Abstract:
Introduction Many surgical suites allocate operating room (OR) block time to individual surgeons. If block time is allocated to services/groups and yet the same surgeon invariably operates on the same weekday, for all practical purposes block time is being allocated to individual surgeons. Organizational conflict occurs when a surgeon with a relatively low OR utilization has his or her allocated block time reduced. The authors studied potential limitations affecting whether a facility can accurately estimate the average block time utilizations of individual surgeons performing low volumes of cases. Methods Discrete-event computer simulation. Results Neither 3 months nor 1 yr of historical data were enough to be able to identify surgeons who had persistently low average OR utilizations. For example, with 3 months of data, the widths of the 95% CIs for average OR utilization exceeded 10% for surgeons who had average raw utilizations of 83% or less. If during a 3-month period a surgeon's measured adjusted utilization is 65%, there is a 95% chance that the surgeon's average adjusted utilization is as low as 38% or as high as 83%. If two surgeons have measured adjusted utilizations of 65% and 80%, respectively, there is a 16% chance that they have the same average adjusted utilization. Average OR utilization can be estimated more precisely for surgeons performing more cases each week. Conclusions Average OR utilization probably cannot be estimated precisely for low-volume surgeons based on 3 months or 1 yr of historical OR utilization data. The authors recommend that at surgical suites trying to allocate OR time to individual low-volume surgeons, OR allocations be based on criteria other than only OR utilization (e.g., based on OR efficiency).

With the development and utilization of biomass energy has been paid close attention. This paper establishes three comprehensive evaluation models on biomass energy utilization’s potential applications in the region, using the method of fuzzy comprehensive evaluation, hierarchical weight analysis and factor analysis by means of MATLAB, SAS software. According these models we analyze biomass energy utilization’s development prospect and geographical distribution and expound several suggestions on adjustment of biomass energy utilization status in China.

We analysed the effect of the sheep grazing, the grass cutting and mixed utilization methods on plant composition, yield and live-stock keeping capacity of extensive Achilleo-Festucetum pseudovinae grassland in the 1996-2000 research period. Analysing the extensive utilizations effects on chanching of the plant composition of extensive grassland established that due to the cutting utilization method the area covering rates of gramineous and usefull weeds were the highest while the grazing utilization-method caused the briggest area covering rate for legumes and unusefull weeds. Analysing the utilized fibrin-fodder yield established that the single utilization methods could not produce the maximum yield and live-stock keeping capacity on the treathea grassland type. Because of things mentioned above we must use the mixed utilization methods to save the maximum phytomass.

Objectives:Magnetic resonance imaging (MRI) and computerized tomography (CT) scanners are high-tech equipment with the highest utilizations and expenditures in Taiwan. This study investigates the factors that affect MRI and CT utilization, such as whether supply-side factors have more impact than demand-side factors, and the effect of utilization control policies.Methods:This study used the nationwide CT and MRI claim data for the years 1998 to 2001 from the Bureau of National Health Insurance (BNHI) in Taiwan. Multiple regression analysis was the statistical method used to determine the relative factors that influence CT and MRI utilization.Results:CT : population ratio, hospital-based physician : population ratio, female ratio, pediatric (≤14 years) : population ratio, and family income significantly influenced CT utilization. MRI : population ratio and hospital-based physician : population ratio, female ratio, and family income significantly influenced MRI utilization. However, the proportion of the aged (≥65 years of age) did not significantly influence CT or MRI utilization after controlling for other factors. The rates of CT and MRI utilization for the outpatient and inpatient settings and the repeated uses of CT or MRI have decreased significantly after a utilization review was implemented by BNHI, which suggests that CT and MRI have been overused.Conclusions:Increases in supply-side factors significantly increase CT/MRI utilization, and supply-side factors impact CT/MRI utilization more than demand-side factors. The results show that the utilization instant report policy effectively decreases repeated uses of CT/MRI.

