Abstract:
Research has shown that access to an asthma specialist improves asthma outcomes. We hypothesized that we could improve access to expert asthma care through a telemedicine link between an asthma specialist and a school-based asthma program. We conducted a prospective cohort study in 3 urban schools to ascertain the feasibility of using an asthma-focused telemedicine solution. Each subject was seen by an asthma expert at 0, 8, and 32 weeks. The assessment and recommendations for care were sent to the primary care physician (PCP) and parents were told to contact their physician for follow-up care. Eighty three subjects participated in the study. Subjects experienced improvement (P<.05) in family social activities and the number of asthma attacks. Ninety four percent of subjects rated the program as good or excellent. This study demonstrates the feasibility and acceptance of a school-based asthma program using a telemedicine link to an asthma specialist.

Most of Nebraska's population resides in the urban centers of eastern Nebraska. Traditionally, how ever, the Cooperative Extension Service has catered to the rural populations of the state. Now, it is imperative that the public and the legislative representatives from these urban areas learn that the research and outreach programs of the University also serve them. A Festival of Color was the vehicle for delivering this information to the urban audience, as well as to Agent Educators and Aides, Natural Resource District personnel, and other professionals, including government officials. Both professionals and the general public responded enthusiastically to the Festival's innovative use of the research site. The educational objective was to answer the public's questions on the environment, including water quality and conservation in the landscape, and reduction of fertilizers and pesticides. Information was presented by Extension Specialists and Assistants, Master Gardeners, Natural Resource- District personnel, Department of Environmental Quality, State Energy Office, and Nebraska Association of Nurserymen.

Abstract:
Objectives The aim of the present study was to provide descriptive planning data for a hospital-based Aboriginal Health Liaison Officer (AHLO) program, specifically quantifying episodes of care and outcomes within 28 days after discharge. Methods A follow-up study of Aboriginal in-patient hospital episodes was undertaken using person-based linked administrative data from four South Metropolitan hospitals in Perth, Western Australia (2006–11). Outcomes included 28-day deaths, emergency department (ED) presentations and in-patient re-admissions. Results There were 8041 eligible index admissions among 5113 individuals, with episode volumes increasing by 31% over the study period. Among patients 25 years and older, the highest ranking comorbidities included injury (47%), drug and alcohol disorders (41%), heart disease (40%), infection (40%), mental illness (31%) and diabetes (31%). Most events (96%) ended in a regular discharge. Within 28 days, 24% of events resulted in ED presentations and 20% resulted in hospital re-admissions. Emergency readmissions (13%) were twice as likely as booked re-admissions (7%). Stratified analyses showed poorer outcomes for older people, and for emergency and tertiary hospital admissions. Conclusions Future planning must address the greater service volumes anticipated. The high prevalence of comorbidities requires intensive case management to address case complexity. These data will inform the refinement of the AHLO program to improve in-patient experiences and outcomes. What is known about the topic? The health gap between Aboriginal and non-Aboriginal Australians is well documented. Aboriginal people have significantly higher hospital utilisation rates, as well as higher rates of complications, comorbidities and discharges against medical advice (DAMA). Aboriginal patients receive most of their specialist services in hospital; however, detailed person-based analyses are limited and planning is often based on crude data. What does this paper add? This is the first analysis of linked data focusing on Aboriginal patient flows and volume and 28-day health system outcomes following hospital admission for all causes in a large metropolitan setting. Because the data were linked, admissions belonging to a single episode of care were combined, ensuring that transfers were not counted as re-admissions. Linkage also allowed follow up across time. The results highlight the main disease groups for which Aboriginal patients are admitted, how this varies by age and the high proportion of patients returning to (any) hospital within 28 days, either through EDs or as booked (pre-arranged) admissions. These data aid in the planning of hospital-based Aboriginal health liaison services. What are the implications for practitioners? The paper outlines the complexity with which many Aboriginal patients present to hospital and the risk of DAMA and re-admission. Clinical and organisational strategies can be put in place in hospitals to address these risks and ensure improved continuity of care with community-based primary health services. The Western Australian South Metropolitan Health Service is reviewing these data and will monitor the impact of the hospital-based AHLO program.

