Abstract:
Twenty-four patients with malignant tumors of the external auditory canal and middle ear, originally seen between 1960 and 1980, were reviewed retrospectively. Seventeen patients had epidermoid carcinoma, one had adenocarcinoma, three had rhabdomyosarcoma, and one had osteosarcoma. At presentation, four had disease confined to the external auditory canal, three had superficial invasion of the bony canal, four had deeply invasive disease, and thirteen had disease that extended beyond the temporal bone. Treatment consisted of radiation, surgery, and combination therapy. Four patients with osteosarcoma or rhabdomyosarcoma received adjuvant chemotherapy. Five years after therapy, one of four patients with external auditory canal tumor died of disease, and two died of intercurrent disorders. One of three patients with superficial temporal bone invasion and two of four patients with deeply invasive tumor died of their disease; another died of intercurrent disorder. Twelve of 13 patients with tumor beyond the temporal bone died.

We evaluated the 10- to 15-year outcome of 252 patients with small-cell lung cancer entered into therapeutic clinical trials with or without chest and cranial irradiation. Thirty-two patients (13%) survived free of cancer for 2 or more years. Twelve patients (5%) survived at least 10 years free of cancer, and 10 patients are currently alive and free of cancer beyond 10 years. Six of these 10 patients currently function at a level comparable with that before diagnosis. The other 22 patients who were cancer-free at 2 years have died. Nine patients died from recurrent small-cell lung cancer 2 to 6.2 years after initiation of chemotherapy. Five died from non-small-cell lung cancer, three died of other malignancies, and five died of causes other than cancer. A small fraction of patients with small-cell lung cancer are cured of their original malignancy, but these patients remain at high risk for second cancers and death from other causes.

Signalment, concomitant diseases, prognostic factors, and mortality were evaluated, retrospectively, in 55 diabetic cats (mean age, 11 years; range, five to 18 years). Sixty-seven percent of the cats were between 7.5 and 15 years of age. One-year mortality (n = 23) was high; most early deaths were due to comorbid disease, and the rate of death diminished in cats surviving beyond one year. The median survival time for all cats was 29 months; among cats that died, the median survival time was 11 months. Of the cats surviving more than one year, 16 were alive at a mean of 41 months. Only 13 of 37 cats died due to diabetes mellitus; the majority died due to concomitant diseases, with renal failure (n = 8) and hepatopathies (n = 6) being the most common causes. No clinical data was identified as being of prognostic significance.

Nonketotic hyperglycinemia is a rare inborn error of glycine metabolism characterized by a severe metabolic encephalopathy with drug-resistant seizures. Here, we report the outcome of nonketotic hyperglycinemia in a cohort of patients diagnosed and followed-up at a tertiary care reference center in Lebanon, between 2000 and 2014.Eight out of 12 patients with nonketotic hyperglycinemia were retrospectively reviewed. The remainders were excluded for incomplete data. The majority of cases presented with seizures and hypsarrhythmia or burst suppression patterns. Half of the patients died. Survival varied between 7 days and 18 years. Seizures remained unresponsive with poor outcome, despite standard supportive care and antiepileptic therapy; however, two patients were responsive to ketogenic diet and one of them became seizure-free.Scarce data on the outcome of nonketotic hyperglycinemia patients from the Middle East and North Africa region are available. The ketogenic diet, in combination with standard therapies, appears to be effective in controlling the seizures in this devastating disorder. Larger multicenter studies are still needed to establish the role of the ketogenic diet in nonketotic hyperglycinemia.

