BackgroundLittle is known about the impact of hospitalisation on prescribing in UK clinical practice.AimTo investigate whether an emergency hospital admission drives increases in polypharmacy and potentially inappropriate prescriptions (PIPs).Design and settingA retrospective cohort analysis set in primary and secondary care in England.MethodChanges in number of prescriptions and PIPs following an emergency hospital admission in 2014 (at admission and 4 weeks post-discharge), and 6 months post-discharge were calculated among 37 761 adult patients. Regression models were used to investigate changes in prescribing following an admission.ResultsEmergency attendees surviving 6 months (N = 32 657) had a mean of 4.4 (standard deviation [SD] = 4.6) prescriptions before admission, and a mean of 4.7 (SD = 4.7; P<0.001) 4 weeks after discharge. Small increases (<0.5) in the number of prescriptions at 4 weeks were observed across most hospital specialties, except for surgery (−0.02; SD = 0.65) and cardiology (2.1; SD = 2.6). The amount of PIPs increased after hospitalisation; 4.0% of patients had ≥1 PIP immediately before pre-admission, increasing to 8.0% 4 weeks post-discharge. Across hospital specialties, increases in the proportion of patients with a PIP ranged from 2.1% in obstetrics and gynaecology to 8.0% in cardiology. Patients were, on average, prescribed fewer medicines at 6 months compared with 4 weeks post-discharge (mean = 4.1; SD = 4.6; P<0.001). PIPs decreased to 5.4% (n = 1751) of patients.ConclusionPerceptions that hospitalisation is a consistent factor driving rises in polypharmacy are unfounded. Increases in prescribing post-hospitalisation reflect appropriate clinical response to acute illness, whereas decreases are more likely in patients who are multimorbid, reflecting a focus on deprescribing and medicines optimisation in these individuals. Increases in PIPs remain a concern.

BackgroundObesity is a global pandemic that threatens the health of the population and the sustainability of publicly funded health care. This randomised controlled trial addresses the gap in the literature surrounding unconscious persuasion and its use in weight loss and weight management. The Slimpod® tool is unlike any of those currently available on the market. Using breakthrough research in ‘nudge’ thinking, it is designed to retrain an adult’s habitual and emotional response to foodstuffs. This therapeutic model allows unconscious thought to be shaped into a manner more consistent with a healthy lifestyle. Candidates can then take control of their eating behaviours to induce a holistic state of wellbeing.AimTo assess the effectiveness of an audio unconscious-persuasion weight loss/weight management intervention (Slimpod®) compared with audio relaxation (control).MethodEighty-two overweight adults were randomised to intervention (n = 41) and control groups (n = 41). Weight was assessed at trial commencement, mid-trial (12 weeks), and trial end (24 weeks). Secondary outcomes were assessed using the Eating Self-Efficacy Scale (ESES), Exercise Confidence Scale (ECS), and Quality of Life Index Generic Version III (QLI-G3) at the start and end of the trial.ResultsReports found a statistically significant difference in mean weight loss between intervention group (1.7 kg at 12 weeks and 4.3 kg at 24 weeks) versus control (0.6 kg and 1.2 kg respectively) at P<0.001. ESES scores showed greater self-efficacy (P = 0.008) in intervention at 24 weeks. No significant differences in ESES negative affect sub-scale score or ECS were observed.ConclusionSlimpod® was effective at reducing weight and increasing eating self-efficacy in overweight adults.

Overlooked or ignored for decades by music historians and specialists in performance practices, the real-life Trapp Family Singers achieved enormous success especially in Canada and the United States during the late 1930s, 1940s, and early 1950s through their appeal to—and consequent reception in terms of—"family values." These values included, but were not altogether limited to, the Trapp's Christian faith, patriotic activities, and contributions to charitable causes, as well as the wholesome image associated with the family's private lives, their Vermont "Sing Weeks," and their more than 1,800 estimated concert appearances.

