Abstract:
The Introduction outlines this issue's special Forum on equity in access to health care, including three Articles and a Postscript. The Forum represents a continuation of the debates raised during a seminar organized by the Oswaldo Cruz Foundation in the city of Rio de Janeiro, Brazil, in 2006, in collaboration with UNICEF, UNDP, World Bank, the WHO Special Program for Research and Training in Tropical Diseases, and the United Nations Research Institute for Social Development. The authors approach health care access and equity from a comprehensive and contemporaneous perspective, introducing a new conceptual framework for access, in which information plays a central role. Trust is proposed as an important value for an equitable health care system. Unethical practices by health administrators and health care professionals are highlighted as hidden critical aspects of inequities in health care. As a whole, the articles represent a renewed contribution for understating inequalities in access, and for building socially just health care systems.

Abstract The Swiss Development Cooperation, Canada’s International Development Research Centre, the Swiss Tropical Public Health Institute, and the UNICEF/United Nations Development Programme (UNDP)/World Bank/World Health Organization (WHO) Special Programme for Research and Training in Tropical Diseases (TDR) collaborated on a project to review, understand and promote the use of multisectoral approaches (MSAs) in the prevention and control of vector-borne diseases (VBDs). The objectives of the project were to support a landscape analysis of how MSAs have been used in the prevention and control of VBDs; to develop a theoretical framework for guiding the implementation of interventions; and to test the recommendations in real-life conditions. To realize these objectives, the project supported several activities, including commissioning a series of scientific reviews on MSAs in 5 thematic areas, sharing the key findings of these reviews in workshops and events, and developing a guidance framework for the implementation of MSAs. These activities have produced the theoretical framework that will be tested in real-life conditions through the support of case studies. The collaboration on implementing multisectoral activities against VBDs will continue among TDR, the Swiss Tropical Public Health Institute, and new partners such as the WHO Water Sanitation and Hygiene Group, UNDP, and UN-Habitat, in order to face the challenges identified and propose solutions tailored to specific contexts. The prevention and control of VBDs require strong and adapted MSAs with the full participation of all relevant sectors.

Urban infrastructures have become imperative to human life. Any damage to these infrastructures as a result of detrimental activities would accrue huge economical costs and severe casualties. War in particular is a major anthropogenic calamity with immense collateral effects on the social and economic fabric of human nations. Therefore, damaged buildings assessment plays a prominent role in post-war resettlement and reconstruction of urban infrastructures. The data-analysis process of this assessment is essential to any post-disaster program and can be carried out via different formats. Synthetic Aperture Radar (SAR) data and Interferometric SAR (InSAR) techniques help us to establish a reliable and fast monitoring system for detecting post-war damages in urban areas. Along this thread, the present study aims to investigate the feasibility and mode of implementation of Sentinel-1 SAR data and InSAR techniques to estimate post-war damage in war-affected areas as opposed to using commercial high-resolution optical images. The study is presented in the form of a survey to identify urban areas damaged or destroyed by war (Islamic State of Iraq and the Levant, ISIL, or ISIS occupation) in the city of Mosul, Iraq, using Sentinel-1 (S1) data over the 2014–2017 period. Small BAseline Subset (SBAS), Persistent Scatterer Interferometry (PSI) and coherent-intensity-based analysis were also used to identify war-damaged buildings. Accuracy assessments for the proposed SAR-based mapping approach were conducted by comparing the destruction map to the available post-war destruction map of United Nations Institute for Training and Research (UNITAR); previously developed using optical very high-resolution images, drone imagery, and field visits. As the findings suggest, 40% of the entire city, the western sectors, especially the Old City, were affected most by ISIS war. The findings are also indicative of the efficiency of incorporating Sentinel-1 SAR data and InSAR technique to map post-war urban damages in Mosul. The proposed method could be widely used as a tool in damage assessment procedures in any post-war reconstruction programs.