AbstractThe article identifies and provides a synthetic overview of various concepts relating to the evolution of the real estate market and property valuation. According to the authors, the processes observed on the real estate market necessitate changes in training programs for property valuers. Real estate appraisers should be able to cope with new consumer expectations and requirements, and they should be well versed in modern technological solutions and analytical tools. The study indicates that, in order to face the challenges of the modern world, the appraisal profession should undergo a paradigm shift to embrace the fact that real estate is a commodity and that globalization is inevitable on the real estate market. Due to the high value of urban areas, a modern specialist determining the value of real estate is particularly needed there. Property valuers should develop new analytical skills, and they should rely on modern data processing tools to collect and process information. Additionally, recent events, including the COVID-19 pandemic, demonstrate that property appraisers should be better prepared for dealing with unprecedented circumstances. The training curricula proposed in this article should increase property valuers’ competencies and effectively support real estate market entities and sustainable urban development.

Abstract:A field-based evaluation is conducted of a Clinical Computerized Information System (CCIS™). Following training, the use of the CCIS™ database, word processing and other programs by thirteen full-time practicing emergency physicians in two urban emergency departments of a University-associated teaching hospital was studied over a one-year period. A tracking program automatically logged frequency and duration of use by the physicians, and user satisfaction was assessed by a reliable and validated questionnaire instrument. Based on utilization data and verbal reports of these physicians, CCIS™ database searching was not only found to be easy-to-leam but was readily accessible during emergency shifts. Individual physicians were found to perform an average of 3.5 searches per month lasting a mean search time of 8 min. Positive notes about the CCIS™ system included ease-of-use, accuracy of data, accessibility of system, and value of output while negative perceptions included a lack of integration with other systems, a lack of system completeness, and a high subscription cost. It was suggested that a less costly telephone link to a high-volume Centre would be desirable in actual implementation of the system.

Telemedicine is the remote practice of medicine through the use of information and communication technologies for the prevention, diagnosis, treatment and management of diseases. In this narrative review, we illustrate how telemedicine technologies are increasingly integrated into pediatric infectious disease programs with the aim of facilitating access to specialist care and reducing costs. There is widespread use of telemedicine for the management of acute and chronic infectious diseases, particularly in countries in which the majority of the population lives in rural areas, far from third-level hospital centers located in large urban centers. Obviously, telemedicine is also used in developed countries, and its importance has been further increased recently given the COVID-19 pandemic. It has many advantages for patients, such as saving time, money and working hours, and reducing cancelled appointments and delays, while there are also many advantages for doctors, allowing collaborations with specialists and continuous updating. Among the disadvantages are the limitation in carrying out an objective examination, which is particularly important for children under 2 years of age, and the need for cutting-edge technology and reliable connectivity. Telemedicine increasingly represents the future and the beginning of a new healthcare system that also will redefine medical care for the treatment of infectious diseases, both acute and chronic. However, the majority of the experience has involved adults, and its validation in pediatric care, as well as its application in real-life practices, are urgently needed.

Самара на 2,15 млрд руб. За это время в Самаре обновлено оборудование на насосно-фильтровальных станциях, городских очистных канализационных сооружениях, насосных станциях подкачки и канализационных насосных станциях, переложено 65,3 км водопроводных и 34,5 км канализационных сетей. Information is presented on the development by the specialists of «Samarskie Kommunal’nye Sistemy», LLC, of an investment program for the construction, reconstruction and upgrade of the municipal water supply and wastewater disposal systems in the Samara Urban District for 2019–2023 in accordance with the «Concept of Development and Reconstruction of the Water Supply and Wastewater Disposal Systems in the Samara Urban District for 2018–2047». For the period of the public-private partnership 2013–2019 «Samarskie Kommunal’nye Sistemy», LLC has implemented the program of constructing, reconstructing and upgrading the public water supply and wastewater disposal systems in the Samara Urban District worth a total of 2.15 billion rubles. During this time the equipment at the pumping filtration stations, boosting pumping stations, and at the municipal wastewater treatment facilities in Samara has been renewed; 65.4 km of the water distribution network and 34.5 km of the sewer network have been relayed. The implementation of the investment program will provide for improving the quality and reliability of the cold water supply, reducing technological damages and failures at the water supply and wastewater treatment facilities in the Samara Urban District, and ensuring the technical ability to connect new construction facilities.

