Abstract:
Background: The current approach to prenatal care, which was established more than 80 years ago, is characterised by a high concentration of visits in the third-trimester of pregnancy which implies that firstly, most complications occur at this late stage of pregnancy and secondly, most adverse outcomes are unpredictable during the first or even the second trimester. Objectives: The objective of this thesis is to provide evidence that most pregnancy complications are predictable as early as 12 weeks’ gestation. The pregnancy complications examined include fetal aneuploidies, fetal structural defects, preeclampsia, preterm birth, gestational diabetes mellitus and fetal macrosomia. Methods: I have critically examined fourteen articles reporting on screening for pregnancy complications at 11-13 weeks’ gestation, where more than 90,000 singleton pregnancies were prospectively assessed at 11-13 weeks’ gestation as part of a routine prenatal visit for screening for trisomy 21. We recorded a series of maternal characteristics and history, measured maternal weight and height, performed a detailed ultrasound examination of the fetus, measured maternal uterine artery Doppler pulsatility index and maternal mean arterial pressure and collected blood for analysis of biomarkers for prospective or retrospective analysis. All data were prospectively entered into our data base as well as the pregnancy outcomes as soon as they became available. Ethical approval was obtained for these studies. Multivariate regression analysis was used to define the contribution of each maternal characteristic and history in predicting each adverse outcome and those with a significant contribution formed an algorithm to estimate the background risk (a priori risk) for each one of these complications. The potential value of biophysical and biochemical markers in improving the performance of the a priori risk in predicting adverse pregnancy outcomes, was evaluated. Results: First trimester effective screening for adverse pregnancy outcomes was provided by a combination of maternal factors and biophysical or biochemical markers. The developed predictive models could correctly identify the vast majority of aneuploidies, early preeclampsia and more than half of the cases of spontaneous preterm birth and gestational diabetes. First trimester prediction of fetal macrosomia was less effective compared with other complications. First trimester examination of fetal anatomy was feasible resulting in a high detection of fetal non-chromosomal defects, including more than half of fetal cardiac defects. Conclusions: Assessment of the mother and fetus at 11-13 weeks’ gestation can provide effective early identification of the high risk group of pregnancies with fetal and maternal adverse outcomes.

In the United States as many as 15% of births occur before 39 weeks because of elective inductions or cesarean sections. This qualitative study employs a grounded theory approach to understand the decisions women make of how and when to give birth. Thirty-three women who were pregnant or had given birth within the past two years participated in key informant or small group interviews. The women’s birth narratives and reflections reveal how they construct the uncertainty of their due dates and how this construction influences their birth decisions. Problematic integration theory is used to analyze this construction and identify points of influence. The results suggest that women construct the uncertainty of due dates as a reason to wait on birth and as a reason to start the process early. The results suggest that information about a baby’s brain development in the final weeks of pregnancy may persuade women to remain pregnant longer. The results demonstrate the utility of using problematic integration theory to understand a medical situation that is the result of epistemological and ontological uncertainty. The analysis suggests the existence of a third type of uncertainty, axiological uncertainty. Axiological uncertainty is rooted in the values and ethics of outcomes.

