Haemodynamic monitoring is a significant component in the management of critically-ill patients. Flow-directed pulmonary artery catheters (PAC) are a simple and rapid technique for measuring several continuous or intermittent circulatory variables. The PAC is helpful in diagnosis, guidance of therapy, and monitoring therapeutic interventions in various clinical conditions, including myocardial infarction and its complications, non-cardiogenic pulmonary oedema and severely ill patients.The catheter is inserted through a large vein. The PAC is advanced, after ballooninflation with 1.5 mL of air, through the right ventricle across the pulmonary valve and into the pulmonary artery (PA). Finally, the catheter is advanced to the ‘wedge’ position. The pulmonary artery wedge pressure (PAWP) is identified by a decrease in pressure combined with a characteristic change in the waveform. The balloon should then be deflated and the PA tracing should reappear. Direct measurements include central venous pressure, pulmonary artery pressure, and PAWP, which during diastole represents the left ventricular end-diastolic pressure and reflects left ventricular preload. Cardiac output can be measured by thermodilution technique. Other haemodynamic variables can be derived from these measurements. Absolute contraindications are rare. Relative contraindications include coagulopathy and conditions that increase the risk of arrhythmias.

The right atrium (RA) is located on the upper right-hand side of the heart and has relatively thin walls. From an anatomical point of view, the RA comprises three basic parts, the appendage, the vestibule of the tricuspid valve, and the venous component (superior and inferior vena cava, and the coronary sinus) receiving the deoxygenated blood. The RA is a dynamic structure dedicated to receive blood and to assist right ventricular (RV) filling. The three components of atrial function are the reservoir function during ventricular systole, the conduit function which consists in passive blood transfer from veins to the RV in diastole, and the booster pump function in relation to atrial contraction in late diastole to complete ventricular filling. Right atrial function depends on cardiac rhythm (sinus or atrial fibrillation), pericardial integrity, RV load and function, and tricuspid function. Right atrial dimension assessment is limited in two-dimensional (2D) echocardiography. Right atrial planimetry in the apical four-chamber view is commonly used with an upper normal value of 18-20 cm2. Minor and major diameters can also be measured. Three-dimensional (3D) echocardiography could overcome the limitation of conventional echocardiography in assessing RA size. Right atrial function has been poorly explored by echocardiography both in physiological and pathological contexts. Although tricuspid inflow and tissue Doppler imaging of tricuspid annulus can be used in the exploration of RA function, 2D speckle tracking and 3D echocardiography appear promising tools to dissect RA function and to overcome the limitations of standard echocardiography.

Modern assessment of left ventricular (LV) diastolic function should be based on the estimation of degree of LV filling pressure (LVFP), which is the true determinant of symptoms/signs and prognosis in heart failure.In order to achieve this goal, standard Doppler assessment of mitral inflow pattern (E/A ratio, deceleration time, isovolumic relaxation time) should be combined with additional manoeuvres and/or ultrasound tools such as: ◆ Valsalva manoeuvre applied to mitral inflow pattern. ◆ Pulmonary venous flow pattern. ◆ Velocity flow propagation by colour M-mode. ◆ Pulsed wave tissue Doppler of mitral annuls (average of septal and lateral E′ velocity).In intermediate doubtful situations, the two-dimensional determination of left atrial (LA) volume can be diagnostic, since LA enlargement is associated with a chronic increase of LVFP in the absence of mitral valve disease and atrial fibrillation.Some new echocardiographic technologies, such as the speckle tracking-derived LV longitudinal strain and LV torsion, LA strain, and even the three-dimensional determination of LA volumes can be potentially useful to add further information. In particular, the reduction of LV longitudinal strain in patients with LV diastolic dysfunction and normal ejection fraction demonstrates that a subclinical impairment of LV systolic function already exists under these circumstances.

