Full textBakgrund: I det postoperativa skedet är patientens förmåga att kommunicera nedsatt. Den postoperativa patienten är därför i stort behov av att informationen som ges är individanpassad. Syfte: Syftet var att undersöka kommunikativa barriärer och underlättande faktorer mellan sjuksköterskan och patienten i postoperativ vård samt beskriva förbättringsområden. Metod: Litteraturstudie bestående av 12 stycken kvalitativa vetenskapliga originalartiklar med fokus på kommunikation mellan sjuksköterskan och patienten i postoperativ vård samt patientens kommunikativa behov och sjuksköterskans förhållningssätt till detta. Resultat: Litteraturstudien visade på att barriärer likt stressiga arbetsförhållanden ledde till kort eller ingen kommunikation mellan sjuksköterskan och patienten samt att den postoperativa vårdtiden därigenom blev förlängd. Olika modersmål, dialekt, kön, kulturell övertygelse och socioekonomisk bakgrund gjorde att informationen från sjuksköterskorna ej uppfattades- eller tolkades korrekt samt att tiden lagd på att få fram budskapet ökade arbetsbördan. Vad som agerade underlättande var patienter som själva var framåt och frågade om sin behandling. Detta resulterade i att dessa patienter fick hjälp samt ökade sannolikheten för kommunikation med sjuksköterskorna. Slutsats: Resultatet av litteraturstudien visade att ett etablerat tydligt kommunikativt samspel mellan sjuksköterskan och patienten, med individanpassad vård, kunde av patienten upplevas som lika viktigt och önskvärt som ett snabbt tillfrisknande från sjukdom.

Full textPatienter som har diagnosen schizofreni är inte unikt samlade inom den psykiatriska vården utan finns inom alla vårdspecialiteter. En viktig del av omvårdnadsarbetet hos de sjuksköterskor vilka möter dessa patienter är etablerandet av en vårdande relation med dem. Det är i denna relation som omvårdnaden tillämpas. Syftet med studien var att sammanställa och belysa forskning som beskriver sjuksköterskans vårdrelation med en patient som har diagnosen schizofreni. I en systematisk litteraturstudie granskades och analyserades åtta vetenskapliga artiklar vilket resulterade i tre huvudteman relaterade till vårdrelationen: ´etablera en vårdande relation´, ´främjar utvecklingen av vårdrelationen´ samt ´hämmar utvecklingen av vårdrelationen´. Innehållet i dessa teman framhäver att vårdrelationen är ett komplex fenomen bestående av många aspekter. Genom en förståelse utav dessa aspekters proportioner samt påverkan kan de utav sjuksköterskor ägnas medvetna åtgärder i syfte att stärka relationen till patienten och öka kvaliteten på omvårdnaden.

Full textBackground: Relatives to patients with mental illness and patients with mental illness experience that it’s hard to get proper healthcare. Lack of communication was a part of the issues that made the healthcare worse. Patients expressed being discriminated against by the healthcare. Aim: To explore nurse’s experiences of working with mental health patients. Method: Systematic literature review with a descriptive synthesis following Evans (2002), twelve qualitative research articles were analysed with an inductive approach. Result: Nurses experienced that communication was an important part of the care, relations were also an important part for the healthcare, both to patients and to relatives of the patient. If this didn’t occur nurses would experience that the healthcare had failed. Conclusion: Nurses experienced that communication and creating relationships between patients was monumental for the outcome to be good. Did this not succeed, nurses felt that they’d failed. It was also important that relatives of the patient could get involved in the healthcare, since it could benefit the recovery of the patient. How nurses treated both patients and relatives were then a good start in getting improvements for how the treatment could be improved for everyone.

