Valentina Martina explores the relation between the plane of linguistic meanings and reality through an analysis of the concept of "system". The article by Artemij Keidan addresses the problem of the definition of deixis and its role in the disambiguation of proposition, with special reference to structuralism and contemporary theories on direct reference. The work of Luca Alfieri takes its cue from recent studies on cognition to demonstrate the unsustainability of the Jacobsonian dichotomy of metaphor and metonymy. Rounding off the book is an essay by Boris Uspenskij on the role of personal pronouns in the structure of language, in semiotics and in human communication, lavishly illustrated with examples and historical curiosities.

As most cancers occur in middle-aged or older adults, only a very small proportion of the overall radiotherapy workload relates to children and young people. As there is a wide spectrum of different cancer types in this age group, not only is paediatric cancer uncommon overall but each individual type is very rare. There are many ways in which to deliver radiotherapy, including advanced photon techniques, proton beam therapy, brachytherapy, and molecular radiotherapy. For these reasons, the care of children and young people requiring radiotherapy is limited to a small number of highly specialist centres. Delivery of high-quality paediatric radiotherapy requires a multiprofessional team including radiation or clinical oncologists, therapy radiographers, physicists, dosimetrists, anaesthetists, and play specialists. This team has to interact very closely with the wider paediatric and adolescent oncology multidisciplinary team, which includes oncologists, surgeons with different anatomical expertise, radiologists, and pathologists. Children, with their developing tissues and organs, are more susceptible to long-term radiation-induced complications, including second cancers, than adults. The art of paediatric radiotherapy, therefore, is to select treatment approaches which offer the maximum chance of cure while minimizing the risk of adverse effects. Careful teamwork, peer review of radiotherapy planning, and quality assurance within a clinical trial framework offer the best chances of achieving this balance. This book covers all these aspects, highlighting the need for highly specialist teams with the extensive knowledge and the broad skillset required to offer children and young people the best possible treatments.

Paediatric urology nurse specialists play an important role in training and supporting young patients and their families. The aims of urological management can be summarized as: protecting upper tract function; providing adequate urine storage capacity at safe pressure; and ensuring adequate emptying. As well as preserving renal function, these treatment goals are intended to provide a socially acceptable degree of urinary continence. Nephronophthisis (NPHP) is a clinically heterogeneous autosomal recessive cystic kidney disease and the leading genetic cause of end-stage renal failure in children and young adults. Whilst enlarged dysplastic cystic kidneys are associated with infantile NPHP, more typically renal ultrasound reveals normal kidney size and corticomedullary cysts in a child with polyuria and secondary enuresis. Extrarenal manifestations occur in 10–15% including retinal degeneration, cerebellar vermis hypoplasia and liver fibrosis, requiring referral to other specialists. Mutations in 18 genes have been identified to cause NPHP, but a genetic diagnosis still cannot be found in many patients. NPHP is classified as a ciliopathy because of the localization of the protein products of the associated genes. Currently there is no specific therapy for NPHP.

This chapter explores how the particular conditions in which conservation biologists conduct their studies can provide a narrow and possibly misleading view of endangered species. Orangutans are a good example. Generally viewed by scientists as ecological specialists of primary rainforests with limited human influence, orangutans may in fact be ecologically and behaviorally adapted to human disturbance, shaped by 60 000 years of co-existence with modern humans. Orangutan scientists have been slow in embracing these views, which has hampered the development of more effective approaches to conservation management, such as the protection of orangutans in selectively logged forests. Also, a narrow focus on habitat loss has hampered efforts to address other important threats such as hunting. This chapter urges young scientists to remain open-minded about their study objectives, question any long-held beliefs, and learn to view species and systems in the broadest context possible.

Idiopathic inflammatory myopathies (IIM) of childhood are rare serious disorders, of which the most common is juvenile dermatomyositis (JDM). It is recognized increasingly that, even within JDM, there is significant heterogeneity. Recently defined biomarkers, such as the myositis-specific autoantibodies (MSA) have been valuable to define juvenile IIM subgroups and may provide novel methods of classification. Although randomized controlled trials in JDM are challenging, an increasing consensus-based body of evidence is being used to drive standardization of treatment and care, with new drugs for severe cases being increasingly tested. Significant differences between adults and children with dermatomyositis (DM) exist, and are reviewed in this chapter. These differences emphasize the importance of specialists in the area of transition of young people who have had JDM to receive their care from a team of health professionals with comprehensive knowledge of JDM, as well as transitional care issues.