The literature has provided substantial evidence for the positive associations between family meals and well-being. The underlying mechanism of this relationship has not been explicitly examined. The Happy Family Kitchen II project was a cluster randomized controlled trial of a community-based family intervention. Using data from this project, this article examined the direct and indirect associations among family meal practices, family communication time and quality, and well-being in Hong Kong. A total of 1,261 participants completed a self-administered questionnaire at baseline, 4 weeks, and 12 weeks. Cross-sectional and prospective mediation analyses showed that family meal and family meal preparation indirectly influenced family health, happiness and harmony, subjective happiness, and mental and physical quality of life through their effects on family communication quality. In contrast, family communication time was a weaker mediator of these effects. These findings highlight the role of quality rather than quantity of family communication in promoting well-being.

BackgroundAlthough the importance of secondary prevention after transient ischaemic attack (TIA) or minor stroke is recognised, research is sparse regarding novel, effective ways in which to intervene in a primary care context.AimTo pilot a randomised controlled trial (RCT) of a novel home-based prevention programme (The Healthy Brain Rehabilitation Manual) for patients with TIA or ‘minor’ stroke.Design and settingPilot RCT, home-based, undertaken in Northern Ireland between May 2017 and March 2018.MethodPatients within 4 weeks of a first TIA or ‘minor’ stroke received study information from clinicians in four hospitals. Participants were randomly allocated to one of three groups: standard care (control group) (n = 12); standard care with manual and GP follow-up (n = 14); or standard care with manual and stroke nurse follow-up (n = 14). Patients in all groups received telephone follow-up at 1, 4, and 9 weeks. Eligibility, recruitment, and retention were assessed; stroke/cardiovascular risk factors measured at baseline and 12 weeks; and participants’ views were elicited about the study via focus groups.ResultsOver a 32-week period, 28.2% of clinic attendees (125/443) were eligible; 35.2% of whom (44/125) consented to research contact; 90.9% of these patients (40/44) participated, of whom 97.5% (39/40) completed the study. After 12 weeks, stroke risk factors [cardiovascular risk factors, including blood pressure and measures of physical activity] improved in both intervention groups. The research methods and the programme were acceptable to patients and health professionals, who commented that the programme ‘filled a gap’ in current post-TIA management.ConclusionFindings indicate that implementation of this novel cardiac rehabilitation programme, and of a trial to evaluate its effectiveness, is feasible, with potential for clinically important benefits and improved secondary prevention after TIA or ‘minor’ stroke.

Postpartum family planning (PPFP) is the initiation and use of contraception during the first year after delivery. Following delivery mothers are encouraged to attend local Baby Clinics where infant follow up and immunization services and contraceptive advice should be available. The general understanding and importance of contraceptive use of postpartum mothers in this period has not been fully explored within our services. The aim of this study was to assess the current knowledge and the utilisation of contraception at the primary level of care among women who delivered 6 to 14 weeks earlier. We also hoped to identify possible obstacles to accessing family planning services. Methods This was descriptive cross-sectional study that was conducted between March and September 2014, in the Metro West of Cape Town. The study included five Baby Clinics in local health centres. Ethics committee approval was granted by the Human Research Ethics Committee of the Faculty of Health Sciences of UCT (HREC REF: 544/2013) and further permission was granted for study sites by the Provincial Health Research Council of the Western Cape Government (RP 004/2014). A total of 228 mothers, who were 6-14 weeks postpartum and attended a Baby Clinic during the study period were recruited. Questionnaire based interviews were conducted and data were entered using Microsoft Excel 2012 spreadsheets and were analysed using SPSS version 22.

Abstract:
The Kenyan society is patriarchal, has an alarming rate of deforestation where rural farmers, especially women, are highly affected by climate change. There is little research on companies operating in dry areas with a mission to address poverty, ecology and women. The objective of this study was therefore to understand how an investment model can alleviate poverty in rural Kenya and what the consequences of doing so are for the local community. This was answered using a qualitative research approach presented in an ethnographic case study, conducting 29 interviews. The field research took place at three different locations at Better Globe, an agroforestry company operating in dry areas who mainly employ and work with women in rural Kenya. Our research, analysed through an ecofeminist lens, demonstrates that the local community benefits on several areas; access to water, employment, firewood and grass, microfinance, training and education. However, there is a big power distance within the company and a high dependency from the workers as Better Globe is the sole big employer in the region. We welcome further research between the merging of ecofeminism and business, operating within the structures of the patriarchal and capitalist society.

