Abstract:

Reikšminiai žodžiai: prieširdžių virpėjimas, vidinė darna. Darbo tikslas. Ištirti prieširdžių virpėjimu sergančių pacientų vidinę darną bei jos ryšį su sociodemografiniais ir medicininiais veiksniais. Tyrimo medžiaga ir metodai. Darbas atliktas Vilniaus miesto klinikinės ligoninės Vidaus ligų klinikos skyriuose. Tyrime dalyvavo 229 sergantys prieširdžių virpėjimu pacientai, 50–70 metų, amžiaus vidurkis 62,36 + 6,35 m., 130 vyrų (56,8 proc.) ir 99 moterys (43,2 proc.). Buvo tiriamos pacientų vidinės darnos sąsajos su sociodemografiniais veiksniais, prieširdžių virpėjimo trukme, tipu, klinikinių požymių sunkumu ir gretutinėmis ligomis. Vidinė darna buvo vertinama A. Antonovsky (1987) vidinės darnos skale („Sence of Coherence, SOC-13“). Klinikinė būklė vertinta remiantis klinikiniu ištyrimu ir ligos istorijų duomenimis. Statistinė analizė. Kokybinių duomenų skirtumams tarp atskirų respondentų grupių vertinti naudotas Chi kvadrato kriterijus (c2). Vidurkių tarp dviejų nepriklausomų respondentų grupių skirtumams vertinti naudotas neparametrinis Mann-Whitney kriterijus. Esant daugiau nei dviem respondentų grupėms, skirtumams tarp jų vertinti buvo naudojamas neparametrinis ranginis Kruskal-Wallis kriterijus nepriklausomoms imtims. Buvo laikoma, kad skirtumai statistiškai reikšmingi, kai apskaičiuotasis reikšmingumo lygmuo p < 0,05. Darbo rezultatai ir išvados. Prieširdžių virpėjimu sergantiems pacientams nustatytas vidutinis vidinės darnos lygis (balų vidurkis – 46,34). Sociodemografiniai veiksniai, turintys teigiamos įtakos bendram VD lygiui sergant prieširdžių virpėjimu, yra lytis, pajamos ir šeiminė padėtis. Vyrai visose srityse pasižymi aukštesne vidine darna negu moterys. Respondentų, turinčių didesnes pajamas, vidinė darna aukštesnė suprantamumo, prasmingumo ir bendro vidinės darnos vertinimo srityse. Nevieniši pacientai pasižymi aukštesniu prasmingumo ir bendru vidinės darnos lygiu. Amžius ir išsilavinimas vidinei darnai įtakos neturėjo. Prieširdžių virpėjimo trukmė, tipas, krūtinės angina ir širdies nepakankamumas 50–70 metų amžiaus grupėje įtakos vidinei pacientų darnai neturi. Žemesne bendra vidine darna ir jos sumažėjimu suprantamumo bei situacijos valdymo ir kontrolės srityse pasižymi III EHRA simptomų klasės pacientai, palyginti su II EHRA simptomų klasės pacientais

Abstract:

Objective To develop an easy surgical approach to facilitate clinical management. Study Design A novel transnasal endoscopic 3-step surgical method for vidian neurectomy was designed and tried in 91 cases with a mild-to-severe degree of allergic and nonallergic rhinitis refractory to routine medical therapy. Setting Endoscopic vidian neurectomy requires accurate localization of the vidian canal. However, it is not easy to localize during surgery because of its deep location and the complex anatomy of the pterygopalatine fossa. Subjects and Methods This technique consists of 3 steps, including transnasal endoscopic perforation of the anterior wall of the sphenoidal sinus as the first step and removal of the anterior wall until the exposure of the vidian canal in the junction between the anterior wall and the floor of the sphenoid sinus as the second step. The last step is the accurate resection and cauterization of the vidian nerve. In some cases in which the sphenoid sinus developed well with a big lateral space, an extended procedure of posterior ethmoidectomy was included to allow good exposure of the vidian canal. Results Using this technique, successful endoscopic vidian neurectomy in this series of patients was confirmed by both histology and Schirmer test, showing its distinct advantages of easy localization of the vidian canal and less risk of injury to the nerve and vessel bundles within the pterygopalatine fossa. Conclusion Taken together, this novel 3-step procedure of endoscopic vidian neurectomy plus an extended procedure guarantees good exposure of the vidian canal and therefore accurate vidian neurectomy.