Full textAtt ena dagen vara frisk och mitt i livet och nästa dag drabbas av akut och allvarlig sjukdom leder för patienten till en känsla av kaos. Under vårdtiden ska sjuksköterskan hjälpa patienten genom detta kaos och förbereda patienten för att lämna sjukhuset och fortsätta med sitt liv. Detta görs genom vårdande samtal som syftar till att informera om sjukdomen, riskfaktorer och livsstilsförändringar. Samtalen har också en stödjande funktion.
Syftet med examensarbetet är att utifrån hjärtinfarktpatienters upplevelser beskriva innebörden av det vårdande samtalet med sjuksköterskan. Det teoretiska perspektivet är det vårdande samtalet. Data utgörs av intervjuer med tio hjärtinfarktpatienter cirka två veckor efter utskrivningen. Lindseth och Norbergs (2004) fenomenologiska hermeneutiska metod användes vid analysen av berättelserna. Resultatet visar att samtalen innebär en möjlighet för patienten att få kunskap om sjukdomen men för att samtalet ska hjälpa patienten vidare kan det inte standardiseras utan det måste fördjupas. Patienten måste få känna att sjuksköterskan har tid att lyssna och vara närvarande för att delaktighet ska uppnås. Först då kan samtalet djupna och få den vårdande innebörd som är så viktig för patienten.
That one day be healthy and in the middle of the life and the next day suffering from acute and severe illness leads for the patient to a sense of chaos. During the time at the hospital the nurse will help the patient through this chaos and prepare the patient to leave hospital and get on with his life. This is done through caring conversation, which aims to provide information on the disease, risk factors and lifestyle changes. The caring conversations also have a supporting role.
The aim of the study is to, on the basis of the heart infarction patient’s perspective, describe the meaning of nurse’s conversation. The frame is the caring conversation. Data consists of interviews with ten heart infarction-patients two weeks after discharge from the hospital. The methodological approach was phenomenological- hermeneutic as described by Lindseth and Norberg (2004). The result showed that the conversation means an opportunity for the patient to get knowledge about the disease, but the conversation can not be standardized if the patient should get help to get on with his life. The patient must feel that the nurse have time to listen, being present and involve the patient in the conversation. At that time, and not before that, the conversation turns to the caring conversation that means so much for the patient’s recovery.

Full textBackground: Nurses gains patients trust through caring relationships. The relation-building process is different each time and demand adjustments to the patient and the situation. Aim: To explore the preconditions and barriers to create caring relationships, among newly graduated nurses in somatic inpatient. Methods: The study is a qualitative descriptive interview study, with semi-structured interviews, following an interview guide. Eight nurses were recruited through snowball selection. Data was analysed with qualitative content analysis. Results: Conditions needed to create caring relationships were, among others, the access of time, that the nurse had an empathetic personality, a person-centered approach and that the patient had an interest in the relationship. Factors that challenged the caring relationship were lack of continuity, stressful environment, lack of communication, lack of trust from the patient or if the patient were instable or had a cognitive impairment. Conclusions: Lack of experience made it difficult to establish caring relationships. God support from colleges decreased stress among the nurses, which made it easier to create caring relationships. Further research of what kind of support that is needed among newly examined nurses to facilitate caring relationships is needed.

Full textAbstract Background: Number of people affected by diabetes has increased during the past decades. Numerous nurses will be needing evidence-based knowledge of how to care for these people in the best way. Previous research has shown patients 'and relatives' experiences of diabetes and diabetes care. The question of what experiences nurses had of caring for diabetics remained. Aim: To describe nurses’ experiences of caring for people who suffer from diabetes. Method: Systematic literature study with descriptive synthesis according to Evans (2002) with an inductive approach. Results: Nurses' experiences were described in two themes with four subthemes. The first theme was to care for the unique human, with subthemes, to support the patient to self-care and to work with a complex care around the patient. The second theme was not to have the conditions to provide good care with subthemes, not to have time and competence a long with to generalize and hold a power position. Conclusion: To facilitate the best care for the growing population of diabetics, this degree project showed, for instance, that nurses should both consider patients' self-determination rights and apply more person-centred care.