Abstract:
Insulin resistance is a precursor to type II diabetes mellitus and in conjunction with dyslipidaemia, hypertension, and obesity, these abnormalities constitute the metabolic syndrome. Insulin resistance usually develops before these other diseases and therefore identifying and successfully treating insulin resistant patients may have potentially great preventive value. Insulin resistance, obesity, and subsequently type II diabetes mellitus have increased dramatically and have reached epidemic proportions. The incidence of diabetes, and in particular type II diabetes mellitus, is increasing in developing countries and throughout the world and this is mainly as a result of increasingly sedentary lifestyle and obesity in an aging population. The specific aim of this study was to explore and describe the effect of a 14-week strength-based resistance training programme on insulin resistance amongst individuals aged 25 to 68 years, who are pre-diabetic, have T2DM, and/or are overweight. The research approach used in this investigation was explorative, experimental, and quantitative in nature. The quasi-experimental design consisted of a pre-test and post-test for an experimental and comparison group who were chosen through convenience and snowball sampling. A total of 30 participants were involved in this study, 15 participants in each group. The following dependent variables were selected, namely: body weight; BMI; body composition; waist-to-hip ratio; total cholesterol levels; triglyceride levels; HOMA-IR; and muscle strength for upper and lower body. Pre-and post-test analysis was performed at the Biokinetics and Sport Science Unit, located at the Nelson Mandela Metropolitan University (NMMU). Blood samples of the participants were drawn by nurses at the Health Clinic at the Nelson Mandela Metropolitan University and these blood plasma samples were stored at the Department of Microbiology and Biochemistry at NMMU for later analysis of glucose and insulin. The experimental group trained three times per week for a period of fourteen weeks, performing strength training exercises with progressive increments in the intensity of the exercise. The control group remained sedentary throughout the intervention period. Analysis of the data was conducted utilizing descriptive and inferential statistics. Analysis of variance (ANOVA) was used as a hypothesis-testing procedure to evaluate the mean differences. The following dependent variables showed a decrease in mean values: body weight, body mass index, body fat percentage, waist minimum, cholesterol and insulin. However these differences in results were not practically and statistically significant. The following dependent variables showed an increase in mean values: hip maximum, arm relaxed, arm flexed, thigh circumference, glucose and HOMA-IR. However these results were not practically and statistically significant. The mean differences in the plasma insulin level, pre- to post-test, between both groups indicated that a significant difference (t = -1.77, p = 0.044) existed between them. Cohen‟s d revealed a value of 0.64, which indicates moderate practical significance. The only dependent variable which showed both statistical and practical significance was sum of skinfolds. The findings for sum of skinfolds revealed that the mean differences, from pre- to post-test, between both groups indicated that a significant difference (t = -2.30, p = 0.015) existed between them. Cohen‟s d revealed a value of 0.84, which indicated a large practical significance. Although the sample size was too small to indicate generalisations to the diabetic population as a whole, strength training should be furthermore explored as an alternative and successful modality in the existing range of options available to the health and exercise professional to address the needs of the person with T2DM. The researcher proposed that a bigger sample size be used for the experimental and control group, the intervention period increased as well as various differences related to frequency, intensity and duration of strength training could possibly result in significant changes.

Abstract:
Stretching is used to maintain mobility, increase range of motion and rehabilitate muscles after injury. It is rarely suggested as a modality for increasing muscle size in humans, although animal studies have reported stretch induced muscle hypertrophy. The purpose of this study was to examine whether six weeks of passive stretching of the plantar flexors would stimulate muscle hypertrophy. The first hypothesis was that stretch training would induce muscle hypertrophy, increase fascicle lengths and decrease pennation angles of the medial and lateral gastrocnemius without altering the Achilles tendon, but changes in muscle architecture would be non-uniform between the gastrocnemii. The second hypothesis was stretch training would decrease maximal force and electromyography (EMG) activity of the plantar flexors through shifting the length tension relationship without changing voluntary activation or reflex activity. Eleven males stretched the non-dominant plantar flexors for six consecutive weeks by using a leg press to passively load the ball of the foot. The load was 20% of the baseline maximal voluntary contraction. EMG was monitored on-line for each session to ensure the subject was stretching and not contracting, which would be indicated by muscle activity. At week zero, three, six and one week after the completion of stretching ultrasound was used to measure muscle architectural and tendon changes. Force, EMG, twitch interpolation, reflex activity, contractile properties and body anthropometry were also measured. Both hypotheses were supported. Muscle depth increased 10.3% (p=0.04) by week three in the stretched leg with a 25% (p<0.001) increase in the length of the muscle fascicles in the muscle tendon junction, and 5.1% (p=0.01) increase in the fascicles of the muscle belly (n=3600). The pennation angle (n=3600) decreased (p=0.02) with no change in the tendon (p=0.95). Muscle force decreased 10.5% (p=0.008) at week six, with a reduction in EMG amplitude (p<0.001) and no change in voluntary activation (95.48% ± 0.92, p>0.05) or reflex activity (p>0.05). These data indicated six weeks of overloaded stretch training of the plantar flexors stimulated muscle hypertrophy but also caused significant reductions in force through an alteration in the length tension relationship.<br>Graduate Studies, College of (Okanagan)<br>Graduate

