Abstract:
Study Design: Prospective cohort study. Objectives: Evidence on predicting the success of indirect decompression via extreme lateral interbody fusion (XLIF) is scarce. The authors investigated if patients who could achieve a pain-free position preoperatively would derive clinical benefit from XLIF without direct decompression. Methods: Data from 50 consecutive patients who underwent XLIF with and without direct decompression by a single surgeon from January 2014 to August 2017 was collected. Primary outcome is the rate of failure of patients who underwent XLIF without direct decompression, characterized by persistence of pain postoperatively that required reoperations within 6 months postoperatively. Secondary outcomes are clinical outcomes and patient-reported quality of life outcome data, including visual analogue scale for leg (VASL) and back (VASB) pain, Oswetry Disability Index (ODI), and Physical Component Score (PCS) and Mental Component Score (MCS) of SF-12, for up to 2 years postoperatively. Results: One patient with preoperative dynamic posture-related pain who underwent XLIF without direct decompression subsequently had a reoperation due to persisting pain. Statistically significant improvement was achieved across all patient reported outcomes ( P < .05): improvement of 68% for VASL, 61% for VASB, 50% for ODI, 33% for PCS, and 11% for MCS of SF-12 at last follow-up. Six patients had thigh symptoms that resolved. Conclusion: The simple clinical criterion based on postural pain status preoperatively may help clinicians in patient selection for indirect decompression of XLIF without the need for direct decompression. Further studies with larger cohorts are warranted to establish the validity of the algorithm.

Background:Lung involvement is very common is systemic sclerosis (SSc). Approximately one quarter of patients develops pulmonary problems within the first 3 years of diagnosis and still represents the leading cause of death in these patients. In a recent clinical trail, the reduction of FVC was not accompanied by a benefit with respect to health-related quality of life and patient-reported outcomes (PROs).Objectives:To assess how the change in Pulmonary Function Test (PFTs) parameters correlates with the Patient Reported Outcomes (PROs) in an observational cohort of patients with Systemic Sclerosis (SSc).Methods:We conducted a retrospective study of 330 clinic episodes from 121 unselected patients diagnosed with systemic sclerosis according to EULAR/ACR 2013 criteria, in annual follow-up (for a total of 165 patients/year) with PFTs, Health Assessment Questionnaire Disability Index (HAQ-DI), Scleroderma Health Assessment Questionnaire (sHAQ), Modified Borg Dyspnea Scale (Borg) and Cochin Hand Function Score (CHFS). We assessed the correlation between the HAQ and the Visual Analogical Scale 1-7 at baseline (VAS1 pain, VAS2 disease severity, VAS3 arthritis activity, VAS4 intestinal problems, VAS5 dyspnea, VAS6 Raynaud’s phenomenon, VAS7 digital ulcers). We evaluated the correlation of PFTs with PROs at every time period and the correlation between the change of PFTs parameters (δFVC, δDLCO) with the change of the PROs over a year of follow-up. Following analysis of distribution, Spearman or Pearson Test were used to determine correlation coefficients, as appropiate (Prism 7).Results:The median disease duration was 5 years (IQR 3-10). The median of 12 months δFVC% and δDLCO% were 0 (IQR -5.81 to 3.28) and -2.439 (IQR -8.76 to 5.98), respectively. The analysis evidenced a strong positive correlation between VAS1-7 and HAQ. We observed also significant correlation between FVC%, DLCO% and HAQ-DI (r= - 0.355 and -0.266, respectively; p<0.0001 for both), Borg (r= -0.403 and -0.379, respectively; p<0.0001) and CHFS (r = -0.355 and -0.256, respectively; p<0.0001). Nevertheless, in longitudinal setting there was no significant correlation between δPROs and changes lung function, as continuous variables, neither there was any significant PROs difference in patients that did or did not lose more than 10% of FVC and DLCO over a year of follow-up.Conclusion:This analysis of a monocentric non-selected population evidenced that the current commonly used PROs in SSc while showing a good correlation with lung function are poorly sensitive to change or to reflect changes in lung function over 12 months. Abstract:
Primary tinnitus has a severe negative influence on the quality of life of a substantial portion of the general population. When acoustic coordinated reset (CR) neuromodulation stimuli are delivered for several hours per day over several weeks a clinically significant symptom reduction in patients with primary tonal tinnitus has been reported by several clinical sites. Here, we reported the first case where CR neuromodulation was delivered through a hearing aid. A 52-year-old man with chronic primary tonal tinnitus was previously considered untreatable with sound therapy. He initially received the classic CR treatment protocol with signals delivered with the separate proprietary device with his hearing aids removed during treatment. He was subsequently treated with the therapy being deployed through a set of contemporary hearing aids. After 5 months of classic CR treatment with the separate custom device, the THI and VASL/Ascores worsened by 57% and 13%/14%, respectively. Using the hearing aid without CR treatment for 5 months no change in tinnitus symptoms was observed. However, after three months of CR treatment delivered through the hearing aids, the THI and VASL/Ascores were reduced by 70% and 32%/32%, respectively.