Haemodynamic resuscitation should target goals that reflect the tissue oxygen needs of an individual patient. Venous oximetry may be such a tool. Oxygen saturation of blood in the pulmonary artery contains venous blood from the whole body and is referred to as mixed oxygen saturation (SvO2). Measurement of oxygen saturation in blood obtained from a central venous catheter is referred to as central venous oxygen saturation (ScvO2). Both values are not identical since a catheter placed into the superior vena cava only represents venous blood draining the upper body. While it is not possible, in the clinical setting, to predict SvO2 from ScvO2, changes in SvO2 are adequately mirrored by changes in ScvO2. Post-operative patients and patients admitted to intensive care with a low ScvO2 show a higher morbidity and mortality. Early goal-directed therapy (EGDT) combines several haemodynamic goals into a treatment algorithm, including a ScvO2 target. However, recent studies do not support the systematic use of this protocolized approach. A normal value of SvO2 or ScvO2 saturation does not always exclude tissue hypoxia, since it is not possible to identify an inadequate oxygen supply in single organs. A further limitation of this technique is that organ dysfunction can progress, or serum lactate increases, despite normal or even increased venous oximetry values.

An increased blood lactate level (hyperlactataemia) is commonplace in critically-ill patients. Lactate is usually measured with the aim of detecting tissue hypoxia, but this is an oversimplification as aerobic processes can also result in increased levels. Understanding of the various anaerobic and aerobic mechanisms of production and clearance is essential for the correct interpretation of hyperlactataemia. Despite the broad differential diagnosis, hyperlactataemia generally predicts adverse outcomes. The consistency of its prognostic value emphasizes its place in the risk stratification of critically-ill patients. Lactate clearance was non-inferior to central venous oxygen saturation as a goal of early resuscitation in patients presenting to the emergency department with severe sepsis or septic shock. Therapy guidance by lactate monitoring significantly reduced hospital mortality in ICU patients admitted with hyperlactataemia after adjustment for predefined risk factors, a finding consistent with important secondary endpoints. These results confirm that lactate monitoring offers clinical benefit and should be incorporated within a goal-directed therapy strategy.

This chapter explores the idea that myths are a key element in the creation of landscape, whereby mere space is transformed into resonant place through the matrix of time. In spite of the local nature and uses of many mythical narratives, it is argued here that mobility is central to understanding the power and value of the mythical past. Not only do the travels of itinerant heroes link together mythical venues, but later travels, such as those of characters in Herodotus’ Histories or of authors themselves, illustrated by Pausanias, activate the resonances of the landscape and give it a temporal dimension by triggering mythical narratives associated with the places passed through. These narratives in turn evoke additional journeys which contribute to the creation of a spatial network that binds together the Mediterranean world. Thus, myth offers a medium through which past and present are linked across space.

This chapter discusses blackface minstrel troupes, British regimental bands and jazz orchestras performing in India from the mid-nineteenth century to the mid-twentieth century. It details their challenges and strategies for success, and suggests that their capacity to facilitate cosmopolitan encounters in the wider world contributed to their popularity and value. It first introduces problems and practicalities in maintaining bands performing British military music in India in the mid- and late-nineteenth century. It then briefly introduces the character and scope of ballroom dance music and blackface minstrelsy in urban centres. To end, it examines the character of jazz orchestras between the 1920s and 1940s, detailing the role of the gramophone industry, entertainment venues such as hotel and cinema hall ballrooms, and the Allied military in Calcutta on their growth and profitability. In each example, it articulates thoughts on the role and usefulness of orchestras and notes issues confronting their musicians.

In modern times, the development of new survey, navigation, diving, and remotely operated vehicle technologies have made the location, exploration, and excavation of historic shipwrecks feasible to the general public. The debate on the value of underwater cultural heritage is recent and the issues of protecting underwater sites are now accepted. The diving community has been engaged in this debate for several decades, and a wide variety of viewpoints have developed. Museums focusing on underwater cultural heritage serve as platforms to foster discussions on submerged cultural resource protection. As any archaeological site, shipwrecks excite the general public. While museums provide a venue to share the story of the wrecks, or the historical contexts in which they existed, there are multiple ways to share this information with the public that will allow them a first-hand experience with a shipwreck. This notion has given rise to the concept of heritage tourism.

