Abstract Several concepts and principles advanced by Widmark over 50 years ago are briefly compared with results of recently published experimental work. His conclusions--that breath alcohol analysis can lead to overestimates of actual blood alcohol concentration, and that +/- 2 standard deviations about the average should be used to estimate certain pharmacokinetic parameters for medicolegal alcohol determinations--are supported by these more-recent experimental results.

Hangover resistance may be linked to an increased risk of continuing harmful drinking behaviours as well as involvement in potentially dangerous daily activities such as driving while hungover, mainly due to the absence of negative consequences (i.e., hangover symptoms) the day after alcohol consumption. The aim of this study was to examine the occurrence of claimed alcohol hangover resistance relative to estimated blood alcohol concentration (eBAC). A total of 1198 participants completed an online survey by answering questions regarding their demographics, alcohol consumption and occurrence of hangover. Two methods were used to calculate eBAC, one based on the modified Widmark Equation (N = 955) and the other from an equation averaging the total body water (TBW) estimates of Forrest, Watson, Seidl, Widmark and Ulrich (males only) (N = 942). The percentage of participants who claimed to be hangover resistant decreased rapidly with increasing eBAC and only a small number of hangover resistant drinkers remained at higher eBACs. Comparisons of the eBACs calculated by the two methods revealed significantly higher BACs when using the modified Widmark equation. These findings suggest that additional research for eBAC calculations is needed to improve accuracy and comprehensiveness of these equations for future alcohol hangover research.

In South Africa of the 1950s, night-clubs frequented by blacks were dangerous places. Fights, shootings and stabbings were commonplace, and some shows even ended in riots. Gangsters were an important catalyst for such events: they terrorised musicians and patrons alike. Miriam Makeba, whose singing career began in the 1950s, remembers what it was like:[T]hese men come in, sit in front, and pull out their bottles. They put these before them on the table. Then they take out their guns and put these in front of them on the table, too. We are all supposed to look, and we can't help ourselves: We do. They are like actors, these gangsters, although they do not play. In South Africa, movies are taken very seriously, and there is a movie in the cinemas now in which Richard Widmark plays a hoodlum. They call him Styles, and he dresses up in a hat, a belted jacket, and those Florsheim shoes. The black gangsters go out and dress just like him. In the movie, Richard Widmark eats an apple after each of his crimes. So, all the African hoodlums have gone and gotten apples, too. Purpose Ethanol as an excipient is used to enhance the solubility of gemcitabine, but, sometimes, the dose of ethanol a patient may be given is much higher than the dose considered to be toxic. We aimed to assess ethanol-related symptoms and signs in patients receiving two formulations of gemcitabine, with and without ethanol. Methods A randomized double blind cross-over study was conducted. All patients being treated with gemcitabine received two consecutive doses of the drug, one diluted from a concentrate for solution for infusion (CSI) containing ethanol and the other from a lyophilized powder, without ethanol, which was used as control group. After each administration, patients were surveyed in order to assess the appearance of any alcohol consumption symptoms (dizziness, difficulty speaking, unsteady walking, impaired balance, mood swings and slower reactions). Widmark formula and the amount of alcohol measured on the breath (breathalyzer) were used to estimate blood alcohol concentration. Results Twenty-four patients received both formulations and were included in the analysis. Mean administered ethanol dose when prepared from CSI was 15.81 ± 2.25 g (mean ± SD). When using CSI gemcitabine, estimated blood ethanol concentration was 0.033 g/dl according to Widmark formula and 0.02 g/dl according to breathalyzer results. Although overall incidence of symptoms was higher in the study group, the difference was not statistically significant (33% vs. 25%; p = 0.53). Conclusions These findings prove there is no difference in the onset of ethanol related symptoms when using CSI instead of lyophilized powder on the reconstitution of gemcitabine.

Abstract An autosampler intended for liquid injection into a gas chromatograph was modified to serve as an automatic headspace sampler by replacing the syringe with a 100-microL gas-tight syringe and shortening the needle that normally enters the vials for sampling. Instead of the usual 1.0 to 1.5 mL of liquid sample, the vials contained 200 microL of liquid; the vapor above the liquid was injected into the gas chromatograph. Blood samples from California drivers were analyzed for ethanol by using the modified autosampler and the values were compared with those obtained by two other methods: the Smith modified version of the Widmark diffusion desiccation oxidation method (J Lab Clin Med 38: 762, 1951) and direct injection of diluted blood into the gas chromatograph. The correlation coefficient between the headspace method described in this paper and the titration method was 0.984; with direct injection, 0.997.

