Abstract:
Dynamic property measurements of the moment-resisting steel-frame University of California, Los Angeles, Factor building are being made to assess how forces are distributed over the building. Fourier amplitude spectra have been calculated from several intervals of ambient vibrations, a 24-hour period of strong winds, and from the 28 March 2003 Encino, California ( ML=2.9), the 3 September 2002 Yorba Linda, California ( ML=4.7), and the 3 November 2002 Central Alaska ( Mw=7.9) earthquakes. Measurements made from the ambient vibration records show that the first-mode frequency of horizontal vibration is between 0.55 and 0.6 Hz. The second horizontal mode has a frequency between 1.6 and 1.9 Hz. In contrast, the first-mode frequencies measured from earthquake data are about 0.05 to 0.1 Hz lower than those corresponding to ambient vibration recordings indicating softening of the soil-structure system as amplitudes become larger. The frequencies revert to pre-earthquake levels within five minutes of the Yorba Linda earthquake. Shaking due to strong winds that occurred during the Encino earthquake dominates the frequency decrease, which correlates in time with the duration of the strong winds. The first shear wave recorded from the Encino and Yorba Linda earthquakes takes about 0.4 sec to travel up the 17-story building.

Numerous pressure modes are currently available on ventilators. The application of microprocessor technology has resulted in sophisticated mode options that are very responsive to patient-initiated efforts, yet little is known about how to use the modes or their effect on patient outcomes. This article describes a wide variety of pressure modes including traditional modes such as pressure support and pressure-controlled ventilation in addition to less traditional new modes such as airway pressure release ventilation, biphasic positive airway pressure, Pressure Augmentation (Bear 1000, Viasys Healthcare, Yorba Linda, California), Volume Support (Maquet, Bridgewater, New Jersey), Pressure Regulated Volume Control (Maquet, Bridgewater, New Jersey), Volume Ventilation Plus (Puritan Bennett, Boulder, Colorado), Adaptive Support Ventilation (Hamilton Medical, Switzerland), and Proportional Assist Ventilation (Dräger Medical, Richmond Hill, Ontario, Canada). The “good, the bad, and the ugly” issues surrounding the application, evaluation, and outcomes of the modes are discussed.

Abstract:
We examined the ability of two transcutaneous devices (Fastrac, Sensormedics Corporation, Yorba Linda, California, U.S.A. and Hewlett Packard M1018A, Hewlett Packard Component Monitoring System, Hewlett Packard, North Hollywood, U.S.A.) to measure arterial Pco2 and Po2 in neonates. Thirty-seven neonates had transcutaneous oxygen measured with the Hewlett Packard (HPo2 group), 38 neonates had transcutaneous carbon dioxide measured with the Hewlett Packard (HPco2 group) and the Fastrac was used on 27 neonates (FTco2 group). Both devices were operated with electrode temperatures of 43.5 °C although an additional ten subjects were studied using the Fastrac with an electrode temperature of 43.0°C. The mean differences (transcutaneous—arterial) and upper and lower limits of agreement were calculated for each group. For the HPo2 group they were 3.78 mmHg (-12.23 to 19.80 mmHg), for the HPco2 group they were 0.40 mmHg (-4.50 to 5.30 mmHg) and for the FTco2 they were - 0.96 mmHg (- 7.85 to 5.92 mmHg). For the Fastrac group at an electrode temperature of 43.0°C the mean difference and limits of agreement were -1.00 mmHg and -4.58 mmHg to 2.58 mmHg. The average sensitivity and specificity for both machines for the detection of hypocarbia were 82% and 92% respectively while for hypercarbia they were 90% and 94% respectively. For hypoxaemia, the sensitivity and specificity were 40% and 94% while for hyperoxaemia the sensitivity and specificity were 83% and 97%. We conclude that both machines provide a useful supplement to arterial Pco2 measurements and the Fastrac performs better at 43.0°C. The measurement of Po2 is less accurate but is still of clinical use.

Background: Ventilator induced lung injury prevention may begin from birth, and respiratory support without endotracheal intubation is an attractive option in preterm baby with Respiratory distress syndrome (RDS). The objective of the study was to evaluate the clinical course and respiratory outcomes in preterm infants with moderate RDS assigned from birth to Nasal Continuous Positive Airway Pressure (NCPAP) or Bi-level Nasal Continuous Positive Airway Pressure (Bi-level NCPAP).Methods: 60 infants of 28-34 weeks GA (<35 wks GA), affected by moderate RDS, were considered eligible and were randomized to NCPAP (CPAP level=6cm H2O, Group A n=30) or to Bi-level NCPAP (lower CPAP level=4.5 cmH2O; higher CPAP level=8 cmH2O, Group B n=30), provided with the variable flow devices (Infant Flow CPAP vs Infant Flow SiPAP™, Viasys Healthcare, Yorba Linda, CA).Results: Length of ventilation, oxygen dependency, need for intubation and occurrence of air leaks were considered as outcomes. Infants showed similar characteristics at birth (Group A versus Group B: GA 30.4±2 wks versus 30.3±2wks, BW 1433±545g versus 1415±560g. Group A underwent longer respiratory support (6,2±2 days versus 3,8±1 days, p=0.025), longer O2 dependency (13,8±8 days versus 6,5±4 days, p=0.027) and was discharged later (GA at discharge 36,7±2,5 weeks versus 35,6±1,2 weeks, p=0.02). All infants survived. No BPD or neurological disorders occurred.Conclusions: Bi-level NCPAP was associated with better respiratory outcomes versus NCPAP, and allowed earlier discharge, inducing the same changes in the cytokine levels. In our population, it was well tolerated and safe

