australia. Leaves survived for up to 13 and 5 years respectively (50% survival was 5.9 and 2.0 years). Leaf specific mass increased with age for both species, due to increase in leaf density. Shading was negligible for 1-year-old B. baueri leaves but increased sharply with age, whereas shading was moderate and similar for leaves of all ages in B. petiolaris. Transpiration rates and stomatal conductance during cloudy autumn weather were higher in B. petiolaris, whereas photosynthesis was lower. Stomatal conductance, transpiration and photosynthesis declined with leaf age in both species, but more steeply in B. baueri. Dawn and midday xylem pressure potentials and hydraulic conductance decreased with leaf age and were higher and declined more gradually in B. Petiolaris. Water-use efficiency declined slightly with age. The water potential at zero turgor, osmotic potential at 100% relative water content and bulk elastic modulus values were closer to zero in B. Petiolaris and changed little with age, while the relative water content at zero turgor and bound water were higher in older leaves of B. petiolaris. No trends in chlorophyll concentrations or fluorescence kinetics were observed with aging except that the oldest leaves of B. baueri had lowest activity. Area based leaf N decreased in B. baueri with leaf age, but was unchanged in B. petiolaris up to 10 years. K and Mg did not vary with leaf age, but P decreased and Ca increased in both species. P levels were extremely low, yet 70% resorption prior to leaf death was apparent. Photosynthetic-N and -P use efficiencies were high in the 1-year-old relative to the 2-year-old leaves, and declined gradually with further aging, but more steeply in B. baueri. We conclude that the exceptionally long-lived leaves of B. Petiolaris are consistent with the substantial investment they represent (large surface area, mass and total nutrient store) and low levels of physiological activity associated with low soil nutrient and moisture availability and moderate illumination at all leaf ages.

Multilevel arterial occlusive disease is often the main cause of critical ischaemia of the lower limb. The aim of this study was to determine the diagnostic criteria that could help select patients for sequential aortofemoropopliteal/distal reconstruction and to compare the results after simultaneous and two-stage surgery. Some 1953 aortofemoral reconstructions were performed during a 6-year period (1987-1992). In 245 cases (12.5%) sequential aortofemorodistal (popliteal or tibial) procedures were performed for critical limb ischaemia Fontaine classification grade III and IV. Two-segment reconstructions were performed in one stage in 161 cases (group A), and two separate operations were performed, the outflow procedure usually following within 12 months after inflow surgery, in 84 cases (group B). The perioperative mortality rates were 3.2% in group A (five deaths) and 5.9% in group B (five deaths). Limb salvage rates were 95.6% at 1 year and 90.4% at 5 years for group A and 88.8% at 1 year and 80.0% at 5 years for group B. Primary inflow bypass patency rates were 97.7% at 1 year and 91.3% at 5 years for group A and 93.4% at 1 year and 76.3% at 5 years for group B, while secondary inflow bypass patency rates were 98.8% at 1 and 5 years for group A, and 95.3% and 88.3% at 1 and five years respectively for group B. Primary outflow bypass patency rates were 91.4% at 1 year and 65.5% at 5 years for group A, and 84.9% at 1 year and 59.4% at 5 years for group B. Secondary outflow bypass patency rates were 92.2% at 1 year and 81.8% at 5 years for group A, and 86.1% at 1 year and 65.9% at 5 years for group B. The data demonstrate that single-stage multisegment reconstruction for multilevel arterial occlusive disease is a safe and effective method of treating critical limb ischaemia.

IntroductionThe symptoms of depression are often found in patients suffering from eating disorders.AimsThe aims of this research were to find out if depression in patients with eating disorders (ED) like anorexia nervosa (AN) or bulimia nervosa (BN) is an additional condition accompanying the ED diagnose, or if it is an integral element being part of emotional problems related to maturation. The estimation of patients’ social condition is also considered in comparison to matched group with no ED diagnose.MethodsSubjects: Group B - 19 females diagnosed, age 12 – 24; group C - matched control group of 30 healthy females, group F - 19 patients out of group B examined in a follow up research two years after hospital treatment. Tools used: Beck Depression Inventory (BDI), Life Quality Questionnaire (Q), Body Mass Index measurement (BMI).ResultsBDI results: depression symptoms are more intense in the early phase of the treatment of ED (group B) than in the follow up -group F (two years after hospitalization) and also more intense than in group C (controls). Q Questionnaire shows no differences between groups B, C, and F.ConclusionsDepression is an additional condition in patients diagnosed with ED mostly in the early phase of the treatment. No differences have been found in the social condition between groups B, C, and F.

