The United Network for Organ Sharing (UNOS) recently changed the policy by which donor livers are allocated to liver failure patients in the United States. Formerly, all liver failure patients were characterized as status 1 and placed at the top of the transplant list. Under the new policy, only patients with liver failure due to acute illness (“ALF patients”) are eligible for status 1; patients with liver failure due to chronic liver disease (“CLF patients”) are characterized as status 2. Since donor organs are allocated first to status 1 patients and then to status 2 patients, the new policy moves all CLF patients down on the waiting list relative to all ALF patients. This means that some livers that would have gone to CLF patients under the old policy will now go to ALF patients. Accordingly, while the new policy will likely increase the number of ALF patients saved, it will also increase the number of deaths among CLF patients waiting for a transplant.

Debates continue to surround the system in the United States for allocating transplantable cadaveric organs, due in large part to the scarcity of such organs in relation to the number of individuals waiting to undergo transplantation. Candidates awaiting transplantation gain access to cadaveric organs by being placed by individual transplant programs on the national list of the Organ Procurement and Transplantation Network (OPTN), overseen by the United Network for Organ Sharing (UNOS). In recent years, the UNOS board has visited (and revisited) the issue of multiple listing, that is, allowing one candidate to be registered on the waiting lists of two or more transplant programs, and has continued to permit multiple listing.

BackgroundCOPD patients account for a large proportion of lung transplants; lung transplantation survival benefit for COPD patients is not well established.MethodsWe identified 4521 COPD patients in the United Network for Organ Sharing (UNOS) dataset transplanted from May 2005 to August 2016, and 604 patients assigned to receive pulmonary rehabilitation and medical management in the National Emphysema Treatment Trial (NETT). After trimming the populations for NETT eligibility criteria and data completeness, 1337 UNOS and 596 NETT patients remained. Kaplan–Meier estimates of transplant-free survival from transplantation for UNOS, and NETT randomisation, were compared between propensity score-matched UNOS (n=401) and NETT (n=262) patients.ResultsIn propensity-matched analyses, transplanted patients had better survival compared to medically managed patients in NETT (p=0.003). Stratifying on 6 min walk distance (6 MWD) and FEV1, UNOS patients with 6 MWD <1000 ft (∼300 m) or FEV1 <20% of predicted had better survival than NETT counterparts (median survival 5.0 years UNOS versus 3.4 years NETT; log-rank p<0.0001), while UNOS patients with 6 MWD ≥1000 ft (∼300 m) and FEV1 ≥20% had similar survival to NETT counterparts (median survival, 5.4 years UNOS versus 4.9 years NETT; log-rank p=0.73), interaction p=0.01.ConclusionsOverall survival is better for matched lung transplant patients compared with medical management alone. Patients who derive maximum benefit are those with 6 MWD <1000 ft (∼300 m) or FEV1 <20% of predicted, compared with pulmonary rehabilitation and medical management.

Abstract Background Improving outcomes of transplant recipients within and across transplant centers is important with the increasing number of organ transplantations being performed. The current practice is to analyze the outcomes based on patient level data submitted to the United Network for Organ Sharing (UNOS). Augmenting the UNOS data with other sources such as the electronic health record will enrich the outcomes analysis, for which a common data model (CDM) can be a helpful tool for transforming heterogeneous source data into a uniform format. Objectives In this study, we evaluated the feasibility of representing concepts from the UNOS transplant registry forms with the Observational Medical Outcomes Partnership (OMOP) CDM vocabulary to understand the content coverage of OMOP vocabulary on transplant-specific concepts. Methods Two annotators manually mapped a total of 3,571 unique concepts extracted from the UNOS registry forms to concepts in the OMOP vocabulary. Concept mappings were evaluated by (1) examining the agreement among the initial two annotators and (2) investigating the number of UNOS concepts not mapped to a concept in the OMOP vocabulary and then classifying them. A subset of mappings was validated by clinicians. Results There was a substantial agreement between annotators with a kappa score of 0.71. We found that 55.5% of UNOS concepts could not be represented with OMOP standard concepts. The majority of unmapped UNOS concepts were categorized into transplant, measurement, condition, and procedure concepts. Conclusion We identified categories of unmapped concepts and found that some transplant-specific concepts do not exist in the OMOP vocabulary. We suggest that adding these missing concepts to OMOP would facilitate further research in the transplant domain.

