The mechanism for maintaining balance in man is complex. Vision, proprioception, and vestibular inputs are integrated in the central nervous system, and modulated by activity from the cerebellum, the extrapyramidal system, the reticular formation, and the cortex. This integrated, modulated information provides one mechanism for control of oculomotor activity, controls posture, gait, and motor skills and allows perception of the head and body in space. Recent evidence also supports an effect upon autonomic function, cognition, and emotion. The complexity of the system is such that pathology in a variety of different bodily systems, including the endocrine system, the cardiovascular system, and the haemopoietic system, can impact upon vestibular activity, in addition to primary otological and neurological pathology.Patients with dysfunction in the vestibular end-organs or vestibular pathways commonly complain of symptoms of dizziness, vertigo, unsteadiness, light-headedness, imbalance, and a plethora of synonyms associated with a sense of instability. Not infrequently, in an attempt to define their ‘unphysiological’ experience, patients use rather vague and imprecise semantics. The clinical distinction between dizziness, a symptom of non-specific pathological significance, and vertigo, a hallucination or illusion of movement, is rarely made, although the latter is a cardinal manifestation of a disorder of the vestibular system (Dix 1973). Ten to 20 per cent of all ‘dizzy’ patients are reportedly seen in neurology clinics (Dieterish 2004), therefore it behoves the neurologist to have a clear diagnostic strategy, including knowledge of detailed neuro-otological examination, to enable appropriate diagnosis and management of the patient with vestibular symptoms.

The cerebellum uses sensory feedback and information about intended actions to ensure coordinated and smooth movements despite changing conditions. An analogy between the cerebellum and an orchestral conductor is elaborated. The cerebellum’s involvement in forming and executing motor memories is presented. Cerebellar circuits through the cerebellar cortex and deep nuclei and the dependence of cerebellar learning on climbing fiber input to Purkinje cells are briefly described. Sensory reafference and motor efference copy are defined and their roles in coordinating movement introduced. Cerebellar symptoms including ataxia, dysmetria and dysdiadochokinesia, are discussed and a possible model for dysmetria is considered. The specific inputs to and outputs from the vermis, paravermis, and lateral lobes are detailed in a description of canonical cerebellar loops. Finally, evidence that the cerebellum is involved in modulating nonmotor functions such as language, affect, social cognition, and visceral control is presented for the reader’s consideration.

AbstractSystemic lupus erythematosus (SLE) is a chronic, relapsing, inflammatory, often febrile multisystemic disorder, characterized by involvement of the skin, joints, visceral organs, and serosal membranes. Symptoms and manifestations vary widely over an unpredictable relapsing and remitting course.The presentation of SLE can range from mild forms to severe disease requiring hospitalization. Most commonly it manifests as a combination of constitutional symptoms, particularly fatigue and fever, with cutaneous, musculoskeletal, mild haematological, and serological involvement; however, when renal, haematological or central nervous system disease predominate it can be more severe, even life-threatening. There is a tendency for the disease pattern present at the time of onset to prevail during subsequent exacerbations.Investigating SLE depends to an extent on the presentation of the individual. However a number of haematological, biochemical and immunological investigations provide useful diagnostic information, either for the disease itself or in context of organ system involvement, and should be performed routinely.The presence of lupus nephritis should be considered in any lupus patient with impaired kidney function, proteinuria, hypertension, or an active urine sediment; the gold standard investigation in this context is a kidney biopsy. Glomerular immune complex deposition is the hallmark of lupus nephritis and underpins the International society of Nephrology/Renal Pathology Society classification of lupus nephritis.The diagnosis of SLE is based upon the presence of clinical and/or laboratory features and immunological markers that meet the various published diagnostic criteria. In 2012, lupus nephritis identified on kidney biopsy became an independent diagnostic criterion.This chapter goes through the clinical manifestations, investigations (including a detailed look at the kidney biopsy) and a review of the latest published diagnostic criteria.