The contemporary development in the field of the digital technology for the media has been witnessed an increasing use in the urban scene of the city. This causes disconnection of the visual references of the place, which are replaced by a simulation of the reality by digital images as an alternative to the physical presence. So, the Availability and spread of the new means of visual communication and information technologies create new symbols of contemporary life, that have different levels of acceptances by users. The problem of research is: “The need to detect the user's evaluation of the use of media technology in the urban scene”. The research proposal is: The excessive use of media in urban scene led to visual pollution and the lack of sense of place. For its methodology, the research follows three steps: First, the review of the architectural writings that have agreement or disagreement of the presence of media in the urban scene, to find the scalable items to measure the user’s evaluation of the urban scene. Second, the implementation of the Questionnaire, which is made by choosing of five case studies representing various urban scenes in Baghdad city (where the media is used), to adopt the questionnaire form designed, as a means of measuring user’s evaluation. The research Sample was 40 persons (not architects) and 70 persons (the architects), who have visited the case study sites several times. Third, by using questionnaire form and excel program, the results are Calculated. The research concluded some negative issues, that should be noticed while using media in the urban scene, to avoid visual pollution and provide the sense of place with the help of the specialists. The ease of recognizing and understanding of the user and the limited use of the media are the most important evaluation factors in user preferences.

19598 Background: The Technology Exchange for Cancer Health Network (TECH-Net) is a collaborative, multi-state effort to implement a systematic care program to improve cancer management in the rural communities of west Tennessee, north Mississippi and east Arkansas. This Health Information Technology (HIT) project uses a two-pronged approach to total clinical decision support: (1) provide access to oncology, hematology, and other specialists through the dedicated telehealth network of the University of Tennessee's Health Science Center in conjunction with the University of Tennessee Cancer Institute; and (2) rely on a distributed electronic health record (EHR) with integrated decision support systems for online management of cancer protocols, electronic orders, and medication management. Methods: Patients with a cancer diagnosis requiring treatment for at least one year are eligible to participate. The ultimate goal for this project is to enroll 250 patients (125 rural, 125 urban) and compare quality, safety, and cost outcomes for patients receiving rural Telehealth care versus “routine” urban care. Telehealth patients see their oncologist in person at the rural sites for initial care, and see their physician via telemedicine for approximately one-half of their subsequent visits. This approach allows the clinician to make in-person clinical assessments on a regular basis while still taking advantage of the time savings associated with telemedicine. Results: 163 patients have been enrolled in the study (127 rural, 36 urban) over 18 months. Patient satisfaction has been high - 95 % of patients indicated their telemedicine visit was as good as or better than an in-person office visit. Cost analysis comparing cost savings (physician travel time) with telemedicine costs (equipment, high speed lines) indicates that the cost-benefit depends critically on distance to the rural facility and number of physician trips avoided. Initial cost data indicate that telemedicine must save at least 5 hours of physician travel time per month to break even. Conclusions: Telemedicine offers a feasible method for increasing access to oncology care in rural areas that is well-accepted by both patients and clinicians. No significant financial relationships to disclose.

Abstract:
In recent years, industrial preservation policies, which aim to preserve urban industrial activity and industrial employment often through the preservation of industrial land, have emerged as a flashpoint in cities across the country that have implemented these policies. While critics contend that industrial preservation policies amount to smokestack chasing in “post-industrial” cities like New York City, industrial preservationists argue that such policies help to preserve well-paying, middle-class jobs and thus represent a tool to mitigate rising income inequalities in cities. Despite considerable attention to these policies, minimal research has evaluated the effectiveness of industrial preservation policies as land use and economic development planning tools. This paper inserts itself into the debate surrounding the utility of industrial preservation policies by evaluating the effectiveness of New York City’s 2006 Industrial Business Zone (IBZ) program. Specifically, this paper uses propensity scoring to evaluate various measures of urban industrial activity in designated IBZs compared to a control group of similar areas. This paper finds that IBZs outperformed the control group in terms of better stemming industrial employment losses and industrial land decline. The control group, however, provided a more favorable climate to industrial business starts and performed about the same as IBZs in encouraging capital investments in industrial infrastructure. These findings suggest that the IBZ program yielded mixed results in its efforts to both attract and retain urban industry.

