Abstract:
This study explores clients’ perceptions of women-only group counselling for problem gambling. The clients surveyed participated in a women-only treatment group through the Alberta Alcohol and Drug Commission (AADAC). The group was a pilot project for AADAC, in that it was the first gambling treatment group for women only; previously, only mixed (male and female) groups had been run. Therefore, this is the first known group of its kind to be conducted in Alberta or Western Canada. A qualitative, thematic analysis using the constant comparison method was conducted using in-depth interviews with a focus group of five women. Themes emerged from this analysis, providing insights into counselling practices for women problem gamblers. The results demonstrated that the women who participated in the group found women-only groups to be helpful. Additionally, the participants reported that, if they had to participate in group treatment for problem gambling, they would prefer women-only treatment in the future. Therefore, further research and exploration of women-only treatment are recommended in order to improve problem gambling treatment for women.

Abstract:
Problem gambling is best understood from a biopsychosocial perspective, whereby multifaceted biological, psychological, and socio-environmental factors interact in ways that may lead to individual risk. Reinforcement contingencies and operant conditioning appear to play particularly important etiological roles. Theoretically, operant conditioning approaches should therefore comprise particularly effective treatment strategies. While operant conditioning in the form of contingency management is known to be an effective treatment for alcohol and substance abuse, it has never been applied by clinical practitioners in community-based treatment for problem gambling. The present pilot study explored the utility of adding concrete reward contingencies to community outpatient treatment, from the perspectives of clinical effectiveness and client/counsellor experiences. At 3-month follow-up, clinical outcomes compared well to typical treatment outcomes, and treatment retention appeared to be superior. Participating clients perceived concrete rewards to be moderately effective in the change process, while active therapist acceptance of this technique appeared to be limited.

Abstract:
Despite growing recognition that impulsivity is associated with pathological gambling (PG), three methodological concerns continue to prevent a meaningful examination of how impulsivity is specifically associated with PG. First, the lack of studies specifically comparing PGs to less severe gamblers leaves the question of whether impulsivity was not also associated with less severe gamblers unexamined. Second, the inadequate control of confounding variables in most studies raises concerns about whether findings that impulsivity is associated with PG may be spurious. Third, many findings that impulsivity is associated with PG are based on unrelated definitions of impulsivity, and are therefore relatively uninformative.
The present series of studies is the first attempt to address all of these concerns at the same time. In terms of appropriate comparison groups, study 1 compared PGs to at-risk gamblers and social gamblers, and study 2 compared severe PGs to moderately severe PGs and social gamblers. In terms of adequate controls, both studies examined PGs who were not in treatment and representative in terms of comorbid drug use. In addition, both studies controlled for age, gender, education, socioeconomic status, psychiatric comorbidity, and general cognitive function. Study 2 furthermore controlled for depression, trait anxiety, aggression, and criminality. In terms of improving how impulsivity is examined, study 1 elaborated a neuroanatomically-based definition of impulsivity and examined how three hypothesized neuroanatomical components of impulsivity were associated with PG. In study 2, different operationalizations of impulsivity, based on the same definition of impulsivity, were examined regarding their association with PG.
Every measure that was directly related to the present definition of impulsivity was significantly associated with severe PGs, and only severe PGs, suggesting that severe PGs may comprise a distinct group of gamblers. Measures which were not directly related to this definition of impulsivity were not significantly different between severe PGs (or PGs in study 1) and less severe gamblers. Severe PGs performed as well or better than social gamblers on retention capacity and attention control measures of working memory.
In conclusion, the inability to inhibit inappropriate reward-seeking is argued to be a primary deficit of severe PGs in our sample.