Abstract:
Abstract Background Pulmonary infection secondary to nontuberculous mycobacteria (NTM) is associated with significant morbidity and mortality, especially in individuals with underlying structural lung disease. Such infections are challenging to treat due to high virulence, antibiotic resistance, and the lack of effective and tolerable therapies. At many transplant centers, the isolation of NTM may be considered a contraindication for lung transplantation. Methods Institutional and referral medical records, microbiology, radiology and pathology databases were reviewed for patients who underwent lung transplantation at the University of Washington between 2006-2020. 8 patients with NTM disease were identified according to the American Thoracic Society (ATS) guidelines. 9 patients with sputum cultures positive for NTM, but not diagnosed with NTM disease were also evaluated. Results Patients with NTM disease continued antimycobacterial therapy pre- and post-operatively. NTM organisms isolated included M avium complex, M fortuitum, M abscessus, M kansasii, and M fortiutum. In the cohort with NTM-disease, one patient died within a year of transplantation (14%), three died within 1-5 years (43%), and four (51%) are still alive 1-9 years post-transplantation. Only one patient clearly died as a direct cause of the NTM infection, and this occurred early post-transplantation due to disseminated M abscessus infection. Of the other deceased patients with pre-existing NTM disease, one died due to graft rejection at 3 years, one died due to graft rejection with concomitant non-NTM pneumonia at 2 years, and one died due to cardiac arrest at 4 years. In the cohort without NTM disease, none died within 1 year of transplantation, 22% died within 1-5 years, 11% died more than 5 years post-transplant, and 66% are still alive 1-14 years post-transplant. The probability of survival more than 1 year and more than 5 years post-transplant and 38% in patients with NTM-disease, and 100% and 56% in patients without NTM disease. Conclusion NTM infection in the lung transplant candidate is uncommon and challenging, however successful treatment can occur, perhaps in the setting of certain subspecies and with prolonged combination antimicrobial therapy. Background/aimsTo determine the mortality within 20 years of diagnosis of chronic open-angle glaucoma (COAG) and visual acuity and visual field progression of a cohort followed for 20 years.MethodsTwenty years following the diagnosis of COAG in 68 of 436 (16%) patients seen in a glaucoma case-finding clinic, visual and mortality outcomes were audited from medical records. Causes of death were obtained from general practitioner records and death certificates. Probability of death was calculated using a Kaplan-Meier survival curve. The visual field of each eye of survivors was graded using a nine-stage severity scale. Visual outcome was analysed at the 20-year follow-up visit.ResultsFrom 68, 14 (21%) were lost to follow-up. In the remaining 54, 20 (37%) were alive 20 years after diagnosis. Of 63% who died, mean age of death was 84 years, most commonly due to vascular disease. Mean age at presentation of those who died was 73.7 years versus 63.2 years for survivors (P=0.001). The median time to death was 16 years. On visual field analysis, nearly half (48.9%) of eyes did not deteriorate, but 28.3% eyes deteriorated by more than two stages. Those who died had worse final visual acuity than survivors (P<0.001). Three who died were registered severely visually impaired mainly from macular disease, but no survivors were registered (P<0.001).ConclusionIn this cohort, approximately two-thirds of patients with glaucoma died within 20 years of diagnosis. In most older patients with glaucoma, the overall goal of preventing visual handicap and blindness is achievable 20 years after diagnosis.

IntroductionUnprovoked pulmonary embolism (UPE) is not rare and it is associated with an unfavorable prognosis in adults. However, the incidence and the prognosis of UPE in older adults have never been studied.Material and methodsThis was a historical prospective study. We reviewed all the medical charts of all older adults (aged 70 years or more) with UPE, provoked pulmonary embolism (PPE), and malignancy-associated PE (MAPE), admitted to a tertiary medical center between 2010 and 2012. The all-cause 3-year mortality rates and cumulative survival following admission were compared between the groups.ResultsThe final cohort included 249 patients with PE: 161 (64.7%) were women; the mean age was 79.8 ±5.7 years. Overall, 36 (14.5%) patients had UPE, 81 (32.5%) patients had MAPE, and 132 (53.0%) patients had PPE. Overall, 39 (15.7%) patients died within 30 days, 76 (30.5%) patients died within 6 months, 101 (40.6%) patients died within 1 year, and 136 (54.6%) patients died within 3 years of admission. Relative to PPE and MAPE patients, the cumulative survival was significantly higher in UPE patients at each time point within 1 year of admission (p < 0.05 and p < 0.001, respectively). However, 3 years after admission, the cumulative survival was comparable between PPE patients and UPE patients, and was significantly lower in MAPE patients (p < 0.001).ConclusionsUPE is not rare in older adults with PE, and it is associated with a favorable prognosis within 1 year of admission in this population.

