Abstract:
Objective Plasma is formed by creating a high-density energy field within an electrically conductive fluid such as saline. Sometimes ablated bits of tissue get stuck between the electrodes of the wand, obstructing the suction channel. The purpose of this study is to investigate the effect of cooling the irrigating saline during ablation of the hypertrophied tongue base in patients with obstructive sleep apnea. Study Design Prospective randomized controlled trial. Setting An otorhinolaryngology department in Main University hospitals. Methods Sixty adult patients with obstructive sleep apnea and tongue base hypertrophy underwent tongue base ablation surgery. Patients were randomly divided into 2 groups of 30 patients each: cooled saline and room temperature saline. The Coblation wand used was the EVac 70 Xtra HP (Smith & Nephew). Results In this study, a significant difference in operative time (mean ± SD) was seen between groups: 21.2 ± 5.5 minutes in the cold group and 47 ± 9.5 minutes in the control group ( P = .001). The wands in the cold group did not obstruct, while all the wands in the control group were obstructed by tissue clogs with variable degrees, hence wasting more time to clean the wands’ tips. Conclusion Cooling the irrigating saline overcame the problem of wand clogs, and the wand tip did not occlude at all during the procedures, thus saving time lost in wand cleaning and demonstrating a faster and safer surgical procedure. Further studies are needed to identify the hemostatic effect of the cooled saline over the regular one.

Abstract:
ABSTRACT Clinical outcomes in arthroscopic hip preservation surgery have improved over the past two decades due to many factors, including advancements in technique and instrumentation. Complications following hip arthroscopy are associated with increased traction and overall surgical times. The purpose of this study was to compare traction and surgical times during hip arthroscopy using two different radiofrequency ablation wands produced by the same manufacturer. The authors hypothesized that the wand with a larger surface area would result in significantly less traction and surgical times. This study was a retrospective comparative investigation on patients who underwent arthroscopic surgery of the central, peripheral, peritrochanteric and/or deep gluteal space compartments of the hip. Both wands are 50-degree-angled probes, but the tip and shaft diameters are 3 and 3.75 mm for Wand A (Ambient Super MultiVac 50; tip surface area 7.1 mm2) compared to 4.7 and 4.7 mm for Wand B (Ambient HipVac 50; tip surface area 17.3 mm2), respectively. There was no difference (P = 0.16) in mean age of Wand A patients (30 females, 20 males; 35.2 years) versus Wand B patients (31 females, 19 males; 32.7 years). Traction time was significantly less in the Wand B group (41 ± 6 versus 51 ± 18 min; P < 0.001), as was surgical time (102 ± 13 versus 118 ± 17 min; P < 0.001). There were no significant differences in the number of labral anchors used or Current Procedural Terminology codes performed between groups. In conclusion, it was observed that the use of a larger surface area wand was associated with significantly less traction and surgical times during hip arthroscopy.

Abstract:
The study discusses the role of a wet nurse regarding the protection of a child in ancient Egypt. There is evidence of the phenomenon of the wet nurse dating from the Old Kingdom’s Pyramid texts, depictions in tombs from the New kingdom, to contracts between the wet nurse and her employer from the Late Period. They were most frequent in royal and elite families and made it possible for the mother to participate in social activities without worrying about feeding her baby, as well as being a symbol for economic wealth.  Wet nurses have been found depicted in funerary contexts, holding ritual protective objects, and been mentioned in protective spells targeted towards children. This shows that the wet nurse had a part in the protection of a child and the intention behind this study is to discuss her participation in it and what it involves. The sources used in this study are to contain two subjects to be of relevance: wet nurse and the protection of a child, which lead to a restricted amount of material to analyse. The material studied contains of amulets, serpent staffs, three apotropaic wands, depictions from two graves and one protective spell. The analysis resulted in a conclusion claiming that the practice of protection is a part of the wet nurse’s occupation and were practiced with at least an apotropaic wand. Moreover, the depiction of her holding apotropaic wands in funerary contexts representing rebirth shows that the wet nurse is present and offering protection in the events of a child’s birth.

Si forniscono, inoltre, spunti per ulteriori studi di approfondimento. Per poter procedere, è stata effettuata innanzitutto la catalogazione digitale delle sue opere dal 1951 ai primi mesi del 2018, non esistendo ancora un catalogo completo. Di questa catalogazione vengono allegati alcuni estratti e la scheda di un'opera quale esempio. Successivamente si è proceduto ad intervistare l'artista sugli aspetti biografici e del vissuto. Estratti dell'intervista vengono riportati in vari capitoli. Successivamente sono stati anaòlizzati: l'evoluzione del percorso artistico tramite le opere che ne sottolineano i passaggi tecnici; i recenti ripensamenti e rifacimenti di alcune; la firma nel corso dei decenni. Ne è emersa una personalità artistica in continua evoluzione, rigorosa, altamente creativa, strettamente connessa col contemporaneo e col sociale.