Abstract:
Background: This study was driven by the belief in the need to expand our understanding of the experience of people with problem gambling. Although there are increasing studies in explaining problem gambling, there is a lack of studies from a meaning-making perspective. Most of the existing models explaining problem gambling place less emphasis on the surrounding system, such as family and peers, which may have an interactive relationship with one’s gambling behaviour. Besides, gambling studies with a focus on culture often involve participants of minority groups and non-mainstream status, which is different from the local context in a Chinese-dominant country. Hence it is worthwhile to understand people with problem gambling in the local context by consideration of their meaning-making toward gambling and their interaction with surrounding systems. Objectives: The study aims to develop understanding of the indigenous pathway(s) of becoming problem gamblers and how such problem behaviour is maintained. The main focuses of this study are how do males develop from social gamblers to problem gamblers, how do they experience themselves as problem gamblers, how may the differences in experiences result in different consequences, and what are some possible factors that initiate and maintain such process. Methods: With reference to the theoretical framework of symbolic interactionism, a qualitative research methodology was employed. In-depth interviews were conducted to allow the researcher to listen to the stories and the subjective experiences of the participants. A rich pool of data in the forms of narratives and sketches of gambling history timelines was collected. QSR NVIVO software was used to help data analysis. Results: The findings revealed how these men experienced the relationship with gambling and made meaning toward their gambling behaviour. Six stages, engagement, enjoyment, immersion, entrapment, separation and termination, were identified with reference to their meaning-making toward gambling. It was identified that there are dynamic interactions among the relationship with gambling, intra-personal relationship and interpersonal relationships in each stage. Vulnerability factors, maintenance factors and exiting factors, which have their roles in driving individuals from one stage to another, were identified and used as significant indicators for the typology of people with gambling problems. This study constructed a five-group typology: a chronic emptiness group, an overly-dependent group, an overly self-demanding group, a prolonged interpersonal distressed group and a brief and intensely distressed group. A process model of Chinese Hong Kong male problem gambling was proposed, which has significant implications for theoretical knowledge and practice. Conclusion: This study makes a significant contribution by widening our understanding of the experience of people with problem gambling. It informs us of the important role of meaning making toward gambling and what purpose gambling has served for the people. Meanwhile, it proposes the importance of looking into the interaction dynamics among one’s relationship with gambling, intrapersonal relationship and interpersonal relationships.

Abstract:

The aim of this study is to gain an increased knowledge and deeper understanding of compulsive gambling and its consequences in today‟s society. Gambling addiction will be described and analysed by a selection of experts, therapists and people with an addictive gambling background. The basis of the empirical research has been undertaken by a qualitative design with interviews consisting of three fundamental themes. The research group of eight participants included four gamblers, two therapists and two specialists. The knowledge perspective focused on the entire „hermenuetisk‟ circle - before, during and after a gambling period. The entry phase into a gambling game showed the reality in which the gambler could feel alive and excited. When participating in gambling, addicts are mentally active and engaged. Therefore, they often experience a sense of being good at what they are doing, which provides them with a purpose. However, the post gambling period sees this sense of meaningfulness dissolve into feelings of guilt, anxiety and failure. During the rehabilitation gamblers experienced anxiety and a powerful lure to play, but if these feelings are conquered they finally feel a sense of release and freedom from gambling. The psychological study showed that the power of gambling provided addicts with a short-term feeling of strength and belonging, as their gambling experiences created an identity. A social study focuses on the gambler‟s own interpretations of their gambling experiences. This provides an insight into the gamblers reality that governs the behavioural patterns of an addict. According to a cognitive perspective, various emotional events provide gamblers with fulfilment during the gambling and then either euphoria from winning or disillusionment from losing.


Studiens syftet var att få ökad kunskap och förståelsen kring spelberoende i dagens samhälle samt följder därav. Följande övergripande frågeställning var i fokus: Hur beskriver och kommenterar ett urval av experter, behandlare samt personer med spelberoendebakgrund sina erfarenheter av spelproblematiken idag? Forskningsunderlagets empiriska material har bearbetats utifrån en kvalitativ design med intervjuer bestående av tre grundläggande teman på åtta deltagare varav fyra brukare, två behandlare och två specialister. Det epistemologiska synsättet fokuserades på helheten av karakteriserande tolkning utifrån en hermenuetisk cirkel samt tematisering före, under och efter en spelperiod. Vägen in i spel visade den faktiska verkligheten som innebar att få uppleva sig själv som existerande. Att spela innebar att vara duktig, aktiv och engagerande men att spela i ett senare skede innebar att meningsfullhet förbyttes mot skuldkänslor, ångest och misslyckande. Vägen ut ur spel visade en falsk känsla av beständighet, att må dåligt under behandlingstiden, känna ångest och ha spelsug men som förbyttes till befrielse efter behandlingen. I existentialistiskt perspektiv visade resultatet att makten över spelet innebar kraft och styrka genom att spelaren byggde upp sin identitet på optimala upplevelser. Utifrån socalkonstruktivistisk perspektiv speglades omgivande faktorer genom spelarens egen tolkning, och upplevelsen var att verkligheten utgjorde den kunskap som styrde beteendet. Enligt kognitivt perspektiv var strukturer sammankopplade i en process som innebar en positiv förankring men som i ett senare skede innebar lust eller olustkänslor.