Abstract:
From November 2001 through to March 2002, 100 boys and 108 girls, aged 12.0 ± 0.8 years were assessed for coronary heart disease (CHD) risk factors. Rural (school 1) and valley (school 2) comprehensive schools (age range 11 to 18 years) were selected to represent differing socio-economic status (SES). Informed written consent was sought from head teachers, parents, and schoolchildren. All measurements were made in the subjects' respective schools, with the author making a total of 60 visits to these schools. Tests were carried out at the same relative time intervals and by an identical team of researchers. Physical and physiological measurements included stature (m), body mass (kg), body mass index (BMI) (kg m'2), skinfold measurements (£ SKF) (mm), waist to hip ratio (WHR), aerobic fitness (20 MST) (shuttles), systolic (SBP) (mm Hg) and diastolic blood pressure (DBF) (mm Hg). Total cholesterol (TC) (mmol-L' 1 ), high density lipoprotein-cholesterol (HDL-C) (mmol-L" 1 ), low density lipoprotein-cholesterol (LDL-C) (mmol-L" 1 ), TC: HDL-C, triglyceride (TG) (mmol-L' 1 ), lipoprotein(a) (Lp(a)) (mg-dL'1), glucose (Glu) (mmol-L' 1), fibrinogen (Fg) (mg-dL" 1), folate (ng-mL"1 ), vitamin Bn (pmol-L' 1), and homocyst(e)ine (Hey) (umol-L" 1 ) were measured for haematological concentrations. Lifestyle assessments included dietary and physical activity analysis. Familial risk of coronary heart disease was also determined. Blood analyses were conducted at the Biochemistry Laboratories of the Royal Glamorgan Hospital, Llamtisant, and Llandough Hospital, Penarth.

Thesis (MTech (Consumer Science: Food and Nutrition))--Cape Peninsula University of Technology, 2016.<br>Objective: To determine the association between the dietary fat knowledge and consumption of foods rich in fat among first-year students in self-catering residence at a university of technology, Cape Town, South Africa. Design: The two concepts – the dietary fat knowledge (represented by dietary fat food knowledge and dietary fat nutrition knowledge) and the consumption of foods rich in fat – were assessed separately. Two norm-referenced, valid and reliable knowledge tests and an intake screening questionnaire were used for the assessments (as subsidiary objectives), before the associations between the concepts were determined (main objective). The dietary fat food and nutrition knowledge as assessed was categorised in the range poor or below average, average and good or above average, and the consumption of foods rich in fat as high, quite high, the typical Western diet, approaching low or desirable. The Pearson‟s chi-square test was applied to these categorical findings to determine if associations (five percent significance) existed between the concepts. Results: The stratified sample included 225 first-year students. Nearly half (48.4%) of them achieved an average fat food knowledge score, while the majority (80.9%) achieved a poor fat nutrition knowledge score. More than half (52.5%) either followed a typical Western diet, a diet quite high in fat or high in fat. While no significant (p > 0.05) association was found between the students‟ dietary fat food knowledge and consumption of foods rich in fat, significant results were found in the association between the students‟ dietary fat nutrition knowledge and consumption of foods rich in fat (p < 0.05) and between their fat food knowledge and fat nutrition knowledge (p < 0.001). Conclusions: An inverse association was found between the students‟ dietary fat nutrition knowledge and fat consumption, a positive association between their dietary fat food knowledge and dietary fat nutrition knowledge, and no association between their dietary fat food knowledge and fat consumption.

Abstract:
As role models and experts in nutrition, registered dietitians have the responsibility of educating the public about appropriate nutrition practices. The purpose of this study was to compare physical characteristics and dietary intake of entry-level dietitians(ELD) and non-entry level dietitians (HELD) in Indiana through a mailed personal history questionnaire and food frequency questionnaire. Most of the dietitians reported a weight that was normal or overweight when compared to their ideal body weight. Very few were underweight, yet an average of 36.2 % of all the dietitians were overweight. Statistically, the findings of this study indicated that there was no significant difference in compliance with the established standards of the Food Guide Pyramid between the ELD and HELD in any of the food groups. Secondly, there was not a significant difference in overall compliance between the two groups of dietitians. These results could indicate that registered dietitians are not forgetting the importance of proper nutrition and are continuing to model positive dietary behaviors as they continue in the profession.<br>Department of Family and Consumer Sciences

