Abstract:
Mechi Eye Care Center was established in a rented building in Nov. 1996 with thehelp of Eye Care Himalaya, The Netherlands. In the last four and half years patientflow has increased gradually and the present facility is stretched to the limit.Provision of efficient and good quality treatment, close monitoring on attainment ofgood surgical quality and visual outcome are the main aims of this eye centre.Community involvement (such as active participation in screening camp and landdonation for new proposed eye hospital) has been exemplary. Trained Ophthalmicassistants are fully utilized thus enabling Ophthalmologists to devote their time insurgery and examination of referred patients. Preventive maintenance of instruments& equipment is a routine practice in this eye center.It is facing the problem with the space and the human resources (mainlyOphthalmologist). Eye Care Himalaya has planned to construct a new building of thehospital and train required human resources. We hope that after completion of thenew hospital building and with proper and sufficient number of trained manpower atleast one thousand patients can be examined and two hundred patients can be operatedevery day. Thus, the Hospital will be a gift to the blind people of this region to preventand cure their blindness and at the same time, it could play a significant role to eliminatethe avoidable blindness in this part of the world and contribute to the Mission - Vision2020:The Right to Sight.

Abstract:
Background: Eye care provision is currently insufficient to meet the population’s eye health needs in Kenya. Many people remain unnecessarily visually impaired or at risk of becoming so due to treatable or preventable conditions. A lack of access and awareness of services are key barriers, in large part due to their being too few eye care providers in the health system for this unmet need. Methods: A hospital-based, retrospective analysis of patients who attended Kitale eye unit, Trans Nzoia County, Kenya from 1st January 2013 to 31st December 2015. Age and sex standardized hospital attendance rates by residence, age group, and sex were calculated for Trans Nzoia county and each subcounty. The changing trends in attendance rates were estimated by calculating the difference between base year and last year. Incidence rate ratios for attendance for each age-group, sex, and residence were estimated using a multivariable regression model. Results: 20,695 patients from the county were seen in Kitale Eye Unit in 2013, 2014 and 2015. In that period, 8.3% had either uncorrected refractive error, cataracts or glaucoma, the priority VISION2020 diseases, and 61.0% had allergic or other conjunctivitis or normal eyes, which could potentially be managed at primary eye care. During the study period, overall average annual attendance rate increased from 609 to 792 per 100, 000 population, incidence rate ratio (IRR) 1.30 (95% confidence interval (CI) 1.26–1.35). Attendance rates increased more in females than males (34.7% vs. 25.1%, respectively), IRR 1.07 (1.04–1.10). Attendance rates increased with increasing age, (highest among the elderly compared to the young). We found that in extreme age groups (>75 years and <15years) females were less likely to attend than males and there was reduced utilization from those based furthest from the hospital. Conclusion: Specialist eye services are heavily utilized by people with conditions that could be managed at the primary health care level. Barriers to accessing eye services were distance and gender, especially among the most vulnerable groups (young and the elderly). Integration of primary and secondary eye care services could lower barriers to essential eye care services to the population whilst lowering pressure on the limited specialist services by ensuring more appropriate utilization.

Abstract:
As India is rich in talent, lagging in research and poor in finance, authors sincerely appeal UGC, NAAC, NPTEL, IIT, AICTE, IISER, IISc and administrative institutions like IIM, UPSC and Supreme Court core committees to take initiative in implementing and maintaining industrial and research oriented certified online audio-visual interactive degree courses across the Indian boundaries. Benefits and implications of this scheme are: 1) Distance education system, Open education system and Open book exam etc can be eliminated. 2) High quality research orientation can be inculcated among teaching staff. 3) Experimental approach and industrial orientation- both can be implemented and maintained simultaneously. 4) Educational stress on teenage students and financial burdens on parents - both can be eliminated. 5) Reservation issues pertaining to caste and seat allocation can be eliminated. 6) Degree level scholarship schemes can be eliminated. 7) All educational institutions can be encouraged to tie up with online degree system with unique pattern and quality and can be forced to focus on laboratory activities rather than teaching. 8) Current educational staff can be classified into five broad categories as Education wing, Call center wing, Research wing, Oration wing and Industrial wing. 9) Students lagging in grasping the key technical points can listen & watch the recorded and edited information several times for a better understanding and thus doubts, subject fear, back logs and detaining issues can be avoided to a great extent. 10) By maintaining educational call centers, interaction with students can be established and online degree courses can be strengthened further. 11) With recorded and edited lab experiments and 3D modeling tools, virtual laboratories can be developed and thus awareness can be inculcated in students on knowledge acquisition on various experiments and experimental setups. 12) As there is no restriction on available number of (electronic) seats, stress oriented and suicide provoking complicated entrance exams can be relinquished. 13) Poor students, Female students, male and female job holders, physically handicapped students and students not interested to attend colleges can get better education in this scheme. 14) For maintaining this online degree system across India, there seems a big scope for new employment. 15) By 2040, India can have a leading role in world science, engineering and technology. Even though the proposal under consideration is interlinked with many political and social issues, with reference to 'Vision2040' program, this proposal can be given a chance in Indian higher education system.