Abstract:
Introduction Insulin resistance in obesity is decreased after successful diet and exercise. Aerobic exercise training alone was evaluated as an intervention in subjects with the metabolic syndrome. Methods Eighteen nondiabetic, sedentary subjects, 11 with the metabolic syndrome, participated in 8 wk of increasing intensity stationary cycle training. Results Cycle training without weight loss did not change insulin resistance in metabolic syndrome subjects or sedentary control subjects. Maximal oxygen consumption (V˙O2max), activated muscle AMP-dependent kinase, and muscle mitochondrial marker ATP synthase all increased. Strength, lean body mass, and fat mass did not change. The activated mammalian target of rapamycin was not different after training. Training induced a shift in muscle fiber composition in both groups but in opposite directions. The proportion of type 2x fibers decreased with a concomitant increase in type 2a mixed fibers in the control subjects, but in metabolic syndrome, type 2x fiber proportion increased and type 1 fibers decreased. Muscle fiber diameters increased in all three fiber types in metabolic syndrome subjects. Muscle insulin receptor expression increased in both groups, and GLUT4 expression increased in the metabolic syndrome subjects. The excess phosphorylation of insulin receptor substrate 1 (IRS-1) at Ser337 in metabolic syndrome muscle tended to increase further after training in spite of a decrease in total IRS-1. Conclusions In the absence of weight loss, the cycle training of metabolic syndrome subjects resulted in enhanced mitochondrial biogenesis and increased the expression of insulin receptors and GLUT4 in muscle but did not decrease the insulin resistance. The failure for the insulin signal to proceed past IRS-1 tyrosine phosphorylation may be related to excess serine phosphorylation at IRS-1 Ser337, and this is not ameliorated by 8 wk of endurance exercise training.

One of the enduring principles in neurobiology is that most sensory activity is processed in the hemisphere opposite (contralateral) to the sensation. After a relatively small stroke, function can re-map to related peri-infarct cortical areas. However, after a large stroke, areas with spared and related function may only be present within the “unaffected” hemisphere leading to ipsilateral (un-crossed) sensory processing. We investigated the mechanism of the ipsilateral cortical representation of tactile information before, and 1 and 8-10 weeks after stroke using voltage sensitive dye imaging of forelimb-evoked sensory responses in mice. In control animals before stroke, we observed bilateral cortical activation in response to unilateral stimulation. Ipsilateral forelimb responses before stroke were dependent on the contralateral cortex and presumably mediated through transcallosal fibers that homotopically connect the somatosensory cortices. Large strokes that affected both sensory and motor cortices led to a shift in processing of forelimb sensory stimuli to predominantly the unaffected hemisphere. Pharmacological silencing of the peri-infarct cortex or thalamus within the stroke-affected hemisphere failed to affect prominent non-crossed responses in animals 8-10 weeks after stroke, yet these treatments blocked ipsilateral responses in control animals. Ipsilateral forelimb responses after 8-10 weeks of recovery from a large stroke were attributed to non-crossed ascending circuits that were dependent on the ipsilateral thalamus and apparently substitute for damaged transcallosal cortico-cortical connections. These results indicate that ipsilateral sensory responses present in animals after 8-10 weeks of recovery from stroke are through a fundamentally different mechanism that involves circuits well upstream of the peri-infarct tissue with little dependence on the thalamus or cortex within the stroke-affected hemisphere.