Der "Hundertjährige Kalender" basiert auf Wetteraufzeichnungen von Mauritius Knauer (1613-1664 ), der als Abt im oberfränkischen Zisterzienserkloster Langheim wirkte. Die zwischen März 1652 und März 1659 fast täglich notierten Beobachtungen ermöglichen eine statistische Auswertung und die - teilweise sogar quantitative - Gegenüberstellung damaliger und heutiger Wetterverhältnisse. Vergleiche mit Niederschriften weiterer zeitgenössischer Autoren belegen den wissenschaftlichen Wert der Knauerschen Aufzeichnungen. Der Beobachtungszeitraum fällt in die "kleine Eiszeit" und zeichnet sich durch eine Reihe sehr strenger Winter aus<br>The "Hundred Year's Calendar" is based on weather recordings of Mauritius Knauer (1613-1664 ), which worked as an abbot in the upper-Frankish Cistercian monastery Langheim. Between March 1652 and March 1659 almost daily noted observations enable a statistical analysis and - partly even quantitative - the confrontation weather conditions at that time and of today's. Comparisons with notes of further contemporary authors show the scientific value of the Knauer recordings. Abstract This chapter sets out to solve the mystery of why Sierra Leone has remained relatively stable 14 years after peacekeepers left the country in 2006, and 18 years after the end of a devastating 11-year civil war in which an estimated 70,000 people died. In doing so, Sierra Leone has defied the fate of so many fragile and conflict-prone states: it has not returned to war, as do more than half of all countries within only five years of a peace settlement. This despite myriad socio-economic challenges, of the kind that often leads to a recurrence of violence and unrest. In investigating this mystery, the chapter highlights how domestic, subregional, and external actors muddled through and improvised one of the rare peacebuilding success stories in Africa.

AbstractIn this chapter, we use censuses, lists of centenarians, and vital statistics to describe the demography of centenarians, defined as the group of people who are 100 years old or older. We also refer to the group of people between the ages of 105 and 109 as semi-supercentenarians. Finally, we refer to those individuals who reach the age of 110 and beyond as supercentenarians. Although our ability to describe centenarians using these data is currently very limited, censuses and the lists of centenarians can be used to determine the number of people who survive to age 100 or older. Moreover, vital statistics provides the number of deaths, by single year of age and by sex, for those who died above the age of 100 since 1899. Our study examines trends in the highest ages at death for each year between 1963 and 2015, and shows increasing trends during this period. Using the number of deaths and applying the extinct cohort method, we estimate the level of mortality of people aged 100 or older, as well as trends in mortality. We observe decreasing trends in mortality levels among people who have reached very high ages, even as mortality continues to increase with age until very high ages.

AbstractThis contribution presents the validation of the age at death of Geert Adriaans BOOMGAARD (GAB), a seaman who reached the age of 110. He was born in Groningen on 21 September 1788 and died in the same city on 3 February 1899. A remarkable number of documents have been found that cover the full span of GAB’s life, and thus make it possible to validate his reported exceptional age. In the first step of the validation, a comparison of the baptism and death records shows that the information provided is consistent, even if the spelling of the surnames of his parents reported in the two records is not identical. The reconstitution of GAB’s family and the dates of birth of his siblings also support the validity of GAB’s reported age at death. The demographic information covers the period between 1818 (the year of his first marriage) and 1837 (the year of birth of his last child). We found few documents that mention him during his early life before his first marriage, including a document from 1791 indicating that his father named his new boat De Jonge Geert as well as a list of conscripts from 1811 where his name appeared. By contrast, we found numerous documents covering the period from 1837 to 1899 that are related to his career as a seaman; the marriages of children; his entry into a nursing home; and various interviews, photos, and articles on his life that appeared in the press. All of these documents support the validity of GAB’s reported year of birth and age at death. Thus, GAB might be considered the first thoroughly validated supercentenarian in the history of humankind.

Lincoln’s Gettysburg Address, delivered on November 19, 1863, included several biblical themes; it even sounded like the King James Bible. The opening—“Four score and seven years ago our fathers brought forth on this continent, a new nation, conceived in Liberty”—recalled texts from the Bible, including the book of Revelation (Revelation 12:5). The Gettysburg Address assessed a year of unprecedented sacrifice on the battlefield. This was also a year of conflicting viewpoints of the war’s moral purpose, especially as deaths multiplied and battles continued with no end in sight. Loss cries out for meaning, and Americans sensed that in 1863. When Americans saw loss on a scale like they did that year, they turned to the Bible to give meaning to the war, much as Lincoln did at Gettysburg.

