Full textGlobalization, financial and political stability and current degree of economical development that Brazil are living (because of these condition Standard & Poor’s gave to Brazil the classification of investment grade on April 30th, 2008) combined with Brazilians income degree are converting Brazil in an attractive market for outside capital. Private health sector regulamentation which started with the Brazilian law number 9656 from 1998, and the creation of Agência Nacional de Saúde Suplementar – ANS (in 2000) let this sector to attract curiosity and nowadays the attention of big stakeholders. Medical and odontological cooperatives have important participation in this market with signification part in share – like 40% on its set. The regulamentation bring on a concentration in the sector by portfolio purchase or until same all firm acquisition – that involves trillion of Brazilian reais. These situations find on medical and odontological cooperatives quirks that diverge from negotiations facilities.

Full textThe objective of the dissertation is to analyze the administration of OPME (Ortheses, Prostheses and Special Materials) in a health care company (UNIMED RS), in the context of the systemic and sectorial conditions of competitiveness and of the strategic choices of the company. Using concepts and analytic outlines of the economy of the strategy, the dissertation seeks to show how important the administration of the OPME is for the trade-off between cost and differentiation, given the strategic choices that characterize the competitive positioning of UNIMED in the ambit of the section of Health Plans and with limitations imposed by the sectorial regulator agent. The progress in the area of medical materials is impressive. The speed that the new surgical materials (Ortheses, Protheses and Special Materials) are introduced in the market is a reflex of the easiness to access information nowadays. These materials are materials of high cost, and there is still doubt about the effectiveness and safety of these materials for the patients, besides there is a geometric increase of the costs of the Health Plans and due to the rigorous governmental control these costs cannot be passed to the beneficiaries. The Unimed System, aware of the need for a structured process that evaluates the incorporation of new technologies in the area of health, and with the objective to maintain its status as an institution that offers its beneficiaries the best medical technology available, created its own Technical Chamber of Medicine Based on Evidences. In this context, this study approaches Unimed's experience in this administration and the absorption of new technologies.

Full textIntroduction: The Primary Health Care (PHC) consisted in a strategy capable to offer health services in an equitable and efficient way to order access to the user. PHC is understood as the coordinator of the healthcare network. Within the assumptions of PHC, access is the first contact condition and corresponds to the use and to the accessibility to health services. It happens in health care spaces for each new problem or even in a new episode of an existing problem. Thus, the users perspective is an important form of information for the construction of assessments of the care process, as well as the obtained results with the applicability of certain care models. Objective: The objective of this study is to analyze the access to Primary Health Care services by means of the adult user perception in the city of Porto Alegre/RS. Methodology: This is a population-based study carried out with users of health service in three administrative-districts in Porto Alegre in the period of September 2016 to May 2017. The studied sample consisted of adult individuals, with a minimum age of 18 years, living in the areas covered by the primary health care public network of the city. This sample was estimated considering 42.4% of the attribute access, as measured between users of PHC services and residents of the area covered by the Partenon-Lomba do Pinheiro District Area in the year 2012, as attribute access by Primary Care Assessment Tool/PCATool-Brazil in previous studies. The final sample size was 214 users. Data were collected through a questionnaire constructed for this study with fragments of PCATool-Brazil adult version, the National Health Research (Acronym in Portuguese is PNS) and the National Sample Survey per Household (Acronym in Portuguese is PNAD). Data were analyzed using SPSS software v.21 (Chicago: SPSS) where were performed Anova, chi-square and t-student tests. Results: 66.0% of the interviewees were women, aged 60 years or more (68.6%), 68.5% from elementary to high school level, married (64.2%) and white self-declared color (59.8%). The average distribution of the PHC access attributes regarding Health Units user's were unsatisfactory. There was an association between the variable age range aged 60 years or more with the First Contact Access/Utilization attribute and, also, with the Access Score, suggesting that elderly, who access the service, qualified it as satisfactory attributes. Conclusion: This study presents the importance of analyzing sociodemographic data for the functioning of PHC and shows that the studied city should direct efforts to the part of the population that presented Access attribute as unsatisfactory. The present study also presents potentially useful information for Unified Health Services (Acronym in Portuguese is SUS) management in order to seek improvements and qualification of access to health services.