The method of this study is using a questionnaire survey to conducting data collection. There are 310 questionnaires have been completed. The results show that there is a negative relationship between the execution degree of labor policies and work-family conflicts. But with the quality of life (physiological function, role limitations due to physical problems), and overall feelings of health have a positive relationship. The quality of life is positively related to the degree of labor policy identity. This study recommends that companies strictly put Five-Day Work Week policies into effect will benefit employees in reducing the Work-Family Conflict, and enhancing the quality of life, and further improving labors’ recognition in new policies.

AbstractIt was only since 1946 [1945] when my uncle moved to Berkeley that I got to know him well. Before this time we had never lived in the same town, and I had only seen him rarely. Otto Stern moved into a house he had bought several years earlier in the Berkeley hills not far from my parents’ house. Because he was a bachelor, he hired a housekeeper who came in six days a week for a few hours to cook and keep house. He loved good food and good wine. The housekeeper for the last years did not keep the house as clean as he would have liked, but her cooking met with his approval, so she stayed for many years. On Sundays, he would have dinner with his sister, Berta (my mother) and family, or he would go into town for dinner in a restaurant and then to a movie. He loved movies, and Shirley MacLaine was one of his favorite actresses.

This chapter offers insights on how caregiving kin can maintain relationships during maternal imprisonment. These insights are especially pertinent in light of the current political and policy climate, which has advocated support for prisoners' family ties. Yet, contact with a person in prison is restricted and mediated by criminal justice policies and processes. The chapter then focuses on the caregivers' experiences of navigating the criminal justice system (CJS) as they followed the mother through court and into prison. There are four main sections: caregivers' experiences of court processes; caregivers' opportunities for re-establishing contact with the mother on the telephone and face to face in the first days and weeks of her sentence; caregivers' perceptions of maintaining contact with the mother; and caregivers' understandings and perceptions of Mother and Baby Units (MBUs) when a child in their family was housed in prison or awaiting confirmation of a place.

If homework assignments that require the use of a computer are given to students, should they be penalized for what their family cannot afford? In this case study, Mrs. Lincoln, who developed her course using a web-based course management system named Moodle, spent time working on her Moodle pages and posting assignments. She then explained to students how the site worked. She also spent a week in the computer lab training her students to become proficient using the Moodle application. After a couple of weeks, Mrs. Lincoln noticed that a quarter of her students were not completing their Moodle based assignments.

Abstract:
Congenital protein C (PC) deficiency is associated with thrombophilia. Heterozygotes with about half-normal plasma PC levels may present with venous thromboembolic events usually beginning during adolescence or young adulthood. A 26-year-old swiss woman had experienced an iliofemoral deep-vein thrombosis without obvious etiology six years.ago. In June, 1986 very low levels of PC antigen (25 %) and activity (27 %) were found when she was six-month pregnant. Three other family members (62, 24 and 19-year-old) had PC levels around 50 % but were symptomfree. Because of the post-thrombotic~syndrome and the pregnancy, ambulatory heparin therapy was immediately started in the patient (10-16'000 IU twice daily, s.c.) in order to maintain a plasma heparin level between 0.2 and 0.5 U/ml six hs after the morning injection. Delivery was induced at full-term whilst heparin was stopped for a few hours. Four weeks after delivery anticoagulation was discontinued and, so far, the patient remained symptomfree. The newborn showed no perinatal problem and the PC antigen level assayed in the umbilical venous blood was 22 % (normal range in the literature 18-46 %). Antithrombin III and protein S levels as well as fibtinolytic potential were within normal values in all family members.Conclusions. 1) Only one out of four heterozygote PC deficient family members had presented with venous thromboembolism. 2) The symptomatic subject had the lowest PC level in the family (activity and antigen around 25 %). 3) This woman experienced an event-free pregnancy and delivery although heparin was started only in the sixth month of pregnancy. 4) Thus, penetrance of the thrombotic trait may be quite variable (and low) amongst PC deficient heterozygotes, an observation which raises the question of the indication of long-term anticoagulation in these individuals. 5) Pregnancy did not affect the PC antigen and activity levels in our patient.