Acute stroke is normally the result of thromboembolism. Such thromboemboli form and extend by the interaction of platelets and fibrin and elicit a fibrinolytic response. Hence laboratory indices of platelet activation, thrombin formation and plasmin formation may be related to thrombus size and progression, and hence to clinical outcome, ie disability and death. We studied 100 patients with acute paretic stroke and followed them for 1 year. Plasma levels of betathromboglobulin (BTG), fibrinogen, fibrinopeptide A (FPA), fibrin(ogen) fragment BB15-42, serum fragment E, high molecular weight cross-linked fibrin degradation products (X-L FDP) D-dimer, total and tissue plasminogen activator activity, tissue plasminogen activator inhibition and serum fibrin(ogen) degradation products (FDP) were related to death and functional recovery in the 1 year follow up period. The levels of BTG, fibrinogen, FPA, BB15-42, tissue plasminogen activator inhibition and serum fragment E were significantly higher on the first day following stroke, in patients who subsequently died within 1 year when compared to patients who survived. Lowered levels of fibrin plate lysis area as well as raised tPA activity, X-L FDP and D-dimer levels did not achieve significance in patients who subsequently died. In the patients who survived only increased BB15-42 and X-L FDP levels were predictive of functional dependence when compared to patients who became independent. Increased fibrinogen, FPA and fibrin plate lysis area, fragment E and tPA activity and reduced D-dimer did not achieve significance in patients with greater disability when compared to patients with minimal disability. We conclude that several measures of activation of haemostasis are predictive of death in the year following stroke, but only BB15-42 and X-L FDP predict disability in survivors.

In a prospective cohort study, we evaluated X-ray VG in the management of non-surgical patients with clinically suspected PE. Thusfar follow up is available on 131 consecutive patients with suspected PE. In all patients a perfusion lungscan (PS) was carried out within 24 hours. In case of a normal PS (group A, n=32), no anticoagulant (AC) therapy was given. In case of an abnormal PS, AC therapy was started (heparin) and a bilateral ascending VG was carried out within 72 hours. In 46 patients (group C) venous thrombosis (DVT) was demonstrated by VG, and these patients were treated with AC for 6 months. In 53 patients with suspected PE and an abnormal lungscan, bilateral VG did not show DVT (group B). AC therapy was discontinued in these patients These patients were then screened for 14 days with fibrinogen legscanning and impedance plethysmography (IPG), followed by IPG alone every 2 months for at least 1 year. In group B, 6 patients died in the follow up period. None of the patients had signs of PE at autopsy. One additional patient in group B developed DVT documented by repeat VG, 6 months after entry into the trial. According to these preliminary data, it seems safe to base the decision whether or not to treat a patient with suspected PE with AC, on the presence or absence of DVT in the lower limbs as demonstrated by VG

Abstract Cancer is one of the most debilitating diseases in the world, affecting over 9.6 million people worldwide every year. Breast cancer remains the second largest cause of death in women. Despite major advances in treatment, over 40,920 women died of breast cancer in 2018 in the United States alone. Early detection of abnormal masses can be crucial for diagnosis and dramatically increase survival. Current screening techniques have varying accuracy and perform poorly when used on heterogeneously and extremely dense breast tissue. Infrared imaging has the potential to detect growing tumors within the breast based on thermal signatures on the breast surface by imaging temperature gradients induced by blood perfusion and tumor metabolic activity. Using clinical patient images, previous methods to estimate tumor properties involve an iterative algorithm to estimate the tumor position and diameter. The details from the MRI are used in estimating the volumetric heat generation rate. This is compared with the published values and the reasons for differences are investigated. The tumor pathology is used in estimating the expected growth rate and compared with the predicted values. The correlation between the tumor characteristics and heat generation rate is fundamental information that is needed in accurately predicting the tumor size and location.