However, the attitude of the teacher-educators and pupil teachers towards Two-Year B. Ed. Programme matters to a great extent for its assessing suitability of the new regulations. Purpose: The present study has been undertaken to review only ten selective research studies only on the attitude of the teacher-educators and pupil-teachers towards the Two-Year B. Ed. Programme, aiming at grasping their attitudinal status or trend in a single frame with a broader spectrum of views value towards the Programme.Methods: The review-based content analysis method was adopted and its approach was qualitative for analysing the attitude of the Teacher-Educators and Student-Teachers towards two-year B. Ed. Programme.Results: the entire gamut of the Two-Year B. Ed. Programme under the NCTE Regulations, 2014 is effective and more promising for preparing professional and accountable teachers from the lens of the teacher-educators. Only the student-teachers need to be oriented with prescribed theoretical knowledge base through viable pedagogical strategies as most of the student-teachers have not hold favourable attitude towards the Two-Year B. Ed. Programme. Conclusion: As a whole, the attitudinal trend among the teacher educators and pupil-students towards the two year B. Ed. The aim of this work was to verify the efficiency of the anticipated selection of superior plants in intrapopulation breeding of passion fruit. The experiment was conducted at the Mato Grosso State University experimental area. Eight populations were evaluated. Planting was carried out in August 2010 following the randomized blocks experimental design, with ten repeats and ten plants per plot. The production characteristics of mass and number of fruits were evaluated from data obtained from two cultivation periods, the first year and the full two-year cycle of culture. Estimates of genetic parameters indicate the possibility of selection gains. The accuracies of fruit production, fruit number and fruit mass characteristics had high magnitude, independent of the selection cycle. Coincidence index estimates were high for all characteristics. Based on the results, early selection (first year) was efficient and reduced the time of selection for the best plants and can be used for passion fruit breeding.

7027 Background: Concurrent chemotherapy and TRT is standard therapy for SCLC if all lesions can be included in a radiotherapy field (LD). Several schedules of TRT are used. One study showed that two fractions a day improved local control and overall survival (OS), but this schedule has not been compared to a commonly used 3 wks schedule. Methods: Eligible pts had LD SCLC and PS 0-2. Pleural fluid was accepted if negative cytology. Pts received 4 cycles of PE (cisplatin 75 mg/m2 IV day 1 and etoposide 100 mg/m2 IV day 1-3 q 3 wks) and were randomly assigned to 3 wks of 3D conformal TRT [A] 42 Gy (2.8 Gy x 1/day) or [B] 45 Gy (1.5 Gy x 2/day). TRT started 3-4 wks after the first PE. All responders received prophylactic cranial irradiation (PCI) 2 Gy x 15 ≤ 6 wks after last PE. Pts reported health related quality of life (HRQoL) on EORTC QLQ C30 + LC13. Primary endpoint: 1-year local failure. Secondary: OS, toxicity and HRQoL (dysphagia and dyspnea; a difference ≥ 10 points was considered significant). 75 pts were required in each arm to show a 30% improvement of local disease control with α=.05 and β=.8. Results: 159 eligible pts were enrolled at 18 sites in Norway May 05 – Jan 11 (A: 85, B: 74). Median age 60 (40-85); 52% men, 84 % PS 0-1, 11% pleural fluid. Mean no. of PE-cycles was 3.8, 97% completed TRT, 82 % PCI (no difference between arms). Response rates were similar (A: 92%, B: 94%; p=.8), but more pts on Arm B had CR (A: 13%, B: 35%; p=.01). There was no difference in local failure as first site of progression at 1 year (A: 17%, B: 12%; p=.4) or 1-year PFS (A: 44%, B: 50% ; p=.4). There was similar grade 3-4 esofagitis (A: 33%, B: 37 %; p=.7) and pneumonitis (A: 6 %, B: 7 %; p=.9). 2 pts (1 on each arm) died from pneumonitis. Pts in Arm B reported more dysphagia (A: 64 points, B: 73 points), but not more dyspnea (A: 29 points, B: 28 points). 1-year OS was similar (A: 77%, B: 76%; p=.9). Currently, 2-year survival among those followed ≥ 2 years (n=130) favors Arm B (A: 40%, B: 55%; p=.09) and so far (all pts followed ≥ 1-year; 103 events) median OS favors Arm B (A: 18.7 mos, B: 26.6 mos; p=.34). Conclusions: Twice daily TRT resulted in more CRs, slightly more dysphagia, similar 1-year local control and 1-year PFS. There are indications of improved 2-year and median OS in this arm.

Three grazing systems with Coastcross-1 bermuda grass (CC) + 100 kg N ha-1 year-1 + common vetch; CC + 100 kg N ha-1 year-1 + arrowleaf clover; and CC + 200 kg N ha-1 year-1 were evaluated. Thirteen grazing cycles were performed during the experimental period (313 days), with two, five, four and two cycles respectively in winter, spring, summer and fall. Lactating Holstein cows were used in the evaluation. Daily accumulation rate, forage production, rate of forage disappearance, agronomic intake, grazing efficiency, herbage allowance and stocking rate were evaluated. Mean rates of forage yield and stocking rate were 20.8; 17.6 and 19.7 t DM ha-1 and 7.0; 6.8 to 6.8 animal units ha-1 day-1 for the respective forage systems. The mixture Coastcross-1 plus common vetch, fertilized with 100 kg N ha-1 year-1 and Coastcross-1 fertilized with 200 kg N ha-1 year-1 provided greater productivity and better distribution of forage throughout the seasons.