Objectives: The purpose of this study was to report the effectiveness of Hoeeum Bee-venom Therapy and Balio Acupuncture Treatment in the management of chronic prostatitis/chronic pelvic pain syndrome.Methods: A patient with chronic prostatitis/chronic pelvic pain syndrome underwent Korean medical treatment, including Hoeeum Bee-venom Therapy and Balio Acupuncture Treatment. The severity of symptoms was assessed with a daily visual analog scale (VAS) for orchialgia and lower abdominal pain. In addition, we measured the National Institutes of Health Chronic Prostatitis Symptom Index (NIH-CPSI) on the first and last days of visit.Results: The patient’s symptoms were improved after treatment. The severity of orchialgia was reduced from VAS7 to VAS2, and lower abdominal pain was improved from VAS6 to VAS1. NIH-CPSI decreased from 21 to 5.Conclusion: Hoeeum Bee-venom Therapy and Balio Acupuncture Treatment are effective in treating chronic prostatitis /chronic pelvic pain syndrome.

Abstract:
This study validated muscle activation estimations generated by OpenSim during the gait of elderly fallers. Ten healthy elderly participants walked on an instrumented treadmill, monitored by motion capture, force platforms, and 12 surface EMG sensors. Static optimization was used to calculate muscle activations, evaluated through cosine similarity, comparing them with EMG signals from 12 muscles of the right leg. Findings revealed varied similarity levels across muscles and gait phases. During stance phase, tibialis anterior (TIBA), peroneus longus (PERL), soleus (SOL), gastrocnemius lateralis (GASL), semitendinosus (SEMI), tensor fasciae latae (TFL), and rectus femoris (RECF) demonstrated poor similarity (cosim < 0.6), while gluteus medius (GMED), biceps femoris long head (BFLH), and vastus lateralis (VL) exhibited moderate similarity (0.6 ≤ cosim ≤ 0.8), and gluteus maximus (GMAX) and vastus medialis (VASM) displayed high similarity (cosim > 0.8). During the swing phase, only SOL demonstrated inadequate similarity, while GASL, GMAX, GMED, BFLH, SEMI, TFL, RECF, and VASL exhibited moderate similarity, and TIBA, PERL, and VASM showed high similarity. Comparing the different 10% intervals of the gait cycle generally produced more favorable similarity results. For most of the muscles and intervals, good agreement was found. Moderate agreement was estimated in the cases of TIBA (0–10%), PERL (60–70%), GASL (60–70%), TFL (10–20%), RECF (0–10%, 80–100%), and GMED (50–60%). Bad agreement was found in the cases of SOL (60–70%), GASL (0–10%), and TFL (0–10%). In conclusion, the study’s validation outcomes were acceptable in most cases, underlining the potential for user-friendly musculoskeletal modeling routines to study muscle output during elderly gait.

Abstract:
Hysteroscopy is an endoscopic diagnostic and therapeutic method traditionally performed under general anesthesia but increasingly under local anesthesia. Today, it is considered the gold standard in gynecology. This minimally invasive procedure allows for a detailed assessment of the uterine cavity’s interior and the removal of abnormal changes within it and is applicable to patients of all ages. Background/Objectives: The purpose of the present study was to evaluate pain during hysteroscopy under local anesthesia at different stages of the procedure, identifying which stage is the most painful (GUBBINI Mini Hystero-Resectoscope; Tontarra Medizintechnik, Tuttlingen, Germany). Methods: The study included patients between the ages of 21 and 80 years. They were divided into two groups: the diagnostic hysteroscopy (HD) and the operative hysteroscopy (HO) groups. Pain measurements on the VAS scale were taken at each stage of the hysteroscopic procedure. After each stage, the operator asked the patient to indicate the maximum perceived pain value: after pericervical anesthesia was administered (VAS1), during the installation of equipment (between the removal of the speculum and the insertion of the hysteroscope into the vagina) (VAS2), after insertion of the hysteroscope and visualization of the external orifice of the cervical canal (VAS3), after passage of the hysteroscope through the cervical canal (VAS4), and after completion of the procedure in the uterine cavity (VAS5). The duration of each stage of the procedure was measured with a stopwatch: administration of pericervical anesthesia (T1), time between the removal of the speculum and the insertion of the hysteroscope into the vagina (T2), insertion of the hysteroscope into the vagina until the outer orifice of the cervical canal became visible (T3), passage of the hysteroscope through the cervical canal (T4), and the hysteroscopy procedure itself (T5). Results: The highest pain rating was for the canal passage stage (VAS4: 2.47 ± 2.48 points), followed by the procedure itself (VAS5: 2.12 ± 2.33 points). Anesthesia was also reported as quite painful, while the lowest pain was noted during the assembly stage. Overall pain scores for the entire procedure (VASmax) ranged from 3.5 ± 2.37. Conclusion: In conclusion, we found that the passage through the cervical canal was the most painful moment. Overall, hysteroscopy under pericervical anesthesia was not associated with significant pain. Special attention should be given to postmenopausal patients, as they experience more pain during the passage of the hysteroscope through the cervical canal. This group may benefit from additional pain management strategies during the procedure.