Introduction: Sao Tome and Principe is an African low-and-middle-income country, where extreme poverty causes major health inequalities. No systematic research has been done on the consumption of alcohol and drugs in Sao Tome and Principe, and only overall statistics are available based on the importation of alcoholic drinks and their distribution among the population. There are also no studies on consumption of alcohol and illicit substances in children and youth and no preventive measures being undertaken. Besides that, manual databases present significant limitations, considering the lack of causes associated with mortality rates (0 - 5 years and > 5), and the difficulty to establish a cause/effect relation between diseases, deaths and life expectancy. No relevant data with burden of life was found in the reports of Centro Nacional de Endemias or the non-governmental, organization Instituto Marques de Valle Flor, a facilitator on healthcare clinical specialties selected on a voluntary basis by doctors from Portuguese hospitals. So, we proposed to provide a first overview of family and housing conditions, and above all, the consumption of alcohol and illegal drugs in young people. Thus, a project, the National Survey on Harmful Consumption of Alcohol and Drugs in Schools of Sao Tome and Principe, will be realized in order to better characterize the situation among children and young students and test public health communication strategies and preventive interventions aimed at this target-population. Interventions were designed taking into consideration local sociocultural realities of target audiences. We considered dialect language, single-parent families (matriarchal structure) and polygamy (mostly) in men and a country and governments led by men (patriarchal structure) and, in which the woman’s role, as Food and Agriculture Organization of the United Nations reports, remains overlooked. Subsequently, we will collect traditional alcohols samples from the two main islands for analysis (at Laboratório de Estudos Farmacêuticos and Laboratório Nacional de Engenharia Civil - Portugal) and to determine heavy metals in the production process and impact on burden of life.Material and Methods: In order to characterise the country’s situation in terms of alcohol and illicit substances consumption a literature review was carried out through a search in several international electronic databases, such as those of the World Health Organization, World Health Organization Africa, United Nation, The Lancet and Lancet Global Health, etc. Available data of the following institutions of Sao Tome and Príncipe was also analyzed: National Institute of Statistics, Ministry of Education, Culture and Training and Ministry of Health and Social Affairs. Several interviews with community and church leaders as well as with members of catholic missions were carried out to better understand the local situation. Following this, a nationwide cross-sectional survey of a sample of 2064 students will be carried out. This will include a questionnaire on socio-demographic characteristics, lifestyles, health behaviors/attitudes, alcohol and illicit substances consumption. Finally, based on the overall diagnosis obtained, some edutainment health communication preventive interventions will be tested in the primary schools of three districts (EDUCA_TURTLE) and on the radio journalists (EDUCA_PRESS). These were evaluated by primary school teachers and by radio journalists.

Clustering emerged as powerful mechanism to analyze the massive data generated by modern applications; the main aim of it is to categorize the data into clusters where objects are grouped into the particular category. However, there are various challenges while clustering the big data recently. Deep Learning has been powerful paradigm for big data analysis, this requires huge number of samples for training the model, which is time consuming and expensive. This can be avoided though fuzzy approach. In this research work, we design and develop an Improvised Fuzzy C-Means (IFCM)which comprises the encoder decoder Convolutional Neural Network (CNN) model and Fuzzy C-means (FCM) technique to enhance the clustering mechanism. Encoder decoder based CNN is used for learning feature and faster computation. In general, FCM, we introduce a function which measure the distance between the cluster center and instance which helps in achieving the better clustering and later we introduce Optimized Encoder Decoder (OED) CNN model for improvising the performance and for faster computation. Further in order to evaluate the proposed mechanism, three distinctive data types namely Modified National Institute of Standards and Technology (MNIST), fashion MNIST and United States Postal Service (USPS) are used, also evaluation is carried out by considering the performance metric like Accuracy, Adjusted Rand Index (ARI) and Normalized Mutual Information (NMI). Moreover, comparative analysis is carried out on each dataset and comparative analysis shows that IFCM outperforms the existing model.