Previous records of Platycopida (Ostracoda) from the Antarctic region of the Southern Ocean include only a few fossil species from the Late Cretaceous to the Palaeocene: Cytherelloidea megaspirocostata Majoran & Widmark, 1998, [sic] Cytherella serratula (Brady, 1880), plus seven species left in open nomenclature. The present study documents the first record of a living platycopid from the Antarctic region and describes Cytherella rwhatleyi sp. nov. as new. Comparison among specimens collected at stations 60° longitude and 10° of latitude apart from each other show that very little intraspecific variation in outline and ornamentation of the valves, as well as on the hemipenis is presented by this new species. Otherwise, clear differences on valve and hemipenis are observed between different species (herein, Jellinek & Swanson 2003). Review of the literature indicates that several species (with great differences in valve outline and ornamentation) have been erroneously assigned to Cytherella serratula (Brady, 1880) demonstrating that this so-called cosmopolitan taxon is in truth most probably restricted to bathyal depths of the Northwestern Atlantic. Finally, the abundances of Cytherella rwhatleyi sp. nov. in the samples studied herein (considering O2 concentration measurements) contradict the proposed relationship between Platycopida and O2 concentration in water masses (Whatley et al. 2003).

Rates of ethanol clearance were measured at rest and with acute exercise in four groups of female Sprague-Dawley rats. Two groups were trained to run on a motor-driven rodent treadmill at 27 m/min, 1 h/day, 5 days/wk and were given a nutritionally balanced liquid diet; one of these groups received 35% calories as ethanol whereas in the other, sucrose was isocalorically substituted for the ethanol. Appropriate sedentary and nonethanol controls were also used. Clearance of a 1.75-g/kg ethanol dose injected intraperitoneally was determined by measuring ethanol levels in the blood each hour and utilizing these values in the Widmark equation (R. Teschke, F. Moreno, and A. Petrides, Biochem. Pharmacol. 30: 1745–1751, 1981) for calculating whole-body ethanol clearance. Rates of ethanol clearance were determined for each rat at 4 and 7 wk of training. The clearance tests at 4 wk included a 60-min period of running exercise, whereas the tests 3 wk later were conducted at rest. The results indicate that both acute exercise and exercise training can increase rates of in vivo ethanol clearance. In addition, the chronic exercise appeared to increase in vitro ethanol metabolism by hepatic microsomes without altering in vitro hepatic alcohol dehydrogenase activity.

Background: Twenty-five to 85% of trauma patients are under the influence of alcohol in addition to experiencing injury-related coagulation impairment. Viscoelastic point-of-care tests (thrombelastography [TEG], rotational thromboelastometry [ROTEM]) are popular tools for rapid hemostasis assessment and therapeutic decision-making in this and other settings. While alcohol affects these tests in-vitro, their specific effects in-vivo are unclear. Therefore, we evaluated the effects of alcohol ingestion on ROTEM parameters.Methods: Twenty volunteers provided informed consent to drinking red wine, whisk(e)y, or vodka to a target blood alcohol concentration of 1 ‰ within one hour, calculated with the Widmark formula. Blood samples were collected before drinking, at a breath alcohol concentration of 0.5 ‰, and at 1.0 ‰, but no later than one hour. After each blood collection, ExTEM and FibTEM tests were performed directly "at the bedside."Results: All participants had a blood alcohol concentration (BAC) of 0.00 ‰ at the beginning. The mean BACs at the second and third collection were 0.48 and 0.76 ‰, respectively. There were no significant differences in the ExTEM parameters. FibTEM measurements showed a significant difference at the A10 value (13.0 vs. 14.0 mm, P = 0.014) and a trend at the maximum amplitude (maximum clot firmness [MCF] 13.7 vs. 16.2 mm, P = 0.075). We saw no significant differences in fibrinolysis parameters and no hyperfibrinolysis in our ROTEM measurements.Conclusions: Ethanol ingestion can impair early fibrin polymerization. These results might be of special relevance in trauma and support routine application of ROTEM/TEG in such cases.

