Eğitim, gelişen teknoloji ve sosyal olaylara göre kendisini sürekli günceller. Genellikle geleneksel metotlarının kullanıldığı eğitim sistemi artık yerini teknolojik eğitime bırakmaktadır. Özellikle dünya üzerinde meydana gelen afetler, salgın hastalıklar, hayatı kısıtladığı ya da kesintiye uğrattığı zamanlarda eğitim sektörünün yardımına teknolojik eğitim sistemleri koşmaktadır. İnternet bağlantısının olduğu her yerde bireylere eğitim alma veya eğitim verme imkânı sunan bu interaktif sistemde müfredata ve ders içeriklerine göre pek çok program ve yazılım geliştirilerek sektörün hizmetine sunulmaktadır. Zamandan ve mekândan bağımsız olarak yapılan bu eğitim sistemi uzaktan eğitim olarak adlandırılmaktadır. Geliştirilen programlar sayesinde sanal sınıflar oluşturularak yapılan bu eğitim, kullanıcılar tarafından interaktif eğitim olarak adlandırılmaktadır. Öğrencinin ve dersin ihtiyacına göre seçilen programlar sayesinde öğrenciler kaliteli ve hızlı bir şekilde eğitim alma imkânına sahiptirler. Özellikle dil öğretiminde öne çıkan uzaktan eğitim sistemi uzakları ve mekanları birbirine yakınlaştırarak mesafe sorununu ortadan kaldırmaktadır.  Ele aldığımız bu çalışmada yabancılara Türkçe eğitiminde okuma, yazma, dinleme ve konuşma becerileri için geliştirilen programlar ele alınarak uzaktan eğitimde hangi amaçlara yönelik kullanıldığı açıklanacaktır. Web 2. 0 araçlarının sanal eğitimde kullanılmaya başlaması giderek artan program çeşitliliği her alanda olduğu gibi dil eğitiminde de öğreticiye ve öğrenciye pek çok yeni imkânlar sağlamakta, eğitimin kalitesini artırarak işleyişi hızlandırmaktadır. Web 2.0 tasarımlarının ele alındığı bu çalışmada, genel manada bir bilgilendirme yapıldıktan sonra Web 2.0 araçlarından hangisinin hangi alanda nasıl ve ne amaçla kullanılacağı açıklanacaktır. Alt başlıklar altıda verilen bilgiler ve kullanım alanları uzaktan eğitim sistemindeki öğreticileri ve öğrencileri yeni gelişmelerden haberdar ederek işlerini kolaylaştıracak ve bu alanda çalışan diğer insanlara büyük fayda sağlayacaktır.

Abstract:
ABSTRACT We used the rotation measure (RM) catalogue derived from the LOFAR Two-metre Sky Survey Data Release 2 (LoTSS DR2) at 144 MHz to measure the evolution with redshift of the extragalactic RM (RRM: Residual RM) and the polarization fraction (p) of sources in low-density environments. We also measured the same at 1.4 GHz by cross-matching with the NRAO VLA Sky Survey RM catalogue. We find that RRM versus redshift is flat at 144 MHz, but, once redshift-corrected, it shows evolution at high significance. Also, p evolves with redshift with a decrement by a factor of ∼8 at z ∼ 2. Comparing the 144-MHz and 1.4-GHz data, we find that the observed RRM and p are most likely to have an origin local to the source at 1.4 GHz, while a cosmic web filament origin is favoured at 144 MHz. If we attribute the entire signal to filaments, we infer a mean rest-frame RRM per filament of RRM$_{\rm 0,f} = 0.71 \pm 0.07 \, \, \rm rad\, m^{-2}$ and a magnetic field per filament of Bf = 32 ± 3 nG. This is in agreement with estimates obtained with a complementary method based on synchrotron emission stacking, and with cosmological simulations if primordial magnetic fields are amplified by astrophysical source field seeding. The measurement of an RRM0, f supports the presence of diffuse baryonic gas in filaments. We also estimated a conservative upper limit of the filament magnetic turbulence of $\sigma _{\rm RRM_{\rm 0,f}} =0.039 \pm 0.001 \, \, \rm rad\, m^{-2}$, concluding that the ordered magnetic field component dominates in filaments.

Large panel structures made of composites are common building units in aerospace industries. In order to increase the stiffness of such structures, the panel or skin is adhered to a flange and supported by a web. Such a stiffened panel is modeled as an arrangement of web, flange and panel with an interface between the flange and the panel. In this paper, three dimensional stress analysis of one such stiffened panel has been carried out using the finite element analysis. The geometric non-linearity has been assumed in the analysis. The effect of material anisotropy and the laminate stacking sequence on the stress components has been studied. Material Graphite Fiber Reinforced Polymeric (GFRP) composite has been used and, two different configurations were considered while the unidirectional prepregs were laid up in quasi-isotropic [0/0/0/0]2 and cross-ply [0/90/90/0]2. Subsequently, the coupled stress failure criterion has been used to predict the critical location of damage onset. When component damage indicator attained the value of 1.0, the component was considered to lose stiffness and structural integrity.