Abstract:
This paper addresses the question of what makes knowledge useful within the framework of anthropological inquiry. Some anthropologists and other social scientists believe that knowledge is intrinsically useful. Others have claimed that knowledge becomes useful only in the social context within which it is to be used; that is, it is the context which makes it useful. Still others claim that the utility of knowledge depends on the process of dissemination, its format or other formal characteristics. In this paper I will first define knowledge utilization, and then discuss anthropological knowledge. The second part of the paper will review a case example in which knowledge based on anthropological research is used in an ethnic minority community in the United States. The conclusion summarizes some central principles or guidelines for the utilization of ethnographic research in community problem solving.

Abstract:
<p><b>Objectives</b>. To study medication utilization and adherence to prescribed therapy in a female population in central Sweden. To study usage of hormone replacement therapy (HRT) in this population and to assess how HRT users compare to non-users regarding symptom reporting, general health and other variables. To evaluate symptom prevalence adjusted for potential symptom affecting variables. </p><p><b>Material and methods</b>. A cross-sectional postal questionnaire study was performed in 1995 in seven counties in central Sweden. A questionnaire was sent to a random sample of 4,200 women aged 35-64, of whom 2,991 responded (71.2%). The questionnaire contained questions on psycho-socio-economic background, quality of life, self-reported health, height and weight, climacteric symptom prevalence, and menopausal status and symptoms. It also comprised questions on medication prescribed during the past year. </p><p><b>Results</b>. 40% used prescribed medication and 12% took four drugs or more. Age, educational level, self-rated health, and BMI remained significantly correlated to drug use in multivariate analysis. Adherence ranged from 15%-98% depending on age, a scheduled check-up, perceived importance of medication, concern about medication, taking cardiovascular and respiratory disease drugs. The highest adherence was found for hormonal medication the lowest for musculoskeletal medication. </p><p>HRT was used by 15% of the women. 13 % used other symptom relieving therapy. HRT users reported higher score of vasomotor symptoms, except for sweating during the daytime. </p><p>Prevalence of general symptoms did not necessarily increase with age. Especially symptoms related to stress-tension-depression decreased with age. Four different symptom prevalence patterns were found. </p><p><b>Conclusions</b>. Age, health status, educational level and body mass index (BMI) appear to affect drug use. Adherence to therapy is highest among elderly women who regard their medication as important and have a scheduled check-up. HRT relieves some vasomotor symptoms but does not affect other symptoms or self-rated health. Prevalence of symptoms related to Stress-tension-depression appears to decrease with age.</p>

Botnets, networks of hundreds to millions of computers, controlled by one or more individuals, increasingly play a part in cybercrimes, with astonishing results. The access of a botnet gives the controller abilities of a large majority of all the cyberattacks over the internet, and with the possibility of buying a complete botnet, this opens the market to nontechnical criminals. The Darknet and the market it provides, enable the buyers to buy and trade everything from botnets and malware to complete schemes.   The increase in cybercriminal activities and organizations has been alarmingly high in recent years, and no wonder, when criminals just need to invest a small amount of money to gain potentially millions of dollars without any advance knowledge of computer science, and with only a slight chance of getting caught due to the anonymity of the internet and botnets.   Based on a literature review combined with a critically reflective analysis of a selection of information about botnets from other sources available on the internet, this paper has identified some of the main types of organizations used in cybercrime and their operations as well as basic information about botnets, the players and stakeholders in this area, the theft and schemes used by botnets and the online money laundering service involved.

Abstract:
Batch fermentations of Corynebacterium dioxydans were performed using aliphatic hydrocarbons as substrates. The production of carboxylic acids was monitored throughout these fermentations. A relationship between the carbon chain length of the hydrocarbon substrate and the carboxylic acids was observed. The maximum yield of carboxylic acid was found to occur in the exponential growth phase, with the initial complete medium. Cell hydrophobicity is the proposed hypothesis for the residual hydrocarbon found in these fermentations. The addition of synthetic surfactant increased the yield of carboxylic acids, but posed problems in extraction of these acids.<br>In spite of a variety of different approaches, it was not possible to avoid contamination of the carboxylic acid products with residual hydrocarbon. The persistence of this residue was attributed to the hydrophobicity of the cell.