This two-part article examines the strengths and weaknesses of various ways of investigating claims of drinking alcohol after driving, commonly known as the hip-flask or glove-compartment defence. In many countries the onus of proof in hip-flask cases rests on the prosecution. With good co-operation from the police and timely sampling of body fluids, such as blood and urine for forensic analysis of ethanol, useful evidence can be mustered to support or challenge the truthfulness of alleged drinking after driving. The person's blood-alcohol concentration (BAC) can be compared with values expected on the basis of the amount of alcohol consumed after driving, according to theoretical Widmark calculations. The actual BAC measured is then adjusted for the additional amount of alcohol consumed in the after-drink. Double blood samples, that is, taking two specimens of venous blood about 30–60 minutes apart and looking at the magnitude and direction of change in BAC provides little or no more information than a single blood specimen. However, the relationship between alcohol in blood and urine is very useful in hip-flask cases whereby the concentration expected in the primary urine is compared with the concentration in the bladder urine voided. The concentration of alcohol determined in a second urine sample collected 30–60 min later gives supporting evidence in hip-flask cases. A graphical method, which entails plotting ethanol concentrations in blood and urine as a function of time provides a robust and practical way to investigate hip-flask defences. In the second part of the review, congener analysis is presented, which entails comparing the concentrations of n-propanol, isobutanol and occasionally other congeners in the alcoholic beverage allegedly consumed after driving with the volatiles present in the suspect's blood and urine determined by headspace gas chromatography.

4 Background: After a median observation time of 7.6 years, Scandinavian Prostate Cancer Group VII randomized trial showed a significant 12% reduction of prostate cancer-specific mortality in patients with locally advanced or histologically aggressive prostate cancer who received three months of total androgen blockade followed by radiotherapy and continuous antiandrogen therapy compared to patients with hormonal treatment only (Widmark et al :Lancet [2009]; 373,1174). Here we provide the 10 (15)-year survival results after a median observation time of 10.7 years. Methods: Between February 1996 and December 2002, 875 patients with locally advanced prostate cancer were randomized (Randomization ratio 1:1). Primary endpoint was prostate cancer-specific survival analyzed by intention to treat. This updated analysis is based on death registry data of the Norwegian patients (2/3 of the population), and on data recorded in CRF database available for the Swedish patients. A Swedish death registry analysis is underway, and will be included in the final analysis at the meeting. Results: Prostate cancer death occurred in 118 out of 439 of the antiandrogen treatment group and in 45 out of 436 men in the combination treatment group (p< 0.0001), with death due to any cause in 210 out of 439 and 161 out of 436 men (p=0.0006), respectively. The 10 (15) year cumulative prostate cancer-specific mortality was more than halved after combined treatment: 18.9% (30.7%) and 8.3% (12.4%) (HR=0.35;[p<4.1E-10 for 15 year results]), and overall mortality was 35.3% (56.7%) and 26.4% (43.4%) (HR=0.70; P=0.0006 for 15 year results), respectively. Conclusions: Addition of local radiotherapy to hormonal treatment in patients with non-metastatic locally advanced or high-risk prostate cancer more than halved the 10 and 15 year prostate cancer-specific mortality and substantially decreased overall mortality.

Jörg Widmann is a German composer who is acutely conscious but certainly not in awe of his musical forebears. His best-known string quartet, no. 3 ‘The Hunt’ makes dramatic use of – and has great fun with – Schumann. His 2011 concerto Flûte en suite, performed at the 2014 Proms, more than nods to Bach and baroque elegance. In Armonica, commissioned by the International Mozarteum Foundation to celebrate Mozart's 251st birthday in 2007, Widmann surprisingly and yet unsurprisingly features the glass harmonica, the distinctive instrument that Mozart featured in his own last chamber piece, the Adagio and Rondo for glass harmonica, flute, oboe, viola and cello, K617. Mozart teamed the instrument with the quieter representatives from the wind and string families; Widmann places it against the whole orchestra.

The world première of Jörg Widmann's new opera Babylon, conducted by Kent Nagano at the Bayerische Staatsoper, on 27 October 2012, was an evening to remember, for both the spectacular production and the musical performances. It began with a poignant announcement from the stage that Widmann had dedicated the performance in gratitude to the memory of his teacher, Hans Werner Henze, who had died that very morning in Dresden at the age of 86. One's thoughts turned to Henze's own wonderful operatic oeuvre, especially The Bassarids, which shared with Babylon an interest in ancient myth and pastiche. Yet curiously, it was a totally different postmodern influence on Widmann that seemed more evident here: that of another mentor, Wolfgang Rihm.

Analiza kontekstu politycznego łączy się z refleksją nad obecnością widm, dlatego metodologicznym punktem odniesienia jest widmontologia Jacques’a Derridy. Propozycja francuskiego filozofa pozwala zrozumieć, jaką rolę pełnią duchy w dziełach tajlandzkiego artysty. W swoim artykule Loska skupia się nie tylko na filmach fabularnych Apichatponga, ale również na jego projektach wideo i instalacjach multimedialnych, by pokazać, na czym polega nawiedzenie i w jaki sposób zmusza ono do przyjrzenia się temu, co wyparte z oficjalnej narracji, czyli przemocy, okrucieństwu i nieprzepracowanej traumie.