Abstract:
For the first time in Ukraine, Woven EndoBridge (WEB) (Microvention – Terumo) for the treatment of intracranial arterial aneurysms was applied at Scientific and Practical Centre of Endovascular Neuroradiology of NAMS of Ukraine on November 29, 2018. To the end of 2019 4 patients were treated with WEB. In all patients aneurysms were accidental findings, diagnosed by magnetic resonance imaging, and confirmed by angio-graphy. Men were 2 (50 %), women – 2 (50 %). The average age of patients was 49 years. All aneurysms were complex for endovascular treatment. In 3 (75 %) cases, the aneurysm was located on the anterior cerebral/anterior connective arteries, in 1 (25 %) case – on the middle cerebral artery. All patients received double disaggregated therapy at the standard dosage (75 mg of clopidogrel and 75 mg of acetylsalicylic acid) during 5 days before surgery in case of an additional intracranial stent implantation. The efficacy of dual disaggregation therapy was monitored by light transmission agrigometry. The size of the WEB depended on the angioarchitectonics of the aneurysm. All surgeries were performed in a routine manner, under general anesthesia. In all cases, the WEB was implanted successfully, there were no intra- and postoperative complications. The next day after surgery, double disaggregation therapy was canceled. All patients were discharged from the hospital in satisfactory condition, under the supervision of a neurologist at the place of residence. A control examination in the form of angiography after 6 months passed all patients. The efficacy of aneurysm occlusion using the WEB embolization device was evaluated on the WEB occlusion scale. Grade 0 and 0’ were achieved in 2 (50 %) patients, Grade 1 – 1 (25 %), Grade 2 – 1 (25 %).

Syndactyly correction results in skin deficiency. Skin grafting is avoided by the use of an extended dorsal interdigital flap for the web and approximation of the side flaps to cover the length of the digits. The technique has been used in 17 cases with all varieties of syndactyly. There were no early complications. The follow-up averaged 2 years. There were two cases of web creep but no contractures. The method combines the advantage of direct suture with an acceptable frequency of web creep.

Abstract:
INTRODUCTION: The Woven EndoBridge (WEB) device is frequently used for the treatment of intracranial aneurysms. Postoperative management, including the use of aspirin, varies among clinicians and institutions, but its impact on the outcomes of the WEB has not been thoroughly investigated. METHODS: This was a retrospective, multicenter study involving 30 academic institutions in North America, South America, and Europe. Data from 1492 patients treated with the WEB device were included. Patients were categorized into two groups based on their postoperative use of aspirin. Data points included patient demographics, aneurysm characteristics, procedural details, complications, and angiographic and functional outcomes. Propensity score matching (PSM) was applied to balance variables between the two groups. RESULTS: Prior to PSM, the aspirin group exhibited significantly higher rates of modified Rankin scale (mRS) mRS 0-1 and mRS 0-2 (89.8% vs 73.4% and 94.1% vs 79.8%, p<0.001), lower rates of mortality (1.6% vs 8.6%, p<0.001), and higher major compaction rates (13.4% vs 7%, p<0.001). Post-PSM, the aspirin group showed significantly higher rates of retreatment (p=0.026) and major compaction (p=0.037) while maintaining its higher rates of good functional outcomes and lower mortality rates. In the multivariable regression, aspirin was associated with higher rates of mRS 0-1 (OR 2.166; 95% CI 1.16 to 4, p=0.016) and mRS 0-2 (OR 2.817; 95% CI 1.36 to 5.88, p=0.005) and lower rates of mortality (OR 0.228; 95% CI 0.06 to 0.83, p=0.025). However, it was associated with higher rates of retreatment (OR 2.471; 95% CI 1.11 to 5.51, p=0.027). CONCLUSIONS: Aspirin use post-WEB treatment may lead to better functional outcomes and lower mortality but with higher retreatment rates. These insights are crucial for postoperative management after WEB procedures, but further studies are necessary for validation.

In this study, the effect of preparing activities with digital literary products in social studies lesson on pre-service social studies teachers' competencies in using Web 2. 0 tools were tried to be determined. For this purpose, a mixed-methods design was adoptedThe sample of the study consists of of 37 pre-service social studies teachers selected in the context of criterion sampling from purposeful sampling techniques. "Repeated measure ANNOWA" test and "Paired Samples T-Test", which are parametric tests, were used to analyze quantitative data, and content analysis was used to analyze qualitative data. According to the results of the quantitative data analysis, a significant difference was found between the mean scores obtained from the Web 2. 0 Tools Usage Competency Scale (WAKYÖ) at four stages. Similarly, this difference also emerged between the first-second, first-third, first-fourth, second-fourth, and third-fourth measurement pairs. However, no significant difference was found between the second and third measurement mean scores. In addition qualitative data analysis identified four main themes related to positive effects: "becoming competent", "most admired", "affective effects" and "using in the profession". Based on this result, it was concluded that social studies research using digital literary products create a positive difference on the competence of using Web 2. 0 tools and this significant difference was also supported by the qualitative data results.

Online self-administered data collection, by reducing the logistic burden and cost, could advantageously replace classical methods based on dietitian's interviews when assessing dietary intake in large epidemiological studies. Studies comparing such new instruments with traditional methods are necessary. Our objective was to compare one NutriNet-Santé web-based self-administered 24 h dietary record with one 24 h recall carried out by a dietitian. Subjects completed the web-based record, which was followed the next day by a dietitian-conducted 24 h recall by telephone (corresponding to the same day and using the same computerised interface for data entry). The subjects were 147 volunteers aged 48–75 years (women 59·2 %). The study was conducted in February 2009 in France. Agreement was assessed by intraclass correlation coefficients (ICC) for foods and energy-adjusted Pearson's correlations for nutrients. Agreement between the two methods was high, although it may have been overestimated because the two assessments were consecutive to one another. Among consumers only, the median of ICC for foods was 0·8 in men and 0·7 in women (range 0·5–0·9). The median of energy-adjusted Pearson's correlations for nutrients was 0·8 in both sexes (range 0·6–0·9). The mean Pearson correlation was higher in subjects ≤ 60 years (P = 0·02) and in those who declared being ‘experienced/expert’ with computers (P = 0·0003), but no difference was observed according to educational level (P = 0·12). The mean completion time was similar between the two methods (median for both methods: 25 min). The web-based method was preferred by 66·1 % of users. Our web-based dietary assessment, permitting considerable logistic simplification and cost savings, may be highly advantageous for large population-based surveys.