Clinical IntroductionAn 82-year-old man with a history of coronary artery bypass surgery, hypertension and small bowel gastrointestinal stromal tumour underwent cardiac risk evaluation prior to surgical resection of his tumour. Abstract:
The paper provides information about the growth and development of somatic structures of two-year-old carp during the fattening period. In variant I, K-III compound feed was used (low-carb-monodiet), in variant II – Carp-38/12 (high-carb-monodiet), in variant III – KIII and Carp-38/12 (in different auto-feeders). The highest proportion of axial somatic structures was observed in fish of variant III (fish feeding on a selective basis) (P &lt; 0.05). White muscles are also better developed in fish that eat complex feed (variant III). Compound feed Carp 38/12 does not increase the mass of white muscles (P &lt; 0.05). The proportion of red muscles in fish that eat mixed feed with a low fat content (3.5%) is 45.5% lower than in variants II and III. The use of feed with different energy values and their combination in one pond affects the growth and development of the structure that determines the growth of fish species – the axial skeletal muscles. At the same time, the two-year-old fish species that received the most energy-intensive diet were the most well-fed. Two-year-old carp that received feed with different energy content and complex composition grow differently. This is primarily due to the growth of muscles, which is evidenced by allometric coefficients. The highest values of this parameter featuring positive allometry, were observed in two-year-old carp that received two compound feeds from different feeders (white muscles b = 1.106; red muscles b = 1.499; all muscles b = 1.125). The bionic method of feeding fish in combination with feeds of different energy values provides for strengthened growth of one of the most important body structures – the somatic muscle system. It is not only a biological component of the body that ensures the welfare of animals, but also a high-quality food source.

Plants of B. serrata and I. Anemonifolius resprout after fire, although the species differ in morphology (single-stemmed small tree, multistemmed low shrub respectively). If fires occur before newly established plants are fire-tolerant, populations will decline. The age of first fire tolerance was found to be lower in B. serrata (6 years) than in I. Anemonifolius (about 13 years). Rates of survival between and during fires were measured in the field along with rates of stem regrowth in fire-tolerant B. serrata juveniles. These results were used to predict rates of population decline under repeated fires sufficiently closely spaced to prevent the survival of newly established genets. In both species, juveniles were more prone to death than adults in fires and high-intensity fires caused most deaths. In B. serrata, adult stems (&gt;2.0 cm d.b.h.) are mostly fire-tolerant, but fires less than 10 years apart can prevent many juveniles which survive from reaching adulthood. This restriction is less likely in I. Anemonifolius. As a result stands of B. serrata may decline more rapidly than I. Anemonifolius under 5-year fire cycles. I. Anemonifolius populations, however, may be more prone to decline when the interval between fires is slightly longer (e.g. 10 years) because lignotubers in young juveniles develop at a slower rate than in B. serrata. Extinction or substantial depletions of adult numbers may be approached in stands of either species after 50 years under some repeated 5- or 10-year fire cycles. The rate of such declines will depend directly on the structure of populations (proportions of adults and juveniles). Declines in populations of these resprouters may be likely under current fire regimes within the Sydney region of New South Wales, although these species are more likely to persist through long runs of frequent fire (&lt;lo year interval) than some cohabiting species of obligate seeders.

&lt;b&gt;&lt;i&gt;Introduction:&lt;/i&gt;&lt;/b&gt; Kabuki syndrome (KS) is a genetic disorder with characteristic facial dysmorphisms, short stature, hypertension, and obesity later in life. The aim of this study was to evaluate catch-up growth and cardiovascular markers before and during growth hormone (rhGH) treatment in KS children. &lt;b&gt;&lt;i&gt;Methods:&lt;/i&gt;&lt;/b&gt; This prospective study included 18 children whose KS was genetically established. Each KS subject received rhGH for a period of 2 years. Several measurements were performed before and during treatment: anthropometry, glucose metabolism, lipid profile, markers for endothelial function, and low-grade inflammation. &lt;b&gt;&lt;i&gt;Results:&lt;/i&gt;&lt;/b&gt; This study found an increase in delta height standard deviation score (SDS) for the whole group of 1.1 SDS after 2 years of rhGH treatment. Baseline metabolic profiles showed no cardiometabolic abnormalities in these children. Although 4 out of 18 children were obese, there were no signs of the metabolic syndrome. During rhGH treatment, serum low-density lipoprotein cholesterol concentrations decreased significantly (2.16–1.91 mmol/L, &lt;i&gt;p&lt;/i&gt; = 0.04). Apolipoprotein B100 concentrations also showed a reduction after 24 months of treatment, but the other lipid and (apo)lipoprotein parameters did not change. While other endothelial function markers were stable, only vascular cell-adhesion molecule-1 concentrations increased (1,084–1,161 pg/mL, &lt;i&gt;p&lt;/i&gt; &amp;#x3c; 0.01) during rhGH therapy. Furthermore, BMI and waist circumference improved during treatment. There were no signs of hypertension. &lt;b&gt;&lt;i&gt;Conclusions:&lt;/i&gt;&lt;/b&gt; At baseline and during rhGH therapy, there were no signs of the metabolic syndrome. This is the first study demonstrating that rhGH treatment in KS children is a safe and effective therapy and that it positively influences linear height without exerting adverse effects on a wide array of cardiovascular risk markers.

