Full textDuring the 19th century the Elementary school and its teachers emerge in the city of Halmstad and in the parish of Veinge as well as in Sweden overall. The aim of the essay is to enthrone these teachers’ conditions and a comparison is done between the teachers in these two places. The mate-rial that is used is for example minutes from the board of education in Veinge and the city council of Halmstad, collected works about the elementary school of Halmstad as well as litera-ture regarding the subject. The essay starts with a short background history about the Elementary school and its teachers. Then it continues to discuss whether it was important to have a teacher’s education or not, how the teachers were treated in the community, the teachers’ work and its biggest problem, and finally, salary, dismissal and pension. The essay shows that differences as well as similarities exist, which proves the complexity of the teachers’ conditions.

Full textThe role of the venous system is central to cardiovascular fonction, yet, in terms of scientific literature, it appears to be the "poor relation" compared to the heart or arterial system. This is true both for the physiological understanding of venous hemodynamics and for the pathophysiological analysis of chronic venous disease and more particularly when the link is made with exercise. The aim of our investigations was to better understand the adaptations of the venous system during physical activity and during the post-exercise recovery period and, to evaluate the impact of chronic venous disease on physical activity and vice versa. We have studied the acute effects of physical activity on venous hemodynamics (evaluation of the calf venous pump), and the effects of various recovery methods ( compression, immersion, calf low frequency electrical stimulation), and we have also explored the consequences of chronic venous disease upon physical activity through the analysis of venous claudication. The main results of our work enabled a better understanding of the adjustments in venous hemodynamics linked to exercise in healthy subjects and to better understand the pathophysiological consequences of high venous pressure on the microcirculation of patients with chronic venous disease. Our findings also provide a detailed understanding of venous hemodynamics during post-exercise recovery and the positive effects of recovery methods on both performance and vascular adjustments. The interdependence of the different organs and tissues (heart, arteries, microcirculation, veins) of the cardiovascular system is an essential element highlighted in this work. Thus, we demonstrated that venous hemodynamics could not be analyzed in isolation, but rather that it should be assessed as a whole by measuring implications on cardiac filling and on tissue exchange, and looking for the part of the splanchnic system and limbs system on systemic venous return. ln conclusion, we have filled some of the gaps in the literature by examining the relationships between veins, physical exercise, recovery and venous pathology. However, questions remain regarding the methodology to be implemented for the evaluation of the venous system and the chronic effects of the intensive physical activity practice on the chronic venous disease prevalence

Full textThe French National Authority for Health recommended since 2012, the use of medical simulators in order to never conduct students’ first medical procedure on a patient during their training. Yet, the haptic realism of anthropomorphic simulators drift too far from reality. The presented work concerns the enhancement of the haptic realism of peripheral venous catheter insertion (PVCI) simulators.Following a literature review made to find the available mechanical data on human skin and vein useful to validate the design of PVCI simulators, a clinical trial was set up to analyze the biomechanics of PVCI gesture when performed by a nurse on the antebrachial area. The kinematic of the practitioner’s gesture and the puncture loads of skin and veins (rupture and friction loads) were obtained thanks to an experimental setup coupling force sensors and digital image correlation.Simultaneously, a sensory study was realized to sensiorally compare silicone-based prototypes and commercially available simulators with the forearms of a panel composed of healthcare students. To this end, a tactile material library composed of tactile descriptors related to PVCI was designed to confine and objectively quantify the sensation of the panel. The descriptors were obtained via an online survey sent to experimented practitioners who performed PVCI on a regular basis. We observed that vein prominence was the predominant descriptor in the realism of forearm substitutes. Preferences according to participants’ gender arose highlighting the need to adapt the mechanical and tribological properties of PVCI simulators to different categories of population. Principal component analysis was subsequently used to analyze the strengths and weaknesses of the presented prototypes resulting in the design of an enhanced prototype.Finally, a comparative study was carried out to mechanically assess the enhanced prototype in regard to human forearms. Results regarding puncture, indentation and friction tests showed that the developed simulator possessed mechanical and tribological properties closer to human skin and vein than venipuncture simulators currently used by healthcare students