Researchers have increasingly adopted a web survey for data collection. Previous studies have examined factors leading to a web survey’s success. However, virtually no empirical work has examined the effects of the three levels of forced responses or the two styles of question items displayed on a web survey’s response rate. The current study attempted to fill this void. Using a quasi-experiment approach, we obtained 778 unique responses to six comparable web questionnaires of identical content. The analysis confirmed that (1) there were statistically significant differences across the surveys with the 100%-, 50%- and 0%-forced responses, and (2) there is not a significant difference between the response rates between surveys with scrolling and those with paging styles. In addition to extending the theoretical insight into factors contributing to a web survey’s response rate, the findings have offered recommendations to enhance the response rate in a web survey project.

Proses vulnerability assessment ditemukan lima kerentanan pada aset yaitu yang bersifat risiko kritis sejumlah 0, risiko tinggi sejumlah 2, risiko menengah sejumlah 2, dan risiko rendah sejumlah 1. Kesimpulan berdasarkan analisis kerentanan situs web serta hasil verifikasi identitas diperoleh bahwa situs web tersebut memiliki sejumlah kelemahan dan kerentanan yang perlu diperbaiki demi menjaga keamanan dan kualitas situs web. Tindakan perbaikan pada konfigurasi website perlu dilakukan seperti pengaturan cookie, SSL, header HTTP, dan lainnya terutama layanan SSL/TLS. Layanan yang tidak membatasi secara akurat pada tahap negosiasi ulang pada sistem mempermudah penyerang untuk melakukan serangan Denial of Service dengan melakukan negosiasi ulang dalam satu koneksi.

Salah satu alternatif untuk menstimulus perkembangan secara optimal pada anak adalah dengan media pembelajaran yang didapat. Namun nyatanya media pembelajaran untuk anak usia dini umumnya masih konvensional seperti menggunakan media kartu, dan beberapa mainan fisik untuk menambah pengetahuan anak serta melatih kognitif dan motorik. Oleh karena itu, salah satu alternatif yang dapat digunakan untuk mengatasi permasalahan ini adalah diperlukannya suatu rancangan media pembelajaran interaktif dan menyenangkan bagi anak usia dini melalui game berbasis web, untuk menstimulus perkembangannya. Experts assess the need for meaningful stimulation so that children's early childhood can develop optimally. The role of stimulation in children must be prepared by educators, starting from parents, teachers, or other adults around the child. One alternative to stimulate optimal development in children is with the learning media obtained. But in fact learning media for early childhood are generally still conventional, such as using card media, and some physical toys to increase children's knowledge and train cognitive and motor skills. Therefore, one alternative that can be used to overcome this problem is the need for an interactive and fun learning media design for early childhood through web-based games, to stimulate their development. This study aims to design interactive learning media for children, in this study the media developed is in the form of educational games based on construct 2 as a medium to support learning in early childhood with material about numbers, colors, animals and musical instruments. In this case the child who acts as a player can learn about the material. The knowledge transfer process is carried out by completing the challenges contained in the game. New technologies are promising for the use of short-term instruments for dietary data collection; however, innovative tools should be validated against objective biomarkers. The aim of the present study was to investigate the validity of a Web-based, self-administered dietary record (DR) tool using protein, K and Na intakes against 24 h urinary biomarkers (24 h U). Of the total participants, 199 adult volunteers (104 men and 95 women, mean age 50·5 (23–83 years)) of the NutriNet-Santé Study were included in the protocol. They completed three non-consecutive-day DR and two 24 h U on the first and third DR days. Relative differences between reported (DR) and measured (24 h U) intakes were calculated from the log ratio (DR/24 h U) for protein, K and Na intakes: − 14·4,+2·6 and − 2·1 % for men; and − 13·9, − 3·7 and − 8·3 % for women, respectively. The correlations between reported and true intakes were 0·61, 0·78 and 0·47 for men and 0·64, 0·42 and 0·37 for women for protein, K and Na, respectively. Attenuation factors, that represent attenuation of the true diet–disease relationship due to measurement error (a value closer to 1 indicating lower attenuation), ranged from 0·23 (Na, women) to 0·60 (K, men). We showed that the Web-based DR tool used in the NutriNet-Santé cohort study performs well in estimating protein and K intakes and fairly well in estimating Na intake. Furthermore, three non-consecutive-day DR appear to be valid for estimating usual intakes of protein and K, although caution is advised regarding the generalisability of these findings to other nutrients and general population.

ObjectiveTo compare contrast-enhanced MR angiography (CE-MRA) and DSA for the follow-up of intracranial aneurysms (IAs) treated with the Woven EndoBridge embolization system DL (WEB DL; Sequent Medical, Aliso Viejo, California, USA).Materials and methodsWe retrospectively identified all patients treated with a WEB DL between November 2010 and February 2013 in 2 hospitals. The IA occlusion was graded on follow-up CE-MRA and DSA by 4 independent readers and by 2 readers reaching a consensus, respectively. Interobserver agreement for MRA and intertechnique agreement was evaluated by calculating linear weighted κ.ResultsFifteen patients with 16 IAs were included. Mean delay between MRA and DSA was 2 months (range 0–16 months). Interobserver agreement for MRA was substantial to almost perfect (κ=0.686–0.921; mean κ=0.809). Intertechnique agreement was moderate to substantial (κ=0.579–0.724; mean κ=0.669). Only three out of five inadequately occluded IAs were detected by MRA.ConclusionsCE-MRA is a useful tool for the follow-up of IAs treated with a WEB DL. However, early follow-up with DSA remains mandatory to detect inadequately occluded IAs.