AbstractIn architecture, landscape architecture, and urban design, site planning refers to the organizational process of site layout. A fundamental step for site planning is the design of building layout across the site. This process is hard to automate due to its multi-modal nature: it takes multiple constraints such as street block shape, orientation, program, density, and plantation. The paper proposes a prototypical and extensive framework to generate building footprints as masterplan references for architects, landscape architects, and urban designers by learning from the existing built environment with Artificial Neural Networks. Pix2PixHD Conditional Generative Adversarial Neural Network is used to learn the mapping from a site boundary geometry represented with a pixelized image to that of an image containing building footprint color-coded to various programs. A dataset containing necessary information is collected from open source GIS (Geographic Information System) portals from the city of Boston, wrangled with geospatial analysis libraries in python, trained with the TensorFlow framework. The result is visualized in Rhinoceros and Grasshopper, for generating site plans interactively.

AbstractThe following paper talks about the studio project [Symbios]City, which is developed as a design research project in 2020–2021 Schumacher’ studio on social ecology of the graduate program in Architectural Association’s design research lab. The project aims to create an assemblage of social ecologies through a rich but cohesive multi-authored urban district. The primary ambition is to generate an urban area with a characterful, varied identity, that achieves a balanced order between unity and difference avoiding both the sterile and disorienting monotony of centrally planned modernist cities and the (equally disorienting) visual chaos of an agglomeration of utterly unrelated interventions as we find now frequently. Through a thorough research process, our project evolves mainly out of three principles that are taken into consideration for the development of our project: topological optimization, phenomenology, and ecology. By “ecology”, we understand it as a living network of information exchange. Therefore, every strategy we employ is not merely about reacting to the weather conditions, but instead it is an inquiry into the various ways we can exploit the latter, a translation of the weather conditions into spatial and programmatic properties. [Symbios]City therefore aims at developing a multi-authored urban area with a rich identity that achieves a balance between the various elements. [Symbios]City began formally from topological optimization, developed based on studies on ecology, and concluded the design following our phenomenological explorations, aiming at a complex design project that unifies the perception of all scales of design: from the platform to the skyscrapers.

Outdoor activities have been installed as a massive phenomenon. In them, people declare behaving—and generally comply—in a respectful way with the natural spaces they visit. However, when time is everyday in a city where the life of today's humanity mostly happens, the ecological ethics tends to be relativized under a myriad of justifications. Caring for wild areas while we are in them and then damaging them from our urban life is something that does not make sense, because cause-effect relationships in nature do not recognize political borders, nor between countries, nor less between urban and non-urban areas; they only recognize geographic borders, which are permeable to the circulation of energy, gases, liquids, matter, information, life, and death. This proposal is aimed at reversing this paradox, sustaining ethics and actions of planetary care present in wilderness areas in urban daily life.

The professional inquiry (a researcher training and development course) was introduced into the Master of Education program at Massey University, New Zealand in 2014 as a practitioner-based alternative to the research thesis pathway. In contrast with traditional, independent, time intensive models of postgraduate research supervision, the authors developed and implemented an innovative blended learning model of postgraduate research training and development to ensure the growing demand of future, predominantly distance, students would be met. Alongside the quick rise of the Internet as a pivotal economic and cultural force in Chinese society, the Chinese government has implemented a two-tiered strategy in coming to grips with the great potentials and underlying risks associated with the network era. This chapter offers a critical, in-depth overview of China’s state-orchestrated Internet surveillance apparatus from the Great Firewall to the latest Green Dam project. It first examines the conceptual and historical evolution of the Golden Shield program, followed by an analysis of the legal framework through which official regulation is justified or rationalized. Next, the chapter looks at the prevalent practice of industry self-regulation among both Chinese and foreign companies engaged in online business in China, and it ends with the discussion of the aborted official effort of extending content control to individual computers with the Green Dam Youth Escort project.