Abstract In higher plants, sexual and asexual reproductions through seeds (apomixis) have evolved as alternative strategies. Evolutionary advantages leading to coexistence of both reproductive modes are currently not well understood. It is expected that accumulation of deleterious mutations leads to a rapid elimination of apomictic lineages from populations. In this line, apomixis originated repeatedly, likely from deregulation of the sexual pathway, leading to alterations in the development of reproductive lineages (germlines) in apomicts as compared with sexual plants. This potentially involves mutations in genes controlling reproduction. Increasing evidence suggests that RNA helicases are crucial regulators of germline development. To gain insights into the evolution of 58 members of this diverse gene family in sexual and apomictic plants, we applied target enrichment combined with next-generation sequencing to identify allelic variants from 24 accessions of the genus Boechera, comprising sexual, facultative, and obligate apomicts. Interestingly, allelic variants from apomicts did not show consistently increased mutation frequency. Either sequences were highly conserved in any accession, or allelic variants preferentially harbored mutations in evolutionary less conserved C- and N-terminal domains, or presented high mutation load independent of the reproductive mode. Only for a few genes allelic variants harboring deleterious mutations were only identified in apomicts. To test if high sequence conservation correlates with roles in fundamental cellular or developmental processes, we analyzed Arabidopsis thaliana mutant lines in VASA-LIKE (VASL), and identified pleiotropic defects during ovule and reproductive development. This indicates that also in apomicts mechanisms of selection are in place based on gene function.

Abstract:
Lipocytes have been classified as vitamin A-storing, desmin-positive cells. In hepatic fibrogenesis, lipocytes transform into myofibroblasts, which express alpha-smooth muscle actin (alpha-SMA) and produce increased amounts of collagen. We isolated a population of vitamin A-poor lipocytes (VAPL) from normal rat liver and examined the morphological and biochemical differences between VAPL and vitamin A-replete lipocytes (VARL). Desmin and alpha-SMA expression were determined by Western blot in quiescent cells and in cells activated by culture on uncoated plastic. Both cell types were alpha-SMA-negative; however, in contrast to VARL, freshly isolated VAPL did not contain desmin. Desmin expression was induced in VAPL on activation. With time in culture, both VAPL and VARL expressed alpha-SMA and produced collagen, indicative of transformation to myofibroblasts. Ferritin receptor expression was demonstrated in cultured VARL after 1 day and in VAPL after 5 days, indicating that this is an early marker of lipocyte activation. After 7 days, VARL and VAPL were indistinguishable in terms of desmin, ferritin receptor expression, and collagen production. This study demonstrates the first isolation and characterization of two distinct quiescent subpopulations of lipocytes from normal rat liver: desmin-negative VAPL and desmin-positive VARL. Both populations of cells can be activated to myofibroblasts, the phenotype associated with hepatic fibrogenesis.