Implementasi ini bertujuan memudahkan sekolah dalam menentukan rekomendasi kelas untuk siswa. Hasil dari algoritma K-Means menghasilkan 4 cluster: Cluster 1 (Bahasa Indonesia, IPS, dan Matematika), Cluster 2 (Bahasa Inggris), Cluster 3 (Bahasa Indonesia dan IPA), Cluster 4 (Bahasa Inggris dan IPA) dengan hasil Silhouette Score memberikan skor 0.6233 yang menunjukkan perehitungan skor berada pada angka 0 bahwa titik data pusat cluster masing-masing.

Tea consumption has inconsistently been shown to be associated with the risk of type 2 diabetes (T2D). The aim of the present study was to conduct a dose–response meta-analysis of cohort studies assessing the association between consumption of tea and risk of developing T2D. Pertinent studies were identified by searching PubMed, Web of Science and EMBASE through 31 March 2013. A total of sixteen cohorts from fifteen articles that reported 37 445 cases of diabetes among 545 517 participants were included. A significant linearly inverse association between tea consumption and T2D risk was found (P for linear trend = 0·02). An increase of 2 cups/d in tea consumption was found to be associated with a 4·6 (95 % CI 0·9, 8·1) % reduced risk of T2D. On the basis of the dose–response meta-analysis, the predicted relative risks of diabetes for 0, 1, 2, 3, 4, 5 and 6 cups of tea consumed per d were 1·00 (referent), 0·97 (95 % CI 0·94, 1·01), 0·95(95 % CI 0·92, 0·98), 0·93 (95 % CI 0·88, 0·98), 0·90 (95 % CI 0·85, 0·96), 0·88 (95 % CI 0·83, 0·93) and 0·85 (95 % CI 0·80, 0·91), respectively. There was a statistically significant heterogeneity within the selected studies (Q= 45·32, P< 0·001, I2= 60·3 %). No evidence of substantial small-study bias was found (P= 0·46). Our findings suggest that tea consumption could be linearly inversely associated with T2D risk. Future well-designed observational studies that account for different characteristics of tea such as tea types, preparation methods and tea strength are needed to fully characterise such an association.

BackroundThe treatment of wide neck bifurcation aneurysms remains challenging despite the introduction of new techniques (Y stenting, waffle cone technique, or flow diverter stents). The Woven EndoBridge (WEB) device is an innovative solution for this type of cerebral aneurysm. A new WEB 17 is now available and has been designed to offer smaller sized devices to optimize navigability and delivery.MethodsBetween February 2017 and April 2018 all patients treated with the WEB 17 device in our center were retrospectively reviewed. 25 patients with 28 non-ruptured aneurysms were identified and analyzed. Three patients with two aneurysms both treated with the WEB device were identified.ResultsThe device was successfully deployed in all cases. Procedure related morbidity was 4% and mortality was 0%. In one case, a delayed postprocedural thromboembolic event occurred owing to device protrusion. Technical success, complications, angiographic outcomes, procedural data, and follow-ups are reported. The modified Rankin Scale score at discharge was 0 for 24 patients (96%). At the 3, 6, or 9 month follow-up, angiograms were taken of 21 of the 25 patients (84%) (24 of 28 aneurysms had been controlled); 3 patients (3 aneurysms) did not receive angiographic follow-up at the time of submission of this work. Complete occlusion was achieved in 22 of 24 aneurysms (91.66%), and 2 of 24 aneurysm (8.33%) showed a neck remnant.ConclusionsThe WEB 17 is safe and technically feasible, according to this retrospective single center analysis. For very small bifurcation aneurysms, the WEB 17 seems to have lower complication rates than stent assisted techniques. However, further studies are needed to evaluate the complication rate and long term efficiency.

Abstract We examine the redshift evolution of the relationship between the neutral atomic hydrogen (H i) content and star formation properties of blue galaxies, along with their location in the cosmic web. Using the COSMOS H i Large Extragalactic Survey (CHILES) and the IllustrisTNG (TNG100) cosmological simulation, and the DisPerSE algorithm, we identify the filamentary structure in both observations and simulations, measure the distance of galaxies to the nearest filament spine D fil, and calculate the mean H i gas fraction and the relative specific star formation rate (sSFR) of blue galaxies in three different cosmic web environments—0 < D fil/Mpc < 2 (filament cores), 2 < D fil/Mpc < 4 (filament outskirts), and 4 < D fil/Mpc < 20 (voids). We find that, although there are some similarities between CHILES and TNG, there exist significant discrepancies in the dependence of H i and star formation on the cosmic web and on redshift. TNG overpredicts the observed H i fraction and relative sSFR at z = 0–0.5, with the tension being strongest in the voids. CHILES observes a decline in the H i fraction from filament cores to voids, exactly the opposite of the trend predicted by TNG. CHILES observes an increase in H i fraction at z = 0.5 → 0 in the voids, while TNG predicts an increase in this time in all environments. Further dividing the sample into stellar mass bins, we find that the H i in log ( M * / M ⊙ ) > 10 galaxies is better reproduced by TNG than H i in log ( M * / M ⊙ ) = 9 − 10 galaxies.

