SynopsisThe effects on depressive thinking and depressed mood of a brief, standardized distraction procedure were examined. In low endogenous patients (scoring 3 or less on the Newcastle Diagnosis Scale (NDS)), distraction significantly reduced the frequency of depressing thoughts. Consistent with Beck's cognitive model of depression, these patients were significantly less depressed after distraction than after a control procedure. In high endogenous patients (scoring 4 or more on the NDS), distraction produced less marked reductions in frequency of depressing thoughts, and no significant change in depressed mood. It is suggested that the relationship between negative thinking and depressed mood differs in the two patient groups.

Studies on depression in the workplace have mostly investigated its impact on individual employees. Little is known about its association with the company as a whole, or the state where the company is based. This is due to the lack of scalable methodologies operationalizing depression in the specific context of the workplace, and of data documenting potential distress. In this work, we adapted a work-related depression scale called Occupational Depression Inventory (ODI), gathered more than 350K employee reviews of 104 major companies across the whole US for the (2008-2020) years, and developed a deep-learning framework (called AutoODI) scoring these reviews on a composite ODI score. Presence of ODI mentions manifested itself not only at micro-level (companies scoring high in ODI suffered from low stock growth) but also at macro-level (states hosting these companies were associated with high depression rates, talent shortage, and economic deprivation). This new way of applying AutoODI onto company reviews offers both theoretical implications for the literature in computational social science, occupational health and economic geography, and practical implications for companies and policy makers.

Introduction The prevalence of depression based on the Patient Health Questionnaire-8 (PHQ-8) may vary depending on the scoring method. Objectives 1) To describe the prevalence of depression in Europe using two PHQ-8 scoring methods. 2) To identify the countries with the highest prevalence according to each method. Methods Data from 27 countries included in the European Health Survey (EHIS-2) for the year 2014/2015 were used (n=258,888). All participants who completed the PHQ-8 were included. The prevalence of depression and its 95% Confidence Interval (95%CI) were calculated overall for the whole of Europe and for each country using a PHQ-8≥10 cut-off point and the PHQ-8 algorithm scoring method. Weights derived from the complex sample design were considered for their calculation. Results The overall prevalence of depression for all Europe was lower using the PHQ-8>=10 cut-off point (6.38%, 95%CI 6.24-6.52) than the PHQ-8 algorithm (7.01%, 95%CI, 6.86-7.16). Using the PHQ-8≥10 cut-off point, the highest prevalence was observed in Iceland (10.33%, 95%CI, 9.33-11.32), Luxembourg (9.74%, 95%CI, 8.76-10.72) and Germany (9.24%, 95%CI, 8.82-9.66). Using the PHQ-8 algorithm the highest rates were observed in Hungary (10.99%, 95%CI,10.14-11.84), Portugal (10.63%, 95%CI, 9.96-11.29) and Iceland (9.80%, 95%CI, 8.77-10.83). Conclusions There is variability in the prevalence of depression rates in Europe according to the PHQ-8 scoring method. These findings suggest the necessity of identify the method of choice for each country comparing with a gold standard measure (clinical diagnosis). Countries with consistent higher prevalence of depression based on PHQ-8 regardless of scoring method deserve further study. Disclosure This work has been funded by CIBERESP (ESP21PI05)

