Diabetic foot ulcers (DFUs) are a common and severe adverse event associated with diabetes, as 25% of diabetic patients will experience DFUs. The lack of effective DFU therapies results in 20% of diabetic patients requiring amputation. Neutrophils are recruited early to the wound bed and, upon activation, they undergo histone citrullination by protein arginine deiminase (PAD)4, exocytosis of chromatin and enzymes as neutrophil extracellular traps (NETs), and death. This thesis investigated the cost-effectiveness of guideline-based care to manage patients with venous leg ulcers in the Australian setting. It showed that both the healthcare system and patients affected by venous leg ulcers would benefit from implementing and funding guideline-based care. This research provided important evidence to policy makers on the most efficient use of scarce health resources to manage venous leg ulcers. It is expected that these results will support changes to the reimbursement structure to increase access to guideline-based care for venous leg ulcers in Australia.

Chronic leg ulcers are a major health problem that affect 1-3% of adults over 65 and are a financial burden on healthcare systems worldwide. In this study biochemical assays were performed on sequential samples collected from both healing and non-healing ulcers. The results demonstrated that inflammatory resolution was necessary for ulcers to heal. Interestingly, in healing ulcers, proteins associated with dampening inflammation did not change over time whilst those keeping the wound in an inflammatory state decreased, suggesting an alternative mechanism for resolution of inflammation.