The incidence of defective T-PA release in 158 young unrelated patients (&lt;45 years old) with deep vein thrombosis was studied and compared to that of PC-, PS-, AT III-, fibrinogen- and plasminogen deficiency. Thrombotic episodes were documented using venography with contrast medium. Venous occlusion test (VO) over 20 min. was performed in all patients, 8-12 weeks after thrombosis. T-PA antigen (Biopool kit) , T-PA activity (on fibimplates) and PAI (T-PA-inhibitor, Biopool kit) were measured before and after VO in the fasting morning samples. Furthermore we investigated the functional and immunologic levels of PC,PS,AT III, fibrinogen and plasminogen. We detected 28 patients (15 females, 13 males)= 17.7% with abnormal T-PA release. In these patients the VO test was repeated three times in an interval of 6-8 weeks. Release of T-PA activity and T-PA-Ag was diminished in all these patients. PAI levels were enhanced in 12 of these 28 patients. The rate of recurrency of thrombosis was 52%. A family history of thrombosis was reported only in 20%The incidence of PC def. was 9.4%, of PS def. 6.3%, of AT III def 5%, of dysfibrinogenemia 0.6% and of plasminogen def. 1.2%. No combined defect of abnormal T-PA release with other known hereditary coagulation or fibrinolysis disorders could be detected.In 11 healthy volunteers we investigated 4 different time periods of VO, 5, 10, 15 and 20 min. in an interval of 10 days in order to find the suitable time for VO. It was striking that T-PA activity (on fibrin plates) did not decrease to the same extent as T-PA-Ag. The different behaviour is demonstrated on table 1. Decrease in % is compared to the values of 20 min. VO.

Abstract:
Chronic diseases affect the quality of life of families of sick people on the level of their physical, economic, mental, and subjective well-being. The aim of the paper is two-folded: a) to determine the economic and social consequences of the child's illness for families and b) to identify the factors that may influence the origin and development of asthma in the child population. The data come from a survey we carried out on a sample of parents with children who are treated in the Children's Speleotherapy Sanatorium in Ostrov near Macocha (a total sample of 201 respondents). We checked for any association by use of correlation analysis, Kruskal-⁠Wallis and Chi-square test. More than half of the families reported that the child's illness negatively influences the involvement in the labour market, families show an increase in financial costs associated with rehab and a financial loss during the care of a sick child. One third of the children’s patients has problems with missing classes at school, the average absence is 8 weeks in the school year, and every tenth child has difficulty in establishing social contacts. Analyses of external factors in relation to asthma show the effect of education on smoking in the family, but did not confirm the effect of age and mother's education on the birth weight of the child or the type of birth. From the results, it is clear that asthma adversely affects the quality of life of the asthmatic child and his family. The disease is thus associated with social and psychological consequences, which limits the performance at school for children and work activities for parents and other activities. All in one, it has not only health, but also socio-economic consequences.

Abstract Engineering analysis of complex structures or mechanical systems typically involves contact with multiple components, large deformation, and material nonlinearity, which requires the application of nonlinear finite element methods. Despite the advancement of commercial software for finite element analysis (FEA), nonlinear FEA of a multi-component mechanical assembly will take hours to days, and even weeks to complete. It is highly desired to develop a reduced-order model for a family of complex structures that can reduce an original problems’ complexity and degree of freedom but has a reasonably small discrepancy with the full model and significantly reduces the computation time. The typical approach to construct a reduced model includes 1) the response surface method via numerical design of experiments and, 2) the simplified physics approach. In this paper, it is proposed to develop a reduced model through the combination of simplified physics, dimensional analysis [1], and numerical design of experiments. The approach is applied to the construction of a reduced model for the analysis of a downhole plug [2]. The developed reduced model is verified by full-scale FEA models and validated through physical tests. The reduced model is implemented in a spreadsheet and takes only seconds to complete a calculation in contrast to hours using a full FEA model, enabling engineers’ quick evaluation of the corresponding designs.