Small-for-gestational-age (SGA) fetuses and neonates are at increased risk of perinatal mortality and morbidity. There is evidence that these risks are reduced in cases identified antenatally, through close monitoring, timely delivery and prompt neonatal management, compared to those not detected. The current method of antenatal screening for SGA fetuses is by maternal characteristics and medical history and serial measurements of symphysis fundal height, but the performance of this method is poor. This thesis aims to develop a model for the prediction of SGA neonates based on maternal characteristics, history, fetal biometry, uterine artery pulsatility index (PI), mean arterial pressure (MAP), and serum biochemical markers at 30-34 weeks' gestation (mean gestation at screening 32.3 weeks, IQR 32.0-32.9). The thesis also aims to examine the value of umbilical artery PI and fetal middle cerebral artery in the prediction of adverse perinatal outcome. This screening study included biophysical measurements in 30,849 singleton pregnancies at 30-34 weeks with 1,727 (5.6%) that delivered SGA neonates. A subset of 9,003 cases with 469 (5.2%) delivering SGA neonates had biophysical and biochemical measurements recorded. The best prediction was provided by a combination of maternal factors, estimated fetal weight (EFW), uterine artery PI, MAP and serum placental growth factor (PlGF). This combination predicted, at 10% false positive rate, 89%, 94%, 96% of SGA neonates delivering at 32-36 weeks' gestation with BW < 10th, < 5th and < 3rd centiles, respectively; the respective detection rates for SGA neonates delivering at > 37 weeks were 57%, 65% and 72%. The use of umbilical artery PI and MCA in the prediction of adverse perinatal outcome was found to be poor. In conclusion, combined screening by maternal factors and biophysical and biochemical markers at 30-34 weeks' gestation can identify a high proportion of pregnancies that subsequently deliver SGA neonates.