642 Background: HER2+ breast cancer (BC) is a well characterized subtype of BC, due to the predictive value of HER2 overexpression for anti-HER2 targeted therapies. Nevertheless, around 50% of HER2+ BC are ER+ and clasiffied as luminal B/HER2+, but their biological and clinical behaviour may be different from HER2+/ER- tumors. Methods: We retrospectively reviewed 347 HER2+ (Herceptest +++ or FISH+) early BC patients (see Table) diagnosed at our institution in 1997-2007, and were divided into two study groups: HER2+/ER+ (group A) and HER2+/ER- (group B). ER+ was defined if expressed in &gt; 10% tumor cells. Results: See table below. Mean age: 54.7 y (44.6 – 65). Mean Ki 67 was higher in B (37,2 vs 22,4%, p&lt;0.0001). At the current FU, n¼ of events were insufficient to reach median DFS/OS. Mean DFS was 6.9 y (3.5 – 10.2); recurrent disease was higher (p 0.003) for B (54 pts, 43%) vs A (62 pts, 28%). 5-year DFS estimates: 78.4 % (95% CI 72.3 – 83.3) and 62.3 % (95% CI 53 – 70.27) for A and B, and 10-year DFS was 73.4% (95% CI 66.6 – 79) and 52.5% (95% CI 42.4 – 61.6) for A and B, respectively (p 0.0006). Most common recurrent sites were local (18) and bone (9) for HER2+/ER+ and liver (8) and lung (8) for HER2+/ER-. Mean OS was 8.03y (5.4 – 10.8); 28 (12,6%) pts died in A, vs 26 (21%) in B (p 0.043). 5-year OS estimates: 93.9 % (95% CI 89.7 – 96.4) and 87.6 % (95% CI 80.3 – 92.3) for A and B, and 10-year DFS was 84.2% (95% CI 77.5 – 89.0) and 74.1% (95% CI 63.4 – 82.2) for A and B, respectively (p 0.01). Conclusions: ER expression in HER2+ early BC defines two clinically distinct diseases with different long-term prognosis. Objective We conducted a multicentre, double-blind, placebo-controlled, randomized study to investigate the efficacy of 2-year lamivudine treatment in hepatitis B e antigen (HBeAg)-negative chronic hepatitis B. Methods One-hundred-and-thirty-nine treatment-naive patients with HBeAg-negative chronic hepatitis B were randomized to receive either lamivudine (100 mg daily) or placebo in a 2:1 ratio for 24 months and were followed for an additional 6 months. The primary endpoint was complete response, defined as hepatitis B virus (HBV) DNA &lt;10,000 copies/ml and normalization of alanine aminotransferase (ALT) levels at month 24. Results On intent-to-treat analysis at month 24, significantly more patients in the lamivudine group than in the placebo group had complete response (56% and 11%, respectively; P&lt;0.001) or negative HBV DNA (26% and 6%, respectively; P=0.006). After adjustment of baseline HBV DNA and ALT, the odds ratio for complete response of the lamivudine group versus the placebo group was 10.8 (95% confidence interval: 3.8–30.2; P&lt;0.001). The median log HBV DNA reduction was 3.21 copies/ml for the lamivudine group compared with 0.47 copies/ml for the placebo group ( P&lt;0.001). Genotypic resistance was detected in 23% and 31% of patients in the lamivudine group at months 12 and 24, respectively. Negative HBV DNA at month 6 was associated with high complete response (84%) and low drug resistance (1%) at month 24. At month 30, there was no difference between lamivudine and placebo groups in the rates of complete response (26% vs 19%, respectively; P=0.38) or negative HBV DNA (10% vs 2%, respectively; P=0.09). Conclusions Two-year lamivudine treatment is effective in HBeAg-negative chronic hepatitis B. However, the response is not sustained after treatment cessation.

Two billion people have been infected with hepatitis B virus. Approximately 1.5 million people are chronically infected. An estimated 820,000 people die each year from hepatitis B and related complications such as liver cancer. <strong>Objective:</strong> The aim of this study is to assess the knowledge, attitude and vaccination status of clinical year students against hepatitis B infection at University of Gondar, Gondar, Ethiopia 2023. <strong>Method:</strong> An institutional based cross-sectional study design was conducted at the University of Gondar. A convenience sampling technique was used to select the study samples of participants. A structured self-administered questionnaire was used to collect data. After data collection data cleaning was done to avoid misleading data. The data collected was entered into and analyzed using statistical package for the social sciences (SPSS, version 20) software. <strong>Results: </strong>This study aims to assess knowledge, attitude and practice towards hepatitis B and associated factors in clinical year students in University of Gondar college of medicine and health science. From total of 465 students, 443 students had responded to the question. According to our study 3.6% of participants had vaccinated the full dose for hepatitis B. <strong>Conclusion: </strong>Our study revealed that 3.6% of study participants received full dose vaccination against hepatitis B virus. We would like to recommend all students in the healthcare professional to be vaccinated before they start clinical attachments, provisions of training for students on infection prevention mechanisms, and universal precautions particularly on HBV.

This study aims to explore the competence level of B.Ed. first year students in reading skill under Tribhuvan University. The data were collected by using two types of testing tools constructed from compulsory English text-book selecting two different reading passages. From the first, 14 objective questions were asked as multiple choice items which carried 14 marks; and from the other reading passage, 12 subjective questions were asked which carried 36 marks. The full mark of the test was fifty and the time allotted to conduct the test was an hour (60 minutes). Two campuses were the institutional samples and forty students from one and sixty students from the other were the samples for the study. Out of these hundred samples, 60 girls and 40 boys were selected proportionally. The marks obtained by the students in objective test item, subjective test item and total are the depended variables and the total number of students, the number of boys and girls are the independent variables. Through the study it was found that the students’ overall competence in reading comprehension was found around 50% in average. The reading competence they performed in objective test item was comparatively higher than subjective test item. In totality, the reading competence level of the boys was slightly better than the girls but the difference does not seem so significant.