The purpose of our study is to evaluate athletic identity (AI) and mental health measures of youth and young adult athletes during the COVID-19 pandemic. This cross-sectional study recruited athletes aged 11–25 years from universities, high schools, and middle schools in California and New York. Participants were emailed a link to an anonymous, cross-sectional electronic survey. The measure included the athletic identity measurement scale (AIMS), the Patient Health Questionnaire-4 (PHQ-4), and demographic variables. Chi-square, Fisher’s Exact Test, and linear regression were used to examine the relationships between AI, symptoms of anxiety, and symptoms of depression by age, gender, and race. The survey was completed by 653 participants. AI was stratified by tertiary percentiles. The odds of positively scoring for symptoms of anxiety were 60% higher for participants in college compared with high school (OR: 1.60, 95% CI: [1.09, 2.35]). Conversely, the odds of scoring positively for symptoms of depression were 68% higher for participants in high school compared to college (OR: 1.68, 95% CI: [1.09, 2.59]). The odds of scoring positively for symptoms of depression were higher for athletes who scored as high AI, compared to those who scored as moderate (OR: 1.72, 95% CI: [1.11, 2.68]) or low (OR: 1.93, 95% CI: [1.20, 3.12]). The odds of scoring positively for symptoms of anxiety on the PHQ-4 were 3.2 times higher for participants who identified as female (OR: 3.19, 95% CI: [2.31, 4.41]), and the odds of scoring positively for symptoms of depression were 2.4 times higher for participants who identified as female (OR: 2.35, 95% CI: [1.56, 3.54]). Female athletes experienced symptoms of depression and anxiety at significantly higher rates than male athletes during the COVID-19 pandemic. High school students experienced fewer symptoms of anxiety, but greater symptoms of depression as compared to the collegiate group, while college students experienced greater odds of symptoms of anxiety. Athletes in the high AI group were more likely to report symptoms of depression than moderate or low identity groups. Female athletes reported lower AI than male athletes, but still had greater symptoms of anxiety and depression.

IntroductionDuring the COVID-19 pandemic, the Albanian authorities declared mandatory stay-at-home measures, closing businesses, schools and public places.ObjectivesTo investigate the impact of these immediate changes on the mental wellbeing of the population.MethodsRespondents (N=1678) from 18 to 60 years old were selected through a convenient sampling method. Questionnaires were administered online reporting time spent daily in the COVID-19 topic and genealities; the Patient Health Questionnaire-9 and Generalized Anxiety Disorder-7.ResultsFindings suggest a significant negative correlation between age and anxiety scoring (r(n=1678)=-.121, p≤.001) and age and depression scoring (r(n=1678)=-.232, p≤.001), shown also on the ANOVA test for age and anxiety (F=6.019, p≤.05) and age and depression (F=20.326, p≤.05). Differences on the level of education resulted in a lower score of anxiety and depression respectively (F=3.524, p≤.05), (F=7.739, p≤.05) on respondents with higher education. Those who were jobless from the pandemic scored higher on anxiety and depression respectively (F=9.760, p≤.05) (M=6.21, ds=4.686) and (F=16.051, p≤.05) (M=8.18, ds=5.791). Significant differences were found related to different amounts of time spent on the COVID-19 topic, respectively for anxiety and depression (F=25.736, p≤.001), (F=5.936, p≤.003), with people who spend less than 1 hour scoring higher on depression (M=7.57, ds= 5.849) and those who spend more than 3 hours scoring higher on anxiety (M=6.76, ds=5.60).ConclusionsHigher education individuals, having a job and being in a romantic relationship relate to lower levels of depression during Covid-19 quarantine in Albania. Spending more time on the COVID-19 topic daily and being a female relate to higher level of anxiety.DisclosureNo significant relationships.

Objective: To study the correlation between depression and blood cytokine levels in lung cancer patients. Methods: 92 patients with advanced lung cancer were evaluated for depression using the scoring index of depression self-rating scale. Lack of depression (n=24), mild depression (n=45), and moderate depression (n=23) were found in the cohort. Meanwhile, 40 healthy subjects were selected as the control group. The levels of IL-10, IL-6, IL-8, and TNF-α in each group were detected by sandwich enzyme-linked immunosorbent assays, and their correlation with the degree of depression was analyzed. Results: The levels of IL-10, IL-6, IL-8, and TNF-α were all higher than those in the control group (P<0.05). Moreover, the depression statuses of patients with lung cancer were positively correlated with IL-10, IL-6, and TNF-α levels (r = 0.705, 0.301, and 0.446, P<0.01); however, the level of IL-8 was not relevant (r=0.136, p>0.05).Conclusion: Serum levels of IL-10, IL-6, and TNF-α are associated with depression scoring in patients with lung cancer.