Full textNon-specific chronic low back pain (NCLBP) is a prevalent condition that causes economical costs for individuals and society. There is a common agreement in its literature saying that NCLBP is multifactorial and although the clinical guidelines recommend managing it based on biopsychosocial orientation for treatment, literature shows that this biomedical orientation is still an influence among health professionals. In Brazil, however, little do we know about the orientation of treatment adopted by physiotherapists in managing NCLBP, mainly in Unified Health System context and the evaluation of attitudes and beliefs of these professionals allow a better understanding of treatment orientation by them in their practices as well as their possible impact in NCLBP management. Moreover, studies show that the work environment, education and age of the health professionals are able to influence their beliefs and attitudes in the management of NCLBP and that knowing them can contribute to propose punctual intervention to the health professionals. Nevertheless, the influence of these factors was not well established yet. Thus, the present master’s work was composed of two studies. The first study systematically reviewed the association of a treatment orientation in managing patients of non-specific chronic low back pain to the demographic and professional profile of physiotherapists. The results of our systematic review showed moderate evidence on the influence of women on the adoption of a biopsychosocial orientation for treatment and private workplace on the adoption of a biomedical orientation for treatment of physiotherapists in the management of NCLBP. Therefore, demographic and professional factors can influence professional practice adopted by physiotherapists in the management of NCLBP. The second study described attitudes and beliefs of the physical therapists who work at Unified Health System in Porto Alegre in the management of patients with NCLBP and identified the relationship between their demographic and professional characteristics and orientation for treatment of NCLBP. This study of inhabitants, cross-sectional and the data were collected through a demographic and professional questionnaire and a Pain Attitudes and Beliefs Scales for Physiotherapists (PABS-PT) questionnaire. The second study included 49 physiotherapists and the results showed a bigger agreement to beliefs and attitudes related to biomedical orientation, the score on this scale being 15,5% higher than in behavior and a regular and positive correlation of time training and behavior scale. We conclude that differently from what international guidelines establish about NCLBP management, beliefs and attitudes related to biomedical treatment orientations still are prevalent among physiotherapists in managing patients of NCLBP from Unified Health System in Porto Alegre.

Full textThe study involves the questioning of the institutionalization process of Integrative and Complementary Practices (ICPs) in Primary Care (PC) and the implementation process of the National Policy on Integrative and Complementary Practices (NPICP) in the city of Porto Alegre, Rio Grande do Sul. Our goal is to analyze the institutionalization and implementation processes of NPICP in PC Health System of Porto Alegre, identifying the offered practices, hindering and facilitator factors of this implementation and how the access and the flow of users in these services on primary care are. For this, we conducted a qualitative study with a) focus group, with all district managers of Porto Alegre; b) semi-structured interviews with primary care practitioners who use ICPs in their care or who are training in ICPs, even if not acting on it; and c) diary at all stages of data collection. In the thematic area on PICs offer, the categories interest/disinterest of the actors emerged. For one side, there is a health professional who is looking for knowledge and alternatives to incorporate into their work process. For the other, managers who prioritize other logic services offer; and informality category, which relates to the lack of professionals on the provision of ICPs, the implementation of the policy and its factors involved, making the provision of such care in Primary Care happen informally, discontinued and with little appreciation. In the thematic area about factors that hinder and facilitate the provision of ICPs, the categories concept of health and invisibility were pointed out. The health conception category is how it is developed the direction of activities and services in health, factor identified as the main element in the implementation difficulty of the NPICP and supply growth of ICPs. The invisibility category concerns the lack of divulgation of PICs offered in the service network for professionals and their routing mode, and the lack of visibility that ICPs have in the community for those who want to access the services. It also refers to the issue of ICPs offer depend exclusively on the health professional initiative. The thematic analysis of the data pointed to a technical care model of health care in Primary Care focused on the biomedical model, and that, despite the managers recognize the limitations of this model, it is kept by reproducing this view, directing health actions. We emphasize the importance that Primary Care has to the visibility and expansion of ICPs, however, the challenges are not small. The ICPs have great potential, but it is not enough to create a policy for the integration of ICPs actually occur in SUS. It is necessary to give visibility and legitimacy to practices that already occur in the services: it is not the case to include new professionals, nor focus on specialized care, but to empower and enable the already practices developed by professionals.

Full textThis ethnography describes the daily life of a Primary Health Care unit located in Porto Alegre and a member of the Community Health Service – this latter being part of the institutional structure of the Conceição Hospital Group. This study takes as its starting point the practice of reception accomplished by members of the Primary Healt Care unit, and directed to its users. In this context, such a practice ultimately prompts up discourses and practices relating to participation: of their employees, as well as of its users, who appear grouped under the heading community. Further, this work describes the segmentation and asymmetries that come up as sources of conflict among members of the healthcare team. In this group, which is guided by a discourse of equal participation among its members, there is evidence of an ongoing tension between hierarchy and horizontality. In addition, multiple meanings and practices of community participation are highlighted by actors who hold different positions in this scenario. The different perspectives from which the different actors convey meaning, as well as give rise to practices of participation are then marked. Finally, users' perceptions regarding reception are placed in the foreground, thus making it noticeable that the health system, as the different concepts and practices of what health is, are not at all an accomplished, unimprovable product, for, they are processes, not states. Therefore, this ethnography addresses the different political dimensions that are embedded in the everyday, distinctively relational fabric of the Unified Health System.
