Full textAn important part of preventive work to maintain health is to prevent healthcare-associated infections. Nursing home for the elderly is an arena for health and care. Healthcare-associated infections refer to infections among persons in care as an outcome of an action of care. A central strategy for maintaining health and promoting public health is to prevent infectious diseases in the healthcare facilities. Factors that affect the adherence of staff to the hygiene routines in municipal care are resources, management, personnel and external factors. Aim of the study was to describe what infection control in a nursing home for the elderly meant from a managerial perspective. The method was a qualitative interview study. Seven managers for nursing homes were interviewed with semi-structured questions. The result was analyzed with a qualitative content analysis. Results and main findings in the study showed that managers in nursing home for the elderly were very aware of that work with infection control is of great relevance to minimize and break the spread of infection. They also highlighted needs such as working more centrally and structured with infection control and hygiene issues. Factors such as knowledge, premises, attitude, and finances sometimes fail and could be more elaborated in work. The conclusion of this study shows that managers in special housing for the elderly are very aware that health care is of great relevance to minimize and break the spread of infection. There is also a need to work more centrally and structured with the health-hygienic issue. Identified difficulties with the health-hygienic work since conditions such as knowledge, premises, attitudes, attitudes and finances sometimes fail.

Full textBackground Good healthcare hygiene can reduce the number of healthcare associated infections by one-third and with current knowledge we can with simple means observe the precautions necessary to ensure good healthcare hygiene. Yet these hygiene rules are often not followed. Aim To elucidate the factors that facilitate for nursing staff in hospital wards in to follow basic hygiene procedures. Methods The study design is a descriptive literature study. This method was chosen to get an idea of the current state of knowledge in the field. The method describes the state of knowledge of a field by reviewing, analyzing and describing selected scientific studies. Results Analysis of data from the articles gave seven themes: sense of responsibility, workload, attitude and behaviour, knowledge and education, responsibilities with regard to hygiene, feedback and availability. The most important factors are “attitudes and behaviour” and “management’s responsibility considering the hygiene”. Conclusion A clear leadership with motivated managers and an encouraging environment gives the best opportunity to change the staffs' behaviour and attitudes. However, to improve compliance with hygiene routines all seven factors need to be considered. Thus patients suffering can be avoided through a reduction in health care associated infections and days of care.

Full textBackground: Statistics showed that in health care and in the community, thenumber of carriers of multidrug-resistant bacteria, such as ESBL has increased.As a protection against communicable diseases basic hygiene routines wasapplied. Local guidelines developed by infection expertise were available on theintranet where they were continuously updated. Objective: The aim of this studywas to investigate were the nurse found the current care routines in the care ofpatients with ESBL. Method: Empirical cross-sectional study with quantitativeresearch approach. Data from the questionnaire were analyzed in Excel 2011 andopen questions were analyzed by manifest content analysis. The results werepresented as descriptive statistics. Sample: Registered nurses at nine wards in asmall hospital in southern Sweden. Results: Out of the 85 who responded, 86%used the hospital intranet as an information source, followed closely byVårdhandboken at 73%. There were 33% who had knowledge of how the ESBLspread and 82% felt secure and safe when they were caring for patients withESBL. Conclusion: The majority of the nurses used the intranet to find currentcare routines. Other repositories used was Vårdhandboken, the hygienenurse andcolleagues.