In 2019, the United Network for Sharing (UNOS), which has been operating the Organ Procurement and Transplantation Network (OPTN) in the United States since 1984, was seeking to design a new national lung transplant allocation policy. The goal was to develop a point system that would prioritize candidates on the waiting list in a way that would yield more efficient and equitable outcomes. Our joint Massachusetts Institute of Technology (MIT)/UNOS team joined forces with the OPTN Lung Transplantation Committee in these policy design efforts. We discuss how our team applied a novel analytical framework, which was developed at MIT and utilizes optimization, regression, and simulation techniques, to illuminate salient trade-offs among outcomes and guide the choice of how to weigh different point attributes in the allocation formula. The committee selected for the allocation formula weights that were highlighted in the team’s analysis. The team’s proposal was implemented as the national lung allocation policy on March 9, 2023 across the United States. Cette these se veut de proposer une alternative aux differentes explications psychologiques qui existent entourant les multicides seriels aux Etats-Unis. Pour ce faire, nous avons opte pour une demarche empirique ou la cueillette de donnees est effectuee par une revue de la litterature. Nous avons opte pour l'approche du conflit-social (marxiste-feministe) qui tient compte des inegalites existant entre les sexes, les races et les classes sociales au sein de la societe americaine et de la domination qui les sous-tendent. Sous cette optique, cela permet de recreer une vue d'ensemble du phenomene plutot que de ne s'attarder que sur l'une de ses composantes. Pour les fins de cette recherche nous avons inclue sept categories: soit femme; enfant; homosexuel; "race"; economique; famille et personne a charge. Elles correspondent a la typologie des victimes orchestree dans une perspective du conflit social. Nous terminons par l'elaboration d'hypotheses qui stipulent que les multicides seriels seraient une des formes d'abus de pouvoir que les hommes blancs et heterosexuels pratiqueraient afin de raffermir leur domination systemique.

Le sujet des inégalités de revenu oppose les économistes néoclassiques aux géographes. Les premiers soutiennent l’hypothèse de la convergence des revenus d’une région à l’autre au fil du temps. Les seconds constatent de façon récurrente la divergence régionale de ces mêmes revenus. La construction de données de panel permet de faire le point sur le cas des États-unis de 1913 à 2003. Toutefois, ces données ne peuvent être homogènes sur le long terme que pour les hauts revenus (les dix pourcents les plus riches, soit le dernier décile). L’on doit faire appel à d’autres sources statistiques pour rendre compte des revenus moyens par état. L’analyse porte sur trois types de convergence : la convergence ? d’un état à l’autre, la convergence ? d’un état à l’autre, et la convergence des revenus, au sein même de la distribution d’un seul état. La convergence ? semble se produire plus à l’intérieur du décile le plus riche qu’entre les revenus moyens, et l’inégalité de revenu d’un état par rapport à la moyenne fédérale est corrélée négativement aux taux de croissance et positivement au revenu moyen de l’état. La convergence ? se caractérise par le déclin de la dispersion des revenus moyens et du pourcentile le plus élevé, sauf en fin de période. Applied to the United States, the dissertation presents the construction of a new homogeneous set of panel data by state cross-sections and annually from 1913 to 2003, using the Statistics of Income publications by the U. S. Internal Revenue Service. This database represents well the top 10 percent of the income distribution, but data from other sources are needed to account for average income. Meanwhile, the top decile database offers an alternative to average income figures used by Barro and Sala-i-Martin to study the same topic. In order to address the issue of income convergence across the United States over the long-run, three types of convergence are distinguished: 1) the ? convergence, 2) the ? convergence, and 3) the convergence of top incomes towards the lower decile of the income distribution. As regards the ? convergence, evidence supporting convergence is stronger within the top decile than among average incomes across states. Moreover, income inequality is positively correlated to average income, and negatively correlated to economic growth rates. The ? convergence features a declining trend for state average incomes and the income of the top percentile. Finally, the estimation of the full income distribution provides a measure of the inequality gap between the lower and upper deciles of the income distribution from 1965 to 2003, based on the hypothesis that the functional form of the Lorenz curve fits the Pareto distribution