Abstract:
Objective: Because of the increasing shortage of general practitioners (GPs) in many countries, this study aimed to explore factors related to GP career choice in recent medical graduates. Particular focus was placed on the impact of specific practice-orientated GP courses at different stages of the medical undergraduate curriculum. Design: Observational study. Multivariable binary logistic regression was used to reveal independent associations with career choice. Setting: Leipzig Medical School, Germany. Subjects: 659 graduates (response rate = 64.2%). Main outcome measure: Choice of general practice as a career. Results: Six student-associated variables were found to be independently related to choice of general practice as a career: age, having family or friends in general practice, consideration of a GP career at matriculation, preference for subsequent work in a rural or small-town area, valuing the ability to see a broad spectrum of patients, and valuing long-term doctor – patient relationships. Regarding the curriculum, after adjustment independent associations were found with a specifi c pre-clinical GP elective (OR = 2.6, 95% CI 1.3 – 5.3), a four-week GP clerkship during the clinical study section (OR = 2.6, 95% CI 1.3 – 5.0), and a four-month GP clinical rotation during the final year (OR = 10.7, 95% CI 4.3 – 26.7). It was also found that the work-related values of the female participants were more compatible with those of physicians who opt for a GP career than was the case for their male colleagues. Conclusion: These results support the suggestion that a practice-orientated GP curriculum in both the earlier and later stages of undergraduate medical education raises medical schools’ output of future GPs. The findings are of interest for medical schools (curriculum design, admission criteria), policy-makers, and GPs involved in undergraduate medical education. More research is needed on the effectiveness of specific educational interventions in promoting interest in general practice as a career.

The examples in this paper show a conflict between private and collective interests. While the planner worry about the extensive and intensive margin effects of trades in a steady state, people want the exhaust the gains from trades immediately, i.e., once in a meeting, consumers prefer spend more for a better output than take the risk of saving money and wait for good meetings in the future. Thus, the conflict can force the planner to choose allocations with a more disperse money distribution, mainly if people are im- patient. When the patient rate is low enough, the planner uses a expansionary policy to generate a better distribution of money for future trades.

AbstractThe magic wand $$\mathbin {-\!\!*}$$ - ∗ (also called separating implication) is a separation logic connective commonly used to specify properties of partial data structures, for instance during iterative traversals. A footprint of a magic wand formula "Equation missing" is a state that, combined with any state in which A holds, yields a state in which B holds. The key challenge of proving a magic wand (also called packaging a wand) is to find such a footprint. Existing package algorithms either have a high annotation overhead or, as we show in this paper, are unsound.We present a formal framework that precisely characterises a wide design space of possible package algorithms applicable to a large class of separation logics. We prove in Isabelle/HOL that our formal framework is sound and complete, and use it to develop a novel package algorithm that offers competitive automation and is sound. Moreover, we present a novel, restricted definition of wands and prove in Isabelle/HOL that it is possible to soundly combine fractions of such wands, which is not the case for arbitrary wands. We have implemented our techniques for the Viper language, and demonstrate that they are effective in practice.

Immersive Computing Technology (ICT) offers designers the unique ability to evaluate human interaction with product design concepts through the use of stereo viewing and 3D position tracking. These technologies provide designers with opportunities to create virtual simulations for numerous different applications. In order to support the immersive experience of a virtual simulation, it is necessary to employ interaction techniques that are appropriately mapped to specific tasks. Numerous methods for interacting in various virtual applications have been developed which use wands, game controllers, and haptic devices. However, if the intent of the simulation is to gather information on how a person would interact in an environment, more natural interaction paradigms are needed. The use of 3D hand models coupled with position-tracked gloves provide for intuitive interactions in virtual environments. This paper presents several methods of representing a virtual hand model in the virtual environment to support natural interaction.