Peygamber’in vefatından sonra bir araya getirilmiş, Hz. Osman’ın halifeliği döneminde de iki kapak arasına toplanıp çeşitli beldelere gönderilmek üzere çoğaltılmıştır. Kur’an’ın yazımı ve muhafazası bağlamında hizmetlerde bulunulduğu gibi onun tilavetiyle ilgili de çeşitli hizmetler yapılmıştır. Kur’an’ın harekelenmesi, noktalanması, bölümlere ayrılması bunlar arasında sayılabilir. Arapçaya vukufiyeti zayıf olan kimselere rehberlik amaçlı tilavet esnasında nerelerde durup nerelerden alması gerektiğiyle ilgili telif edilen eserler de söz konusu hizmete matuftur.  Vakf ve ibtidâ dediğimiz mesele işte Kur’an’ın anlam bakımından doğru tilaveti için neşet etmiş bir alandır. Ancak bu alanla ilgili kalem oynatan ulemânın yaklaşımı farklı olabilmiştir. Kur’an’ın tilaveti esnasında anlamın uygun olduğu yerlerde durmak anlamına gelen vakf ve yine uygun yerden başlamak manasında kullanılan ibtidâ ile ilgili günümüze değin birçok bölümlendirmeler yapılmıştır. Bunlar arasında en meşhur olanı İbnü’l-Enbârî, Nehhâs ve Dânî’nin dört kısım şeklinde ele aldığı yaklaşımdır. Sonraki dönemlerde farklı kavramlarla ve okuyucuya kolaylık sağlayacak şekilde rumuzlarla vakf sistemi zenginleştirilmiş ve daha pratik tarzda uygulanagelmiştir. Çalışmamızda öncelikle vakf-ibtidânın tanıtımı yapılarak başlangıcıyla ilgili öne sürülen görüşler zikredilmiştir. Ardından bahsi geçen âlimlerin eserlerinde temas ettikleri vakf-ibtidâ çeşitlerine yer verilmiş, ayrıca sonraki dönemde kavramsallaşan muhtelif vakf-ibtidâ kısımları da ele alınmıştır.

This paper is an attempt to interrogate the supposed &lsquo;political turn&rsquo; that Faiz Ahmad Faiz&rsquo;s poetry is assumed to have taken in<em>&nbsp;Naqsh-e-faryādī (</em>Remonstrance) through his popular nazm &ldquo;Mujh se pahlī sī mohabbat mirī mahbūb na maaṉg&rdquo; It is understood as marking a departure from his writings on the beloved (<em>mahmoob</em>) to turn towards the concerns of the society steeped in oppression. However, I argue against reading it as a &lsquo;departure&rsquo; and instead see this as emerging from his love for his beloved, through a reading of &ldquo;<em>Raqīb se</em>,&rdquo; a nazm from&nbsp;<em>Naqsh-e-faryādī</em>.&nbsp; In doing so the paper engages with the poetics and politics of Faiz Ahmad Faiz&rsquo;s poetry, in order to argue that his writings enmesh the themes of love and politics in a way that makes a separation between the two impossible. Lastly, the article argues, in reference to Faiz&rsquo;s writings, that the concept of unrequited love,<em>&nbsp;hijr&nbsp;</em>(separation),&nbsp;<em>vasl</em> (union) of the traditional/classical Urdu poetry can itself be read politically. The paper thereby concludes that Faiz never really &lsquo;departed&rsquo; from Classical Urdu poetry and neither did he make a fresh entry into the &lsquo;political&rsquo;, since the essence of love and affection remains, blends and informs his poetries on resistance and revolution.

<strong>Objective:</strong>&nbsp;To evaluate and compare the costal cartilage lateral crural strut graft&rsquo;s (LCSG) ability to support a weak lateral crus in patients with external nasal valve dysfunction (EVD) undergoing primary versus revision functional rhinoplasty.&nbsp;<strong>Methods:</strong>&nbsp;This is a prospective cohort study of 26 patients (mean [SD]: 40.23 [6.75] years of age; 10 [38%] females) with clinically diagnosed EVD, who underwent primary versus revision functional rhinoplasty with the use of a costal cartilage LCSG (10 [38%] primary functional rhinoplasty patients and the 16 [62%] revision patients). Preoperative and 12-month postoperative subjective and objective functional measurements along with statistical analysis were performed.&nbsp;<strong>Results:</strong>&nbsp;While all baseline demographic and preoperative functional measurement scores were similar between the 2 groups, the primary cohort&rsquo;s preoperative scores were higher overall. Follow-up was a mean of 14.58 months. The primary group demonstrated a greater difference in score improvement postoperatively in all categories. All patients had significantly improved visual analog scale (VAS), Nasal Obstruction Symptom Evaluation Scale, 22-Item Sinonasal Outcome Test, and nasal peak inspiratory flow (NPIF) scores. When comparing the overall score outcome and surgical efficacy of the LCSG, both groups had near equal final score outcomes with the exception of VASL and NPIF.&nbsp;<strong>Conclusion:</strong>&nbsp;The LCSG is a viable and versatile option in the management of EVD for both primary and revision rhinoplasty patients. &nbsp; &nbsp; &nbsp;