Full textNosokomiala infektioner förekommer världen över och är en stor orsak till sjukdomar och dödlighet. Även i Sverige är nosokomiala infektioner ett problem, både på sjukhus och i andra vårdformer. Basala hygienrutiner är den viktigaste åtgärden för att förebygga uppkomst av infektionssjukdomar. Syftet med denna studie var att undersöka och jämföra hemtjänstens undersköterskor och vårdbiträden, gällande deras kunskaper och följsamhet till basala hygienrutiner, samt deras kunskaper angående den vanligaste smittvägen inom vården. Datainsamlingen gjordes med en enkät som innehöll 22 frågor. Fyra hemtjänstgrupper ingick i studien, där totalt 66 vårdbiträden och 30 undersköterskor inkluderades. Resultatet visade att följsamheten till handtvätt och handdesinfektion efter ett omvårdnadstillfälle var hög, dock var följsamheten till handdesinfektion före ett omvårdnadstillfälle mindre bra. Följsamheten till handskar vid personlig hygien var mycket god, resultatet visade dock att de flesta endast använde plastförkläde/skyddsrock ibland. Många av respondenterna i båda undersökningsgrupperna kände inte till vilka områden som ingår i begreppet basala hygienrutiner, däremot kände många till den vanligaste smittvägen inom vården. Endast en statistisk signifikant skillnad kunde påvisas mellan undersköterskor och vårdbiträden. Slutsatsen var att begreppet basala hygienrutiner måste tydliggöras och att mer information behövs om vid vilka tillfällen händerna ska tvättas eller desinficeras. Ytterligare information behövs även om varför och vid vilka tillfällen plastförkläde eller skyddsrock ska användas.
Nosocomial infections is a big cause of disease and mortality worldwide. In Sweden nosocomial infections also is a big problem, both in hospitals and other care facilities. Hygiene guidelines is the most important measure to prevent infection diseases. The aim of this study was to measure and compare licensed practical nurses and nursing assistants in homemaker services, concerning their knowledge and compliance to hygiene guidelines and their knowledge about the most common way of spread of infection in healthcare. The data collection was conducted with a questionnaire including 22 questions. Four groups in homemaker services were included in the study, totally 66 nursing assistants and 30 licensed practical nurses. The result showed that the compliance to hand washing and hand disinfection after a care opportunity was high, however the result showed that hand disinfection before a care opportunity was less good. The compliance of using gloves was very good in personal hygiene opportunities. The result also showed that most of the respondents only used aprons occasionally. Many of the respondents didn’t know which areas are included in hygiene guidelines, however many had knowledge about the most common way of spread of infection in healthcare. Only one statistical significant distinction was established between nursing assistants and licensed practical nurses. The conclusion showed that hygiene guidelines must be elucidate and that more information is needed about when hands should be washed or disinfected. Further information is also needed about why and in which occasions aprons should be used.

Full textBackground: Healthcare professionals are often carriers of microorganisms in the event of incorrect hand hygiene and hygiene routines taken. Lack of healthcare hygiene among healthcare professionals increases the spread of infection and increases the risk of suffering from healthcare-associated infections, resistant bacteria and the risk of patient suffering.Aim: The aim of the study is to describe factors that affect nurses` compliance with hygiene routines.Methods: The literature review was written with 15 articles using qualitative, quantitative and mixed methods. The articles were searched through the databases CINAHL and PubMed, were quality reviewed and analyzed in five steps.Results: The results show factors that affect nurse’s compliance to hygiene routines where the main factors are attitudes, knowledge, barriers, prerequisites and incentive.Conclusion: The consequence of low compliance to hygiene routines is patient suffering related to the increase in care-related infections. Preventive measures towards influencing factors are of great importance and are required of all health care workers.

Full textBackground: A hospital-aquired infection is one of the most common complications suffered by patients treated in a hospital or other health care facilities; about 10 % of the patients get a hospital-aquired infection. Routines for basic hygiene is an effective method for preventing contact transmission, and thereby reduce the spread of VRI. According to the regulations for basic hygiene from the National Board of Health and Welfare, the staff is obligated to comply with these measures at any examination, treatment or other direct contact with the patient, to limit the spread of VRI. Aim: The purpose of the study was to chart the nursing staff’s knowledge of the routines for basic hygiene and whether there are factors that affect the adherence to these routines. Method: The study had a quantitative approach with non-experimental design, where the staff's knowledge of the routines for basic hygiene were described and measured. Hypotheses were designed and tested. Descriptive and correlation analyses were performed on the result. Result: The level of knowledge in the staff is relatively high, but there is room for improvement. There is a tendency of higher adherence when the level of knowledge arises. To some extent certain factors affect the adherence to the hygiene routines, for example access of disinfectants and sinks, and acute situations. Conclusion: Extent of knowledge about the subject affects adherence to the routines for basic hygiene, and because of that the authors of this study consider that education should be included as a mandatory element for employment at Skaraborg Hospital, and that there should be a plan for refreshing the nursing staff’s knowledge of routines for basic hygiene.