Abstract:

Abstract OBJECTIVE The vidian canal, the conduit through the sphenoid bone for the vidian nerve and artery, has become an important landmark in surgical approaches to the cranial base. The objective of this study was to examine the anatomic features of the vidian canal, nerve, and artery, as well as the clinical implications of our findings. METHODS Ten adult cadaveric specimens and 10 dried skulls provided 40 vidian canals for examination with ×3 to ×20 magnification and the endoscope. RESULTS The paired vidian canals are located in the skull base along the line of fusion of the pterygoid process and body of the sphenoid bone. The canal opens anteriorly into the medial part of the pterygopalatine fossa and posteriorly at the upper part of the anterolateral edge of the foramen lacerum. The vidian nerve, when followed posteriorly, reaches the lateral surface of the anterior genu of the petrous carotid and the anteromedial part of the cavernous sinus where the nerve is continuous with the greater petrosal nerve. The bone surrounding the upper part of 12 of 20 vidian canals protruded into the floor of the sphenoid sinus and one canal had a bony dehiscence that exposed its contents under the sinus mucosa. Nine petrous carotid arteries (45%) gave rise to a vidian artery, all of which anastomosed with the vidian branch of the maxillary artery in the vidian canal or pterygopalatine fossa. The vidian canal can be exposed by opening the floor of the sphenoid sinus, the posterior wall of the maxillary, the posterior part of the lateral wall of the nasal cavity, and the medial part of the floor of the middle fossa. CONCLUSION The vidian canal and nerve are important landmarks in accessing the anterior genu of the petrous carotid, anteromedial part of the cavernous sinus, and petrous apex.

Abstract:

Background Vidian neurectomy was an option for treating allergic rhinitis in the past but outcomes varied. A modified transsphenoidal approach is proposed to simplify endoscopic vidian neurectomy. The postoperative evaluation of rhinorrhea, sneezing, and recurrence was investigated. Methods A total of 317 patients with refractory allergic rhinitis underwent 414 transsphenoidal vidian neurectomies from September 2006 to December 2010. A rigid nasal endoscope was used through a transsphenoidal approach to reach the vidian canal inside the sphenoid sinus (type I) or through its anterior opening into the pterygopalatine fossa (type II) and to cut or cauterize the vidian nerve. The surgical outcomes were analyzed for patients with at least 6 months of follow-up. Results Our approach was successful in 90.3% of the 414 vidian neurectomies. Vidian neurectomy was successful via the type I approach in 27 sides and type II approach in 347 sides. The short-term surgical outcomes of 163 patients who underwent a total of 236 vidian neurectomies with at least 6 months of follow-up were analyzed. Immediate, complete cessation of sneezing and rhinorrhea occurred uniformly. Three recurrences were detected during the 1–2 years of follow-up. The symptom of dry eye was reported for 172 surgical sides, but only 6 had persistent symptoms for > 6 months. Conclusion The transsphenoidal approach in a vidian neurectomy is a simple method that removes the need for sphenopalatine artery ligation and causes less surgical morbidity. However, the possibility of recurrence of this condition in the long term needs further investigation.

Abstract:

Abstract BACKGROUND Endoscopic endonasal surgery of the skull base requires expert knowledge of the anatomy and a systematic approach. The vidian canal is regarded as a reliable landmark to localize the petrous internal carotid artery (pICA) near the second genu, which can be used for orientation in deep skull base approaches. There is controversy about the relationship between the vidian canal and the pICA. OBJECTIVE To further establish the vertical relationship between the vidian canal and the pICA to aid in surgical approaches to the skull base. METHODS We utilized a collection of institutional review board-approved computed tomographic (CT) angiograms (CTAs). Fifty CTAs were studied bilaterally for 100 total sides. The vidian canal was visualized radiographically to determine whether it terminates below, at, or above the level of the pICA. RESULTS Sixty-six of 100 vidian canals terminated inferior to the pICA (66%), which was the most common relationship observed. The average distance inferior to the pICA was 1.01 mm on the right, 1.18 mm on the left, and 1.09 mm of the total 66 sides. Less commonly, the vidian canal terminated at the level of the pICA canal in 34 sides (34%). The vidian canal was not observed to terminate superior to the pICA in any of the 50 CTAs studied. CONCLUSION The vidian canal terminates inferior to the pICA most commonly, but often terminates at the level of the pICA. Careful drilling clockwise inferior to superior around the vidian canal should allow for safe pICA localization in most cases.

