Abstract:
The aim of this research was: to explore the published literature and the experiences of people working with multiple sclerosis (MS); to use information collected to develop and trial a vocational rehabilitation (VR) service; to evaluate the cost utility and impact on service users; and to implement a randomised control trial (RCT). The Medical Research Council’s framework ‘Framework for Development and Evaluation of RCTs for Complex Interventions to Improve Health’ was used to structure the study and facilitated the use of mixed methodologies. These included focus groups, patient reported outcome measures, semi-structured interviews and an RCT. Barriers to working with MS were identified in the literature and in the focus group discussions. Focus group participants helped design the VR service. This service cost a mean of £730 to provide and appeared effective in maintaining participants in their working roles. The need for early intervention was described at all stages of this study and formed the basis for the VR service trialled in the RCT. The RCT has a five year follow up period and therefore only preliminary results are reported here. This study demonstrates how service users can be usefully engaged in the development of a service. The results define the VR intervention offered and analysis shows the service was relatively inexpensive to provide. Although only small numbers were recruited at each stage of the study the results still add to the growing evidence for the provision of VR for people with long term neurological conditions.

Abstract:
This dissertation investigated rates of detected and undetected substance abuse among vocational rehabilitation applicants. The Substance Abuse Subtle Screening Inventory (SASSI) was administered to adults who applied to one Rehabilitation Services Administration (RSA) office during a consecutive four-month period. Subjects supplied demographic data and information regarding their disabilities. RSA disability codes, DSM III-R codes assigned by psychologists, and reasons for case closures were provided by counselors. Results of the study indicated a 41% rate of substance abuse among the 134 applicant-subjects. Within this sample 31% of the subjects evidenced some indication substance abuse in combination with another disability, while 10% had only indications of alcoholism or drug abuse. Seventeen percent of the sample self-disclosed problems with alcohol or drugs. Among the 41 applicants determined eligible to receive vocational rehabilitation services, a 27% rate of undetected substance abuse was found. For purposes of this study undetected substance abuse was defined by a self-disclosure, SASSI profile, or DSM III-R code indicative of substance abuse that was not reflected in the subject's assigned RSA disability code(s).

Looking for work is complex. It involves a synthesis of decisions relating to many separate jobseeking tasks, requiring discrimination of how an individual’s skills match the job description, whether the salary offered is acceptable, location of the work and the logistics of arranging suitable transport. These job-seeking tasks are even more difficult for the disabled, and despite the Web Access Initiative (WAI), there are no Web-enabled work searching systems designed as vocational rehabilitation tools. An opportunity exists among the educational technologists to devise a customizable learning platform that can supply a much needed life-long learning resource. It is becoming increasingly obvious that a range of computer supported collaborative learning (CSCL) systems may provide a ubiquitous approach to fulfilling the desire for reskilling the general population. This paper describes a research project in progress, designed to provide an interactive Web-enabled work searching resource for those undergoing vocational rehabilitation.