Full textBakgrund: Alzheimers sjukdom förändrar minnet hos den drabbade, detta innebär att de blir beroende av vårdande. Närstående vill oftast inte lämna dem, utan blir deras naturliga vårdare i hemmet. Detta upplevs vara en trygghet för den drabbade, då minnesstörning oftast förvärras av att flytta hemifrån. Oavsett om det handlar om naturlig vård eller professionell vård är den vårdande relationen viktigt då den lindrar lidandet. Problem: Människor drabbade av Alzheimers har svårt att uttrycka sig verbalt, något som kan förhindra att den vårdande relationen uppstår. Detta skulle kunna bidra till att deras lidande inte lindras. Syfte: Är att belysa hur den vårdande relationen gestaltas i drama, då anhörig är naturlig vårdare till en människa drabbad av Alzheimers sjukdom. Metod: En latent innehållsanalys användes för att analysera filmen ”En sång för Martin”. Resultat: I resultatet presenteras fyra teman, ”Som att spela ett spel”, ”Jag vill bara hjälpa dig”, ”Ensam fastän vi är två” och ”De heligaste löftena”, för att belysa syftet. Slutsats: Båda parter kan i den vårdande relationen växa tillsammans och finna en väg ut ur lidandet. Utan den vårdande relationens närvaro blev de ensamma i sin smärta, rädsla och oro.
Background: Alzheimer´s disease changes the memory of the sufferer, this means that they become dependent of caring. Relatives do not often want to leave them, but rather become their natural caregivers in their home. This is experienced as security for the sufferer, because moving often disturbs a memory disorder. Regardless of whether it is natural care or professional care the caring relationship is important because it relieves suffering. Problem: Human beings affected by Alzheimer's, has difficulty expressing them verbally, which may prevent a caring relationship. This could contribute that their suffering will not alleviate. Aim: To illuminate how the caring relationship shapes in drama, when a relative becomes a natural caregiver to a person suffering from Alzheimer's disease. Method: A latent content analysis was used for analyzing the movie “En sång för Martin”. Results: The results are presented in four themes, “Like playing a game”, “I just want to help you”, “Solo though we are two” and “The most sacred promises”, to illuminate the aim. Conclusion: Both parts can in the caring relationships grow together and find a way out of suffering. Without the caring relationship presence they were left alone in their pain, fear and anxiety.

Full textBackground: Psychological ill health is steadily increasing in Sweden. By the Public Health Authority's measurements in 2016, the result showed that 16 percent of the population suffered from mental health. It has been found that through talks with a nurse, the person suffering from mental health can get support and power to strengthen his / her own abilities. A prerequisite for being able to provide a good care is to create a good care relationship between caregivers and patients. Purpose: The purpose of this study is to highlight what nurses in psychiatric outpatients experience as carers in conversation with patients. Method: Ten semi- structured interviews have been conducted and these have been analyzed with a qualitative content analysis. Results: The result is presented in three categories; To feel commitment to the patient, describing the importance of listening and hearing the patient's story and understanding the patient based on his life. It is described that a purpose of the caring communication is considered important. Respect for the patient's integrity and the ability to adapt the conversation from each individual is described. Create and deepen the relationship, be sure that safety and trust in the care relationship is a prerequisite for a caring communication. It is expressed that a constant contact is favorable to the relationship and the conversation. Space for development highlights the importance of the patient's wishes in the conversation. As well as the conversation as a possibility of change. Conclusion: The study shows that nurses experience the caring communication as significant. In order to succeed, insight into the importance of caring communication needs to be taken and that it may take time to get to know the patient. Based on the study, the conclusion can be drawn that nurses have a good value base with the patient in focus.

Full textBackground: A caring relationship is not the same as a friendship relationship, but still requires intimacy and commitment. Personal commitment is also necessary to develop as a health care worker. Grief is a natural reaction to loss of relationships, but caregivers do not necessarily know if they are allowed to or should feel and express grief when patients die. There is no complete picture of health care workers’ grief over dying and deceased patients. Objective: The objective was to describe how health care workers’ own grief for the dying or deceased patients manifests itself. Method: A descriptive literature review of eleven articles, analyzed with a form of qualitative content analysis. Findings: Health care workers’ grief manifests itself as inner feelings such as pain, burdensome feelings, and guilt and anger. Grief can also manifest itself as an outward expression where the caregiver for example cries or feels restless. It can also manifest itself through time, when the health care worker’s grief for a particular patient is changing over time, but also in a life-time perspective, when the health care worker’s past experiences affect how he or she experiences the grief. Clinical significance: This study may provide support to health care workers who are wondering if they are allowed to or should feel and express grief for dying and deceased patients. It can also be used by health care workers who have reached the expert stage and are supervisors themselves. At this level of expertise, according to the results of this study, encouragement and support should be given to less experienced health care workers, to help them gain the courage to feel and express their grief.