Abstract:

Salah satu cara yang biasa digunakan untuk menambah daya tarik sebuah iklan adalah dengan memakai pendekatan retorika visual. Iklan obat antiseptik Vidisep adalah salah satu iklan yang menggunakan pendekatan retorika visual. Iklan Vidisep meraih penghargaan tertinggi kategori iklan media cetak dalam Citra Pariwara tahun 2016. Hal ini menandakan sebagai sebuah karya visual, iklan ini dianggap telah memenuhi persyaratan dari segi kreativitas dalam visualisasinya. Namun demikian belum diketahui bagaimana korelasi antara iklan yang kreatif dengan respons dari konsumen. Penelitian yang dilakukan bertujuan untuk mengetahui respons konsumen terhadap iklan yang memakai pendekatan retorika visual. Metode yang dipakai dalam penelitian ini adalah metode kualitatif. Pengumpulan data dilakukan melalui wawancara mendalam. Pada tahap awal dilakukan penelitian yang ditujukan untuk mengidentifikasi jenis retorika visual dalam iklan Vidisep. Sedangkan tahap kedua merupakan penelitian utama, yaitu berusaha mengetahui respons konsumen terhadap iklan. Hasil analisis pada penelitian tahap pertama menunjukkan bahwa iklan Vidisep memakai mode figuratif retorika jenis trope, dengan operasi destabilisasi. Sementara hasil penelitian tahap kedua menunjukkan respons konsumen yang berkaitan dengan sikap terhadap merek menunjukkan bahwa reputasi produsen mampu mengurangi keraguan akan kualitas produk. Sikap terhadap iklan dan eksekusinya hanya mampu memberikan hiburan. Pemaknaan yang terjadi pun tidak berpengaruh banyak untuk mengatasi keraguan pada merek.

Abstract:

✓ The purpose of this study was to describe the technique used to safely identify the petrous carotid artery during expanded endonasal approaches to the skull base. A series of 20 cadaveric studies was undertaken to isolate the vidian artery and nerve and to use them as landmarks to the petrous internal carotid artery (ICA). Twenty-five consecutive paraclival endoscopic cases were also reviewed to determine the consistency of the vidian artery in vivo as an intraoperative landmark to the ICA. These data were then correlated with results from a separate study in which computed tomography scans from 44 patients were evaluated to delineate the course of the vidian canal and its relationship to the petrous ICA. In all 20 cadaveric dissections and all 25 surgical cases, the vidian artery was consistently identified and could be reliably used as a landmark to the ICA. The correlation between anatomical and clinical data in this paper supports the consistent use of the vidian artery as an important landmark to the petrous ICA.

Abstract:

Object Gamma Knife surgery has recently been used to treat patients with cluster headaches. Both the trigeminal nerve root and the pterygopalatine ganglion (PPG) have been targeted. However, there are no clear-cut anatomical landmarks on computed tomography (CT) scans or magnetic resonance images that accurately identify the PPG. Therefore, the authors performed microsurgical dissections on latex-injected cadaver heads to expose the PPG and correlated the findings with thin-slice axial CT scans obtained in the same heads to determine how best to target the PPG. Methods Three cadaver heads (five sides) previously injected with colored latex were dissected using skull base approaches and microsurgical techniques to identify the PPG and surrounding structures. Measurements were then made to different osseous anatomical landmarks such as the foramen rotundum, vidian canal, and so on. The PPG was marked with a radiopaque marker and thin-slice CT scans were obtained in the cadaver heads to develop some correlates that could be used to identify where the PPG is located on CT scans. Results The PPG was clearly identified in all specimens and had a mean diameter of 3.58 ± 0.6 mm. The PPG was always located in the same plane (lateral and vertical) as the vidian canal and was located on average 2.7 ± 0.3 mm from the end of the canal. The vidian canal was clearly identified on coronal CT scans and had a diameter of 3.05 mm. Conclusions There was a clear and constant relationship between the PPG and vidian canal. The vidian canal is easily identified on coronal CT scans and can be used as a landmark to target the PPG with the Gamma Knife.