L'intention est de protéger les intérêts des citoyens émiriens contre les non-citoyens tout en imposant le droit des décideurs dans un cadre juridique arabo-musulman. Le contexte (acteurs et espèces) détermine l'adaptation du droit. Les E.A. U. ont signé des traités internationaux qui les obligent à mettre le droit local en conformité. Or, les réserves stipulées et le manque de clarté retardent ce processus. Localement, l'application du droit dépend de la variété des sources, notamment du droit islamique et de ses différentes écoles, d'une procédure législative opaque et d'un système judiciaire complexe (trois types de tribunaux, statut personnel discriminant fondé sur la notion de citoyenneté). Les limites fluctuantes entre les sphères publiques et privées font enfin planer une incertitude dans des domaines relevant de l'intime ou de la diffamation par exemple. Les autorités émiriennes exercent un contrôle sur le droit en s'appuyant sur une organisation tribale et un système de permis à tous les étages de la société émirienne. On the local level, a diversity of legal sources with Islamic law as primary source of law with its four interpretations in Sunni Islam, a non-transparent legislative and court system, three parallel court systems and persona! status law distinguishing citizens from non-citizens make it challenging to predict what the law may be. Furthermore, the notions of private and public in law are in constant flux in the jurisdiction indicating how what appears to be private might be public in intimate relations and how to be private in public domains through defamation legislation. Finally, this work shows how the authorities have established a system of control of rights based on traditional tribal organizations through a license system in law of rights in several spheres of society. This system of license creates hierarchy of rights, control of exercise of rights and at times outsources monitoring of other unacceptable behaviour. The sphere of expressions is examined in detail in this work as the regional upheavals in 2011 resulted in more legislation beyond licensing to protect the stability of the rulers. However, what the law is on the ground is also affected by local interpretations of rights and this will be illustrated through the right to education. Additionally, the license system in employment of non-citizens is based on a fusion between the interests of the authorities and citizens creating a system where citizens may willingly monitor non-citizens on behalf of the state. Against the backdrop of growing economic success in the UAE, outsiders were questioning the price of the boom resulting in attention from human rights activists and media. But, the government quickly learnt how to use its license system to protect itself by establishing a front organization to retaliate on its behalf. Other organizations with links to the authorities would appear. On the back of growing influence of the state, the authorities changed its way of responding to the view of outsiders on its law and rights. Foreign aid provided the state with status and influence as a humanitarian state making it harder to question the fluidity of law in the jurisdiction. While the unpredictable nature of the law in the UAE might seem chaotic from the outside and on the ground, I argue that the fluidity of law legitimises the state and makes it resilient

The use of medical devices and drugs is constantly increasing in the United States. New techniques are developed, pharmaceutical companies manufactured thousands drugs and medical devices each year, these products are put on the market immediately; therefore, the consequences can be terrible.In the US, thousands even millions of people suffer from personal injuries because they use a defective medical product; this is referred as a mass tort. The class action procedure is often used in order to repair these personal injuries. Plaintiffs always ask for the use of this procedure, but the federal American courts always refuse the certification of the class because the conditions of Rule 23(b)(3) are not fulfilled. This is what is called "the new trend of American courts".
The non application of the class action procedure leads to terrible results; victims of a defective medical product cannot receive compensation for the injury they are suffering from. These victims choose the class action procedure because of its advantages, if the class action is not certified they won't sue individually. If the real problem was the non respect of Rule 23(b)(3)'s conditions, a modification or a reform of the Rule would have been done. Unfortunately the laxity of the federal judges tends to influence the American Congress who ignores the need for a reform of this rule. This makes us wonder what the real justifications motivating this refusal of certification are.

On the back of growing influence of the state, the authorities changed its way of responding to the view of outsiders on its law and rights. Foreign aid provided the state with status and influence as a humanitarian state making it harder to question the fluidity of law in the jurisdiction. While the unpredictable nature of the law in the UAE might seem chaotic from the outside and on the ground, I argue that the fluidity of law legitimises the state and makes it resilient

For example, at least six of the United Nation’s Sustainable Development Goals, namely zero hunger, clean water and sanitation, affordable and clean energy, industry, innovation and infrastructure, sustainable cities and communities and responsible consumption and production, overlap directly with most ‘definitions’ of sustainability and will require a range of new technologies to implement. Technological innovation geared towards environmental or sustainable factors (which could also loosely be referred to as ‘green’ technology) will therefore be pivotal in achieving the UN’s goals.

AbstractIn the 1950s, the UN began to use the concept of social development, in contrast to economic development. Social development as adapted to Japan brought a dramatic change in Japanese policy debates on human resources and social security. In this chapter, I discuss the role of the idea of social development in promoting the policy debate towards introducing child allowance in Japan. Social development as a Japanese concept was combined with an interest to improve “the quality of population”, in particular through the state’s role in family planning, while the UN’s concept encompassed a more general humanism or psychosocial well-being.

Organ allocation for transplantation across the United States is administered by the United Network for Organ Sharing (UNOS). UNOS recently approved a major policy change of the system used to allocate hearts for adult transplant candidates. The main objective of this study is to investigate the impact of the new policy on geographic disparity measured by four performance indicators (waiting time before a transplant, transplant rate, pre-transplant mortality rate, and average distance traveled by donated hearts). The current policy and the new policy were evaluated using the thoracic simulation allocation model. The results show that the new policy improves the median waiting time, transplant rate, and pre-transplant mortality rate. The overall predicted improvement in geographic equity is modest except in terms of waiting time. The findings highlight the need for a targeted approach for donor heart allocation to achieve equal access to heart transplantation in the US.

