Abstract:
<p>Any struoture in navigable waters constitutes a hazard to shipping and is itself vulnerable to damage or destruction in the event of vessel collision. Worldwide vessel traffic and the average size of vessels continue to lncrease. At the same time, ever more bridges crossing navigable waterways are being planned and constructed, sometimes with inadequate navigation clearance and/or lnadequate protection.<p> The objective of this publication is to provide information and guidelinesfor engineers charged with the planning and design of new bridges, navlgation channels, and prevention and protection measures. Lt offers advice on up­grading and retrofrtting existing bridges and navigation channels. And lt provides the means to evaluate the safety of bridges, vessels, persons and the environment. <p>After reviewing some basics o! navigatlon and vessel traffic, and consider­ing risk acceptance and collision risk, the publication examines vessel impact forces on bridges and proposes appropriate bridge design criteria. Prevention measures, such as regulations and management systems. And protectlon measures and systems are also described. Major international research projects have provided the analytical basis for the publication, including the development of vessel collision guide specifi­c-atrons for the Federal Highway Administration in the USA and the vessel colllsion design crrteria developed for the Great Bell Crossing in Oenmark. <p>Prepared by Ole Damgaard LARSEN, Chairman of the IABSE Working Group "Ship Collision with Bridges'', lhis 132 page publlcation is a must for any engineer dealing with structures in navigable waters.

Abstract:
This chapter highlights the importance of considering people as integral to foodwebs. Despite extensive recent research on coupled human-natural systems, lacking are models that incorporate human behavior in a way that yields pragmatic insights into the management of multispecies fisheries. Using the US West Coast commercial fisheries system as a case study, this chapter develops a novel network approach of linking the social system (i.e., fishing communities) to the ecological system (the fish). The analysis reveals that fisheries that seem unconnected biologically, such as benthic Dungeness crabs and pelagic tuna, can in fact be strongly linked by fishing vessels that are active in both fisheries. Understanding how human behavior connects seemingly disparate ecological systems has important implications for fisheries managers seeking to balance human well-being with sustainable populations of fish.

Abstract:
Ischaemic stroke is the most common cause of stroke around the world. It is a complex disease with a range of causes, manifestations, outcomes, and treatments. As the therapeutic time window to rescue or ‘protect’ the brain from ischaemic damage is extremely short, effective treatment requires coordinated systems of care, which commence in the prehospital paramedical setting and continue through the emergency department into the critical care environment, neurology ward, rehabilitation, and re-settlement back home. Successful outcomes from ischaemic stroke can be achieved through the effective use of thrombolytic therapy to re-canalize an occluded vessel and re-perfuse the ‘at risk’ area of the brain. Other aspects of management include the prevention of complications of the neurological (cerebral) disability, timely introduction of rehabilitation, realistic goal-setting towards satisfactory recovery, and secondary prevention measures to reduce the high risk of recurrent stroke and other serious vascular events.

This chapter reviews advances in pathogenesis; European League Against Rheumatism/American College of Rheumatology (EULAR/ACR) classification criteria with clinical, laboratory, and ultrasound criteria for classification as polymyalgia rheumatica (PMR); the heterogeneity and overlap between PMR, inflammatory arthritis, and large-vessel vasculitis as illustrated by representative cases; recent guidelines on early and correct recognition, investigations, and management of PMR; the scope of disease-modifying agents; socio-economic impact, outcomes, and patient experience in PMR. It also discusses areas for future research including clinical trials with biological agents and newer steroid formulations, standardized outcome assessments, and the search for better biomarkers in PMR. PMR is one of the common inflammatory rheumatic diseases of older people and represents a frequent indication for long-term glucocorticoid (GC) therapy. It is characterized by abrupt-onset pain and stiffness of the shoulder and pelvic girdle muscles. Its management is subject to wide variations of clinical practice and it is managed in primary or secondary care by general practitioners (GPs), rheumatologists, and non-rheumatologists. The evaluation of PMR can be challenging, as many clinical and laboratory features may also be present in other conditions, including other rheumatological diseases, infection, and neoplasia. PMR is usually diagnosed in the primary care setting, but standard clinical investigations and referral pathways for suspected PMR are unclear. The response to standardized therapy is heterogeneous, and a significant proportion of patients do not respond completely. There is also an overlap with inflammatory arthritis and large-vessel vasculitis for which adjuvant disease-modifying medications are often used. Prolonged corticosteroid therapy is associated with a variety of side effects, especially when high-dose glucocorticoid therapy is employed. Giant cell arteritis (GCA) is also often linked to PMR. It is a vasculitis of large- and medium-sized vessels causing critical ischaemia. GCA is a medical emergency because of the high incidence of neuro-ophthalmic complications. Both conditions are associated with a systemic inflammatory response and constitutional symptoms. The pathogenesis is unclear. The initiating step may be the recognition of an infectious agent by aberrantly activated dendritic cells. The key cell types involved are CD4+ T cells and macrophages giving rise to key cytokines such as interferon-γ‎ (implicated in granuloma formation), PDGF (intimal hyperplasia), and interleukin (IL)-6 (key to the systemic response). The pathogenesis of PMR may be similar to that of GCA, although PMR exhibits less clinical vascular involvement. The mainstay of therapy is corticosteroids, and disease-modifying therapy is currently indicated in relapsing disease.

Abstract:
Diagnosis and treatment of acute stroke has advanced considerably in the past 2 decades. Most notably, in cases of ischemic stroke, intravenous alteplase has become the standard of medical treatment despite its multiple contraindications and limited time window. More recently, trials have proven that endovascular thrombectomy is superior to medical therapy alone, advancing the standard of care for patients who present with acute ischemic stroke from a large vessel occlusion and salvageable brain tissue. The treatment of hemorrhagic stroke now involves the use of novel pharmacological agents and advanced minimally invasive technology. Important changes have also occurred at the levels of hospital organization and treatment decision-making. Such changes in organization and designation of hospitals with distinct levels of stroke care and the variety of stroke protocols now requires team work of emergency medical services (EMS), Emergency Department, stroke neurologists, neurosurgeons, and neurointerventionalists. This book provides an overview of the modern medical and surgical options for the treatment of patients with acute ischemic and hemorrhagic strokes. The pivotal role of EMS in prehospital evaluation and triage of a stroke patient and the levels of stroke systems of care are discussed. In addition, the current guidelines on the management of acute stroke, with the focus on early care of acute stroke patients at the Emergency Department and the first 24 hours of hospital admission, are reviewed. Each chapter contains a discussion of common clinical scenarios including initial management steps, practical points, and common pitfalls.