Full textAtrial fibrillation (AF) is the most common form of cardiac arrhythmia in Humans. Pulmonary vein isolation (PVI) by radiofrequency (RF) ablation is the mainstay treatment for patients with symptomatic and drug refractory paroxysmal atrial fibrillation (PAF) as ectopic beats (triggers) from pulmonary veins (PVs) initiate AF, however PVI alone had limited role in treating patients with persistent atrial fibrillation (PsAF), due to additional involvement in left atrium (LA) by electrical and anatomical remodeling, creating more complex substrate (fibrosis) that maintain AF and therefore necessitate non-PV sites ablation to modify the arhythmogenic substrate. Atrial fibrillation recurrence mainly due to pulmonary vein (PV) electrical reconnection is common and remains the current issue that limits long term procedural success and generates extra costs due to repeated hospital admissions and repeated procedures. Point by point ablation using single tip catheter can be challenging, complex and time consuming, enhancing electrical reconnection as stable lesions are difficult to achieve. To overcome these limitations, new type of catheters such as balloon (cryoablation) and multi-electrode circular ablation catheters like PVAC and nMARQ, are now available. However, cryoablation balloon is limited by inability to adapt to anatomic PV variability and to ablate non-PV sites. This means that an additional ablation catheter has to be used for non-PV targets, and it certainly adds to the costs. The PVAC catheter lacks of irrigation increases the risk of thromboembolic complications. On contrary, nMARQ is an irrigated circular ablation catheter which in addition to its role in PVI, allows for non-PV/LA substrate ablation. Cardiac mapping and imaging tools are now commonly used to plan and guide AF ablation, such as invasive (Carto 3) and noninvasive (ECVUE) electroanatomic mapping (EAM), computed tomography (CT) and magnetic resonance imaging (MRI), respectively. We hypothesized that different strategies and technologies could improve PVI procedure and outcome by producing better lesions and by allowing for direct visualization of lesions. We therefore, assessed the role of circular, irrigated mapping and ablation catheter (nMARQ) in PAF and PsAF with or without noninvasive ECGi guidance, and advanced imaging technologies (MRI) after PVI. For that purpose, PVI data were obtained and processed acutely and at 3 months. Clinical follow up was evaluated at 1 year. The thesis consists of 3 parts: Part 1: How to improve pulmonary vein isolation lesion formation in patients with paroxysmal atrial fibrillation? Part 2: Circular catheter and persistant atrial fibrillation ablation. Part 3: The role for magnetic resonance imaging in assessing pulmonary vein reconnection after pulmonary vein isolation?

Full textPancreticoduodenectomy (PD) for adenocarcinoma was safer during the last decades but did not improve survival. We sought to determine if technical changes during PD could improve survival.A) In a first study, we determine the presence of K-ras mutation in the venous margin of 23 PD's specimens. Thirteen specimens had K-ras mutation (kras+ group) and 10 specimens did not (kras- group). Except K-ras mutation status, tumors of the 2 groups were not different when comparing major histological findings (margin status, lymph node invasion, perineural invasion…). Overall 1- and 3-years survival of patients of kras- group versus kras+ group were 80% versus 84,6% and 16,7% versus 0% (p=0,03), respectively. Median survival of patients of kras- group versus kras+ group were 24 months versus 16 months (p=0,04), respectively.B) In a second study, we compared 19 patients with “excessive” portal vein resection during PD (PV group) with 19 matched patients who underwent PD without venous resection (control group). Median survival of patients of PV group versus control group were 42 months versus 22 months (p=0,04), respectively.In conclusion, we showed that the « genic margin » concept was consistent. Systematic portal vein resection could avoid positive genic margin and might be benefic for patient who underwent PD for resecable adenocarcinoma

Full textPart I: Objectives. This work reports an anatomic study of swine heart in order to produce technical recommendations and achieve successful experimental cardiac surgical procedures. Methods. 16 swines were studied. Coronary artery vessels were surgically (n=13) and angiographically (n=10) assessed. Coronary venous vessels were studied by anatomic dissections (n=13) and retrograde injection of methylene blue through the coronary sinus (n=8). Results. Specific pericardial positioning of swine heart dramatically differs from human heart resulting in a limited access to ascending aorta and right atrium, requiring surgical precautions to perform a safe sternotomy and canulation of ascending aorta with an antegrade cardioplegia. Arterial coronary pattern is similar to that of humans (right dominant supply: 70%). Pig coronary sinus receives 4 main branches vs 3 in human sinus. Preliminary ligation of the left azygos vein is required to visualize the surface distribution of methylene blue within the venous vessels, thereby confirming an optimized perfusion of the left ventricle whereas the right ventricle remains poorly perfused. This asymmetry of perfusion results from a specific venous drainage of the right ventricle through small cardiac veins disconnected from coronary sinus. Conclusions. Anatomic knowledge of swine heart validated surgical guidelines for designing the model of self-myocardial retroperfusion and safely performing experimental cardiac surgical procedures. Part II: Background. Retrograde perfusion into the coronary sinus is used to deliver cardioplegia. We developed an in-vivo porcine beating-heart model of self-myocardial retroperfusion (SMR) using the venous route to supply myocardial oxygenation and sought to assess hemodynamic and cardiac responses triggered by SMR before and after a prolonged occlusion of the LAD.Methods. A bypass-line between the ascending aorta and the coronary sinus was made to perform a selective retrograde perfusion of the great cardiac vein with oxygenated blood (SMR). A Control group (n=6) was assigned to collect baseline data, and an SMR group (n=6) was dedicated to undergo SMR with occlusion of LAD for 240 minutes. Cardiac output (CO), maximal pressure in the LV (Pmax in-LV), stroke volume (SV), left ventricular ejection fraction (LVEF), diastolic durations, heart rate, and arterial systemic pressure were evaluated with conductance catheters for the following periods: basal (before SMR), SMR with patent LAD, and SMR with occluded LAD. In order to assess peripheral perfusion, patterns of sublingual microcirculation were analyzed. At the end of the procedures, the hearts were harvested for histology. Results. Echographic LVEF evaluation was affected by sternotomy, but conductance catheter evaluation was not. Following pericardiotomy, CO decreased by 7.51% (P < 0.05). SMR with patent LAD showed inotropic properties with improvements in CO, SV, Pmax in-LV and LVEF (P < 0.0001). Following LAD occlusion, SMR supplied myocardial oxygenation with hemodynamic compensation and preserved the peripheral perfusion. Histology confirmed no signs of infarct. Conclusions. SMR showed capacities to produce inotropic effects and protect against ischemia, opening interesting potential applications