Intakes of energy and of twenty-four selected nutrients assessed by both methods were compared.SettingQuébec City metropolitan area.SubjectsFifty-seven women and fifty men (mean (sd) age: 47·2 (13·3) years).ResultsEquivalent proportions of under-reporters were found with the R24W (15·0%) and the FR (23·4%). Mean (sd) energy intake from the R24W was 7·2% higher than that from the FR (10 857 (3184) kJ/d (2595 (761) kcal/d) v. 10 075 (2971) kJ/d (2408 (710) kcal/d); P<0·01). Significant differences in mean nutrient intakes between the R24W and the FR ranged from –54·8% (i.e. lower value with R24W) for niacin to +40·0% (i.e. higher value with R24W) for alcohol. Sex- and energy-adjusted deattenuated correlations between the two methods were significant for all nutrients except Zn (range: 0·35–0·72; P<0·01). Cross-classification demonstrated that 40·0% of participants were classified in the same quartile with both methods, while 40·0% were classified in the adjacent quartile and only 3·6% were grossly misclassified (1st v. 4th quartile). Analysis of Bland–Altman plots revealed proportional bias between the two assessment methods for 8/24 nutrients.ConclusionsThese data suggest that the R24W presents an acceptable relative validity as compared with the FR for estimating usual dietary intakes in a cohort of French Canadians.

Objective Type 2 diabetes mellitus(T2DM) is closely related to sarcopenic obesity(SO). Body composition measurement including body weight, body mass index, waist circumference, percentage body fat, fat mass, muscle mass, visceral adipose tissue and subcutaneus adipose tissue, plays a key role in evaluating T2DM and SO. The weight reduction effect of sodium-glucose cotransporter 2(SGLT-2) inhibitors has been demonstrated. However, there are warnings that SGLT-2 inhibitors should be used with caution because they may increase the risk of sarcopenia. The effect of SGLT-2 inhibitors on body composition in T2DM is inconclusive. In this work, a meta-analysis of randomized controlled trials was conducted to evaluate the effect of SGLT-2 inhibitors on body composition in T2DM. Methods PubMed, the Cochrane Library, EMbase and Web of Science databases were searched by computer. All statistical analyses were carried out with Review Manager version 5. 3. Results were compared by weight mean difference(WMD), with 95% confidence intervals(CI) for continuous outcomes. A random effects model was applied regardless of heterogeneity. The I2 statistic was applied to evaluate the heterogeneity of studies. Publication bias was assessed using Funnel plots. Results 18 studies with 1430 participants were eligible for the meta-analysis. We also reviewed reference lists from retrieved articles. We included studies that reported risk estimates (including relative risks, odds ratios and hazard ratios) with 95 % confidence intervals for the association between breakfast skipping and the risk of T2D.SubjectsEight studies involving 106 935 participants and 7419 patients with T2D were included in the meta-analysis.ResultsA pooled adjusted relative risk for the association between exposure to breakfast skipping and T2D risk was 1·21 (95 % CI 1·12, 1·31; P=0·984; I2=0·0 %) in cohort studies and the pooled OR was 1·15 (95 % CI, 1·05, 1·24; P=0·770; I2=0·0 %) in cross-sectional studies. Visual inspection of a funnel plot and Begg’s test indicated no evidence of publication bias.ConclusionsBreakfast skipping is associated with a significantly increased risk of T2D. Regular breakfast consumption is potentially important for the prevention of T2D.

Background Poor anticoagulation management of warfarin may lead to patient admission, prolonged hospital stays, and even death due to anticoagulation-related adverse events. Traditional non–web-based outpatient clinics struggle to provide ideal anticoagulation management services for patients, and there is a need to explore a safer, more effective, and more convenient mode of warfarin management. Objective This study aimed to compare differences in the quality of anticoagulation management and clinical adverse events between a web-based management model (via a smartphone app) and the conventional non–web-based outpatient management model. Methods This study is a prospective cohort research that includes multiple national centers. Patients meeting the nadir criteria were split into a web-based management group using the Alfalfa app or a non–web-based management group with traditional outpatient management, and they were then monitored for a 6-month follow-up period to collect coagulation test results and clinical events. The effectiveness and safety of the 2 management models were assessed by the following indicators: time in therapeutic range (TTR), bleeding events, thromboembolic events, all-cause mortality events, cumulative event rates, and the distribution of the international normalized ratio (INR). Results This national multicenter cohort study enrolled 522 patients between June 2019 and May 2021, with 519 (99%) patients reaching the follow-up end point, including 260 (50%) in the non–web-based management group and 259 (50%) in the web-based management group. There were no observable differences in baseline characteristics between the 2 patient groups. The web-based management group had a significantly higher TTR than the non–web-based management group (82.4% vs 71.6%, P<.001), and a higher proportion of patients received effective anticoagulation management (81.2% vs 63.5%, P<.001). The incidence of minor bleeding events in the non–web-based management group was significantly higher than that in the web-based management group (12.1% vs 6.6%, P=.048). Between the 2 groups, there was no statistically significant difference in the incidence of severe bleeding and thromboembolic and all-cause death events. In addition, compared with the non–web-based management group, the web-based management group had a lower proportion of INR in the extreme subtreatment range (17.6% vs 21.3%) and severe supertreatment range (0% vs 0.8%) and a higher proportion in the treatment range (50.4% vs 43.1%), with statistical significance. Conclusions Compared with traditional non–web-based outpatient management, web-based management via the Alfalfa app may be more beneficial because it can enhance patient anticoagulation management quality, lower the frequency of small bleeding events, and improve INR distribution.