The United Nations (UN) is the foremost international body helping uphold world peace through peacekeeping missions, ranging from deployments that enforce peace treaties, monitor conflicts, and protect civilians; However, determining when and how to intervene is complex. The updated UN General Debate Corpus (UNGDC), cataloging every speech from the UN's inception in 1946 to 2022, is a treasure trove of national policy, as the UNGD is the only body where every country can speak. We propose a discourse-driven intervention recommendation framework that categorizes ongoing conflicts based on UN precedent and recommends the magnitude of funds and forces that should be committed to addressing a conflict. We employ natural language processing techniques to tokenize, preprocess, and analyze word stem frequencies in the UNGDC, generating a timeseries of the number of UN mentions for any given country. Paired with historical analysis, we show that debate in the UNGDC is a potent indicator to determine UN intervention and response mechanisms for conflicts in Africa; further, by aggregating mention statistics across periods of active conflict, we provide quantitative backing for the correlation of mention dynamics and the presence of an active conflict, for a given country. Finally, we present and test an interpretable, shallow decision tree model that can perform intervention type classification and response magnitude recommendation with 91.7% accuracy. Our results, established by computational experiments and statistical testing, suggest that corpus analysis and broader computational diplomacy methods can drive intervention recommendations to improve the UN’s decision-making.

The large-scale COVID-19 pandemic is a severe public health emergency which poses distressing social and economic challenges to the international community as a whole. In order to provide immediate and effective support to affected welfare and healthcare systems as well as to build their lasting, inclusive and sustainable recovery, both the European Union and the United Nations have introduced a number of urgent measures aiming to help and protect citizens and economies. This paper looks into the specificities of urgent procedures launched and carried out by the two most influential international organisations with a view to rapidly respond to the unprecedented COVID-19 crisis. More specifically, it focuses on the involved institutions and steps of urgent procedures as well as on their most remarkable outcomes. In the case of the European Union, the emphasis is put primarily on two Coronavirus Response Investment Initiatives (CRIIs), adopted during the Croatian Presidency of the Council in one of the fastest legal procedures in the history of the European Union, and the Recovery Assistance for Cohesion and the Territories of Europe (REACT-EU) as an extension of the CRIIs’ crisis repair measures. The overarching United Nations’ response is assessed through an analysis of its urgent policy agenda developed on the premise that the COVID-19 pandemic is not only a health and socio-economic emergency but also a global humanitarian, security and human rights crisis. This particularly includes procedures foreseen by the Global Humanitarian Response Plan (GHRP) and the Strategic Preparedness and Response Plan (SPRP). In addition, the aim of the paper is to provide a critical overview of the subject by highlighting three pivotal elements. First, the paper sheds light on the financial aspects of the urgent fight against the COVID-19 pandemic, necessary for turning words into action. Notably, this refers to funds secured by the Multiannual Financial Frameworks 2014-2020 and 2021-2027, and the Next Generation EU recovery instrument, on the one hand, and the UN COVID-19 Response and Recovery Fund, the UN Central Emergency Response Fund and the Solidarity Response Fund, on the other hand. Second, it offers a comparative evaluation of the end results of the European and global emergency procedures in mitigating the impacts of the COVID-19 pandemic. Finally, it summarises the underlying elements of measures governing the aftermath of the ongoing crisis, i.e. those promoting a human-centred, green, sustainable, inclusive and digital approach to future life.

XX, la tecnología digital toma la ventaja en casi todas las fases de la producción de cine de animación, haciendo posible entre otras cosas, la unión de cine de acción real con animación, dando lugar a series animadas de estética híbrida.El presente artículo centrará su atención en uno de estos ejemplos híbridos, El asombroso mundo de Gumball (2011), serie de animación televisiva creada por Ben Bocquelet (1983), tras proponer a Cartoon Network reunir en una única producción a todos los personajes que él había creado y que fueron rechazados por estudios de animación a lo largo de los años.Partiendo de esta unión de elementos diferentes (explicación presente en la idea base, pero no en el argumento de la serie), se analizarán los recursos que se utilizan para unir a personajes de estética dispar en un "universo gráfico" coherente. Este concepto de "universo" aparece definido por Raúl García en Actores del Lápiz (2000), como la unidad de estilo (y movimiento) en la que coexisten los personajes y elementos de diseño dentro de una serie de televisión, regidos por unas reglas que hay que mantener durante toda la creación, para hacerla verosímil ante el espectador. United Nations peace operations are generally considered a poor vehicle for counter-terrorism. Yet, UN peace operations are regularly deployed by the Security Council to settings affected by terrorism and violent extremism. The confluence of terrorism and conflict make this co-existence inevitable, but some missions have also engaged more directly in countering these elements. For example, the UNSC continues to support the mission in Somalia, which actively faces terrorist threats, and has also deployed a peacekeeping mission to Mali amid a jihadist insurgency. At the same time, in places like the Democratic Republic of Congo, peace operations have engaged in offensive military operations that target groups labelled as “terrorists” by their host government partners. Academics and peacekeeping experts have raised concerns and noted the initial impacts of counter-terrorism efforts on UN peace operations. Building on this work, this brief draws on illustrative examples from the field to examine how the UNSC’s counter-terrorism framework has impacted the mandates and practice of the UN’s peace operations, particularly the large stabilization operations deployed in Africa. It shows that counter-terrorism efforts at the level of the UNSC have blurred the normative distinctions between peace operations and counter-terrorism to the detriment of the former. This brief concludes by providing recommendations to ensure that UNSC responses to terrorism and violent extremism do not unintentionally undermine the effectiveness of UN peace operations.