The Republic of Ghana (former British colony of the Gold Coast) inherited ethnic heterogeneity of its population from the colonial period along with its state borders. The southern and central regions of Ghana are inhabited by Akans, whose potestarianpolitical culture had the most serious impact on the formation and functioning of the political culture of the whole country. The article is devoted to the identification and characterization of the main components of the traditional potestarian-political culture of the Akans, among which the following are the most significant: 1) matrilineality of inheritance of status and property; 2) avunculate; 3) co-ruling of the chief (ohene) and his female sibling (ohema); 4) the institute of elders (panin), who influence power-andgovernance relations through the decisions of the councils of elders (of the village communities, chiefdoms or unions of chiefdoms); 5) the institute of okyeame — intermediaries (“linguists”) between the chiefs and their subjects; 6) the formations of the traditional order of battle as the basis of the administrative-territorial division of polities (omans); 7) the paramilitary nature of the potestarity apparatus and political structure of the omans; 8) special symbols and attributes of traditional power: stoolthrones, umbrellas, swords, wands and gold jewelry; 9) Odwira (Apafram) and Adae festivals associated with the consecration of a new crop of yams and the veneration (commemoration and propitiation) of ancestors. The use of surface markers to calculate kinematics of diarthrodial joints is subject to error from relative motion between the markers on the skin and the underlying bones. It has been demonstrated that markers placed over the knee and hip joints could move 10–30 mm with respect to the skeleton which can have significant effects on the accuracy of calculated knee kinematics (Cappozzo et al., 1996, Manal et al., 2000). Several methods have been developed to calculate knee kinematics from marker locations and decrease error resulting from skin motion. The Helen Hayes method uses markers attached to the skin over anatomical landmarks and on wands to calculate the locations of the hip, knee, and ankle joints (Vaughan et al., 1999). The location and orientation of the thigh and shank segments can then be calculated based on anthropometric correlations. Other methods, such as that developed by the Cleveland Clinic, record the position of the femur and tibia using triads of markers attached to each segment (Manal et al., 2000). Additionally, a Point Cluster Technique was developed using clusters of markers attached to each segment to reduce error from skin motion (Andriacchi et al., 1998).

Abstract:
Esta arte experimental tentou produzir um clima descolonizado de pesquisa e criação de um espaço digital tridimensional (3D) de Realidade Virtual (RV), baseado na integração de conhecimento transdisciplinar. Aplicou o conceito de Transformação Digital (DX) (O’Brien e Reinitz, 2021), dentro de processos de prática computacional educativa (Tissenbaum et al. 2019) para capacitar pessoas (Freire, 2016), por meio da concepção e execução de um espaço 3DRV, utilizando técnicas de programação de computação gráfica 3D para o desenvolvimento de comunicação visual (Cunningham, 2007) com suporte do emprego de tecnologias baseadas em Web3D (Web3D Consortium, 2021) como Recursos Educacionais Abertos (REA) (UNESCO, 2019). As tecnologias Web3D acessíveis e de baixo custo permitiram praticar, analisar e estender uma investigação de longo prazo nos níveis de educação K-12, referente ao compartilhamento de conhecimento, com o objetivo de inspirar o envolvimento dos indivíduos em práticas de computação que abrangem habilidades de codificação e alfabetização visual (Franco e Lopes, 2012; Franco, 2020), incluindo a arte e suas oportunidades de aprendizagem e expressão na era digital (Wand, 2007). Com o apoio de tecnologias Web3D e experiências de aprendizagem e ensino ao longo da vida, por meio da participação no curso “Projeto Espetáculo em Realidade Aumentada”, na Fábrica de Cultura Diadema, o desenvolvimento contínuo desta obra ampliou nosso processo de pesquisa através da produção de um material descolonizado, ao interconectar o conhecimento conceitual relacionado à arte visual da mistura de culturas afro-brasileiras, ameríndias e europeias de Rubem Valentim (Mendes Wood DM, 2021) e a representação simbólica da cultura afro-americana de Jacob Lawrence (Smithsonian American Art Museum, 2021). Ambos os artistas têm uma trajetória de aprendizagem ao longo da vida e usam geometria e formas coloridas em sua composição artística. Suas obras expressaram conhecimentos relacionados às culturas afro-brasileira e afro-americana, contribuindo para diminuir uma lacuna de invisibilidade na educação oficial (Fernandes, 2016), no ensino das contribuições da cultura negra para o mundo das artes, das ciências e da tecnologia. A interação com a trajetória e trabalho destes artistas tem levado a pesquisar, aplicar e compartilhar conhecimentos relacionados à África antiga, tendo como referência o uso da matemática, da geometria pela civilização egípcia, em certa medida (geometria sagrada, Calter, 1998) conhecimentos de formas e cores para construção e decoração de pirâmides e outros monumentos. Tal confluência de conhecimento transdisciplinar fez parte da pesquisa e formação de bibliotecas e técnicas de computação gráfica, permitindo, por meio de práticas educativas de programação de computadores 3D, integrar nesta realidade virtual 3D (RV) - (3DRV) recursos de arte de escultura digital, instalação e net art (Wands , 2007), de forma autônoma e/ou por meio de uma interface baseada em blog. Assim, esta confluência de conhecimentos tem gerado, conforme investigado e materializado por meio da computação gráfica 3D em (T’ÒSÚN, 2014), a construção e visualização de representações simbólicas de adornos e instrumentos sagrados de Seres Encantados Afro-Brasileiros, Europeus e Ameríndios, incluindo o aprimoramento das habilidades cognitivas e técnicas das pessoas.
