Abstract:
Acquired cicatricial narrowing of the vagina, which is quite rare, is of great interest in the therapeutic relationship for the obstetrician and gynecologist. What a serious obstacle is vaginal stenosis for a pregnant uterus during childbirth - can be seen from the article by Merz, posted in Archif fr Gyncologie, Bd XLV: for 230 cases of rupture of the pregnant uterus collected from the literature over the last 20 years, 16 caused vaginal stenosis. Timely removal of this obstacle would save 16 lives! It must be assumed that a huge number of cases, in addition, remain unpublished; in November l

Abstract:
Vaginal outlet obstruction commonly occurs with congenital anomalies resulting in accumulation of menstrual blood in the vagina or both vagina and uterine cavity. Childbirth injuries such as cervical, vaginal and perineal lacerations as well as episiotomies are common and may result in complications like vaginal stenosis and excessive perineal scarring. Reported here is a rare case of complete occlusion of the vaginal introitus secondary to a badly healed injury resulting from childbirth.

This paper presents two cases of women who had extensive vaginal adenosis from prenatal DES exposure, extending almost halfway down the vaginal canal. Both women were followed for decades with annual exams and Pap smears until after menopause. Clinical examination in both cases initially showed an absent pars vaginalis of the cervix, vaginal adenosis, and shallowness of the fornices. Several decades of annual exams showed these stigmata of DES exposure gradually disappear as the upper vagina progressively contracted. After menopause the upper vagina in both cases transformed into what appeared

GVHD can manifest in various areas, including the mouth, skin, and genitals, with vaginal symptoms present in 26% of cases. These may include dryness, itching, discharge, and labial fusion. Resection of the perineal area is challenging. Not only is there a component of loss of skin and muscle, but one must also consider reconstruction in an immediate or delayed fashion; whether a vaginal resection will result in partial or a total defect; and, if partial, whether the vaginal loss will be on the anterior, posterior, or distal vaginal wall. The pudendal flap may not be the best option for some of these defects. Loss of the vaginal introitus as well as part of the vault requires careful planning to provide a flap design with adequate length and width to prevent stenosis. Fraught wit

Valvular heart disease is a frequent underlying heart disease in pregnant women. Mitral stenosis is often rheumatic in origin, while aortic stenosis is more often congenital. Stenotic lesions carry a high risk of complications. Mitral stenosis with a valve area less than 1.5 cm2 should be considered for pre-pregnancy intervention, especially when balloon valvulotomy is an option, since the risk of maternal and fetal complications is high. For women with aortic stenosis, pre-pregnancy intervention should be considered when the woman is symptomatic, or when left ventricular function o