Background: A carotid web (CaW) is a shelf-like lesion in the posterior aspect of the internal carotid bulb and represents an intimal variant of fibromuscular dysplasia. CaW has been associated with recurrent strokes and conventionally treated with surgical excision. We report a multicenter experience of stenting in patients with symptomatic CaWs. Methods: Retrospective review of consecutive patients admitted to 5 comprehensive stroke centers who were identified to have a symptomatic CaW and treated with carotid stenting. A symptomatic CaW was defined by the presence of a shelf-like/linear, smooth filling defect in the posterior aspect of the carotid bulb diagnosed by neck CT angiography (CTA) and confirmed with conventional angiography in patients with negative stroke workup. Results: Twenty-four patients with stented symptomatic CaW were identified (stroke in 83% and transient ischemic attack in 17%). Their median age was 47 years (IQR 41–61), 14 (58%) were female, and were 17 (71%) black. The degree of stenosis by NASCET was 0% (range 0–11). All patients were placed on dual antiplatelets and stented at a median of 9 days (IQR 4–35) after the last event. Closed-cell stents were used in 18 (75%) of the cases. No periprocedural events occurred with the exception of 2 cases of asymptomatic hypotension/bradycardia. Clinical follow-up after stent placement occurred for a median of 12 months (IQR 3–19) with no new cerebrovascular events noted. Functional independence at 90 days was achieved in 22 (91%) patients. Follow-up vascular imaging (ultrasound n = 18/CTA n = 5) was performed at a median of 10 months (IQR 3–18) and revealed no stenosis. Conclusions: Stenting for symptomatic CaW appears to be a safe and effective alternative to surgical resection. Further studies are warranted.

ABSTRACT We analyse the IllustrisTNG simulations to study the mass, volume fraction, and phase distribution of gaseous baryons embedded in the knots, filaments, sheets, and voids of the Cosmic Web from redshift z = 8 to redshift z = 0. We find that filaments host more star-forming gas than knots, and that filaments also have a higher relative mass fraction of gas in this phase than knots. We also show that the cool, diffuse intergalactic medium [IGM; $T\lt 10^5 \, {\rm K}$, $n_{\rm H}\lt 10^{-4}(1+z) \, {\rm cm^{-3}}$] and the warm-hot intergalactic medium [WHIM; $10^5 \lt T\lt 10^7 \, {\rm K}$, $n_{\rm H} \lt 10^{-4}(1+z)\, {\rm cm^{-3}}$] constitute ${\sim } 39$ and ${\sim } 46{{\ \rm per\ cent}}$ of the baryons at redshift z = 0, respectively. Our results indicate that the WHIM may constitute the largest reservoir of missing baryons at redshift z = 0. Using our Cosmic Web classification, we predict the WHIM to be the dominant baryon mass contribution in filaments and knots at redshift z = 0, but not in sheets and voids where the cool, diffuse IGM dominates. We also characterize the evolution of WHIM and IGM from redshift z = 4 to redshift z = 0, and find that the mass fraction of WHIM in filaments and knots evolves only by a factor of ∼2 from redshift z = 0 to 1, but declines faster at higher redshift. The WHIM only occupies $4\!-\!11{{\ \rm per\ cent}}$ of the volume at redshift 0 ≤ z ≤ 1. We predict the existence of a significant number of currently undetected O vii and Ne ix absorption systems in cosmic filaments, which could be detected by future X-ray telescopes like Athena.

Objective To discuss the presentation, evaluation, and management of pediatric laryngeal web. Study Design Retrospective case series. Setting Single tertiary care center. Subjects All patients with laryngeal web at Boston Children’s Hospital in the past 22 years. Methods No exclusion criteria. Charts mined for age at presentation, presenting symptoms, degree/location of web, associated syndromes, number/type of surgical procedures, and postoperative outcomes. Results Thirty-seven patients were included (13 male, 24 female). Average age at diagnosis was 3.7 years (0-19.5 years). Mean follow-up was 4.4 years (range, 0-16.4 years). There were 26 congenital webs (70.2%) and 11 acquired webs (29.8%). Presenting symptoms were vocal (29 patients, 78.4%) and respiratory (22 patients, 60%). Underlying syndromes or synchronous airway lesions included the following: premature (n = 5), congenital heart disease (n = 18), subglottic stenosis (n = 5), 22q11.2 deletion syndrome (n = 10), and recurrent respiratory papillomatosis (n = 4). There were 20 type 1 webs, 6 type 2 webs, 8 type 3 webs, and 3 type 4 webs; 10 had subglottic extension of the laryngeal web. Twelve patients were managed conservatively with observation. Eighty-four interventions were performed: 18 open and 66 endoscopic (sharp division, 32; dilation, 33; mitomycin C, 14; laser, 5; keel, 6; triamcinolone injection, 8; stent, 15; removal of granulation tissue, 5). Tracheotomy was required in 11 patients, and 5 patients were decannulated. Voice improved in 12 patients, with respiratory symptoms in 12 patients. Web recurred in 17 patients. One patient died due to airway complications. Conclusions Pediatric laryngeal web is an uncommon but challenging lesion. Patients need to be evaluated for comorbid syndromes and synchronous airway lesions. Management includes open and endoscopic procedures. Procedures should be tailored to the child’s presentation.