BACKGROUND Postoperative instruments supporting decision-making for optimal care level are lacking. OBJECTIVES This study tested the reliability/validity of the postoperative track assessment tool (PoTra-tool) in a postanaesthesia care unit (PACU) and a specialised peri-operative intermediate care unit (IMCU). DESIGN A prospective observational study. SETTING The PoTra-tool consists of two visual analog scales that rate estimated indication (VASi) and benefit (VASb) of care in an IMCU. The tool was tested on day 0 in both units of a Swiss university hospital, and on day 1 in the IMCU. PARTICIPANTS Raters were nurses with two levels of competency (noncertified or certified) and physicians with three levels of competency (noncertified, certified, consultant). MAIN OUTCOME MEASURES Reliability, between raters and across days, was assessed using intraclass correlation coefficients (ICCs) or weighted kappas. Validity was evaluated with validated scores (severity of illness, nurse workload and predicted mortality). Statistical analysis was carried out using analysis of variance (ANOVA) or Kruskal–Wallis tests. RESULTS In total, healthcare professionals performed 4206 ratings for 879 patients with a median [IQR] age of 72 [61 to 81] years) and for American Society of Anesthesiologists physical status scores 4 [4 to 4]. The median [IQR] VASi for PACU patients was 12 [2 to 30] and for IMCU patients, 76 [57 to 89]. Median VASb was 7 [1 to 19] for PACU patients and 66 [43 to 84] for IMCU patients. Internal consistency between VASi and VASb was high (Pearson correlation: 0.90). For each type of rater, reliability of VASi and VASb was acceptable to moderate for inter-rater reliability (weighted kappas: 0.59 to 0.67). The associations tested for construct validity were consistent with expected patterns, although the effect sizes were small. CONCLUSION The PoTra-tool may support the decision-making process in deciding the most appropriate peri-operative level of care for an individual patient: PACU with standard care or IMCU with specialised care. Background/Objective: The prevalence and character of female sexual dysfunction (FSD) in polycystic ovary syndrome (PCOS) have not been precisely determined. The aim of this study was to assess FSD using the Changes in Sexual Functioning Questionnaire (CSFQ-14) in women with PCOS and their partners compared to a control group, as well as correlations between five subscales, the total score of the CSFQ, and seven questions of the Visual Analogue Scale (VAS). Methods: The study sample (N = 160) comprised two groups: (1) women with PCOS and their partners (n = 91) and (2) women without PCOS and their partners (control group; n = 69). Results: The total scores of the CSFQ did not reveal FSD in either group of women. Regarding all subscales and the total score, the analysis showed a statistically significant difference between women and their partners (in all cases: p &lt; 0.001). The discrepancy in arousal between women and men in the PCOS group was large (the mean difference was −2.32; t = −11.29, p &lt; 0.001, Cohen’s d = −1.26). The importance (VAS1), the level (VAS7) of sexual satisfaction, and the intensity of sexual thoughts (VAS2) correlated with almost all domains of the CSFQ. Conclusions: In conclusion, normal sexual function in PCOS does not mean proper sexual functioning in a sexual relationship.

SUMMARY Flushing the vasa deferentia (ductus deferentes)a at the time of vasectomy reduced to zero the number of intact spermatozoa by postvasectomy day 6 in the dog and by postvasectomy day 7 in the cat and shortened the time from vasectomy to azoospermia in the dog, but not in the cat. The fluid used to flush the vasa deferentia was not eliminated through the penile urethra, but flowed into the urinary bladder, indicating that the least resistant pathway for the exit of vasal content in the anesthetized dog and cat is toward the urinary bladder. Both control and treated dogs and cats had spermatozoa in the urine obtained by cystocentesis immediately after ejaculation or ejaculation and flushing of the vasa deferentia. Flushing the vasa deferentia at the time of vasectomy is easy to do, safe, and can be used in clinical practice to decrease the time from vasectomy to the safe utilization of dogs and cats as teasers. The procedure has potential application to males of other species.

This study was conducted in the plastic house - Nursery of the Duhok University - Duhok governorate-Kurdistan region-Iraq from the period 10th Apr. 2021 to 10th Jan. 2022. This study aimed to investigate the effect of supplemental light (light and without light), pinching (pinching and without pinching) and number of plants per pot (2, 3, and 4) plants/pot on the growth and flowering of chrysanthemum (Dendranthema grandiflora Tzvelev) plant. The results showed that supplemental light for one month delayed the flower bud emergence (223.08) days, and flower anthesis (241.58) days compared with the control (without supplemental light) that gave (198.83, 218.44) days for the two characteristics respectively. Further, supplemental light gave the best results for flower length, flower diameter, flower number, and vase life compared to (without supplemental light). Pinching delayed the flower bud emergence and decreased the plant high, growth index, flower diameter, and vase life of cut flower significantly while it significantly increased the flower diameter and vasa life. Increasing the number of plants per pot to 4 plants caused a significant increment in flower number (8.25) compared with 2 plants which gave 5.64 plants/pot. The triple interaction showed significant differences in all characteristics and number of days to flower bud emergence and flower anthesis were the highest mean for the supplemental light with or without the pinch and 2 plants/pot. Also, the best plant height (72.33) cm and growth index (26761) cm3 were recorded without supplemental light, without pinch with 2 plants/pot. However, the maximum vase life of 26.33 days was found from the interaction of supplemental light, pinch, and 2 plants/pot.