Blade tuning is one of the key tasks of the modern aircraft engine design. Generally, a couple of vibration modes of a blade need to be tuned by changing the distribution of its mass and/or stiffness to avoid the resonance at some specified rotating speed. The widely used traditional method for blade tuning is a loop-based process. It requires a number of design iterations between the aero engineer and the structure engineer. It usually takes a few weeks or up to a few months, depending on the experiences of the engineers, to reach an acceptable new design. The uncertainties caused by the non-convergence associated with the loop-based iteration process have led to serious issues for both project management and quality control. To avoid the problem, this paper presents a robust one-piece flow process based on the principle of the set-based concurrent engineering and applying the concept of Lean &amp; DFSS. Following the concept of Lean &amp; DFSS process, the one-piece flow blade tuning starts with defining key parameters and objectives. These parameters consist of a set of design variables relevant to both structure and aero disciplines. The variation range of each parameter is predefined by the manufacture feasibility and the past design experiences. Based on the principles of DOE, only a definite number of FEA models are developed and analyzed. From these analysis results, important parameters are selected and a family of response functions of those concerned objectives is formulated. The achievable ranges of the objectives are well determined accordingly. Guided by these functions, the structure and aero engineers are then able to evaluate instantly any new design from the various combinations of the above-mentioned design parameters. A robust design can be obtained from the given sensitivity of the variance of those design parameters. This methodology can be applied to blade and vane tuning for fans, compressors and turbines alike. In this paper an example of the application of this process is presented for a hollow fan blade tuning, which is accomplished within a week as planned. The methodology can be standardized and used in many other related engineering design processes as well.

Restriction fragment length polymorphism (RFLP) analysis has been used to perform family studies, including prenatal diagnosis, in 21 haemophilia A kindred.Two intragenomic RFLPs were studied in conjunction with one linked RFLP. The intragenomic BgII RFLP,situation 3' to exon 26 was detected with cDNA probe C (Genetics Institute) giving bands of 20kb (17% of X chromosomes) and 5kb (83%), and the intragenomic Bell RFLP, situated 3' to exon 18, was detected with the genomic DNA probe pi 14.12 from Genentech. The frequency of this RFLP in the local population was 23% (1.1 kb allele) and 77% (0.88kb allele). The linked probe DXS15 (DX13) was used to detect a Bglll RFLPwith alleles of 5.8kb (45%) and 2.8kkb (55%). A recombination rate of approximately 5% has been estimated between the factor VIII and DXS15 lociCarrier studies were performed in 15 kindreds. 25 obligate carriers were identified and of these, 20 were potentially informative (heterozygous and phase known) for at least 1 RFLP (9 for Bgll, 9 for Bell and 7 for BgIII). 34 possible carriers were studied, of which 13 were diagnosed as normal (6 by BgII, 6 by Bell and 5 by BgIII). 17 were diagnosed as carriers (2 by Bgll, 12 by Bell and 10 by Bglll) and diagnosis was not possible in a further 4 cases. Of these diagnosed as carriers 3 were non-informative for all RFLPs, and 14informative for at least one RFLP (3 by Bgll, 8 by Bell and 8 with BgIII).Prenatal diagnosis was attempted by analysis of DNA extracted by chorionic villussampling in 6 cases of male fetuses at risk of havinghaemophilia A. 1 fetus was diagnosed as being affected (Bell) and was electively terminated. Three otherfetuses were diagnosed as normal by the BgIII/DXS15 RFLP, but the two intragenomic RFLPs were non-informative. Because of the possibility of a crossover allthree patients opted for mid-trimester fetoscopy andmeasurement of fetal factor VIII at Kings College Hospital, London (Dr Reuben Mibashan), where the diagnoses were confirmed. In the 4th case a normal fetus was diagnosed by the Bgll RFLP analysis, but a spontaneous abortion at 12 weeks prevented confirmation of this result. In the final case of twin male fetuses, none of the RFLPs was informative and both were diagnosed as normal by fetal blood sampling at fetoscopy.