Uno de los mayores retos que enfrentan los sistemas de salud en el mundo es el de proveer servicios y tecnologías en salud de manera sostenible y eficiente. En este estudio se presenta evidencia de los efectos de cambiar el esquema de remuneración a las aseguradoras, al pasar de un esquema retrospectivo a uno prospectivo. A partir del análisis de un experimento natural, presentado en Colombia, y generado por el cambio en la forma de remuneración de servicios y tecnologías de salud financiados a través de la Unidad de Pago por Capitación (UPC), utilizamos los registros de la totalidad de medicamentos que se comercializaron en el país, entre el año 2011 y el 2018, para implementar múltiples especificaciones de modelos de diferencias en diferencias, con tratamiento escalonado, para estimar efectos sobre la media y sobre la distribución de precios, cantidades y gasto de los medicamentos. Encontramos evidencia relacionada con que la inclusión de medicamentos, en el esquema de remuneración prospectivo (UPC), desde el retrospectivo (recobros), generó reducciones en los precios de los medicamentos y aumentos en las cantidades comercializadas. Los efectos en el gasto son positivos para los percentiles inferiores a la mediana y negativos en los percentiles superiores a la mediana, con significancia económica, pero no estadística. Se encuentra evidencia relacionada con que la inclusión de medicamentos en la UPC tiene efectos sobre sus sustitutos terapéuticos, ocasionando aumentos en sus precios y cantidades.

In 2020, WSSCC’s India Support Unit (now UNOPS) piloted a new participatory approach called Community Leave No One Behind (CLNOB) to support the Swachh Bharat Mission Grameen (SBM-G) Phase II. The pilot took place in five districts in India (Mirzapur in Uttar Pradesh, Ranchi in Jharkhand, Kamrup in Assam, South 24 Paragnas in West Bengal and Purnea in Bihar). A Prerak (facilitator) was appointed in each district to support this process and work within villages at community level. The Sanitation Learning Hub supported an accompanying learning component of the pilot, facilitating learning sessions between the preraks and the development of a Handbook based on the experience. This learning brief outlines the purpose of CLNOB, the actions generated by the pilot and our reflections of the CLNOB approach. The CLNOB Handbook, a handbook on Community Leave No One Behind, accompanies this Learning Brief. CLNOB was designed to ensure a participatory method to enable sustained access to safely managed sanitation facilities for people who have been ‘left behind’ or left out of the first phase of India’s national sanitation campaign.

Diez años que se expresan en 10 fanzines o números donde cada uno representa una arista desde donde mirar esta primera década. Ubicada en el corazón de Tlatelolco, la UVA ha acompañado en sus procesos de iniciación, formación y creación artística a más de 10,000 personas. A través de nuestros talleres libres y nuestra adorada biblioteca comunitaria “Alaíde Foppa” hemos sido cómplices de niñas, niños, jóvenes, adultos y personas mayores. En un contexto en el cual los hogares y empresas ya estaban pasando dificultades económicas por la crisis del COVID-19, los precios internacionales de los combustibles comenzaron a aumentar con el fin de las medidas de confinamiento y la reactivación de la actividad productiva. Posteriormente, tras comenzar la guerra en Ucrania, dicho aumento se aceleró, superando claramente los niveles prepandemia. En respuesta a ello, los países de Centroamérica, Panamá y República Dominicana (CAPRD), al igual que otros de dentro y fuera de Latinoamérica, empezaron a adoptar medidas para mitigar el impacto sobre los consumidores. Sin pretender realizar una rigurosa clasificación conceptual, estas medidas han sido de tres tipos: i) congelamiento de precios, instrumentado por distintas vías; ii) rebaja de precios por una cuantía determinada, directamente o vía reducción de impuestos; y iii) subsidios fijos, ya sea de monto total o por unidad de consumo. Uno de los objetivos principales de este trabajo es presentar estimaciones del costo fiscal directo en 2022 de las medidas tomadas por los países de CAPRD. Con la información disponible a la hora de escribir este documento, el costo promedio en CAPRD fue del 0,5% del PIB hasta agosto incluido. Atendiendo a los diversos combustibles, en promedio de CAPRD la mayor cuota del costo fiscal corresponde al diésel, seguido del GLP, la gasolina superior y, finalmente, la gasolina regular. Respecto a la distribución por tipo de medidas, el congelamiento de precios ha sido la que ha tenido mayor costo fiscal total, seguida de la reducción de impuestos, la rebaja directa de precios y, finalmente, los subsidios. En cuanto al tipo de costo, en promedio, las medidas con costo variable han tenido una mayor participación cuantitativa que las de costo fijo. No obstante, todos estos resultados muestran una alta variabilidad entre países. Finalmente, hemos realizado un análisis distributivo estimado de las medidas adoptadas para tres países: Costa Rica, Panamá y El Salvador. Empleando un indicador estándar de progresividad, dichas medidas han sido progresivas en los dos primeros, aunque las causas son distintas, y regresivas en El Salvador.