Abstract:
According to the UN, several problems are expected to arise due to the longer life span of future generations. In conjunction with a reduced amount of healthcare personnel, lifted by the WHO, a predictable crisis is yet to come. To counteract this problem many nations have turned to digitalisation. For example, Sweden is currently working towards "Vision e-hälsa 2025", which among others implies becoming the world leader in using digitalised solutions by 2025. Digitalisation in the healthcare sector implicates to upgrade existing systems or implementing new Healthcare Information Systems (HIS). This master thesis focuses on the implementation processes of HIS, which need to be improved in order to achieve the earlier mentioned goal. A literature review was conducted where relevant strategies and barriers within implementation processes in healthcare were identified. Furthermore, semi-structured interviews with practitioners resulted in the finding of ten categories and four themes (group of categories) through qualitative analysis. The categories and themes are circumstances that affect implementations; the following were the most influential in an implementation: Planning, Communication, Practical Training and Follow-up & End-user involvement. In conclusion, the improvement of one or all of the circumstances above could contribute to a faster digitalisation in the healthcare sector. Finally, the authors suggested some of the selected strategies and other solutions as tools for improvement. This study contributes to current knowledge about implementations by discovering important gaps between knowledge and practice. Moreover, this study contributes partially to a methodological discovery in visualisation analysis.

Abstract:
The research reported here is an investigation into the problems of social and economic development of a multiethic and multicultural country which has the added challenge of adopting a non-indigenous code to facilitate the development process. Malaysia's power to negotiate outcomes favourable to the interest of the country is critical for the successful attainment of the goals and objectives of VISION2020. Therefore the mechanisms of the human resource development programme have to be efficacious. The three hypotheses of this study are as follows: 1. there is a fear that the problems and challenges posed by the development plans, have been conceptually trivialised; 2. based on (1) above there is a concern that solutions proposed are inadequate and inappropriate and 3. the outcome of both (1) and (2) can lead to the potential underachievement of national goals and objectives. The study proposes a complex model for conceptualising the problem which looks at the relationship between society and language, which any solutions proposed must take into proper consideration. The study looks at the mechanisms available for the smooth absorption of new Malaysian members to new and international communities. A large scale investigation was undertaken with the researcher functioning as a participant observer. An in-depth study of one particular educational ecology yielded approximately 38 hours of interviews and 100 questionnaires. These data were analysed both for explicit information and implicit implications. By some criteria national policies appear to be having the desired effect, and can be given a clean bill of health. By others it is clear that major adjustments would be necessary if the nation is to achieve its objectives in full.

Abstract:
The aim of this study is to examine the situation of converging technologies - nano technology, biotechnology, information technologies, and cognitive science - in Turkish foresight study and to analyze situation of Turkey. The research on convergence in Turkish foresight is carried out in the first foresight study of Turkey, Vision2023 consisting of Delphi Report, Panel Reports and Strategy Reports. All of these reports are investigated thoroughly to find out the statements about the converging technologies to have an insight what Turkey is planning to do in the following years. Additionally, to compare the perspective of Turkey in terms of converging technologies, the findings of this study are compared with the findings of the USA NBIC Report. Moreover, as a base of comparison of findings, the foresight study (ManVis Study) whose participants was 22 European countries one of which is Turkey about manufacture vision is taken into consideration. The findings of the study indicate that in different reports, there are some visions that are not mentioned in Vision2023 Study. A detailed investigation of these, shows that most of these visions are related to cognitive science. Only a few are nano technology oriented visions. However, according to the findings of the study, in biotechnology related statements, the position of Turkey and expertise of Turkish scientists are considered as "
very good"
by Turkish experts. As far as information technology in Turkey is considered, Turkey is in an average position among other countries according to the findings of the study.