Successful transitioning involves input from both paediatric and adult multidisciplinary teams with overlap between the two services and tailored to the needs of each individual. This includes varying the duration of the preparatory phase and the timing of transition and eventual transfer based on the chronological and developmental age, maturity, medical stability, and psychosocial issues of each individual patient. Buddy systems and peer support may aid smooth transitioning for some, with the promotion of patients attending a clinic with similar age groups, which can include the formation of a young adult clinic. Effective collaboration between children’s and adult services is required to obtain successful transition of adolescent patients. Moving from one paediatric hospital to different adult specialists in different hospitals requires excellent communication between various teams. Some staff members from each adult nephrology unit should specialize in adolescent medicine and construct their own transitional care pathways to ensure effective communication and collaboration with appropriate paediatric units and facilitate continuity of care with ongoing educational and social programmes.

Clinical electromyography (EMG) refers to the diagnostic tool in the electrophysiological evaluation of disorders of peripheral nerve and muscle. This introductory chapter defines the terms of the discipline and its scope. Clinical EMG used in the evaluation of Clinical EMG is utilized by a variety of physicians, including specialists in the field of neurology, physical medicine and rehabilitation, orthopedics, hand surgery, neurosurgery, spine, rheumatology and pain management. The scope of the EMG Examination includes nerve conduction studies and needle EMG. It also includes other specialized testing such as late responses, repetitive nerve stimulation and single fiber EMG. This chapter discusses the referral process to the EMG laboratory and guides the readers to the best practice in the EMG evaluation of patients with neuromuscular disease. Special attention to testing young children and testing patients in the intensive care unit is given. The generation, format and final layout of the EMG report is also advised.

Childhood cancer in adults outlines the uncommon challenge presented by the occurrence of these pathologies in young adults, and the need for shared expertise in their successful management. The embryonal tumours, medulloblastoma, retinoblastoma, neuroblastoma, and Wilms’ tumour are considered first, emphasising similarities and differences in the pathology and management of each when adults are compared with children. The soft tissue sarcoma, rhabdomyosarcoma, is frequently best managed with chemotherapy and surgery. The important role of specialist cytogenetics in all these tumours is emphasised.

Abstract:
This chapter provides a critical analysis of the role that vocational education and training plays in preparing young people for the labour market in two contrasting systems – Australia and Germany. In Germany, this occurs mainly within the structure of the “dual system”. In Australia, it occurs within a system of comprehensive high schools, where vocational studies are located within the senior secondary certificates. In Australia, it also occurs to an extent in the adult sector VET institutions and in some specialist providers which focus on school-aged youth. The concepts of education logic and employment logic and the type of welfare state, whether neocorporatist or neoliberal, are used to analyse important differences between Australia and Germany. The chapter argues that the skills formation of young people requires both symbolic and financial support and an approach to upper secondary education which is not captive to the sorting and selection mechanisms of universities.

The cartoon is an educational tool that can be enjoyed by youth and adults alike, whether integrated into youth centre curricula or non-governmental organizations civic education campaigns. The booklet is designed primarily for Myanmar and the characters and scripts are tailored to this context. The information shared in the cartoon, however, is relevant for people interested in learning and teaching about constitutions all around the world.

Traumatic brain injury (TBI) is a leading cause of death and disability worldwide and although young male adults are at particular risk, it affects all ages. TBI often occurs in the presence of significant extracranial injuries and immediate management focuses on the ABCs—airway with cervical spine control, breathing, and circulation. Best outcomes are achieved by management in centres that can offer comprehensive neurological critical care and appropriate management for extracranial injuries. If patients require transfer from an admitting hospital to a specialist centre, the transfer must be carried out by an appropriately skilled and equipped transport team. The focus of specific TBI management is on the avoidance of secondary injury to the brain. The principles of management are to avoid hypotension and hypoxia, control intracranial pressure and maintain cerebral perfusion pressure above 60 mmHg. Management of increased intracranial pressure is generally by a stepwise approach starting with sedation and analgesia, lung protective mechanical ventilation to normocarbia in a 30° head-up position, maintenance of oxygenation, and blood pressure. Additional measures include paralysis with a neuromuscular blocking agent, CSF drainage via an external ventricular drain, osmolar therapy with mannitol or hypertonic saline, and moderate hypothermia. Refractory intracranial hypertension may be treated surgically with decompressive craniectomy or medically with high dose barbiturate sedation. General supportive measures include provision of adequate nutrition preferably by the enteral route, thromboembolism prophylaxis, skin and bowel care, and management of all extracranial injuries.
