This paper presents six cases who had a contracture of the long flexor tendons of the fingers and exhibited Volkmann’s sign due to a chronic abscess or cysticercosis in the belly of the flexor digitorum profundus. All of them were treated conservatively, with full functional recovery in all the cases and with no recurrence.

Alguns autores apontam o empreendedorismo como um importante vetor de sustentabilidade socioeconômica nos países do mundo inteiro. No entanto, acredita-se que o papel do empreendedor pode ir além desses fatores socioeconômicos observados por estes autores. Com uma mudança de valores, se poderia constituir uma nova noção de empreendedorismo orientado para a sustentabilidade também ambiental, sendo essa a discussão central deste artigo. Tendo em vista a constatação de que os produtos advindos de práticas agrícolas biodinâmicas podem ser considerados social e ambientalmente corretos, pretende-se, neste artigo, analisar as inovações desenvolvidas por um empreendedor que é um dos principais produtores de arroz biodinâmico brasileiro. Neste artigo, se assume uma abordagem qualitativa visando captar, com maior profundidade, detalhes relacionados ao caso desse empreendedor. Entende-se que, além de criatividade e liderança, o empreendedor orientado para a sustentabilidade deve possuir valores que estejam de acordo com essa perspectiva ao conduzir as atividades da organização.

Abstract:

A contractura isquémica congénita de Volkmann é uma entidade rara, observada em recém-nascidos logo após o parto, caracterizada clinicamente por uma placa necrótica que atinge, geralmente, um dos membros superiores e que se faz acompanhar de paralisia flácida. Ocorre no período intrauterino e surge devido a compressão mecânica do membro. O seu mau posicionamento, a constrição por banda aminiótica, a existência de oligoâmnios e de distocias são alguns dos possíveis factores impli- cados. A compressão conduz a um síndrome de compartimento que induz isquemia e necrose da pele, músculos e nervos. O tratamento é cirúrgico, implicando desbridamento da placa de necrose e eventual recurso a um enxerto cutâneo. Relatamos o caso clínico de um recém-nascido de 2 dias de vida com uma contractura isquémica congénita de Volk- mann do membro superior esquerdo, induzida pela presença de oligoâmnios.PALAVRAS-CHAVE – Contractura Isquémica Congénita, ; Recém-nascido; Úlcera Cutânea; Necrose Gorda.

Abstract:

In 1826, Lisfranc practiced a perineal circular incision, without opening the peritoneum. 50 years later, Verneuil and then Dolbeau and Denonvilliers practiced more extensive resections. In 1907, Lockart-Mummery described the operation that bears his name. In 1875, Kocher and Volkmann used the sacral approach that has been popularized by Kraske. Later, it became well known in the whole world and, with some little changes, it has been used in Europe by Bardenhaver, Hochenegg, Billroth, Rehn, Heinike and Rygidier and in the USA by Harrison Cripps, Edwards, Bevan and Grey Turner. Almost all of them resected the rectum and reestablished the continuity with a primary anastomosis. It seems that Volkmann practiced the combined abdominal and perineal approach for the first time in 1877, followed by Czerny and Köenning. Between 1896-1899, Quenu popularized this technique. In 1908, Ernest Miles began to use the technique and in 1914 he published the results. The resection of the posterior wall of the vagina in a rectum cancer has first been made by Hildebrant in 1879. The abdominal approach has been used by Hartmann in 1923. In 1930, Dixon together with Mayo, Waugh, Black and Judd described the “Mayo clinic operation”, meaning the resection of the rectal tumor with extraperitoneal anastomosis. They called that “anterior resection” and in 1948 they published their 20 years’ experience results and they definitely popularized the technique.

Abstract:

For 40 years we have been observing clear changes in the interest of pediatric orthopedists. Some malformations and diseases have almost disappeared in pediatric orthopedic departments, such as developmental hip dislocation, multiple congenital clubfoot surgeries, torticollis, varus of the shin, Blounts disease,Volkmann syndrome, shin mower amputations and others described in this article. The reason for this phenomenon is usually the emergence of new diagnostic methods (e.g. hip joint ultrasound), new treatment methods (e.g. botulinum toxin) or new birth techniques or technical progress in agricultural machinery.

Abstract:

Rhetoric is not only a methodology of the humanities. It is the most important of the sciences the ancient thinkers left us. It defines not only the methods of argumentation from uncertain premises, and not only is it a theory of text, but most of all it is a tool for organising our thoughts and preparing texts, the aim of which is to convince, but in a way which ensures knowledge and not opinion (Kant). However, the language that we use often fails us. Both Plato and Aristotle warned us about this. At the same time, they developed the foundations for téchne rhetoriké, the systemic shape of which we owe to Quintilian. Rhetoric, as posited by its ancient as well as contemporary researchers, is not a lexicon of gimmicks (Volkmann). It is rather an analogue of logic (Volkmann). As Wilhelm Windelband said: “thanks to grammar and rhetoric we have learnt the correct rules of thinking.” The author discusses not only the history of the studies of rhetoric, but also of language, mainly referring to the traditions of the Vienna Circle (Der Wiener Kreis). Though the achievements of contemporary linguistics and the contemporary studies on the philosophical foundations of linguistic studies are considered, the perspectives defined by Immanuel Kant and the researchers from the Vienna Circle are dominant. The aim of this study was not only to reiterate Windelband’s thesis, but also to verify it more comprehensively.