AbstractBackgroundAbnormal visceral sensory function has been demonstrated in patients with diabetes mellitus and diabetic autonomic neuropathy seems to be involved in the development and progression of gastrointestinal tract dysfunction. The possibility of multimodal (e.g. mechanical, electrical, thermal and chemical) stimulation in, e.g. the esophagus has developed visceral pain research. The major advantage is involvement of distinctive receptors, various sensory nerves and different pain pathways mimicking clinical pain that favours investigation of central pain mechanisms involved in allodynia and hyperalgesia. Hence the rationale of the presents study was to explore the nervous system by assessment of esophageal sensitivity to multimodal stimulations.MethodsThroughout an euglycemic clamp, 31 healthy volunteers (age 44.3 ± 10.6 (mean ± SD) years; 11men) and 31 patients (age 46.3 ± 11.7 years; 10 men) with insulin dependent diabetes mellitus (duration 31.3 ± 13.1 years) were included in the study. By use of a multimodal oesophageal probe, sensitivity to heat, mechanical distension and electrical stimulation was assessed in the lower oesophagus.ResultsFor heat stimulation patients had increased sensitivity in the sensory range with shorter stimulus duration until pain tolerance threshold (122 ± 3.8 sec vs. 136 ± 3.7 sec; p = 0.006) and larger area under the temperature curve (2380 ± 1847 vs. 1409 ± 1450; p = 0.03). There were no differences between groups for mechanical stimulation (maximum pressure (39 ± 57mmHg vs. 24 ± 48mmHg; p = 0.3; maximum volume 59 ± 21ml vs. 62 ± 25 ml; p = 0.56). As an overall finding, patients tolerated higher electrical stimulation intensities (p = 0.02), dominated by discrimination in the sensory range: At sensory detection threshold (VAS1) 21.1 ± 12.4 mA vs. 16.3 ± 5.5 mA (p = 0.03); at moderate sensation (VAS3) 27.5 ± 13.3 mA vs. 21.5 ± 5.0 mA (p = 0.03) however at pain detection threshold (VAS5) 31.6 ± 13.1mA vs. 28.8 ± 5.2 mA; the trend was insignificant (p = 0.3).ConclusionPatients with insulin dependent diabetes mellitus had modality-specific alterations of esophageal sensitivity. Heat stimulation activates selectively mucosal receptors whereas on the contrary electrical stimulation depolarizes the free nerve endings non-selectively. Hence, due to the different sensitivity profile, the esophageal neuropathy is most likely a result of both peripheral and central neuropathy caused by longstanding diabetic neuronal damage.

Background and Objectives: Remifentanil is a potent synthetic μ-opioid receptor agonist known for its rapid onset and ultrashort duration of action, making it a popular choice for intravenous labor analgesia. The analgesic effectiveness of remifentanil patient-controlled analgesia (Remifentanil-PCA) may vary based on the stage of labor and parity, potentially influencing satisfaction with labor analgesia. This study aimed to evaluate the degree of pain reduction achieved with remifentanil-PCA, considering different cervical dilations in both nulliparous and multiparous women. Material and Methods: Women who were ≥37 weeks pregnant with singleton cephalic fetuses, either by spontaneous onset or induction of labor, were included in the study. Data were collected from the Labor Record form, which included demographic and obstetric information, as well as the onset of analgesia categorized by cervical dilation (1–3 cm, 4–6 cm, 7–9 cm, and full dilation). Additionally, data on analgesia onset and duration (the time interval between the start of analgesia and the delivery of the baby), initial numerical rating scale (NRS0) for pain intensity, NRS after the first hour of analgesia (NRS1), the lowest recorded NRS during labor (NRSmin), and pain reduction during the first hour of analgesia (NRS0–VAS1), satisfaction with labor analgesia (rated 0 for dissatisfied, 1 for moderately satisfied, 2 for very satisfied), and complication rates were obtained from the remifentanil-PCA form. Results: A total of 513 nulliparas and 523 multiparas who gave birth between 1 January 2019 and 31 December 2019 were reviewed. No significant differences were found between the two groups regarding age, body mass index, labor induction rates, occipito-posterior positioning, blood loss &gt; 500 mL, or neonatal outcomes. Nulliparas exhibited a higher gestational age (p = 0.021), longer labor duration (p &lt; 0.001), and increased rates of cesarean sections (p &lt; 0.001) and vacuum extractions (p = 0.002). Remifentanil-PCA consistently provided mild to moderate pain intensity reduction. No differences were found in VAS0, VAS1, or pain intensity reduction (VAS0–VAS1) regardless of the stage of labor or parity. Significant differences in VAS min were observed among nulliparas at different stages of labor (p &lt; 0.026). However, a higher proportion of multiparas reported moderate (24.7% vs. 9.5%, p &lt; 0.001) and high satisfaction (90% vs. 75%, p &lt; 0.001) with remifentanil-PCA compared to nulliparas. Importantly, no serious complications in mothers or neonates attributed to remifentanil-PCA were observed during the observational period. Conclusions: Remifentanil-PCA demonstrates consistent effectiveness regardless of the stage of labor or parity. This indicates that remifentanil-PCA can be administered at any point during labor. Coupled with its rapid availability and immediate analgesic effect, this feature enhances the flexibility of its use in clinical practice.