A 24-year-old woman with a diagnosis of Glanzmann's Thrombasthenia (GT) type I (confirmed in our Hemostasis Section by sodium dodecyl sulphate-polyacrylamide gel electrophoresis) and with a history of several transfusions, was seen in our Service since the second month of her first pregnancy.In the first visit a strong anti-Rh(D) antibody was found in her serum. In 28th week an ammiocentesis was done. Seventy two hours later, she was hospitalized because of disminution of foetal movements. Then, a platelet-allo-antibody, IgG+IgM type (title IgG 1/512, IgM 1/2) was detected in her scrum by immunofluorescence test, as well as an anti-HLA antibody (by lymphocytotoxicity test) which reacted with her husband lyin-phocites.The platelet antibody wasn't EDTA or Paraformaldehide (PFA) dependent and reacted with all normal platelets of a panel of known platelet phenotypes and, also although significantly weakly, with GT type II platelets. The negative results were only observed with GT type I platelets. Therefore, it seemed to recognize an antigenic site located in GP IIb-IIIa. Besides, this antibody inhibited the normal platelet aggregation.A caesarian was advised because of foetal suffering an was performed without immediate complications. The patient was protected by our hemotherapy established support and delivered a boy with a severe hemolytic disease (Hematocrit: 6%, bilirrubine: 1,9 mgs./lOCimls., platelets: 40.000/ mm3), who died 48h. later. His platelets had a direct positive test IgG type and in his serum the platelet antibody as in his mother's was detected.We discuss the specificity of the antibody detected and also, its possible implication in the neonatal thrombocytopenia.

Protein S deficiency has been demonstrated in 5 members from the same family with a history of severe recurrent venous thrombosis over three generations. The propositus, a 16 year old female, had a first spontaneous thrombotic episode at age 15. Phlebography revealed a total obstruction of her left ilio-femoral vein with an extension to the vena cava. She was treated with heparin followed by oral anticoagulant therapy. The four other affected members (mother, aunts and uncle of the propositus) had also presented recurrent thrombosis with onset at a young age. The grandfather, not tested, had died from massive pulmonary embolism at age 54. The immunological assay of protein S was performed in plasma by Laurell, using a monospecific antiserum to human protein S, or by an ELISA, using a kit from Diagnostica Stago (Asserachrom Protein S). In order to separate free protein S, the functionally active form, from protein S complexed with C4-binding protein, plasma was adsorbed with 3.75 % polyethyleneglycol (PEG 6000). Following PEG precipitation, the levels of free protein S antigen remaining in the supernatant were quantitated by the usual immunological methods. In addition, two-dimensional immunoelectrophoresis (DDIE) also provided information on the distribution of both forms. In plasma protein S levels were decreased (40 to 55 % of the normal range) in two untreated patients and lower levels (17 to 20 96) were observed in the three others, including the propositus, who were under dicoumarol therapy. In PEG treated-plasma, protein S was undetectable (less than 5 %) in all patients, indicating a lack of free protein S. This was confirmed by DDIE : whereas protein S migrated as two distinct peaks, corresponding to free and complexed protein S in normal plasma, only a single peak of complexed protein S was observed in all affected patients. These results clearly demonstrate a total lack of free protein S which appears to be responsible for the thromboembolic disorder in this family as there was no deficiency of the other plasma inhibitors (antithrombin III, heparin cofactor II and protein C). According to the classification recently proposed by Comp et al., this family belongs to type I protein S deficiency, with an autosomal dominant mode of inheritance.

SSHAP convened a virtual roundtable of expert advisors on Friday 12 March 2021 to discuss the outbreak of Ebola in Guinea declared on 14 February 2021. At the time of writing (19 March 2021), there have been 18 cases (14 confirmed, 4 probable), 9 deaths (including 5 in the community; CFR 50%) and 6 recoveries. Six of the 7 first cases identified were from the family of the first case, a 51-year-old nurse from Gouecke who died in N’Zérékoré on 28 January. Vaccination was launched on 23 February, and as of 17 March, 3,492 people had been vaccinated. The last new case was reported on 4 March 2021.

Long-term bird banding data were collected from 1996–2019 (except in 2008) to assess the New River riparian zone avian community in one location in New River Gorge National River (NERI). The NERI banding station has banded over 4,500 individuals over 10,700 net hours in the twenty-three years it has been in operation and has captured 80 different species. Total captures, capture rate, and total species have been declining annually over the study period. Species associated with early-successional/shrubland habitat also declined over the study period, which is consistent with regional trends during the same time frame. Species richness of habitat guilds did not change over the study period within specific major habitat types. Capture metrics of Louisiana Waterthrush, an obligate riparian species, did increase over the study period. Continued banding will further provide information to assist in local management and contribute to regional data.