Objective Prospective studies on relationships between hedonic hunger and BMI (Body Mass Index) during weight management are lacking. This study examined if hedonic hunger reduced during a behavioural weight management programme, and if hedonic hunger predicted future BMI. Methods Participants were 594 community-dwelling, UK-based adults(396 female; age 56.43 years, s.d. = 12.50, range 20–83 years); 490 participants (82.5%) had obesity. Participants were randomised to a 12- or 52-week behavioural weight management intervention (WW12 or WW52, respectively) or a brief self-help intervention (BI). Relationships between hedonic hunger and BMI over 24 months (baseline, 3, 12, 24 months) were analysed using an autoregressive cross-lagged model. Results Hedonic hunger scores decreased from 2.71 (s.d. = .91) at baseline to 2.41 (s.d. = .88) at 3 months (p &lt; .001, CI .22 to .38), remained reduced to 24 months, and were not affected by intervention arm at any time point (p’s&gt;.05). Baseline hedonic hunger scores predicted 3-month scores (B = .76, SE = .03, p &lt; .001, CI .71 to .82), 3-month scores predicted 12-month scores (B = .76, SE = .03, p &lt; .001 CI .72 to .80), and 12-month scores predicted 24-month scores (B = .72, SE = .03, p &lt; .001, CI .64 to .77). Higher hedonic hunger at 3 months predicted higher BMI at 12 months (B = .04, SE = .02, p = .03, CI .01 to .07) but not at 24 months (p&gt;.05). BMI at 12 months was lower in WW52 30.87kg/m2, s.d. = 5.02) than WW12 (32.12 kg/m2, s.d. = 5.58, p = .02, CI .16 to 2.34) and BI (32.74 kg/m2, s.d. = 4.15, p = .01, CI .30 to 3.45). BMI was not affected by intervention at any other time point (p’s&gt;.05). Conclusion Hedonic hunger reduced during weight management irrespective of intervention. Early reductions in hedonic hunger appear to be associated with lower BMI in the medium-term. Identifying ways to reduce hedonic hunger during weight loss could aid weight management for some people.

During annual influenza epidemics, influenza B viruses frequently co-circulate with influenza A viruses and in some years, such as 2005, large outbreaks have occurred while in other years, the virus virtually disappears. Since 1987 there have been two lineages of influenza B viruses co-circulating in various countries and causing disease in humans. The proportions of these two lineages vary from year to year and country to country. For example, in 2005, the B/Victoria/2/87 lineage was predominant in New Zealand while in Australia the B/Yamagata/16/88 lineage was more common. Antigenic and genetic analysis has revealed gradual movement in the both lineages. Careful monitoring of the two virus lineages is important, as they are antigenically distinct. This is an important consideration for influenza vaccine formulation decisions, as only one influenza B component is traditionally included in the annual trivalent influenza vaccine. Species-specific production rate per unit biomass (P/B, per year) ratios were calculated for 79 freshwater fish species of eastern Canada. P/B (per year) ratios were calculated using two methods, which were based on allometry with fish weight-at-maturity and life expectancy, respectively. P/B (per year) values obtained by the two methods were significantly correlated, as expected from life history theory, since the two predictors (longevity, size-at-maturity) were themselves correlated. Species-specific P/B (per year) ratios were also significantly correlated with field observations of P/B from published sources. The estimation of P/B based on allometry with fish size is recommended because of its utility; the predictive equation is P/B (per year) = 2.64Wmat-0.35, where Wmat is weight-at-maturity. Both the coefficient 2.64 and the exponent -0.35 of this equation are provisional and require further validation from field studies of fish production. More accurate estimates of P/B are possible if population-specific information on size-at-maturity or mean size is known. The product of average fish biomass and estimated P/B coefficients (habitat productivity index (HPI) = B × P/B) is a proposed measure of habitat productive capacity.

ObjectiveWe wanted to evaluate efficacy on inflammatory parameters of rituximab (RTX)-personalized reinfusion scheme using a memory B cell–based treatment regimen.MethodsThis is a prospective, uncontrolled, open-label study including patients with MS treated with RTX in 2 Italian MS units. All patients were treated with RTX induction, followed by maintenance infusion at the dosage of 375 mg/m2, according to memory B cell repopulation (0.05% of peripheral-blood mononuclear cells [PBMCs] for the first 2 years, 0.1% of PBMC for the third year). MS activity was assessed as clinical or MRI activity.ResultsOne hundred two patients were included in the analysis. Mean follow-up was 2.40 years (range 0.57–7.15 years). The annualized relapse rate (ARR) was 0.67 in the year before RTX start and decreased to 0.01 in the 3 years after RTX initiation (global ARR). The proportion of patient with MS activity (i.e., relapse or MRI activity) was 63.16% in the year before RTX start and decreased to 8.7% (0–6 months), 1.3% (6–12 months), 0% (12–24 months), and 0% (24–36 months). Annualized RTX infusion rates were 1.67 (95% confidence interval [CI]: 1.43–1.94), 0.76 (95% CI: 0.58–0.98), and 0.78 (95% CI: 0.52–1.12) for the first 3 years after RTX initiation, respectively. Patients were reinfused with a mean infusion interval of 367 days (range 181–839 days).ConclusionThe results of this study show that the memory B cell–based RTX reinfusion protocol is able to reduce the mean number of RTX reinfusions with persistent reduction of disease activity.Classification of evidenceThis study provides Class IV evidence that for patients with MS, a memory B cell–based RTX reinfusion protocol can reduce the mean number of RTX reinfusions with persistent reduction of disease activity.