Thesis (MMed)--Stellenbosch University, 2012.<br>ENGLISH ABSTRACT: Introduction - Accurate computerized analysis of the fetal heart rate (FHR) pattern has become more feasible and clinically relevant in recent years. Taking into account advances in neonatal care resulting in a declining lower limit for fetal viability as well as research into fetal effects of maternal high risk behaviour and escalating intra-uterine exposure to harmful toxins and drugs, fetal heart rate patterns need to be more accurately described in earlier gestations than what is currently available in literature. With advancing technology it is becoming possible to accurately record and interpret the FHR patterns from gestations as early as 20 weeks' gestation. By using the Monica AN24 transabdominal electrocardiographic fetal monitor and product specific software, we analysed early FHR patterns according to the Dawes-Redman criteria, as used in later gestations. Methods - The aim of our study was to describe patterns of FHR (short-term variability, basal heart rate, accelerations and decelerations) at 20-24 weeks' gestation. Physiological data were obtained from the routine second trimester fetal assessment by the Monica AN24 monitor as used in the Safe Passage Study. As of December 31, 2009, 411 participants completed their first fetal assessment and met our inclusion criteria. Because our aim was the description of patterns in pregnancies with a normal outcome, we excluded all adverse neonatal outcomes, preterm deliveries, babies with low birth weights as well as cases where delivery data were lost. After recordings of poor technical quality were also removed from our data pool, 281 recordings remained for analysis. Results - Distinct FHR patterns and quantifiable parameters of heart rate variability were consistently observed. In contrast to what has previously been published, accelerations of the FHR and reassuring baseline variation are present from as early as 20 weeks. Conclusion - Information from this study provides an important foundation for further studies of early FHR patterns and it will help us better assess the fetus at a crucial age for indicators of good outcome at early delivery.<br>AFRIKAANSE OPSOMMING: Inleiding - Akkurate gerekenariseerde ontleding van die fetale hartpatroon het in die afgelope paar jaar meer tegnologies haalbaar met toenemende kliniese toepassing geword. Met vooruitgang in tegnologie is dit nou moontlik om deur nie-indringende elektrokardiografie, fetale hartpatrone te registreer en te interpreteer van so vroeg as 'n swangerskapsdurte van 20 weke. Deur die gebruik van die Monica AN24 transabdominale monitor en produk-spesifieke programmatuur, kon ons vroeë fetale hartpatrone ontleed volgens die Dawes-Redman kriteria wat gewoonlik in later swangerskapsduurtes gebruik word. Metodes - Die doel van ons studie was die beskrywing van verskeie fetale hartpatrone (naamlik korttermyn variasie, basale hartspoed, versnellings asook vestadigings) rondom 20 – 24 weke swangerskapsduurte. Fisiologiese data is in die tweede trimester verkry deur die Monica AN24 monitor, soos gebruik word in die voortgaande Veilige Geboorte Studie by Tygerberg hospitaal. Tot en met 31 Desember 2009, het 411 deelnemers hulle eerste fetale evaluasie vir die Veilige Geboorte Studie gehad en ook voldoen aan die insluitingskriteria van hierdie projek. Aangesien ons fokus die beskrywing van hartpatrone in normale swangerskappe was, het ons alle nie-wenslike neonatale uitkomste, voortydse verlossings, babas met lae geboorte gewig asook gevalle waarvan die geboortedata nie beskikbaar was nie, uitgesluit. Alle opnames met sub-standaard tegniese kwaliteit is ook verwyder uit ons finale data vir ontleding, wat ons met 281 opnames gelaat het vir hierdie studie. Resultate - Duidelike fetale hartpatrone en meetbare afmetings is deurgangs opgemerk. In teenstryd met wat voorheen gedokumenteer is, is die teenwoordigheid van versnellings asook gerusstellende basislyn variasie meetbaar vanaf 20 weke gestasie. Samevatting - Informasie vanaf hierdie studie verskaf 'n belangrike grondslag vir verdere projekte in die ontleding van fetale hartpatrone met die fokus op ondersoek van vroeë voorspelling van goeie neonatale uitkomste.

Determining the chorionicity of twin pregnancies is extremely important as this influences the frequency of surveillance, timing of delivery and management of complications. Monochorionic twins have 2.5 times the perinatal mortality of dichorionic twins, and in the case of a single intra-uterine fetal demise, the surviving twin of a monochorionic pair is at significant risk of neurological damage compared to a dichorionic pregnancy. Chorionicity can be accurately determined before 14 weeks gestation using the lambda or T-sign. After 14 weeks, these ultrasonographic signs become less reliable and the pregnancy may be assumed to be monochorionic for management purposes. The implication of this assumption is that on occasion premature dichorionic fetuses may be delivered unnecessarily. In South Africa, many women have their first antenatal visit after the first trimester or are not scanned by an experienced sonographer until after 14 weeks. There is thus a need for an accurate means to determine chorionicity in the second and third trimesters.

According to the World Health Organization, breastfeeding is natural and the most effective way of nourishment to feed infants and young children to ensure child health and survival. Currently, the World Health Organization, the American Academy of Pediatrics, and other health organizations recommend exclusive breastfeeding during the first 6 months of life. Although exclusive breastfeeding has been shown to reduce the occurrence of adverse health outcomes to the infant and mother, the duration of exclusive breastfeeding remains relatively low in the United States. The theory of planned behavior was used as a theoretical framework for this study. The purpose of the project was to identify the barriers to exclusive breastfeeding among mothers during the first four weeks after delivery. A descriptive research design and a convenient sampling method were used to conduct this study. A questionnaire was used to collect the data from 75 mothers who met the inclusion criteria and who attended 3 selected obstetric and gynecologic private practice physicians' offices. Data analysis was performed by using descriptive and correlational statistics. The findings showed that only 8 mothers continued exclusive breastfeeding during the first 4 weeks postpartum. The major maternal problems identified for not continuing exclusive breastfeeding were (a) insufficient breast milk, (b) sore or painful nipples, (c) return to work or school, and (d) poor latching. Findings suggested that healthcare professionals use the model of the theory of planned behavior to develop interventions that promote a positive attitude toward breastfeeding. A positive attitude toward breastfeeding will create a social change within the community to promote exclusive breastfeeding.