Full textIntroduction: In today's intensive care, patients who are mechanically ventilated are more often lightly sedated. Patients are deprived of their verbal communication and treated in an environment that they can find frightening. This puts greater demands on the nurse, in order to respond to the patient's psychosocial needs in a positive way and establish a caring relationship. Aim: To illuminate nurses’ experience of creating a caring relationship with an awake or lightly sedated patient undergoing mechanical ventilation in an intensive care unit. Method: A qualitative inductive design was used and data was collected with four focus groups, with a total of 14 female registered nurses with at least two years professional experience of intensive care. The study was conducted at three intensive care units in western Sweden. Data was analysed with a qualitative content analysis. Result: Nurses' experience of creating a caring relationship with an awake or lightly sedated patient undergoing mechanical ventilation, was interpreted on the basis of the following themes to highlight the various factors that are of importance; communication in the meeting, to highlight the person behind the patient, the room of caring and when the relationship is complicated. Conclusion: Using communication to create a common understanding, to get to know the patient and orient the patient in the present, to provide care with respect and impart a sense of comfort, and finally strive to resolve any challenges that may arise along the way, were all key factors in creating a caring relationship. As future intensive care nurses, the authors consider that the study provides important insights into the problem area that may be useful in their future profession.

Full textThe concept heteronormativity was used in the literature for the first time in 1993. This term can be found in dictionaries and encyclopedias from 2006 but only in a few of the most modern publications. The term currently lacks an exact definition. The aim of concept analysis is to identify characteristics for a term by studying its use. Healthcare relations can and should be developed into a caring relation, which is dependent on a connection involving body, mind and soul between the caregiver and the patient. The aim of this work was to describe and deepen the understanding of the term heteronormativity from a healthcare perspective. Rodgers’ evolutionary concept analysis of 23 scientific papers and common newspapers resulted in seven defined attributes: assumptions, norms, social hierarchies, sanctions for those deviating from the norm, gender roles, sexual relationships and stereotype family apprehension. The analysis also identified what precedes the concept, two women working with the concept and five related concepts: equality, gender, power, discrimination and homophobia. A number of consequences of which all were negative for both individuals and the society were found. Healthcare is negatively influenced in several ways by heteronormativity rendering the caring relation impossible. Conclusion: Heteronormativity leads to a number of problems and everyone are both victims and supporters of the system.

Anorexia Nervosa är en komplex sjukdom av psykisk och fysisk karaktär och vårdpersonal spelar en viktig roll för dessa patienters tillfrisknad. Målet med denna studie var att nå en djupare förståelse för den mänskliga kontaktens betydelse i omvårdnaden av patienter med Anorexia Nervosa samt att belysa essensen i en vårdande relation. Syftet med studien var att beskriva Anorexia Nervosa patienters erfarenheter av den mänskliga kontaktens betydelse i möten med vårdpersonalen. En kvalitativ ansats valdes och datainsamlingen grundade sig på sex självbiografier, skrivna av sex unga kvinnor som har varit sjuka i Anorexia Nervosa. En kvalitativ textanalys användes för att tolka data. Åtta teman framkom i resultatet: ”att känna förtroende” ”att få hjälp med att gå till botten med problemet”, ”att inte bli förstådd”, ”att inte våga öppna sig”, ”att inte bli sedd”, ” att få bekräftelse”, ”att känna maktlöshet” och slutligen, ”att någon talar till den friska delen”. Dessa teman speglade dessa personers erfarenheter av den mänskliga kontaktens betydelse i möten med hälso- och sjukvårdspersonal. Denna studie kan bidra med att ge en ökad förståelse för den mänskliga kontaktens betydelse i omvårdnaden av patienter som lider av Anorexia Nervosa samt att hjälpa vårdpersonal att tillmötesgå dessa patienters behov.