&lt;b&gt;&lt;i&gt;Background:&lt;/i&gt;&lt;/b&gt; The prevalence and natural history of progressive fibrosing interstitial lung diseases (PF-ILDs), and their response to commonly used treatments in real life are largely unknown. &lt;b&gt;&lt;i&gt;Objectives:&lt;/i&gt;&lt;/b&gt; The aim of the study was to describe the prevalence, clinical characteristics, management, and outcomes of PF-ILD patients attending 2 Italian referral centers (San Gerardo Hospital, Monza, and San Giuseppe Hospital, Milan) from January 1, 2011, to July 31, 2019. &lt;b&gt;&lt;i&gt;Methods:&lt;/i&gt;&lt;/b&gt; From a cohort of non-idiopathic pulmonary fibrosis fibrosing ILD patients with at least 2-year follow-up, we selected only those with progressive disease, defined as per the INBUILD trial, collecting their demographical, clinical, and functional data. &lt;b&gt;&lt;i&gt;Results:&lt;/i&gt;&lt;/b&gt; Out of the 245 fibrosing ILD patients, 75 (31%) were classified as PF-ILDs (median age 66 years, 60% males), most frequently idiopathic non-specific interstitial pneumonia (28%), followed by connective tissue disease-associated ILD (20%), chronic hypersensitivity pneumonitis, and sarcoidosis (17% each). Most patients (81%) were categorized as PF-ILDs because of forced vital capacity (FVC) decline ≥10%, while 19% experienced a marginal FVC decline (between 5 and 10%) associated with worsening respiratory symptoms or increasing extent of fibrotic changes on high-resolution computed tomography. Disease progression occurred after a median of 18 months from ILD diagnosis. The vast majority (93%) of PF-ILD patients received prednisolone, alone (40%) or associated with steroid-sparing agents (52%), and 35% of treated patients developed treatment-related adverse events. After ILD progression, the median survival was 3 (interquartile range (IQR) 2–5) years, with a 2- and 3-year mortality rate of 4 and 20%, respectively. &lt;b&gt;&lt;i&gt;Conclusions:&lt;/i&gt;&lt;/b&gt; In a real-life setting, approximately one-third of the fibrosing ILD patients showed a progressive course despite treatment. Studies aimed to better phenotype this subgroup of patients are needed.

In this sense, this research aimed to identify a parent's knowledge about breastfeeding. This is an exploratory and descriptive study with a qualitative approach, performed in the Family Health Strategy in Bom Jesus, Rio Grande do Norte State, Brazil. Data were collected through semi-structured interview with the guiding question: what is your understanding of breastfeeding? The population was 15 men with aged over 18 years old and preserved mental faculties, who lived with his wife and the child under one year of age and performed development and growth consultations in the Family Health Strategy. The interviews were conducted in place previously defined parent. The information was organized as content analysis Bardin, giving rise to two categories and analyzed according to the symbolic interacionism. It was observed that parents of participating antenatal clinics have more knowledge about it and recognize the benefits of breastfeeding in this way can encourage their wives in order to exclusive breastfeeding.

The aim of the study was to investigate rural–urban differences in depressive symptoms in terms of the risk factors among older adults of two regions in Myanmar to provide appropriate intervention for depression depending on local characteristics. This cross-sectional study, conducted between September and December, 2018, used a multistage sampling method to recruit participants from the two regions, for face-to-face interviews. Depressive symptoms were assessed using the 15-item version of the Geriatric Depression Scale (GDS). Depressive symptoms were positively associated with living in rural areas (B = 0.42; 95% confidence interval (CI): 0.12,0.72), female (B = 0.55; 95% CI: 0.31,0.79), illness during the preceding year (B = 0.68; 95% CI: 0.45,0.91) and non-Buddhist religion (B = 0.57; 95% CI: 0.001,1.15) and protectively associated with education to middle school level or higher (B = −0.61; 95% CI: −0.94, −0.28) and the frequency of visits to religious facilities (B = −0.20; 95% CI: −0.30, −0.10). In women in urban areas, depressive symptoms were positively associated with illness during the preceding year (B = 0.78; 95% CI: 0.36, 1.20) and protectively associated with education to middle school level or higher (B = −0.67; 95% CI: −1.23, −0.11), middle or high wealth index (B = −0.92; 95% CI: −1.59, −0.25) and the frequency of visits to religious facilities (B = −0.20; 95% CI: −0.38, −0.03). In men in rural areas, illness during the preceding year was positively associated with depressive symptoms (B = 0.87; 95% CI: 0.33, 1.42). In women in rural areas, depressive symptoms were positively associated with illness during the preceding year (B = 0.83; 95% CI: 0.36, 1.30) and protectively associated with primary education (B = −0.62; 95% CI: −1.12, −0.12) and the frequency of visits to religious facilities (B = −0.44; 95% CI: −0.68, −0.21). Religion and wealth could have different levels of association with depression between older adults in the urban and rural areas and men and women. Interventions for depression in older adults should consider regional and gender differences in the roles of religion and wealth in Myanmar.