Abstract. This article explores Vasyl Korchovyi’s artistic heritage, his unparalleled style, and notable manner of working. Vasyl Korchovyi is a Ukrainian sculptor, graduate from National Academy of Fine Arts and Architecture, representative of the Academic school of sculpture creation. This sculptor has not been neglected today and is an often and widely recognizable guest and/or participant of/in numerous exhibitions, art competitions, and sculpture symposiums including the latest one, the Xth Kaniv International Sculptural Symposium (September–October, 2021). Despite that, his artworks have been discussed in a quite occasional and random way in artistic articles and other kind of publications. This is the reason of why this study focuses on the major subjects and gentres used by Korchovyi for his artworks, as well as on his manner and techniques for working with stone. The studу is one of the first-ever attempts to use the art criticism approach to evaluate three sculpture works by Korchovyi: with two of them being complete and one piece still unfinished: the work from the Kaniv sculptural symposium above. First, Altar of Dionysus, an indoor sculpture, carved of sandstone, is discussed. It depicts a bacchante, a woman who holds a bowl and a grapevine, with her hair ingrown in the branches and the two goat heads that flank the composition on both sides; the attributes associated with fer­tility, sweet delight and also with wild Dionysian origin, while calm or even being asleep at the moment. Second sculptural work is the tombstone of Vasyl Korchovyi’s father, Ivan Sobko. For this reason, its design is marked with Korchovyi’s personal feelings and sentiments. The composition of the sandstone-made sculptural part of the tombstone represents an angel with outstretched wings, inscribed in the area of a trefoil cross. The openwork pattern of weaves of winding shoots with remains of leaves in some places adds depth to the background framed with smoothly polished parts of the surface, where the figure of angel is located, and carefully surround the upper part of the medallion with a photo of the deceased one; the composition that brings the feeling of a deep anguish and imminence, the painful emotions to be eventually accepted.