Full textBackground: Transmission of Meticillinresistenta Staphylococcus Aureus has increased in recent years, which places greater demands on health professionals’ adherence to guidelines for infection control. Several studies shows that adherence to guidelines is low which is causing the patient suffering and higher costs for healthcare service. Aim: To illuminate factors that influence health professionals’ adherence to guidelines on MRSA in hospital care. Method: This literature review is based on qualitative (n=3) quantitative (n=7) and mixed method (n=3) scientific articles. Findings: Four categories emerged; knowledge, work environment, organization and negative approach, with nine subcategories. Conclusion: To increase adherence to guidelines regarding MRSA, education, communication and information is required continuously. Increased knowledge would improve health professionals’ approach to patients, relatives and employees, which in turns create confidence in their profession. This might also create opportunities to reduce the staffs concerns of being infected or spreading the infection to family members. Healthcare service is facing a great challenge where understaffing, overcrowding and placement of equipment deficiencies. The organization has a significant responsibility to create structure and conditions for an active and committed leadership to improve factors that may affect the spread of MRSA.

Full textBackground: Due to the demographic development in the world with an increasing proportion of older people and thus more individuals in need of regular care the interest in home health care and nursing homes has increased. At the same time, the problems with healthcare-related infections are growing in both hospitals and in home care environment. This entails new infection prevention challenges for healthcare professionals who have an obligation to always provide safe care. As a nurse in home health care and in nursing homes, delegation to care staff, care recipients and relatives is required. In the preventive work, knowledge and participation in self-care are thus seen as important for safe care. Aim: The aim of this study was to compile health care professionals´ experiences of infection prevention in home health care and in nursing homes.Method: Literature review with a qualitative approach. Literature searches were performed in the databases PubMed and CINAHL. Quality review of selected studies was carried out with SBU's review template for qualitative studies and the content was analyzed with content analysis. Twelve studies remained and were the result of the present literature review. Results: The results emerged three main themes which were Environment and the surroundings, Knowledge and education and Organization. Based on these, another six sub-themes were extracted. Conclusion: The connection between infection prevention and care in the home environment is seen as a challenge for care staff. The home environment makes it difficult for care staff to apply basic hygiene routines and the lack of cleanliness in care recipients is seen as a risk of infections. Lack of knowledge among both care recipients and care staff also contributes to impaired work with infection prevention and makes the possibility of self-care more difficult.

Full textIntroduktion: Vårdhygien inom djursjukvården är liksom inom sjukvården viktigt för att förebygga och motverka utvecklingen av antibiotikaresistens vilken kan påverka folkhälsan. Basala hygienrutiner är en av de viktigaste vårdhygieniska åtgärderna för att förhindra smitta inom sjukvården och djursjukvården. En god följsamhet till basala hygienrutiner är mycket viktig att uppnå men på grund av liten kunskap om hur det förhåller sig inom djursjukvården är det svårt att skräddarsy insatser. Traditionellt riktas många vårdhygieniska insatser och ansvar till sköterskor i djursjukvården. Syfte: Undersöka om följsamheten till basala hygienrutiner skiljer sig mellan olika yrkeskategorier inom djursjukvården. Metod: Genom kvantitativ ansats utifrån 313 svar på en webenkät med ett utskick på 360-2600 enkäter om basala hygienrutiner undersöka självskattad följsamhet och med hjälp av statistikprogrammet SPSS beräkna om skillnader finns mellan olika yrkeskategorier inom djursjukvården. Resultat: Signifikanta skillnader i följsamhet till basala hygienrutiner mellan yrkeskategorierna veterinärer och sköterskor (djurvårdare och djursjukskötare) fanns inte. Bättre följsamhet sågs på större arbetsplatser samt om personalen fått utbildning på arbetsplatsen. Handdesinfektion före patientkontakt var svårast att följa samt användning av skyddsförkläde/rock vid risk för kontakt med kroppsvätskor. Slutsats: I motsats till tidigare praktisk erfarenhet visade resultaten att signifikant skillnad i följsamhet mellan veterinärer och djurvårdare eller mellan veterinärer och djursjukskötare inte kunde ses. Därmed kan konstateras att fler studier behövs för att identifiera dolda faktorer som påverkar följsamheten bland djursjukvårdens personal. Utifrån resultaten kan dock konstateras att interventioner som innebär förbättrade utbildningsprogram i vårdhygien på arbetsplatsen, särskilt på små till medelstora kliniker, bör genomföras. Dessa bör fokusera på handdesinfektion före patientkontakt. Mer effektiv övervakning behövs.