Male breast cancer is a rare clinical entity accounting for approximately 1 per cent of all breast cancers. The present study investigated changes in patient characteristics, disease patterns, treatment, and outcomes over a 30-year period. A retrospective chart review was performed on male breast cancer patients treated between 1975 and 2005 at Eastern Virginia Medical School, Norfolk, VA. Demographic, pathologic, treatment, and survival information was collected. To facilitate comparison of trends, the patients were divided into two groups: Cohort A (1972–1991, previously reported) and Cohort B (1992–2005). Both cohorts included 28 male patients. Comparing the cohorts, no statistical differences were noted in median age, ethnicity, presenting symptoms, or progesterone receptor status. In Cohort A, 70 per cent of patients were estrogen receptor positive, compared with 100 per cent of Cohort B ( P = 0.02). Her2/neu was positive in three of five patients in Cohort B. There was a trend toward more conservative surgery, with no radical mastectomy or orchiectomy performed in Cohort B. Only two patients had sentinel lymph node mapping, both from Cohort B. Infiltrating ductal carcinoma was more prevalent in Cohort B ( P = 0.04). For Cohort A and B, 5-year survival was 43 per cent and 51 per cent, respectively, which was not statistically significant. For male breast cancer, radical mastectomy is no longer a common treatment modality. Male breast cancer of today is more hormonally responsive which may have important implications for therapy. Survival has not significantly improved over the previous 30 years. Compilation of multi-institutional data of male breast cancer is needed to advance the treatment of this uncommon disease.

We investigated prospectively the relationship of Type A behavior and its subcomponents with cardiac mortality and recurrent nonfatal cardiac events in a 2-yr. follow-up of 90 patients [69 men and 21 women, M age = 56.4 yr., SD = 8.4] after acute coronary syndrome. Type A behavior was assessed via the general Bortner Type A Index. Each patient completed the Bortner's scale before hospital discharge During the first 2-yr. follow-up, there were 14 cardiac deaths among patients with myocardial infarction. 8 patients had recurrent cardiac events and were hospitalized, and 19 patients had an effort-induced angina pectoris. Patients with acute myocardial infarction who died during follow-up had a significantly lower Bortner score than patients with a secondary cardiac event. Bortner scores of patients with acute myocardial infarction who died indicated Type B behavioral patterns. Mortality was significantly higher in the patients classified as showing Type B (21.8%) behavior than in the patients classified as showing Type A (12.0%) behavior. Patients with a secondary cardiac event had more common Type A behavior patterns and higher Bortner scores than patients without a secondary cardiac event. The items on Bortner's scale “very competitive, ever rushed, tries to do too many things at once, fast in daily activities and expresses feelings” were inversely associated with cardiac deaths. These findings suggest that patients with acute coronary syndrome classified by scores on the Bortner scale as Type B behavior have a greater probability of death, and patients classified on the Bortner scale as Type A behavior have a greater probability of secondary cardiac events during follow-up. This finding may have implications for the treatment of patients with acute coronary syndrome. The inferior survival of patients with Type B personalities argues against attempts to modify Type A behavior in postinfarction patients.

The appearance of these rare types in the same patient and the same period makes the treatment of the disease difficult. But both types respond well to anthracyclines and immunotherapies. A 74-year-old woman was treated with R-CHOP (Rituximab, cyclophosphamide, doxorubicine, vincristine, prednisolone) without surgery and/or radiotherapy, and no recurrence at 2 years follow-up. Despite the poor prognosis of these types of lymphomas, treatment responses are quite good as they are in other subtypes.

Membranous glomerulopathy is a well-recognized extrahepatic manifestation of chronic hepatitis B virus (HBV) infection. The authors report two cases of HBV-related nephrotic syndrome treated with lamivudine. A 46-year-old Chinese man had a hepatitis B e antigen seroconversion along with improvement in his nephrotic syndrome after lamivudine therapy. Two years after treatment was discontinued, a reactivation of HBV was successfully treated again with lamivudine. A 44-year-old Chinese woman, who was intolerant of interferon, was treated with lamivudine for 15 months without a virological response. However, two years after completing lamivudine, her nephrotic syndrome resolved. Implications for the treatment of HBV-related glomerulopathy and a review of the literature are presented.