To address the grand challenge of the severe shortage of qualified engineering workforce and equally important educators, engineering and education departments at Manhattan College created a holistic program called Engineering Scholars Training and Retention (STAR) program. Engineering STAR program created a collaboration among undergraduate education and engineering majors, the professors who teach them and current STEM teachers and their students in local urban middle and high schools. We developed three new academic programs (engineering education minor and certificate programs for both undergraduate and graduate engineering and education majors) to support engineering and education students who are passionate about promoting engineering for 6–12 grade students and become qualified and competent engineering educators. In addition, through partnership with local middle/high schools we developed an engineering ambassadors’ program where students from engineering and education majors develop hands-on design projects and present them to middle/high school students to encourage and inspire more students to study engineering. Next, we engaged in a professional development program to support current STEM teachers to develop skills in engineering and increase the number of teachers who possess the pedagogical content knowledge to prepare students to be successful in engineering fields. All three aspects of the STAR program employees engineering design projects to introduce engineering to students and teachers. This integrative model could serve initially as a template to design such programs.

Recent terrorist attacks have prompted considerable interest in predicting damage to structures that could result from explosive blasts in densely populated urban environments. This is a particular concern for government and military organizations wishing to improve the safety of facilities and insurance providers who want to quantify risks. Blast waves from explosions are characterized by a shock front propagating into the surrounding air, followed by an exponential decay in pressure. Structural damage can be caused by either the magnitude of the peak pressure or the impulsive loading over time. Thus, any assessment of damage requires accurate computation of the entire pressure history on the structure. Semi-empirical approaches, such as CONWEP, although able to predict free-field and single-reflected pressures accurately, are unable to account for the effect that the urban environment has on amplifying, dissipating or focusing the blast wave. This paper describes a numerical finite difference approach, using the non-linear dynamics program AUTODYN, which allows an accurate prediction of the pressure fields that develop as a blast wave propagates through an urban environment by recursively remapping the solution through numerical regions that expand to track the evolving shock front. Data for specific urban layouts can be imported into AUTODYN from geographic information system (GIS) services.

As conventional cadastral maps only show building perimeters, they contain no information about the city’s internal structure—about the complex interplay of architecture and its socio-economical use. Thus urban planning seems to spare little thought for what really takes place inside the buildings lining a street, although we perfectly know that the potentials of ground floor use and the structure of the correlating public street space are directly related. The Urban Parterre Modelling UPM-method refers to the city’s “parterre” as a holistic urban system: it covers both built-up and non-built-up areas. Thus street, ground floor and courtyard are treated as entity, so that their interrelations can come to light. Technically the method represents the merging of a common 3D-city-model and a Comprehensive Ground Plan Survey CGPS—a researching technique used in the 1960s until the 1990s (mostly in Italy and Switzerland). This new urban research method has been developed and tested in a pilot study by means of an in depth exploration of an exemplary historical street in Vienna. In September 2015 a new four years research program was financed by the Austrian Science Fund (Austria's central funding organization for basic research, FWF) and launched at the Department of Urban Design at Vienna University of Technology. Within this operational framework a variety of different street-level environments in Vienna are being examined. Gasoducto del Pacífico is a pipeline that carries natural gas from southern Argentina to Chile, through the Patagonian Andes. During previous years of operation only minor sign of soil movement were registered by the monitoring program, taking the form of ditch settlements. However, on September, 2005 a landslide took place producing a gas leak, at a segment that was previously characterized as a non-critical one. An initial office review of available information and site pictures would not yield a clear reason of land movement initiation given the terrain mild slope. Actions were taken aiming at three different time frames. Immediate remedial actions were taken such as ditch opening to free stresses and replacement of buckled pipeline. Integrity specialist flew to the site to conduct geological survey and a stress analysis was conducted at a lab, allowing to asses and manage present risk before transportation began. Mid-term solutions were implemented as the ditch remained open until the next summer season, when access to the site was guaranteed. Finally, two new river crossings were constructed to mitigate this natural hazard permanently. Lessons from this incident revealed design aspects that need to be reconsidered to assure mountain pipelines integrity.