Tables and identification keys were compiled to determine whether plants and seeds belong to the Brassicaceae family, the Thlaspi genus and the T. arvense species. It was concluded that the identification of seeds of the field pennycress usually presents no difficulties due to the presence of characteristic features of their morphology.

The research was performed in 2019 on the fields of the Nemchinovka research center. Infestation was represented by two types of monocotyledonous weeds from the Bluegrass family (Grasses): annual bluegrass (Póa Annua) and barn grass (Echinóchloa crus-gálli). The use of anti-slag herbicides reduces the number of weeds in the experiment and their air-dry mass. Herbicide treatment increased the wheat grain yield by 1.19-1.67 t / ha and increased protein by 0.5-3.3%.

Abstract:
This paper presents the results of a demonstration of a unique water distillation system at the Musco Family Olive Plant in Tracy, California. The Musco REWS (Renewable Energy/Water System) was developed by Combined Solar Technologies (CST) of Carmel Valley, California using its SteamBoy™ brine boiler technology. The system is powered using solar thermal and biomass energy. The notoriously hard to clean waste water from the olive canning process was a challenge for CST, a start-up company who had previously done successful technology demonstration projects for the California Department of Water Resources (agricultural drainage water) and Clint Eastwood’s Tehama Golf course Water Treatment Plant (Reverse Osmosis Brine). The olive waste water at the Musco plant contained a high concentration of salts as well as organic materials. CST successfully treated water directly from the plants waste water stream (12,000 TDS) as well as the water from the plants Title 27 ponds (67,000 TDS). The CST processed water averaged 250 TDS. The CST REWS at the Musco olive facility uses one of the plants waste (olive pits) streams to provide the heat energy for the process. The olive pits are combusted in fluid bed burners that are specially designed for olive pit combustion. The Musco plants 200 tons per week of olive pits contain 8800 btu/lb (dry). The olive pits are fed into the CST burners where the heat energy is used to heat a heat transfer fluid (HTF) to 450F. The HTF is then directed to the SteamBoy™ brine boilers. The heat from the HTF evaporates the plants waste water under pressure. The resulting pressurized saturated steam is then used to power SteamBoy™ steam engine/generator sets. The engines are set up to leave large quantities of heat in the exhaust. That heat is used to take the remaining solids and sludge to a dry state in custom built drying pans that separate the minerals from the water, leaving a very high concentration of organics in the remaining water. That water is used in the burners for temperature control and as a way to eliminate the organics with combustion. The exhaust from the combustion of the olive pits goes first though an electrostatic particle separator and then to a selective catalytic reduction system. From there the exhaust is directed to bubbling algae tanks that reduce the CO2 by over 50%. The system has completed a successful demonstration phase and is now being upgraded to a full scale plant (50,000 GPD).