AbstractIntroduction:To present a case of a child with a desmoplastic fibroma of the vidian canal, compressing the vidian nerve.Case report:A 12-year-old girl with several years' history of right-sided facial pain was referred to our institution. Magnetic resonance imaging and computed tomography scans showed an expansile mass involving the right vidian canal. The patient underwent a complete endoscopic surgical resection aided by the FusionTM ENT navigation system. This was performed through a transnasal, trans-septal, trans-sphenoidal route via the right nostril, and achieved macroscopic clearance with minimal peri-operative morbidity. A biopsy of the lesion showed a fibro-osseous lesion consistent with desmoplastic fibroma.Conclusion:Diagnosis and resection of this rare lesion at an earlier stage would have avoided delays in resolving the child's disabling pain. This emphasises the importance of early referral of unusual cases to tertiary centres.

We have developed several detector systems for use in observational astronomy. These include a rapid scanning spectrometer (Jeffers and Weller, 1973), a rapid chopping spectrophotometer (Stiff and Jeffers, 1978) and an intensified, silicon vidicon detector for use either for spectrophotometry (Jeffers, Stiff and Weller, 1983) or direct imaging (Jeffers, 1982). These detector systems have been used for rapid variability studies of emission line strengths and profiles in Wolf-Rayet and Of stars (Weller and Jeffers, 1979; Jeffers, Stiff and Weller, 1985) and for low dispersion spectrophotometry of these stars (Jeffers and Weller, 1985). Here we report on the spectrophotometric performance of the intensified, silicon vidicon detector (RCA 4804H) when used with a low dispersion spectrograph.

Odkar se slovenski etnografi vedno bolj lotevajo socioetnografskih študij krajev, občin in manjših predelov, so se povečale možnosti načrtnega zbiranja nekaterih ljudskih znanj, ki so zanimiva ne le za etnografijo, temveč tudi za lokalno geografijo, jezikoslovje in lokalno zgodovino. Za ta posvet je bil predviden poseben referat o zbiranju toponimov vseh vrst, za kar obstoja že razmeroma precejšnja literatura v jezikoslovju, geografiji, v planinskem vestniku itd. Abstract:

Aiming at the problem that the dynamic attitude of small UAVs can’t be measured accurately, this paper describes a ground target positioning method based on the multi-images forward intersection. The parameters of the corresponding image rays are carried out in the local geographic coordinate system, and then these parameters are used to get the target 3-D coordinates. The flight tests have been finished. The test system is composed of a small UAV with a wingspan of 1.2 meters, a miniature vidicon and the micro autopilot developed by our laboratory. The miniature vidicon is calibrated with the active method and a red target is placed on the ground beforehand. Straipsnyje teigiama, kad mokslas ir menas yra pagrindiniai dvasinės kultūros elementai, kurių egzistavimas neatskiriamas nuo žmogaus vidinės kultūros. Kadangi jie nevienodai susiję su vidine kultūra, todėl juos skirtingai nulemia ir visuomenės-ekonominės formacijos. Mokslo ir subjektyvios realybės vienovė egzistuoja tik mokslinio tyrimo stadijoje: euristinis tyrėjo ieškojimas neatskiriamas nuo subjekto intuityvios pasąmonės. Mokslinė tiesa, objektyvuota teorijoje (schemoje, modelyje), reiškia minimalų ryšį su mokslininko vidine kultūra ir maksimalų su materija (objektyviais daiktų ir žmonių tarpusavio santykiais). Estetinis vidinės kultūros momentas glūdi žmogaus psichikos giluminiame sluoksnyje – pasąmonėje. Vidinė kultūra, susijusi su kūrybiniais ieškojimais moksle, pasireiškia magijos, kulto ar tabu neigimu. Subjektyvios realybės simbolinės ar archetipinės išraiškos potencija neigiama mokslinių tiesų. Humaniškais siekiais pagrįstoje socialistinėje visuomenėje mokslas padeda plėtoti dvasinę kultūrą. Mokslinių galimybių praktinis panaudojimas kiekvienam gamintojui garantuoja vis ilgėjantį laisvalaikį, būtiną dvasinės kultūros egzistencijai.