Full textBackground: The good relationship is stated to be characterized by reciprocity and acceptance of the other. To make the relationship work in a good way, communication and interaction between the care provider and the patient is needed. This study is focused on the good relationship in psychiatric nursing context, which means care of persons with mental health problems. Aim: To illustrate what nurses believe is important in creating a good relationship with patients in psychiatric care. Method: A literature study has been made, based on ten scientific articles which has been analyzed and quality checked. The material of this study, is collected from four different databases. Results: The study led to four themes; to mediate security, to create trust, to show respect and to be available and committed. To mediate security meant to give information to the patient concerning his care, that the care was individualized, genuine and had continuity. To create trust meant that the patient could trust the nurse and talk about their feelings and thoughts, and also that the nurse listened and related to what the patient expressed. To show respect meant to have patience, and needed to be mutual, for the cooperation to work between nurse and patient. To be available and committed meant that the nurse needed to be reachable, give the patient space to talk, support and give feedback. Discussion: The result of the study is discussed against Peplaus theory of nursing, articles illuminating patients view of the good relationship, the background and also barriers in creating a good relationship.

Full textBackground: The pandemic caused by COVID-19 has forced the intensive care to make several adjustments. As part of restrictions to prevent spread of infection personal protective equipment (PPE) has to be used when in contact with infected patients. The use of PPE has introduced questions of its impact on the caring relationship between the patient and intensive care nurse. Aim: The aim with this study was to illustrate the intensive care nurses experience of how working in PPE during the pandemic has affected the caring relationship with the patient. Method: Semi- structured interviews was conducted with six intensive care nurses. The interviews served as data for an interpretative phenomenological analysis (IPA). Results: Four superior themes emerged from the analysis. In the theme focus on communication emerged different way of which the PPE has affected communication. The experience of being shielded emerged in the theme focus on distance where closeness was expressed as essential. In the theme focus on suffering was the PPE´s impact on touch described and also its potential cause of suffering. Emotions of inadequacy in regards to the caring relationship and powerlessness in regards of not being able to change or affect emerged in the theme focus on emotions. Conclusion: The caring relationship has been affected by the use of PPE. PPE reduces the possibility of establishing a caring relationship which endangers the patient’s active participation. There is need for further research about affect with focus on prevention. An organizational awareness regarding PPE´s effect on the caring relationship could lay the foundation for interventions supposed to reduce its negative impact.

Aim: The aim of this study was to describe patients and nurses experiences of what enables and aggravate a caring relationship in forensic psychiatry care. Methods: The results of the study are based on interviews of five patient in a forensic clinic and five nurses who works on the clinic. The study was performed with an inductive approach. Data were analyzed based on a qualitative content analysis. Results: In order for a caring relationship to be established the results indicates that the patients find it important to be seen as humans, that the carers do ”that little extra” and that the carers and patients works at the same direction. The nurses found it important to familiarize themselves in the patient, to have a stabilized work with the team, to do “the little extra” and get the trust to cooperate. Conclusion: The results corresponds to the aim of the study. Both patients and nurses agreed that the nurse had the responsibility of the caring relationship. A similarity were shown between both of the groups such as time aspect, presence and a genuin interest from the carrier. Full textBackground: About 1500 people die from suicide each year in Sweden. Suicide is increasing in adolescent and young adults and is the second largest cause of death in this age group. Mental illness, suicidal behavior and suicide among young people are associated with a great deal of suffering and are a significant health problem. Suicide prevention is in need of research and knowledge where medicine and nursing is the keystone. Aim: The aim of the study was to increase the knowledge of conversation about lack of lust for life in outpatient psychiatric center. Method: Qualitative interviews were conducted with seven expert researchers and healthcare developers in near clinic suicide prevention. The data was interpreted through a qualitative content analysis according to Elo and Kygnäs (2007). Results: The study resulted in three themes; Handle fears, Show personal commitment and inspire hope for change and Procedures and flexibility. The result describes how the fear of health professionals' can be a hindrance when young people with a lack of zest for life shall be identified during short meetings. The study showed that the way of responding is of the importance. It´s important to show interest, understanding and acknowledgment for what the adolescent says in the call to get the young to talk about a lack of lust for life and the. Discussions: The result has been discussed with help of Travelbees interaction theory which it emerged that the communication and relationship is of great importance in the encounter with the young. The results also show that health professional’s fear must be addressed and overcome when young with a lack of zest for life will be identified.