Introduction. Hemodialysis patients often have chronic volume overload, hypervolemia, which may cause severe complications. In some patients hypervolemia is masked, without any signs and symptoms, such as hypertension, edema and bibasilar crackles on lung auscultation. Lung ultrasound can be used to detect these patients. Pre- and post-dialysis lung ultrasound can be used to quantify lung congestion using the B line score. High post-dialysis B line score can identify patients with residual hypervolemia and adequate measures can be taken (increasing ultrafiltration, extended duration of hemodialysis, additional dialysis sessions). Case Reports. The first patient was a 57-year-old male. The hemodialysis vintage was 4 years. His interdialytic weight gain was 2.8 kg. The lung ultra?sound was performed before and after dialysis and B line score was calculated. The pre-dialysis score was 15 and post-dialysis score was 2. The second patient was a 72-year-old male. The hemodialysis started 5 years before. This patient was noncompliant with the medical advice of his physician regarding diet and medications. His interdialytic weight gain was 5.6 kg. His pre-dialysis score was 26 and post-dialysis score was 15. Both patients were without signs and symptoms of hypervolemia after dialysis. Nevertheless, the second patient was 1.6 kg over his dry weight after dialysis. An additional dialysis session was scheduled, after which his post-dialysis B line score fell to 5. Conclusion. Lung ultrasound can be used to assess volume status in dialysis patients. It can identify hypervolemia in asymptomatic patients and allow necessary corrections.

In this study, we established whether controlling backfat thickness at breeding over a long time period can result in optimized reproductive performance in sows. Two commercial herds were used: herd A (322 purebred Landrace sows) and herd B (337 cross-bred Yorkshire-Landrace sows). Backfat thickness at breeding and farrowing, along with reproductive data [live Born (LB), stillborn, mummified, piglets alive at 48 h (LB48) and the weaning-to-estrus interval (WEI)] were collected over nine parities. The herd B producer was more successful in maintaining a steady backfat thickness at breeding than was the herd A producer. At breeding, the backfat thickness of sows from herd A showed a marked decrease between parities 2 and 5. During their first parity, these sows gained the least backfat during gestation and lost the most backfat during lactation. Sows from herd B had more LB and LB48 than sows from herd A. In herd A, a longer WEI was found in first and second parity sows. Our results demonstrate that maintaining backfat thickness throughout the reproductive cycle is more important than fixing this parameter at breeding alone. This is particularly true for gilts, which are prone to mobilize fat tissue reserves, a condition associated with declining reproductive performance. Background and objectives: The aim of this study was to evaluate the therapeutic results in patients with exudative AMD treated with ranibizumab and aflibercept intravitreal injections over a two-year observation period. Materials and methods: A retrospective observational study was conducted in a clinical hospital on a group of patients who randomly qualified for treatment with Aflibercept (group A) and Ranibizumab (group B) as part of the Polish National Health Fund Medical Program for exudative AMD. Group A consisted of 90 patients, and group B contained 54 patients. The choice of drug in a patient depended solely on the availability of the medication at the time. Before each injection, best corrected visual acuity (BCVA) on the ETDRS scale and central retinal thickness (CRT) were assessed using optical coherence tomography (OCT). Patients from both groups were treated in the first year of treatment with a rigid scheme of 3 doses of 2.0 mg Aflibercept (group A) and 0.5 mg Ranibizumab (group B) at monthly intervals, followed by 4 doses at bimonthly intervals. In the second year, a “pro re nata” scheme was applied. The aim was to evaluate changes in BCVA and CRT after three injections, after 7 injections (about 12 months), and after the second year of therapy (24 months) with reference to the baseline and to compare the effectiveness of the medications. The influences of the following factors were studied: age, gender, initial BCVA, and initial CRT, as well as the number of injections received. Results: No significant statistical differences were found between patients receiving Aflibercept and Ranibizumab therapy in terms of achieving improved visual acuity and reducing retinal thickness after two years of therapy. Conclusions: Both aflibercept and ranibizumab were found to be effective for treating exudative AMD.

This study evaluated the association between risk factors and severity of problems related to drug use in secondary school adolescents. This study had the participation of 1192 students from 6th to 9th year of a city in the South of Minas Gerais, Brazil. Data collection occurred through a questionnaire containing: socio-demographic data and the Drug Use Screening Inventory. Drug use was prevalent in adolescents aged 14 and 15 years, atheist, with good family relationships, living with friends/institutions, attended parties once a month, one or two times a week and three and four times a week. There was main damage in the areas of psychiatric disorders, family system and social competence among those who made use of drugs (except alcohol and tobacco). The results point to the need for implementation of preventive strategies of drug use and health promotion in the school context, whereas consumption was associated with significant damage.

A 1-year study was carried out to investigate the prevalence of Salmonella in two abattoir environments coded “A” and “B” in Gaborone, Botswana. The total number of environmental samples collected from abattoirs A and B was 250 and 300, respectively. The samples were taken from soils in the corrals, knife blades, saw blades, cattle-drinking water, cattle feces, and feed. Preenrichment, enrichment, and selective/differential media, which enabled the favorable growth of Salmonella, were used in the study. Salmonellae were present in all sampled environments. The most common serotypes found in the environment at abattoir A were E1, C1, C2, and B. Serotypes B, C1, C2, C3, and E1 were common in abattoir B. Antigenic characterization of the salmonellae isolates showed that Salmonella Anatum, Salmonella Azteca, Salmonella Saintpaul, Salmonella Cerro, and Salmonella Westhampton were predominant in abattoir A, whereas Salmonella Anatum, Salmonella Mbandaka, Salmonella Molade, Salmonella Reading, and Salmonella Oranienburg were dominant in abattoir B. Implementing hazard analysis critical control point principles in work procedures would definitely reduce the gross contamination taking place in abattoirs.