AbstractOverweight and obesity may increase risk of disease progression in men with prostate cancer, but there have been few studies of weight loss interventions in this patient group. In this study overweight or obese men treated for prostate cancer were randomised to a self-help diet and activity intervention with telephone-based dietitian support or a wait-list mini-intervention group. The intervention group had an initial group meeting, a supporting letter from their urological consultant, three telephone dietitian consultations at 4-week intervals, a pedometer and access to web-based diet and physical activity resources. At 12 weeks, men in both groups were given digital scales for providing follow-up weight measurements, and the wait-list group received a mini-intervention of the supporting letter, a pedometer and access to the web-based resources. Sixty-two men were randomised; fifty-four completed baseline and 12-week measurements, and fifty-one and twenty-seven provided measurements at 6 and 12 months, respectively. In a repeated-measures model, mean difference in weight change between groups (wait-list mini-intervention minus intervention) at 12 weeks was −2·13 (95 % CI −3·44, −0·82) kg (P = 0·002). At 12 months the corresponding value was −2·43 (95 % CI −4·50, −0·37) kg (P = 0·022). Mean difference in global quality of life score change between groups at 12 weeks was 12·3 (95 % CI 4·93, 19·7) (P = 0·002); at 12 months there were no significant differences between groups. Results suggest the potential of self-help diet and physical activity intervention with trained support for modest but sustained weight loss in this patient group.

Background: Many studies explored the prognostic value of the modified Glasgow Prognostic Score (mGPS) in urothelial carcinoma (UC), but the results are controversial. This study aimed to quantify the relationship between pretreatment mGPS and survival in patients with UC. Methods: A systematic literature search was conducted using Embase, PubMed, and Web of Science to identify eligible studies published before August 2022. Pooled hazard ratios (HRs) with 95% confidence intervals (CIs) were used to assess the association between pretreatment mGPS and the prognosis of UC. Results: Thirteen eligible studies involving 12,524 patients were included. A high mGPS was significantly associated with poor overall survival (mGPS 1/0: HR = 1.33, 95% CI 1.12–1.58, p = 0.001; mGPS 2/0: HR = 2.02, 95% CI 1.43–2.84, p < 0.0001), progression-free survival (mGPS 1/0: HR = 1.26, 95% CI 1.03–1.53, p = 0.021; mGPS 2/0: HR = 1.76, 95% CI 1.12–2.77, p = 0.013), recurrence-free survival (mGPS 1/0: HR = 1.36, 95% CI 1.18–1.56, p < 0.0001; mGPS 2/0: HR = 1.70, 95% CI 1.44–2.000, p < 0.0001), and cancer-specific survival (mGPS 2/0: HR = 1.81, 95% CI 1.30–2.52, p < 0.0001). A subgroup analysis of OS also yielded similar results. Conclusions: Evidence suggests that high pretreatment mGPS in UC is closely related to poor survival. Pre-treatment mGPS is a powerful independent prognostic factor in patients with UC.

Emerging adults are at high risk of obesity but behavioral weight loss programs do not meet their needs. Emerging adults ( N = 52, age = 22.3 ± 2 years, body mass index = 34.2 ± 5.5 kg/m2, 46.2% non-Hispanic White) were randomly assigned to one of three behavioral weight loss programs adapted based on formative work: face-to-face behavioral weight loss, web-based behavioral weight loss, or web plus optional community sessions (Hybrid). Assessments occurred at 0 and 3 months. Engagement and self-monitoring were highest in Hybrid. Intent-to-treat weight losses were −2.8 ± 2.9 percent in face-to-face behavioral weight loss, −2.2 ± 4.5 percent in web-based behavioral weight loss, and 4.8 ± 4.9 percent in Hybrid. Percent achieving ⩾5 percent weight loss was highest in Hybrid (63%). Findings suggest potential for adapted behavioral weight loss to promote engagement and weight loss in emerging adults.

The electronic catalog of infrared and optical photometry in the Hubble Deep Field South (NICMOS) identifies galaxies at redshifts ranging from z near 0 through z greater than 10. In this paper we try to investigate the formation and evolution of different structures in the Universe, using cosmological N-body simulations. By means of 2563/5123-point, 5–25 Mpc simulations, we traced the relation among the evolution of first order filamentary web structures, galactic and cluster structures, and second order filamentary web structures. These simulations have been made in order to derive the environmental effects (first/second order collapse, heating/cooling mergers) in the early Universe (10 > z > 2), closely related to galactic and cluster evolution.

Background Groin wound infections in vascular surgery are still a common complication and challenging problem. This systematic review aimed to establish a complete view of patient characteristics and clinical outcomes for infected groin wounds following vascular surgery reconstruction using muscle flaps and to evaluate the differences in outcomes between the sartorius muscle flap (SMF), rectus femoris muscle flap (RFF), and gracilis muscle flap (GMF). Methods PubMed, Scopus, and Web of Science were systematically searched from inception to April 2021. Random-effects meta-analysis for comorbidities and outcomes and subgroup analyses for outcomes were performed. Results Thirty studies were included in qualitative and quantitative syntheses. Overall pooled data showed the following outcome rates: 4.5% muscle flap necrosis (95% confidence interval [CI], −3.4–12.3%; I 2 = 0%), 21.8% overall complications (95% CI, 15.8–27.7%; I 2 = 0%), 8.0% limb loss (95% CI, 1.9–14.1%; I 2 = 0%), 15.4% graft loss (95% CI, 5.0–25.3%; I 2 = 37.9%), and 7.4% 30-day mortality (95% CI, −.9–15.6%; I 2 = 0%). The rates of overall complications were 20.3% (95% CI, 12.1–28.2%; I 2 = 0%), 23.2% (95% CI, 11.2–34.5%; I 2 = 10.2%), and 18.0% (95% CI, −3.537.8%; I 2 = 0%) for the SMF, RFF, and GMF, respectively. The rate of limb loss was highest for the GMF (17.2%; 95% CI, −4.237.2%; I 2 = 0%). The rate of graft loss for the RFF was the highest (20.7%; 95% CI, .6–39.1%; I 2 = 53.9%). The rate of 30-day mortality was the lowest for the SMF (5.3%; 95% CI, −6.1–16.6%; I 2 = 0%). Conclusions The effectiveness and safety of muscle flap reconstruction for infected groin wounds following vascular surgery are clearly positive. This review indicated a tendency for lower complication rates with the SMF than with other muscle flaps.