This Doctoral research reports on a qualitative case study conducted to explore the adaptation experience of a cohort of Mexican international students pursuing a postgraduate degree at a prestigious university in England during the 2016-2017 academic calendar. Because the early stage of the transition has been acknowledged as a critical period (Ward, Furnham and Bochner, 2001; Brown, 2008) and a holistic understanding of the participants' experience is to be captured by considering the pre-arrival stage (Schartner, 2014), this thesis focused on the pre-arrival period and the first four weeks of the participants' stay in the UK. Additionally, although the numbers of students in international mobility have augmented considerably during recent decades (British Council, 2012), and with it research to document the international experience has flourished (Delgado-Romero and Sanabria, 2007), international students of Latin American origin appear to be an understudied population (Reynolds and Constantine, 2007). Therefore, this research aimed to redress such gap by looking at Mexican postgraduate international students in the second largest importer of international mobility, the UK (UIS, 2016). It explored the academic, sociocultural, and affective challenges faced, the coping strategies employed, and the institutional services resourced for their adaptation during their first four weeks. Empirical evidence was gathered through a qualitative survey, a questionnaire, focus groups, and interviews. Bearing in mind the 'here and now' implementation of the instruments to facilitate the recalling of recent events (Lincoln and Guba, 1985; Munn and Drever, 1990) data was collected at four different points in time. In the pre-arrival stage (early to late September 2016) 12 students completed an online qualitative survey, on arrival (early October 2016) 25 students responded in-situ an online questionnaire; in the third stage (mid October) 20 volunteers took part in three focus groups; finally, 7 student participants (end of October) collaborated on an interview. The results reveal the challenges faced by the participants seemed to be related to various elements such as the extent of pre-arrival preparations, prior experience overseas, individual, and societal characteristics. Institutional support appeared to be mainly steered towards administrative concerns prior to departure whilst on arrival little assistance tailored for their needs, as international students seemed to be provided. The presence of a consolidated group of conational fellows in the host institution acted as a buffer during the participants' first weeks in the UK.

Populations that utilize deep water running (DWR) are described in Chapter I. A review of the literature concerning studies comparing peak physiological variables of water exercises (swimming, DWR, & land based running) to land based exercises (cycle ergometer, walking, & running) are presented in Chapter II. The protocols utilized for obtaining peak values on land and in the water along with subject characteristics, statistical methods and description of the training regimen are discussed in Chapter HI. The results, presented in Chapter IV, indicate no interaction between any of the variables measured but a main effect for treadmill V02 peak for the pre- and post testing. Chapter V discusses factors which may limit physiological changes within each training group. Chapter VI contains suggestions for further research.

The object of this study was to get a wider picture of the articles published in the Swedish online newspaper Nyheter24. The newspaper is the biggest newspaper only coming out online in Sweden today and basically the idea was to get an overview of their content. The study was based upon two research questions: “What does Nyheter24 write about?” and “What type of journalism does Nyheter24 make?” My research method was a quantitative content analysis and the study contains 550 articles in total, gathered during two weeks time from the website. The results show that Nyheter24 writes about various topics but they tend to produce news stories with an odd or sensational angle. More than half of the articles studied in the news section were rewrites from other news media and less than 20 % of the articles in the same section had quotes from interviews done by the newspaper themselves. In the sports section, just one of all the articles published in the section during the two weeks contained a quote from an interview Nyheter24 had done. The study does also show that investigative journalism is rare at Nyheter24. But it was even more uncommon, zero articles in the news section, for the journalists to do reports of places or news events by going outside the newsroom. What played a bigger part in the news from Nyheter24 was social media. Especially the sports section and the entertainment section published stories where social media was the origin of the story. Generally the results show what several other researchers have found out about online journalism. There is a big recycling of news and the journalists tend to be stuck behind a computer, screening websites and doing rewrites to publish a lot of material quickly. Doing interviews to the stories seemed to have a lower priority.