This paper deals with the historical overview of the castles and forts and what are the rules and foundations on which they arose. Castles constitute tourist attractions at the global and local levels as well as studying the variables and elements of spatial organization, in addition to the concepts of urban development strategies for historical regions. Salah al-Din Citadel was elected in Cairo because of the historical and urban depth it carries today. By studying how it originated and what changes occurred on it after the explorations and excavations that are taking place (design and development strategies) and then the practical study and analysis of the results of the practical study (General conclusions). The aim of the study is to analyze the urban fabric of Salah al-Din Citadel and define indicators for the characteristics of spatial organization using the Geographic Information Systems Program (GIS) and (Space Syntax) analysis, as they have an impact on the development of city centers and their importance in shaping the urban body of the city in an attempt to reach the strategies for urban development and possible urbanization of historic city centers (especially the cities of the castles). From the review of studies that dealt with the study of the organizational characteristics of urban spaces, the research problem crystallized (The lack of a clear perception of studying the characteristics of the spatial organization of castles and their impact on the development of urban development strategies) and the research hypothesis was formulated with (The study of the characteristics of the spatial organization of castles affects the integration of the castle with its urban surroundings and its historical characteristics ). The research was based on the development of the elements of a theoretical framework on the characteristics of spatial organization and then applied in practice to the castle of Salah al-Din as a model.

As a game of imperfect information it involves obstacles not present in games like chess and Go, and requires totally different techniques. No program had been able to beat top players in large poker games. Until now! In January 2017, our AI, Libratus, beat a team of four top specialist professionals in heads-up no-limit Texas hold'em, which has 10^161 decision points. This game is the main benchmark challenge for imperfect-information game solving. Libratus is the only AI that has beat top humans at this game. Libratus is powered by new algorithms in each of its three main modules: 1. computing blueprint (approximate Nash equilibrium) strategies before the event, 2. novel nested endgame solving during play, and 3. fixing its own strategy to play even closer to equilibrium based on what holes the opponents have been able to identify and exploit. These domain-independent algorithms have potential applicability to a variety of real-world imperfect-information games such as negotiation, business strategy, cybersecurity, physical security, military applications, strategic pricing, product portfolio planning, certain areas of finance, auctions, political campaigns, and steering biological adaptation and evolution, for example, for medical treatment planning.

This paper describes a multi-year program to assess pipeline crossings of sensitive watercourses along a major pipeline project. During the Front End Engineering and Design (FEED) phase a sensitive watercourse assessment team (SWAT) was established to provide a biophysical and construction assessment of selected watercourses to be crossed by a proposed pipeline project in western Canada. The SWAT comprised a fisheries biologist, a pipeline watercourse construction specialist and other technical support personnel. The field work included assessing biophysical data, fish habitat values, access to the crossing location, construction issues, site-specific mitigative measures and potential habitat compensation options, as well as providing photo documentation and a conceptual crossing sketch. The advantages of the SWAT assessment at a crossing site were: • It provided an effective and efficient field assessment of the proposed watercourse crossing in the early phase of the project. • It was a multidisciplinary assessment. • It provided a recommendation as to a preferred crossing location at the site. • It provided a recommendation as to preferred crossing method and timing of construction at the site. • The data were site-specific to the preferred crossing location. Three consecutive years of baseline biophysical field data were compiled and site reports generated using a custom designed database. Over 200 sensitive watercourses were identified based on environmental, geotechnical, and constructability factors and were visited by the SWAT team, sometimes more than once, for a total of 271 individual site assessments. Data collected during the FEED phase included site-specific information that can be used for ongoing project discussions, regulatory and community consultations, permitting and Fisheries and Oceans Canada (DFO) authorizations. The SWAT program also provided recommendations for minor or significant shifts in crossing location for 40% of the sites visited, resulting in changes to the pipeline alignment during the route evolution process.