On the occasion of the centenary of the First World War, remembered across the world from 2014 until the end of 2018, many aspects and experiences of this global conflict have been re-examined or brought to light for the first time, as we honour the memory of those estimated 16 million soldiers and civilians who perished in what was then known as the ‘Great War’, or the ‘War to End All Wars’. So many of these died on the infamous fields of Flanders, where Allied and Central Forces dug themselves into trenches for the better part of four years. Over the past few years, new research has brought to light many insights into the plight of animals in this War, which – for the younger readers amongst you – was fought at the dawn of motorised warfare, using anything powered by two or four feet or paws, from the homing pigeons delivering secret messages across enemy lines, to the traction provided by oxen and mules to pull cannons and other heavy artillery, to the horses of the cavalry. Not least among these roles was the supply of animal protein to the troops, whether this came through the specific designation of animals for this purpose or as the result of a failed attempt at delivering any of the above services. Several leading publications today have documented the role (and suffering) of animals in ‘La Grande Guerre’. Less so the role of veterinarians in the ‘War to End All Wars’. Who were they? How many? How were they organised? What did they do, on either side of the enemy lines? The present article is a humble attempt to shed some light on these veterinary colleagues, based on available, mostly grey, literature…

In this profile, we investigate trends in birth intendedness among women 15-44 between 1997 and 2018 using the 2002, 2006-10, 2011-15, and 2015-19 cycles of the National Survey of Family Growth1. Birth intendedness is based on a series of questions in which women were asked to characterize each birth as on time, mistimed (wanted but occurring earlier than desired), or unwanted (the respondent did not want any births at all or no additional births). When births were reported as mistimed, women were asked how much earlier than desired the birth occurred, and we categorize mistimed births into two groups: slightly mistimed (less than two years earlier than desired) or seriously mistimed (two or more years too early). This profile is an update of FP-17-08 and is the first in a three-part series on unintended fertility in the U.S.

In this Insight Note, we report results of a phone survey that the RISE Tanzania Research team conducted with 2,240 parents (or alternate primary care-givers) of primary school children following the school closures in Tanzania. After the first case of COVID-19 was confirmed in Tanzania on 16 March 2020, the government ordered all primary schools closed the following day. Schools remained closed until 29 June 2020. Policymakers and other education stakeholders were concerned that the closures would lead to significant learning loss if children did not receive educational support or engagement at home. To help stem learning loss, the government promoted radio, TV, and internet-based learning content to parents of school-age children. The primary aims of the survey were to understand how children and families responded to the school closures, the education related activities they engaged in, and their strategies to send children back to school. The survey also measures households’ engagement with remote learning content over the period of school closures. We supplement the findings of the parent survey with insights from interviews with Ward Education Officers about their activities during the school closures. The survey sample is comprised of primary care-givers (in most cases, parents) of students enrolled in Grades 3 and 4 during the 2020 school year. The survey builds on an existing panel of students assessed in 2019 and 2020 in a nationally representative sample of schools.4 The parent surveys were conducted using Computer Assisted Telephonic Interviewing (CATI) over a two-week period in early September 2020, roughly two months after the re-opening of primary schools. We report the following key findings from this survey: *Almost all (more than 99 percent) of children in our sample were back in school two months after schools re-opened. The vast majority of parents believed it was either safe or extremely safe for their children to return to school. *Only 6 percent of households reported that their children listened to radio lessons during the school closures; and a similar fraction (5.5 percent) tuned into TV lessons over the same period. Less than 1 percent of those surveyed accessed educational programmes on the internet. Households with access to radio or TV reported higher usage. *Approximately 1 in 3 (36 percent) children worked on the family farm during the closures, with most children working either 2 or 3 days a week. Male children were 6.2 percentage points likelier to work on the family farm than female children. *Households have limited access to education materials for their child. While more than 9 out of 10 households have an exercise book, far fewer had access to textbooks (35 percent) or own reading books (31 percent). *One in four parents (24 percent) read a book to their child in the last week.