Costa Rica, tiene una población de 4.7 millones de habitantes distribuido en 51,100 Km2. Un sistema de salud público que cubre a 86% de la población y brinda servicios de salud mediante 103 áreas de salud de atención primaria y 29 hospitales. El presente estudio tiene como objetivo calcular el número de hospitalizaciones por condiciones sensibles a atención primaria asociadas a cada uno de los grupos diagnósticos establecidos en la lista de Alfradique et al (2009) y la proporción de estas en relación al total de hospitalizaciones en el año. Los datos se obtienen del Seguro Público de Salud costarricense (SPS). Se estudian 13 categorías diagnósticas agrupadas en enfermedades prevenibles por inmunización, agudas y crónicas. La unidad geográfica de análisis son las 103 áreas de salud y el periodo de estudio es 1997-2008. Una de cada diez hospitalizaciones es evitable. El 68.7% de estas hospitalizaciones se concentran en seis categorías diagnósticas: enfermedad de vías aéreas inferiores, infección de la piel y tejido subcutáneo, infección en el riñón y vías urinarias, asma, diabetes mellitus e infecciones gastrointestinales y complicaciones. Las tasas de hospitalización por ambulatory care sensitive conditions (ACSC) presentan un nivel de variabilidad geográfica sistemática relativamente alta y creciente en el período 2000-2010. La magnitud de las diferencia entre áreas, medida por el coeficiente de variación de las razones de hospitalización suavizadas, se incrementan en un 24% en hombres y 14% en mujeres. Los resultados del modelo bayesiano espacio-temporal permiten determinar la presencia de patrones persistentes de sobrehospitalización en la zona Sur, parte del Pacífico Central y parte de la provincia de Alajuela, tanto en hombres como en mujeres. Además, se registran patrones persistentes de infrahospitalización en la zona central del país y parte de la provincia de Guanacaste. En la Zona Sur del país se presentan una concentración de las áreas de salud con tendencia creciente y que están ubicadas dentro de zonas de alto riesgo, las cuales podrían estar asociadas a factores de riesgo constantes en dichas zonas. Estas zonas deberían tener un interés desde el punto de vista de la vigilancia epidemiológica y de planificación de servicios de salud.

Desde el Instituto para la Integración de América Latina y el Caribe (INTAL) del Sector de Integración y Comercio (INT) del Banco Interamericano de Desarrollo (BID) se realizó un análisis de la opinión que los ciudadanos de América Latina tienen sobre la integración regional, así como sobre otros fenómenos vinculados al comercio como la inversión extranjera directa, la digitalización y la inmigración. Los datos utilizados para el estudio corresponden principalmente a los resultados del año 2020 de la encuesta Latinobarómetro, que constituye uno de los principales bancos de datos de opinión pública en América Latina. Se trata de una encuesta de carácter presencial y anual en 18 países de la región, con muestras representativas de cada país, que aplica un cuestionario común con una unidad metodológica y técnica. La encuesta permite captar las diferentes opiniones, actitudes, comportamientos y valores de los latinoamericanos encuestados. Los resultados abarcan una muestra representativa de 600 millones de habitantes y los temas principales de su abordaje y seguimiento son la democracia, el estado de la economía, el desarrollo social de los países y los esfuerzos de integración regional, entre muchas otras temáticas.