Abstract:
This essay utilises two different but related concepts: “poetics and politics of literary memory” by Lars Eckstein and “adaptation” by Linda Hutcheon, to examine Yoruba narrative convention, characterisation and the “spirituality of Being and object(s)” in two films, Saworoide (1999) and The Narrow Path (2006), by the Nigerian filmmaker, Tunde Kelani, who has distinguished himself as one of the leading contemporary Nigerian and African cinema icons and storytellers. The essay argues that as one of the most significant voices in Nollywood, the history of film and cinema in Nigeria and beyond, Kelani has not only turned the adaptation of materials from page to screen into an art and a veritable source of history, he has also shown ways in which the process functions as the recollection of the fading glorious past of his race. In order to achieve its aim, the essay is divided into two parts: the first part examines the cultural and political considerations of memory and the aesthetics of adaptation in relation to Kelani’s body of works, and the second part discusses the two films by drawing from arguments that are developed in the first part.

Abstract:
Objective pulmonary function (PF) evaluation is essential for the diagnosis, monitoring, and management of many pediatric respiratory diseases as seen in the emergency room, intensive care, and outpatient settings. In this paper, the development and testing of a new noninvasive PF instrument, pneuRIPTM, which utilizes respiratory inductance plethysmography (RIP) are discussed. The pneuRIPTM hardware includes a small circuit board that connects to the RIP bands and measures and wirelessly transmits the band inductance data to any designated wirelessly connected tablet. The software provides indices of respiratory work presented instantaneously in a user-friendly graphical user interface on the tablet. The system was tested with ten normal children and compared with an existing system, Respitrace (Sensormedics, Yorba Linda, CA), under normal and loaded breathing conditions. Under normal breathing, the percentage differences between the two systems were 2.9% for labored breathing index (LBI), 31.8% for phase angle (Φ), 4.8% for percentage rib cage (RC%), and 26.7% for respiratory rate (BPM). Under loaded breathing, the percentage differences between the two systems were 1.6% for LBI, 4.1% for Φ, 8.5% for RC%, and 52.7% for BPM. For LBI, Φ, and RC%, the two systems were in general agreement. For BPM the pneuRIPTM is shown to be more accurate than the respitrace when compared to manually counting the breaths: 13.2% versus 36.4% accuracy for normal breathing and 16.9% versus 60.7% accuracy for breathing under load, respectively.

Abstract:
More research is needed to be able to fully understand the role that social media plays in elections, specifically in local elections. Candidates need to understand how it works and how they can effectively use this new communication medium. By exploring Diffusion of Innovation Theory, Social Information Processing Theory, and the Two-Way Symmetrical Model of communications this study sought to answer one overarching question: how should a candidate employ social media in a local election? This qualitative, single case study explores the 2014 recall and general election in Yorba Linda, California. Councilman Tom Lindsey and candidate Matt Palmer are the primary subjects of study. Observations were made through analysis of documentation, interviews, and participant and direct observation. The researcher was employed as the campaign manager for both Lindsey's and Palmer's campaigns. The findings support the use of social media in local campaigns on a case by case basis. Determining use depends on the demographics of the voters and the abilities of the candidate. The data suggests that social media needs to be part of comprehensive strategy that includes traditional communication tools. Observations from the case study illustrate the need for candidates to engage in two-way communication that is monitored and regulated. This study begins to establish social media as a tool that candidates can use to inexpensively reach voters in a way that showcases the candidate's personality and allows them to connect on a personal level with constituents. Social media will play a role in politics at all levels.

Abstract:
This chapter introduces the book’s main claims by analyzing three examples where religion inspired political controversy in recent US politics: Amina Wadud’s decision to lead a mixed-gender Friday prayer in New York City in 2005, the protests outside an Islamic Circle of North America fundraiser in Yorba Linda, California in 2011, and Mormons Building Bridges’ inaugural march in the Salt Lake City Pride Parade in 2012. Public discussions of these cases exemplify the dominant approaches American citizens use to evaluate religion’s roles in democratic politics. The chapter contrasts these approaches with the alternative defended in this book: a way-of-life conception of democracy. The way of life-conception implies a framework for evaluating religion in politics that is democratic, liberal, and pluralistic. Approaching religion in politics with this conception avoids prominent concerns about evaluating religion: that extant perspectives flatten and deform religious phenomena.