Trucks serve significant amount of freight tonnage and are more susceptible to complex interactions with other vehicles in a traffic stream. While traffic congestion continues to be a significant ‘highway’ problem, delays in truck travel result in loss of revenue to the trucking companies. There is a significant research on the traffic congestion mitigation, but a very few studies focused on data exclusive to trucks. This research is aimed at a regional-level analysis of truck travel time data to identify roads for improving mobility and reducing congestion for truck traffic. The objectives of the research are to compute and evaluate the truck travel time performance measures (by time of the day and day of the week) and use selected truck travel time performance measures to examine their correlation with on-network and off-network characteristics. Truck travel time data for the year 2019 were obtained and processed at the link level for Mecklenburg County, Wake County, and Buncombe County, NC. Various truck travel time performance measures were computed by time of the day and day of the week. Pearson correlation coefficient analysis was performed to select the average travel time (ATT), planning time index (PTI), travel time index (TTI), and buffer time index (BTI) for further analysis. On-network characteristics such as the speed limit, reference speed, annual average daily traffic (AADT), and the number of through lanes were extracted for each link. Similarly, off-network characteristics such as land use and demographic data in the near vicinity of each selected link were captured using 0.25 miles and 0.50 miles as buffer widths. The relationships between the selected truck travel time performance measures and on-network and off-network characteristics were then analyzed using Pearson correlation coefficient analysis. The results indicate that urban areas, high-volume roads, and principal arterial roads are positively correlated with the truck travel time performance measures. Further, the presence of agricultural, light commercial, heavy commercial, light industrial, single-family residential, multi-family residential, office, transportation, and medical land uses increase the truck travel time performance measures (decrease the operational performance). The methodological approach and findings can be used in identifying potential areas to serve as truck priority zones and for planning decentralized delivery locations.

In this Insight Note, we report results of a phone survey that the RISE Tanzania Research team conducted with 2,240 parents (or alternate primary care-givers) of primary school children following the school closures in Tanzania. After the first case of COVID-19 was confirmed in Tanzania on 16 March 2020, the government ordered all primary schools closed the following day. Schools remained closed until 29 June 2020. Policymakers and other education stakeholders were concerned that the closures would lead to significant learning loss if children did not receive educational support or engagement at home. To help stem learning loss, the government promoted radio, TV, and internet-based learning content to parents of school-age children. The primary aims of the survey were to understand how children and families responded to the school closures, the education related activities they engaged in, and their strategies to send children back to school. The survey also measures households’ engagement with remote learning content over the period of school closures. We supplement the findings of the parent survey with insights from interviews with Ward Education Officers about their activities during the school closures. The survey sample is comprised of primary care-givers (in most cases, parents) of students enrolled in Grades 3 and 4 during the 2020 school year. The survey builds on an existing panel of students assessed in 2019 and 2020 in a nationally representative sample of schools.4 The parent surveys were conducted using Computer Assisted Telephonic Interviewing (CATI) over a two-week period in early September 2020, roughly two months after the re-opening of primary schools. We report the following key findings from this survey: *Almost all (more than 99 percent) of children in our sample were back in school two months after schools re-opened. The vast majority of parents believed it was either safe or extremely safe for their children to return to school. *Only 6 percent of households reported that their children listened to radio lessons during the school closures; and a similar fraction (5.5 percent) tuned into TV lessons over the same period. Less than 1 percent of those surveyed accessed educational programmes on the internet. Households with access to radio or TV reported higher usage. *Approximately 1 in 3 (36 percent) children worked on the family farm during the closures, with most children working either 2 or 3 days a week. Male children were 6.2 percentage points likelier to work on the family farm than female children. *Households have limited access to education materials for their child. While more than 9 out of 10 households have an exercise book, far fewer had access to textbooks (35 percent) or own reading books (31 percent). *One in four parents (24 percent) read a book to their child in the last week.

In Kenya, although 45 percent of maternal deaths occur within the first 24 hours after childbirth and 65 percent of maternal deaths occur during the first week postpartum, health-care providers continue to advise on a first check-up six weeks after childbirth. The early postpartum period is also critical to newborn survival, with 50–70 percent of life-threatening newborn illnesses occurring in the first week. Yet most strategies to reduce maternal and perinatal morbidity and mortality have focused on pregnancy and birth. In addition to the heavy workload of providers who do not assess the mother post-delivery when she may bring her infant for immunization, lack of knowledge, poverty, cultural beliefs and practices perpetuate the problem. The only register that exists for mothers post-delivery is for family planning, thus perpetuating the lack of emphasis on the early postpartum period with no standardized register to record care given. To address this gap in service delivery, the Population Council defined the minimal services a mother and baby should receive from a skilled attendant after birth. As stated in this brief, the development of a standardized postpartum register is one step toward advocating for providing early postpartum care among health-service providers.