Estamos ante una encrucijada global de proporciones nunca vistas. El planeta se calienta más rápido de lo esperado y enfrentamos condiciones climáticas cada vez más extremas, lo que pone en riesgo la sostenibilidad de la vida humana en el mediano y largo plazo. El nivel de los océanos está aumentando, los arrecifes de coral están muriendo, las especies se están extinguiendo, los glaciares se están derritiendo y las condiciones climáticas extremas se hacen cada vez más frecuentes con intensas olas de calor, inundaciones, huracanes, incendios y/o sequías. Para hacer frente a esta compleja situación, todos, personas e instituciones en los ámbitos nacional, regional y global, tenemos que hacer nuestra parte para evitar llegar a un punto de no retorno. Aunque Colombia solo genera el 0,6 % de los gases de efecto invernadero (GEI), es uno de los veinte países más amenazados por la crisis climática. Nuestro país alberga el 50 % de los páramos del mundo y es catalogado como una potencia en agua, biodiversidad y ambiente, aloja alrededor del 10 % de la fauna y flora del mundo, teniendo por ello un rol central en los esfuerzos globales que se realizan para mitigar los efectos del cambio climático. También es reconocido por ser el segundo país con más alta presencia en biodiversidad en la tierra, goza de dos océanos, cinco vertientes hidrográficas, ríos, lagunas y ciénagas, siendo de esta manera el agua un recurso fundamental para el desarrollo de las generaciones futuras. El país cuenta con grandes extensiones de páramos, los cuales son la fuente del 70 % de agua dulce en nuestro país y comparte al sur de nuestras fronteras con la Amazonía, considerada la selva tropical más extensa del planeta y el pulmón del mundo. Una porción representativa de ese 10 % del total de la biodiversidad mundial que tiene nuestro país, se encuentra dentro del Sistema Nacional de Áreas Protegidas (SINAP), del que hace parte el Sistema de Parques Nacionales Naturales y las Reservas Forestales (más de 17 millones de hectáreas), una fuente importante de bienes y servicios ecosistémicos, entre los cuales, el suministro del recurso hídrico incluye más del 62 % de los nacimientos de los acuíferos nacionales y abastece a casi un 80 % de la población colombiana. Así mismo, protege lagunas y ciénagas que contienen el 20 % de los recursos hídricos que abastecen la generación de energía eléctrica del país. En estas áreas protegidas se conservan muestras representativas de los ecosistemas naturales marinos y continentales, los cuales, además de albergar un gran número de especies de fauna y flora, contribuyen a la regulación del clima y protegen las cuencas hidrográficas. Es así como por su especial importancia ecológica, los Parques Nacionales Naturales tienen la función de conservar, proteger y salvaguardar sus ecosistemas de especial valor por medio del Estado y de los particulares. Esta gran riqueza ambiental del país está siendo amenazada por el uso indebido de los recursos naturales, lo que genera un acelerado deterioro de los ecosistemas, alterando su funcionalidad y poniendo en riesgo la biodiversidad. Esta problemática se ha convertido en un reto de primer orden para el Estado colombiano, sus autoridades ambientales y la sociedad en general. El Sector Defensa no ha sido ajeno a esta realidad, por lo que en el marco definido por la ley, la Presidencia de la República y el Ministerio de Ambiente y Desarrollo Sostenible, ha reconocido el potencial desestabilizador de las afectaciones causadas por las diversas actividades ilícitas de los Grupos Armados Organizados (GAO), los Grupos Delincuenciales Organizados (GDO) y ciudadanos que hacen un uso indebido de los recursos naturales. Fenómenos como la siembra de cultivos ilícitos, la extracción ilícita de minerales, la tala indiscriminada, la ganadería extensiva, el acaparamiento de tierras, la pesca ilegal, la contaminación, el vertimiento de sustancias peligrosas y el tráfico ilegal de flora y fauna, han debilitado los ecosistemas nacionales. Estas actividades ilegales afectan los recursos hídricos del país, los suelos, los páramos y la atmósfera, produciendo efectos negativos que generan deforestación, pérdida de hábitats, extinción de especies, la destrucción de fuentes de agua, el deterioro de las tierras de cultivos e impactos negativos en las reservas forestales y las áreas que integran el Sistema Nacional de Áreas Protegidas (SINAP). La degradación ambiental que hemos visto no solo es utilizada por las organizaciones criminales para obtener recursos que les permiten sostener su accionar delictivo en diversas zonas del país, más grave aún, se constituye en un atentado directo contra el porvenir de las generaciones futuras. Es por ello que el Sector Defensa ha reconocido el agua, la biodiversidad y el medio ambiente como activos estratégicos de interés nacional, con un carácter principal y prevalente, buscando apoyar con sus capacidades a las autoridades ambientales del país, con el fin de realizar acciones cada vez más eficaces contra las organizaciones criminales responsables del deterioro ambiental. Es en ese contexto surge la idea de este libro, que busca presentar la problemática que se enfrenta y visibilizar las acciones realizadas por el Sector Defensa en coordinación con las autoridades ambientales, con el fin de apoyar la defensa de estos activos estratégicos, así como prevenir los daños futuros a los mismos, en aras de trabajar en la protección de los recursos naturales, bajo una visión multidimensional de la seguridad. El Ministerio de Defensa (MDN) ha sido consciente de los desafíos climáticos