This chapter’s examination of the registers of the Comédie-Italienne provides more detailed context to Goldoni’s career at the Hôtel de Bourgogne by focusing specifically on the status of the author in this theatre. A detailed account of the payments made to the opéra-comique authors working alongside Goldoni illustrates his particular value to the troupe. This data modifies the straightforward vision of the Comédie-Italienne as a lucrative commercial venue; however, a consideration of relative timescales for payment also suggests that although payments there were often low, the theatre’s accessibility with respect to the Comédie-Française could nonetheless still make it attractive. Goldoni’s employment by the Italian troupe is posited as not only an attempt to address increasingly pressing economic concerns, as recent re-evaluations suggest, but also as part of a move by the theatre’s administration to re-enter an older logic of prestige and historical status.

This chapter, which yanks the reader from mid-twentieth-century Detroit to early twenty-first-century Guangdong Province, recounts the author's discovery that the labor question can have many different configurations, especially when some of the most important and characteristic enterprises of our day are the big-box retailers, whose employee rolls and annual revenues now far outrank those of the largest manufacturing companies. It appears that the essence of the twenty-first-century labor question no longer resides at the point of production in a struggle between workers and the owners of the factories in which they labor. Instead, the site of value production is found at every link along a set of global supply chains, in which the manufacturer and the warehouse operator, the ports and the shipping companies, as well as the retailers and their branded vendors jockey for power and profit. In this disaggregated system, legal ownership of the forces of production has been divorced from operational control, making accountability for labor conditions diffuse and knowledge of the actual producers far from transparent.

Antenatal and intrapartum fetal monitoring aim to identify the beginning of the process of fetal hypoxia before irreversible fetal damage has taken place. Fetal movement counting by the mother has not been reported to be of any benefit. The biophysical profile score, incorporating ultrasound and fetal heart rate monitoring, has not been proven to reduce perinatal mortality in randomized trials. Doppler ultrasound allows the exploration of the perfusion of different fetal organ systems and provides data on possible hypoxia and fetal anaemia. Maternal uterine artery Doppler can be used to select women with a high risk for intrauterine growth restriction and pre-eclampsia but does not directly provide information on fetal status. Umbilical artery Doppler has been shown to reduce perinatal mortality significantly in high-risk pregnancies (but not in low-risk women). Adding middle cerebral artery Doppler to umbilical artery Doppler does not increase accuracy for detecting adverse perinatal outcome. Ductus venosus Doppler demonstrates moderate value in diagnosing fetal compromise; it is not known whether its use adds any value to umbilical artery Doppler alone. Cardiotocography (CTG) reflects the interaction between the fetal brain and peripheral cardiovascular system. Prelabour routine use of CTG in low-risk pregnancies has not been proven to improve outcome; computerized CTG significantly reduces perinatal mortality in high-risk pregnancies. Monitoring the fetus during labour with intermittent auscultation has not been compared to no monitoring at all; when compared with CTG no difference in perinatal mortality or cerebral palsy has been noted. CTG does lower neonatal seizures and is accompanied by a statistically non-significant rise in caesarean delivery. Fetal blood sampling to detect fetal pH and base deficit lowers caesarean delivery rate and neonatal convulsions when used in adjunct to CTG. Determination of fetal scalp lactate has not been shown to have an effect on neonatal outcome or on the rate of instrumental deliveries but is less often hampered by technical failure than fetal scalp pH. Analysis of the ST segment of the fetal ECG (STAN®) in combination with CTG during labour results in fewer vaginal operative deliveries, less need for neonatal intensive care, and less use of fetal blood sampling during labour, without a change in fetal metabolic acidosis when compared to CTG alone.