Aim The purpose of this study was to assess muscle hypertrophy outcomes from high and low load strength training routines performed to muscular fatiguein a unilateral design. Method 14 well-trained men and women (age 26.4 ±4.4 years, weight 79.9 ±10.7 kg, height 179.4 ±76 cm) volunteered to participate in eight weeks of fully supervised training two times per week. Subjects had their legs randomized to a HL protocol performing 3-5reps and LL protocol performing 20-25 reps. Training was performed in leg press and leg extension and all sets were performed until volitional failure. Subjects were measured for muscle thickness by ultrasound at mid and distal portion of vastus lateralis before and after the study. Paired sample t-test and independent sample t-test was used to establish differences in pre and post and between leg differences. Results Muscle thickness increased inHL: MID 8% p=0,002 and DIST 14% p=0.009, in LL: MID 7% p=0.004 and was unchanged at DIST 4% p=0.512. Total muscle thickness for both measurement sites combined increases significantly in both legs, HL: 9% p=0.001 and LL: 5% p=0.045. There were no between leg differences at MID (p=0.404) or DIST (p=0.989) after eight weeks. Conclusion We conclude that high and low load strength training performed unilaterally to volitional failure yields equal increases in muscle thickness after eight weeks for well-trained men and women.<br><p>Kursen Projektarbete.</p>