a los que se enfrenta, no solo el Sector, sino el territorio nacional, en cuanto a los impactos cada vez mayores derivados de las condiciones meteorológicas extremas y la explotación de los recursos. Por ello, el Sector ha trabajado para alinearse con la Política Nacional de Cambio Climático, los compromisos del país ante la Contribución Nacionalmente Determinada (NDC), el cumplimiento del CONPES 4021 de diciembre de 2020 para el Control de la Deforestación y la Gestión Sostenible de Bosques, para así aportar con acciones de adaptación, mitigación y gestión del riesgo ante el cambio climático y variabilidad climática mediante del Plan de Gestión de Cambio Climático del Sector Defensa (PIGCCSD). Este libro reúne los principales resultados del Sector Defensa en materia de protección del medio ambiente y resalta la articulación de las políticas, así como de la operativización de las mismas por parte de las Fuerzas Militares y la Policía Nacional, por la protección de los recursos naturales de la Nación, una responsabilidad que ha quedado claramente plasmada en la “Estrategia Artemisa”, que es un esfuerzo permanente, sostenido, conjunto, coordinado, e interinstitucional que permitirá proteger y defender el agua, la biodiversidad y el medio ambiente como activos estratégicos de la Nación, a partir de la lucha contra la deforestación, sus causas directas o subyacentes y contrarrestar los efectos del cambio climático, como también la lucha contra la explotación ilícita de minerales, contra los cultivos ilícitos que afectan el medio ambiente y el conjunto de actividades de control y vigilancia que apoya nuestra Fuerza Pública para la defensa de los recursos naturales. Trabajando de forma articulada con la Fiscalía, el Ministerio de Medio Ambiente y Desarrollo Sostenible, el Instituto de Hidrología, Meteorología y Estudios Ambientales (Ideam), y la Unidad de Parques Nacionales Naturales, en diversas zonas del país, la Estrategia Artemisa ha permitido brindar apoyo a las autoridades ambientales y administrativas, para preservar y defender el agua, la biodiversidad y el medio ambiente en las áreas de reserva forestal, áreas protegidas y 59 Parques Nacionales Naturales. Mediante la Directiva Permanente 008 del 22 de marzo de 2022, el MDN institucionalizó e impartió lineamientos e instrucciones al Comando General de las Fueras Militares, la Dirección General de la Policía Nacional y la Unidad de Gestión General del MDN, con el propósito de implementar medidas, desplegar operaciones, actividades de apoyo y gestión ambiental, para la implementación de la Estrategia Artemisa. De esta forma y bajo un enfoque sostenido, conjunto, coordinado, interinstitucional y multilateral, se trabajará para incluir dentro de la doctrina militar y policial las acciones relacionadas con la protección del agua, la diversidad y el medio ambiente, al tiempo que se concentrarán los esfuerzos de inteligencia en la identificación del modus operandi de las organizaciones criminales que afectan el medio ambiente, desarrollando operaciones contra los GAO y los GDO que atentan contra el medio ambiente. Desde el 2019 a la fecha se han realizado diecisiete (17) operaciones sobre las áreas de los Parques Nacionales Naturales, especialmente en las regiones de la Amazonía y la Orinoquía, como son los Parques Nacionales Naturales (PNN) Serranía de Chiribiquete, PNN La Paya, PNN Tinigua, PNN Picachos, PNN Sierra de La Macarena, la Zona de Reserva Forestal de la Amazonía y la Reserva Natural Nukak. Se han dispuesto, para esta campaña, 22.300 hombres de la Fuerza Pública, que, desde sus respectivas unidades militares y policiales resguardan las áreas protegidas del territorio nacional. Estas unidades incluyen: 10 batallones de alta montaña (páramos), una brigada contra el narcotráfico, una brigada contra la minería ilegal, unidades de guardacostas, infantería de marina y efectivos de la Policía Nacional. El Sector Defensa es consciente de que a futuro será fundamental continuar fortaleciendo las capacidades de la Fuerza Pública para, en el marco de sus competencias, continuar apoyando a las autoridades ambientales, entes territoriales y a la comunidad en la defensa y preservación del agua, la biodiversidad y el medio ambiente como activos estratégicos de la Nación. Será fundamental profundizar la disrupción del delito de la explotación ilícita de minerales, mediante el desmantelamiento de las economías ilícitas que se lucran de ella y de la afectación de la cadena criminal. En igual medida, será vital apoyar la recuperación de los ecosistemas más afectados por las actividades ilegales, por medio de actividades de restauración de áreas, del trabajo articulado con las comunidades, la construcción y mantenimiento de viveros forestales y el fortalecimiento a las investigaciones científicas lideradas desde la Armada Nacional y la Dirección General Marítima para la protección de los océanos, el recurso hídrico y sus ecosistemas. De esta forma será posible continuar trabajando en la reducción de los riesgos que se ciernen sobre los ecosistemas del país y disminuir los índices de deforestación, y tras la búsqueda de soluciones que contribuyan a la reducción y mitigación de los GEI. Esta estrategia que ha puesto en marcha el Sector Defensa y que es recogido en esta obra, presenta los aportes sectoriales del trabajo interinstitucional que se han venido realizando en los últimos años, siendo un ejemplo de la forma en la que se pueden sumar esfuerzos para contribuir a la superación de la encrucijada global que nos afecta a todos y que debe ser enfrentada con un esfuerzo común.