Full textBackground: A large number of patients do not fulfill the treatment provided for eating disorders due to a feeling of misunderstanding from the nurse; this can lead to a suffering for the patient. The works of nurses are significant since the therapeutic relationship is seen as more healing than the medical treatment. A well-established relationship between care provider and caretaker is of great importance for a good quality care and a successful treatment of the patient suffering from an eating disorder. Aim: The aim of the study was to shed light on nurses' experiences of caring for patients suffering from an eating disorder. Method: A literature review was conducted with support of Evan's (2002) method of analysis. An analysis of 10 scientific studies which contributed to the aim of this study. Findings: Four main themes were identified that shed light on the nurses' experiences regarding attitudes and behaviors, a therapeutic relationship, knowledge and feelings. Conclusion: The relationship between patient and caregiver is very important but also very complex. It is our opinion that all nurses try to ease the patients suffering at the best of their ability although the knowledge of how to achieve this is not very specific. That nurses sometimes abandon core values like compassion, love and understanding; is a fact. Clinical impact: The nurses often sought support and help from other, more experienced colleagues to take note of their experiences and knowledge to be able to provide better care. The study is there for an essential contribution since it gives an overview of experiences on a greater scale than the collected experience that can be found in just one ward.

Full textBackground: Nurses are daily faced with ethical dilemmas, complex situations, suffering and grief. Empathy and compassion are considered fundamental in nursing, even though today there is uncertainty about the meaning of the concepts. Laws and regulations point to empathy and compassion as important components for good care. The quality of care according to patients is determined by the experience of communication, understanding and compassion, which has also proved crucial for the outcome of the medical treatment. Aim: To describe nurses' experiences of empathy and compassion in the nursing practice. Method: A literature study with descriptive design. Articles were searched in MEDLINE via PubMed. A total of 14 qualitative articles were included in the results of the literature study. Main results: Through thematic analysis, the literature study resulted in nurses' experiences of empathy and compassion consisting of two main themes and six sub-themes. The first main theme describes how internal factors such as personal characteristics and values of nurses affect their empathy and compassion. The second main theme describes how external factors such as organization and context affect nurses' empathy and compassion. Conclusion: Regardless of whether there is uncertainty or not about empathy and compassion as concepts, the results show that empathy and compassion as a phenomenon are affected by internal and external factors according to nurses. The phenomenon is perceived to affect the work of nurses, just as well as nurses are affected by the experiences that nursing work entails. The results most clearly show that organizations, together with personal values, has a crucial role to play in raising awareness and creating opportunities for improvement.

Full textThe aim of this study was to describe what is required for presence in the caring relationship, based on caregivers and care receivers experience. To look at what gives a presence that touch, gives an interaction and soothes suffering. Twelve scientific articles with qualitative methodology approaches were analyzed, with the aim to describe the content from this paper´s point of view. The content analysis resulted in four categories: Seeing the whole person, Spiritual and physical touch, The path to connection and The assets of the caregiver. The result shows that confirmation of the care receiver, caregivers attitude, physical and mental presence and communication is essential for how people experienced presence. Presence can create an interface that takes place between caregivers and care receivers in the caring relationship. In order to meet, time, physical and mental presence is required. Touch affects humans and thus caring. To look at the invisible that is happening in the light of presence, can provide insight to caregivers, which can be used in daily nursing care, in meetings with the care receivers.