Abstract    Aim    The main purpose of this study was to investigate Cross Education (CE), and how gender, detraining and leg dominance affects CE in previously untrained subjects when conducting a unilateral resistance training program. We also investigated if unilateral resistance training can give a hypertrophic response.    Method   Twenty healthy previously untrained individuals, 10 females and 10 males, were recruited as volunteer participants. The participants were randomly assigned to train either left or right leg. The training intervention was 10 weeks (34 sessions) of unilateral resistance training in the leg press and leg extension, sixteen of the participants fulfilled the criteria for inclusion. After two initial familiarization the participants trained conventional resistance training three times a week (week 1-3, 5-7 and 9-10) and Blood Flow Restriction Training (BFRT) five times a week (week 4 and 8). One repetition maximum for both legs in the leg press and leg extension was tested pre-, post and post20 to the training intervention as well as ultrasound measurements of muscle thickness.   Results    The ten-week training period resulted in a significant increase of maximal strength for the untrained leg 18,9 %, (16,6) (p &lt; 0,01) in the leg press and 6,7 %, (3,7) (p &lt; 0,05) in the leg extension. When comparisons between gender were made only men had a significant increase 26,5 %, (16,7) (p &lt; 0,01) in the leg press and 9,9 %, (4,7) (p &lt; 0,05) in the leg extension. Also, we saw a significant difference between women and men on a group level. Comparisons of dominant vs non-dominant leg showed that training the dominant leg resulted in a significant increase of maximal strength in the untrained leg in both the leg press 22 %, (17,9) (p &lt; 0,01) and leg extension 10,1 %, (4,3) (p &lt; 0,05). The maximal strength in the untrained leg was not significantly altered by the detraining period and a significant increase of muscle thickness could be seen in the untrained leg at MP50 4,7 %, (1,3) (p &lt; 0,01).   Conclusion    The conclusions are that a ten week unilateral resistance training intervention results in a CE effect for men but not for women and that this type of training also can result in an increased muscle thickness in the untrained leg. Our findings also supports that training the dominant limb has superior effect on achieving a CE effect. Lastly we conclude that a twenty week detraining period did not affect the CE achieved strength.<br>Abstrakt    Syfte     Studiens huvudsakliga syfte var att undersöka Cross Education (CE) och hur kön, viloperiod och ben-dominans påverkar CE hos otränade individer när man undergår ett unilateralt styrketräningsprogram. Vi undersökte även om ett unilateral styrketräning kunde ge ett hypertrofisvar.    Metod    Tjugo friska otränade och för närvarande inaktiva individer, tio kvinnor och tio män rekryterades som frivilliga deltagare. Deltagarna randomiserades för att träna antingen vänster eller höger ben. Träningsperioden var tio veckor (trettiofyra pass) av unilateral styrketräning i benpress och benspark, sexton deltagare uppfyllde kriterierna för inkludering. Två initiala familjäriseringspass hölls varefter träningen delades in i två typer av träning, dels konventionell styrketräning tre gånger i veckan (vecka 1-3, 5-7 och 9-10) och dels Blood Flow Restriction Training (BFRT) fem gånger i veckan (vecka 4 och 8). Före, efter och efter tjugo veckor testades one repetition maximum för båda benen i benpress och benspark samt att ultraljudsmätningar för muskeltjocklek utfördes.     Resultat    Den tio veckor långa träningsperioden resulterade i en signifikant ökning av den maximala styrkan för det otränade benet 18,9 % (16,6) (p &lt; 0,010) i benpressen och 6,7 % (3,7) (p &lt; 0,050) i bensparken. När jämförelser gjordes mellan könen så hade enbart män en signifikant ökning, 26,5 % (16,7) (p &lt; 0,010) i benpressen och 9,9 % (4,7) (p &lt; 0,050) i bensparken. Vi fann även att det var en signifikant skillnad mellan kvinnor och män på gruppnivå.   Jämförelser mellan dominant och icke-dominant ben visade att träning av det dominanta benet resulterade i en signifikant styrkeökning i både benpress 22 % (17,9) (p &lt; 0,010) och benspark 10,1 % (4,3) (p &lt; 0,050). Den maximala styrkan i det otränade benet påverkades inte signifikant av en viloperiod och en signifikant ökning i muskeltjocklek kunde ses i det otränade benet i MP50 4,7 % (1,3) (p &lt; 0,010)    Konklusion    Slutsatserna är att en tio veckors unilateral styrketräningsintervention resulterar i en CE effekt hos män men inte hos kvinnor, och att denna typ av träning kan resultera i en ökad muskeltjocklek i det otränade benet. Våra fynd styrker att träning av den dominanta lemmen har större effekt på CE. Slutligen drar vi slutsatsen att en tjugo veckors viloperiod inte påverkar CE-styrkan.

Our findings are: (1) the occurrence rate is 41.5%; (2) older pregnant women, pregnant women whose jobs require long use of power of abdominal muscles, pregnant women with more births, having constipation during pregnancy or having stress incontinence tend to have incontinence in six weeks after delivery. Women correctly ad continuously having floor muscles of pelvic exercises reduce the possibility of having postnatal urine incontinence. Conclusions: this study helps understand the occurrence and relevant factors of postnatal urine incontinence. Medical personnel can emphasize the issue and provide preventive measures such as instructing floor muscles of pelvic exercises. Training potocol was 8 weeks, 3 sessions a week, 4-7 sets per sessions, 20 seconds per set. The study revealed that the muscle activition of the extenors, the flexors, the brachioradialis muscle, flexibility, the grasp ability and the control ability were significant improvement in the VIB group. The VIB group of change percentage in muscle activition of the brachioradialis muscle, the grasp ability and the control ability significant higher than the CON group. However, only the grasp ability significant increased in the MVC group and its change percentage higher than the CON group. The findings suggested that 8-weeks handy vibration training improved neural drive, range of motion, precise ability and control ability in fine-motor muscular. Therefore, handy vibration is an effective training method for athletes or someone who needs excellent precise and control movement.