Abstract:
OBJECTIVEThe Woven Endobridge (WEB) device has been in clinical use for the treatment of brain aneurysms for the past 4 years. Observational studies to assess clinical outcome and related complications have been published. Clear evidence is required to better understand the safety profile of the WEB device. The authors here present a multicenter series that provides a detailed safety analysis focused on patient selection, procedural events, and technical issues of treated patients throughout the United Kingdom (UK).METHODSA nationwide password-protected database was set up to collect anonymous information across the UK (14 centers). Complications and clinical outcome were analyzed for the initial 109 patients (112 procedures). An independent root cause analysis classified the complications into groups (procedural, disease, device, ancillary device, and other). The modified Rankin Scale (mRS) was used as a marker of clinical outcome.RESULTSEach of the 109 patients had 1 aneurysm suitable for WEB treatment (109 aneurysms). Three patients had 2 procedures, making a total of 112 procedures performed. Eight procedures were abandoned because of access issues; 2 patients went on to have a successful procedure. All 109 patients had a preprocedure and discharge mRS scores recorded. One hundred patients had a recorded mRS score from a > 3-month follow-up.Deployment of the WEB device was successful in 103 (94.5%) of 109 patients and 104 (92.9%) of 112 procedures. One patient had 2 successful WEB procedures on separate occasions. Patients without a successfully implanted WEB device were included in the analysis. Selection analysis showed that the average patient age was 56.5 years among 34 men and 75 women. The percentage of incidental aneurysms was 58.7%, acute 16.5%, symptomatic 18.3%, and recurrent 6.4%. Further results analysis showed that 40 (36.7%) of 109 patients had recorded adverse events, including those unrelated to the WEB device. Events that could be related to the WEB device numbered 17 (15.6%) among the 109 patients. Two patients with device-related complications were symptomatic. Overall, 11 patients (10.1%) had persistent clinical sequelae. Thromboembolism was the most prevalent event, affecting 15.6% of the patients (17 of 109), and 6.4% of the patients (7 of 109) with a thromboembolism were symptomatic.Overall mortality before discharge was 0% and at the > 3-month follow-up was 5% (5 of 100 patients). Morbidity was defined as an mRS score increase to > 2. Overall morbidity at discharge was 1.8% (2 of 109) and at the > 3-month follow-up was 6% (6 of 100). No device-related morbidity or mortality was associated with this group.CONCLUSIONSThe UK data show that the WEB device is safe for clinical use. Thromboembolic complication adds a risk that should be minimized with appropriate anticoagulation and correct sizing of the device. There is scope for further evaluation and standardization of an anticoagulation regimen for the WEB device.

Perkembangan teknologi informasi berkembang pesat seiring dengan pertumbuhan penggunanya. Contoh dari perkembangan teknologi adalah penggunaan website untuk mendukung kegiatan pembelajaran. Website merupakan kumpulan halaman web yang dapat diakses secara publik. Website dapat terdiri dari teks, gambar, video, dan media suara lainnya. Namun dengan berkembangnya suatu teknologi, maka perkembangan kerentanan atau serangan terhadap teknologi tersebut juga bertambah. Berdasarkan laporan tahunan monitoring keamanan siber tahun 2021 oleh Badan Siber dan Sandi Negara (BSSN), terdapat lebih dari 1,6 miliar serangan siber yang telah terjadi di Indonesia. Penelitian ini akan menggunakan Acunetix Web Vulnerability Scanner (WVS) untuk mengaudit keamanan website SMK Muhammadiyah 2 Terpadu Pekanbaru (SMK MUTI). Penelitian ini akan mengkaji kelemahan keamanan website SMK MUTI dan membahas bagaimana Acunetix Web Vulnerability dapat membantu dalam meningkatkan tingkat keamanan website tersebut. Metode Vulnerability Assessment (VA) yang digunakan adalah analisis deskriptif, yaitu data yang diperoleh disajikan dalam bentuk tabel, sehingga memungkinkan untuk memperjelas hasil analisis yang dilakukan dalam meng-audit. Berdasarkan data yang diperoleh dari hasil scanning iterasi 1 yang dilakukan, website SMK MUTI berada pada level ancaman 3 tergolong tinggi dengan ditemukan 192 peringatan atau kerentanan, dimana 2 diantaranya berada pada level tinggi dan 11 berada pada level sedang. Berdasarkan audit, dilakukan perbaikan dan pengujian pada penelitian di situs SMK MUTI ini, hasil yang telah dilakukan tingkat ancaman yang dicapai berada pada level 1, dimana pada level tinggi, jumlah kerentanan menjadi 0 dan tingkat dukungan juga menjadi 0, sehingga dapat disimpulkan bahwa situs SMK MUTI saat ini dengan status level 1 dapat bebas dari kerentanan keamanan.

Siantar Utara District Office is one of the sub-districts in the Pematangsiantar city, North Sumatra Province. Through this sub-district office, residents can take care of various forms of licensing. At the subdistrict office also routinely held awards to employees which are held every three months which were previously counted on the excel application. The calculation process determines the best employee that the writer does, namely with the Excel application and uses the web-based ELECTRE method. Based on the final results obtained from the calculation results in both the excel application and the web-based application, alternative A1 with Ekl = 4 value, alternative A2 with Ekl = 4 value, alternative A3 with Ekl = 0 value, alternative A4 with Ekl = 2 value , alternative A5 with the value Ek = 2 From this decision support system, the final result is obtained with alternative A1 with the value Ekl = 4 as the best employee.