Hajnal, the Government Publications Specialist at the University of Toronto and the author of many reference books and articles relating to the publications of international organizations, has compiled a list of 110 publications he believes to be currently the best, and most useful sources of information produced by international organizations in general and the United Nations and its specialized agencies in particular, as well as books written about such organizations and their publications.This list was originally prepared for the Conference on the United Nations: Law and Legal Research sponsored by and conducted at the Institute for Comparative and International Legal Research, Center for International Legal Studies, St. Mary's University School of Law, San Antonio, Texas. At this important conference, which took place on February 20–22, 1991, a number of specialists discussed different aspects of the United Nations and described the publications, as well as other information activities, of the United Nations and its specialized agencies. Mr. Peter I. Hajnal spoke about United Nations publications. In conjunction with his lecture he distributed to the participants of the conference the excellent list reproduced below. This study examinesthe perceptions of participants in UN police monitors courses about the effectiveness of TED (technology, entertainment and design)talksin developing their listening. The researchersused a qualitative research design on25 purposefully-selected participants in the police monitor course held at the Jordanian police peacekeeping institute. The TED talks-based program aimed at offering the participants opportunities for practicing listening and raising their awareness of the benefits of using TED talks for academic purposes. The training lasted for four weeks during the first police monitor course of 2018. The findings show that TED talks were reported to positively affect the participants' perceptions of the effectiveness of these talks in improving their listening. The participants were reportedly highly satisfied with the content, method and time of training as well as their interaction, motivation and benefit from it. The study concludes with several recommendations and implications for EFL teachers, educational policy makers, and textbook designers.

How should we teach the users of official statistics? Many government advisors lack adequate mathematical and statistical skills. The authors were part of different teams that developed a National Certificate of Official Statistics comprising four modules taught face-to-face with competency-based assessment that included a workplace project, and three free downloadable web apps in official statistics hosted on the International Statistical Literacy Project website. They also supplied draft content for a United Nations Institute of Training and Research (UNITAR) e-learning course Understanding data and statistics better – for more effective SDG decision making comprised of five self-teaching modules. Key common elements in this progression from a country specific paper-based course to an international e-learning course targeted at the United Nations’ Sustainable Development Goals, included: • establishing cooperative teams, including information technology and course design experts • de-mathematising statistical content, in favour of conceptual understanding • using visual tools, such as dynamic and interactive graphics, to interpret data • recreating as little material as possible with use of freely available internet tools. There were challenges to be overcome: how to make the content internationally applicable to an international audience; how to keep material up-to-date and relevant and how to provide appropriate formative and/or summative assessment.

Solving the world’s great crises and taking advantage of the world’s greatest opportunities requires innovation and capacity. E-governance uses Information & Communication Technologies (ICT) to broaden participation and make problem solving more effective. Environmental issues represent some of the world’s greatest crises and most significant opportunities. The United Nations Environment Programme (UNEP) is a global leader in tackling such issues. UNEP’s Global Environmental Outlook (GEO) relies on contributions from many experts from all regions of the world. Because capacity development is critical to UNEP’s ability to fulfill its mandate, UNEP collaborated with the International Institute for Sustainable Development (IISD) to commission more than 50 experts to develop training resources for integrated environmental assessment and reporting at the sub-global level. These resources were adapted to an eLearning format, significantly broadening their utility and reach. All materials are organized onto the Moodle Learning Management System and use a facilitated interaction model. The eLearning curriculum and approach has been peer reviewed and pilot tested. This research has further developed a blended learning, Train-the-Multipliers program to train facilitators. This eLearning implementation has clearly demonstrated potential and is advancing e-governance at global, regional, national and sub-national levels in the area of environmental assessment and reporting. Although UNEP’s position as a strong proponent of global environmental governance is unique, the detailed approach described for the eLearning programme is generic and therefore, would be a useful model for others who wish to develop eLearning curricula.

“The problem with a tent is that when you use it you throw it away, so it’s money that melts.”—Alejandro Aravena The social project of architecture has long been fascinated with emergency and refugee housing as a primary unit of architectural and urban development. For decades, architects have proposed alternatives to the United Nations’ blue tent cities that are the principal image associated with humanitarian aid and its resulting urbanism. During the 2016 Venice Biennale Reporting from the Front, curator Alejandro Aravena challenges architects to reconsider the discipline’s relationship to society’s most urgent challenges. The ongoing European refugee crisis is one such ‘Front’and this research examines the viability of an alternative to the polyvinylchloride (PVC) tarp as the default condition of emergency and refugee housing. The authors propose that waterproof paper surfaces and members, treated with a proprietary nano-coating can perform as well as traditional materials, but with reduced environmental impact and improved user comfort. A collaboration between researchers in Material Science and Architecture combines ongoing scientific research with digital design tools and methods. Following is a brief history of building with paper, an introduction to hydrophobic nano-coatings, and several fabricated prototypes. This project expands upon initial applications from the Tokyo Institute of Technology (2012) where researchers successfully prepared paper surfaces with a nano-particle coating, repelling water and maintaining structural integrity.