AbstractSurgical treatment of chronic rhinosinusitis is indicated after failure of correctly conducted maximal drug therapy or the occurrence of complications. Radical maxillary sinus surgery has been abandoned nowadays, to the detriment of endoscopic sinus surgery, for several reasons, such as: increased incidence of complications, decreased healing rate compared to the endoscopic technique. The literature cites many situations in which the Caldwell-Luc procedure is used as a first-line surgical technique: recurrent chronic rhinosinusitis, malignant tumors of the maxillary sinus extending to the lateral wall of the nasal fossa and the pterygomaxillary space, the cases where an extensive approach to the pterygopalatine fossa is required – for ligation of the internal maxillary artery or the approach of the vidian canal in vidian neurectomy.The authors highlight the use of Caldwell-Luc procedure in endoscopic sinus surgery era, by reviewing the complications rates, indications and long-term effectiveness of the two surgical techniques.

The detective quantum efficiency (DQE) characterizes the measurement uncertainty introduced by a detector. A formulation of the DQE for CCD and vidicon-based X-ray integrating detectors is developed and illustrated with five model detectors. Introduction Identifying the internal carotid artery (ICA) when managing petroclival and infratemporal fossa pathology is essential for the skull base surgeon. The vidian nerve and eustachian tube (ET) cartilage come together at the foramen lacerum, the vidian–eustachian junction (VEJ). The ICA position, relative to the VEJ is described. Methods Endoscopic dissection of adult fresh-frozen cadaver ICAs and a case series of patients with petroclival pathology were performed. The relationship of the VEJ to the ICA horizontal segment, vertical segment, and second genu was assessed. The distance of the ICA second genu to VEJ was determined in coronal, axial, and sagittal planes. The length of the vidian nerve and ET was measured from the pterygopalatine fossa (PPF) and nasopharyngeal orifice to the VEJ. Results In this study, 10 cadaver dissections (82.3 ± 6.7 years, 40% female) were performed. The horizontal petrous ICA was at or behind VEJ in 100%, above VEJ in 100%, and lateral to VEJ in 80%. The vertical paraclival segment was at or behind VEJ in 100%, above in 100%, and medial in 100%. The second genu was at or behind VEJ in 100% (3.3 ± 2.4 mm), at or above in 100% (2.5 ± 1.6 mm), and medial in 100% (3.4 ± 2.0 mm). The VEJ was successfully used to locate the ICA in nine consecutive patients (53.3 ± 13.6 years, 55.6% female) where pathology was also present. The VEJ was 15.0 ± 6.0 mm from the ET and 17.4 ± 4.1 mm from the PPF. Conclusion The VEJ is an excellent landmark as it defines both superior and posterior limits when isolating the ICA in skull base surgery.

AbstractBackground:Vidian neurectomy has been proposed as a surgical option for rhinitis refractory to medical treatment. However, the evidence base for its benefit remains controversial.Methods:A systematic review was performed. Studies reporting original data on patients with rhinitis treated by vidian neurectomy were included. Patient-reported outcome measures were the primary outcome investigated; specific peri-operative morbidities were the secondary outcome.Results:A total of 1012 articles fulfilled the search criteria, 32 of which were included in the study. Patient-reported outcome measures were compared before and after surgery in eight studies. There were 529 patients represented in these trials. Significant improvement in rhinorrhoea was reported in all eight studies. Temporary dry eyes was reported in 24.63 per cent of cases (272 out of 1104 cases). There was no report of cranial nerve deficit or eye movement disturbance.Conclusion:Endoscopic vidian neurectomy does have a role in the surgical management of refractory rhinitis, particularly in patients with non-allergic rhinitis, but a well-designed cohort trial would be advantageous to clarify long-term outcomes.