Objective To assess the efficacy of Yamamoto's acupuncture method on pain, drug intake, functional capacity and quality of life for the treatment of acute non-specific low back pain (ANLBP). Methods A prospective, randomised, parallel-group, double-blind, placebo-controlled trial was performed in 80 men and women with ANLBP who were randomly assigned to five acupuncture sessions (intervention group (IG), n=40) and to five non-penetrating acupuncture sessions (sham group (SG), n=40). Patients were evaluated at baseline and at 3, 7, 14, 21 and 28 days. The measurements used were: visual analogue scale (VAS) for cumulative pain (before intervention, VAS1) and immediate pain (after intervention, VAS2); function (Roland–Morris Disability Questionnaire (RM)); quality of life (SF-36); improvement rating; and number of anti-inflammatory tablets taken. The primary endpoint was a decrease of at least 2 cm in VAS1. Results Pain VAS improved significantly in the IG from day 14 onwards compared with the SG, but the difference did not reach the prespecified clinically relevant value of 2 cm. The IG was significantly superior to the SG in the following outcomes: cumulative pain, function, pain (SF-36) and vitality (SF-36) at days 14, 21 and 28 (p&lt;0.05); limitation in physical aspects (SF-36) at all times (p=0.007 and p=0.02); and functional capacity (SF-36) at days 21 and 28 (p&lt;0.05). The IG also took significantly fewer anti-inflammatory tablets than the SG (p=0.004) at all evaluation times and the improvement rating was better than the SG (p&lt;0.001). Conclusions Yamamoto's new scalp acupuncture was more effective than sham treatment with regard to decrease in pain and anti-inflammatory intake as well as improving functional status and quality of life for patients with ANLBP. To compare different scores in assessment of risk of left atrial or its appendage thrombosis (LAAT) in patients with non-valvular atrial fibrillation (AF) or atrial flutter (AFL) undergoing transesophageal echocardiography before catheter ablation (CA) or cardioversion (CV).Material and methods. In retrospective, single-center, case-control study medical records of 1994 patients with non-valvular AF or AF from the period 2014-2019, who underwent transesophageal echocardiography before CA or elective CV, were analyzed. LAAT was detected in 33 of them. For the control group, 167 patients without LAAT were randomly selected from this database. CHADS2, CHA2DS2-VASc, R2CHADS2, R-CHA2DS2-VASc, R2CHA2DS2-VASc, CHA2DS2-VASc-RAF, mCHA2DS2-VASc, CHA2DS2-VASc-AFR, ATRIA scores were evaluated and compared as potential predictors of LAAT.Results. Mean age of studied patients (n=200) was 60.3±10.9, 55% of them were males. Most of patients had non-valvular AF (87,5%) and were assessed before CA (84,0%). All risk scores were associated with LAAT and CHA2DS2-VASc-RAF score was most informative [AUC 0.84; 95% confidence interval (CI) 0.76-0.91]. According to multivariate analysis in a logistic regression model among studied risk scores CHA2DS2-VASc-RAF score was the single independent predictor of LAAT [odds ratio (OR) 1.37; 95% CI 1.21-1.55; p=0.004). OR of LAAT among patients with CHA2DS2-VASc-RAF &gt;3 was 12.8 (95% CI 3.75-43.9; p&lt;0.0001) with sensitivity, specificity, positive and negative predicting values 90.6%, 57.1%, 30.2% and 96.7% respectively Conclusion. In the studied group of patients with non-valvular AF or AFL, without severe structural heart disease and severe concomitant diseases CHADS2, CHA2DS2-VASc, R2CHADS2, R-CHA2DS2-VASc, R2CHA2DS2-VASc, CHA2DS2-VASc-RAF, mCHA2DS2-VASc, CHA2DS2-VASc-AFR, ATRIA scores were associated with LAAT. Abstract:
One of the critical quality criteria in cut flower marketing is vase life. The neck bending issue observed in gerbera flowers can substantially reduce vase life. This study aimed to identify effective vase solutions to prolong the vase life in the ‘Yeliz’ and ‘Rosalin’ gerbera varieties. Seven different vase solutions were evaluated: sodium dichloroisocyanurate (DICA), sodium hypochlorite (klorak), sodium hypochlorite + sucrose, 8-hydroxyquinoline citrate (8-HQC), perlan + sodium hypochlorite, and perplex + sodium hypochlorite and potable water (Ks: control). Parameters such as vase life, total and daily solution uptake, relative fresh weight, and microbial density in vase solutions were assessed. Vase life ranged from 8.33 to 13.22 days for the ‘Yeliz’ variety and from 10.00 to 14.67 days for the ‘Rosalin’ variety. DICA provided the longest vase life for both varieties among the seven different vase solutions. All vase solutions except for flowers in control improved relative fresh weight within the first 3 days. DICA was also the most effective in reducing microbial density in the vase solutions. Furthermore, all treatments significantly reduced microbial density compared to the control. The findings suggest that substances such as DICA, perlan, and perplex could serve as viable alternatives in vase solutions for extending the vase life of gerbera flowers.

Abstract:
Guillermo Martínez was born in Bahía Blanca, in the province of Buenos Aires, Argentina, in 1962, four years before General Onganía came into power. In 1982 he was awarded the first prize in the National Short Story Competition ‘Roberto Arlt’ for his book La jungla sin bestias (The Beastless Jungle); six years later he received the first prize from the Fondo Nacional de las Artes for his second collection of short stories, Infierno Grande (Vast Hell). His first novel, Acerca de Rodorer (Concerning Rodorer) was published in 1992. Martínez belongs to the generation of writers who grew up in the midst of the Argentina of the ‘dirty war’ between the military dictatorship and the guerrilla, a war that left the country shattered and from which Argentina has not recovered in spite of the present government's attempts to erase all memory of those past atrocities. The war did violence to everyone and everything, including the Argentine language. The writers of Martínez's generation were forced to reconstruct a tongue destroyed by the abuse of power, by irrational violence, by forced stupidity which infected words like a virus infects the blood. Their task was not only to bear witness and to build imaginary landscapes for their chronicles which are not, it must be said, mere documentaries. First they had to rescue the words themselves from debasement, using a pared-down, clear-cut language, free from the rhetoric, far-fetched metaphor and bombast so dear to the military heart.
