Abstract:
Advances in imaging technology allow ever more complex yet minimally invasive diagnostic and therapeutic interventions to take place in the genitourinary tract. Imaging provides precise targeting for tissue biopsy to facilitate rapid and accurate diagnosis—the basis of all subsequent treatment regimes. Percutaneous renal intervention is invaluable in the treatment of complex stone disease and for renal preservation in the patient with malignant or benign urinary tract obstruction. Antegrade ureteric procedures allow strictures, stones, and tumours to be tackled, often with much greater ease than an alternative retrograde approach. Although interventional radiological procedures are generally safe, they do come with risks such as bleeding and sepsis, as well as the longer-term complications related to indwelling drains, catheters, and stents. It is important for the urological surgeon to be familiar with interventional uroradiological techniques so as to appropriately counsel and care for patients who require these procedures.

Abstract:
Obstructive uropathy refers to the clinical sequelae of impairment of free urinary flow in the urological tract. The clinical manifestation of obstructive uropathy depends on the level and duration of obstruction, whether it is partial or complete, and whether infection has supervened. Obstruction may be clinically silent—and incidental radiological, biochemical, or examination findings may suggest the diagnosis. The radiological techniques used to demonstrate obstruction can be divided into anatomical and functional studies. These include ultrasound, urography, cross-sectional imaging, micturating cystourethrography, the Whitaker test, nuclear renography, and urodynamic techniques. Presence of infection with obstruction constitutes a urological emergency and urgent decompression is warranted. The pathophysiological effects of unilateral or bilateral upper tract obstruction differ and have important implications for clinical management of patients. The management of numerous upper urinary tract obstructive conditions is discussed.

Abstract:
Ultrasound is an invaluable tool in the diagnosis and management of many urological disorders. It has the advantages of not involving ionizing radiation, allowing rapid real time imaging and being relatively inexpensive. It can also be performed at the patient’s bedside if necessary. There are limitations, however, and it is best used as an adjunct to clinical assessment, often alongside other complementary imaging modalities. While many ultrasound studies are undertaken by urological surgeons, it is often performed by imaging specialists; close liaison between these two groups is essential. A brief, clinically relevant, introduction to ultrasound physics is included and the use of Doppler techniques and ultrasound contrast agents will be discussed. It is not possible to cover all the urological conditions for which ultrasound is used in one chapter, so some recommended texts are included in the reading list for further study.

Abstract:
Of all the major urological injuries, injury to the kidney is the most common. Indeed it is the most commonly injured intra-abdominal organ. Blunt or penetrating external injury is almost always the cause. The diagnosis and treatment of renal trauma has changed dramatically in the last 30 years or so reflecting the development of CT scanning for diagnosis and of endovascular interventional radiological techniques for treatment, at the expense of intravenous urography and surgery, respectively. Complications can be divided into early and late. The commonest early complications are bleeding and extravasation of urine, which may in turn lead to a urinoma, infection, and abscess formation. The best way of handling bleeding is by selective microembolization or surgery if necessary, which is usually only when embolization fails. A urinoma can usually be drained percutaneously through the flank, but may require surgery occasionally.