Increased accessibility and availability of gambling opportunities around the world have contributed to more people having problems with gambling. Current gambling studies have focused on the etiology, prevalence and mechanism for the development of pathological gamblers with few studies examining the processes of change of pathological gamblers into non-gamblers.
Qualitative case analysis has been used in understanding the inner, personal and subjective changes of Chinese male pathological gamblers when concerning their process of change and their difficulties in regulating their gambling behaviour. Participant observations were conducted to collect background information about pathological gamblers and build up trustful relationships with potential respondents. Fourteen recovered gamblers or pathological gamblers have been interviewed. The researcher has utilized narrative analysis to explore the meaning of the narratives of the ex-gamblers and pathological gamblers about their relpases, their identity formation, the role of human agency and about how others have storied their lives into a meaningful configuration when facing their gambling problems. There has been a lack of qualitative studies to understand the narratives of Chinese male gamblers in accounting for their changes from gamblers to non-gamblers. In particular, no studies on Chinese male gamblers have examined the issue of relapses and how pathological gamblers manage relapses.
This study aims to understand the interpretations and narratives made by pathological gamblers during their change. This study will give insights on how Chinese male pathological gamblers are able to abstain from pathological gambling. As relapses are common among pathological gamblers, this study would like to understand why and how relapses occur. Further, the study would like to understand the factors that motivate and maintain changes. Also, it tries to understand how the identified factors of changes such as interpersonal support, the formation of a new identity and meaning of life, the role of spirituality on gamblers' motivation, their connection with family members, have interacted with one another to constitute change.

Abstract:
The objectives of this study were to provide a detailed biopsychosocial description of the characteristics of a sample of 100 individuals screened and referred by the National Responsible Gaming Programme helpline for their outpatient treatment programme over an eighteen month period, and, importantly, to measure the success of this specific treatment programme at set intervals, up to a one year follow-up period. While 80% of the sample did not relapse during the six-week treatment programme, the number of treatment seekers without any gambling relapses during each follow-up period declined, and those falling back into gambling increased as time went on. After one year 47% of treatment seekers managed not to revert back to gambling – total abstinence. A further 28% reported having relapsed once or twice or that their gambling was controlled. 25% of treatment seekers reported that they reverted back to gambling fulltime which leaves the success rate of the treatment at 75%. Treatment seekers reported an overall reduction in gambling participation, debt and expenditure and an overall improvement in social and vocational functioning. There is evidence in this study to support the perspective that pathological gambling is a multidimensional disorder and that certain sub-groups of gamblers have distinct gambling behaviour.

Abstract:
This study is an exploration, through ethnographic and auto-ethnographic inquiry, of the personal world, gambling experiences and underlying biopsychosocial vulnerabilities of three individual case studies - one male and two females - each representing a different sub-type of pathological gambler. It comprises the integration and implementation of a psycho-structural stage matching model to explore comorbidity and identify certain biopsychosocial manifestations in the respective stages of pathological gambling. Long term treatment strategies were identified and patient treatment matching was explored. Further, it comprises my personal relationship and therapeutic treatment of these sub-types of gamblers over a period of one year and longer. In this study it is hypothesized that formulating appropriate matching long term treatment strategies should be based on the stage of change, the phase in the psycho-structural model, as well as the gambler's underlying vulnerability. From this a comprehensive gambling disposition profile can be completed with proper intervention matching approaches. A number of other hypotheses emerged from this study that could provide valuable information and serve as a guideline to those working with pathological gamblers.