This master thesis report describes signal processing parameters of electroencephalography (EEG) signals with a significant difference between the signals from the animal model of clinical depression and the non-depressed animal model. The signal from the depressed model had a weaker power in gamma (30 - 80 Hz) than the non-depressed model during awake and it had a stronger power in delta (1.5 - 4 Hz) during sleep. The report describes the process of using visualisation to understand the shape of the signal which helps with interpreting results and helps with the development of parameters. A generic tool for time-frequency analysis was improved to cope with the size of the weeklong EEG dataset. A method for evaluating the quality of how well the EEG parameters are able to separate the strains with as short recordings as possible was developed. This project shows that it is possible to separate an animal model of depression from an animal model of non-depression based on its EEG and that EEG-classifiers may work as indicative classifiers for depression. Not a lot of data is needed. Further studies are needed to verify that the results are not overly sensitive to recording setup and to study to what extent the results are translational. It might be some of the EEG parameters with significant differences described here are limited to describe the difference between the two strains FSL and SD. But the classifiers have reasonable biological explanations that makes them good candidates for being translational EEG-based classifiers for clinical depression.

The world is facing a shortage of professional medical staff, a situation which has been exacerbated by the COVID-19 pandemic which has significantly increased challenges globally and has had an adverse impact on the health care system. This has also led to additional barriers to patient care access, specifically for individuals who are in need of constant special care. To address the issue of limited access to medical professionals, medical assistance can be provided to patients in the form of a chatbot which acts as a proxy between psychiatrists and patients and is available and accessible 24/7. Although there has been a degree of success in developing medical chatbots, many medical professionals believe that the use of chatbots in early depression detection needs to be more practical which will require further research. In this research, we address the well-known and common shortcomings which have been discussed in the recent literature. Three of these shortcomings are as follows: firstly, there is a lack of open-ended questions to enable participants to interact openly and without any restrictions about their moods and emotions as most bots in the literature constrain the participants’ responses by limiting them to multiple choice questions which means the participants are not able to open up and describe their real feelings freely. Secondly, there is a lack of semantic analysis to draw exact meaning from a text. Thirdly, there is a requirement for participants to make a long-term commitment in terms of their involvement in the research. This research introduces a depression analysis chatbot, DEPRA, which aims to resolve some of these shortcomings and challenges by asking open-ended questions, providing semantic analyses and automatic depression scoring. DEPRA is developed using contemporary bot platforms, Dialogflow on Google cloud-based infrastructure, and is integrated with social network platforms such as Facebook. Most chatbots today are designed for therapeutic purposes. However, the DEPRA chatbot is designed with a focus on the detection of depression in its early stages. DEPRA is designed based on a structured early detection depression Standard Interview Guideline the Hamilton Depression Scale (SIGH-D) and Inventory of Depressive Symptomatology (IDS-C), which is used by professional psychiatrists in triage sessions with patients. DEPRA has been trained with personalized utterances from a focus group. This research utilizes Natural Language Processing (NLP) to identify the depression level of participants based on their recorded conversation. DEPRA uses a scoring system to determine the participant’s depression level and severity. This research also details a non-clinical trial with 50 participants who interacted with the DEPRA chatbot. Due to the ethical limitations of this research, such as only residents of Australia and participants to be in the age group of 18 to 80 years old, we have approached a dataset with only 50 participants. This size of dataset was suitable to conduct and run the research. However, the future studies will target a more comprehensive dataset. This study was a first stage of utilizing Chatbot for early detection of depression. In this stage our goal was to develop the system not to run the clinical trial. Therefore, we required a sample that could assist us mainly to identify the accuracy of the system developed. Future work is to access evaluation by human expert which goes into the next phase of the project and could also include extending the sample and/or enhancing the system further and the assistance would be offered to western health. Therefore, at this stage 50+ sample sufficed to capture various responses by the people that had participants of different level of depression. To evaluate the autoscoring feature of DEPRA, the accuracy of the Machine Learning (ML) algorithms is calculated. Accordingly, manual scoring is compared with the calculated depression scores. The average accuracy of the 27 questions related to the linear SVC of the 26 participants’ experiment is 88%, the SGD algorithm of 40 participants’ experiment is 80%, and the linear SVC of 50 participants’ experiment is 87%. Furthermore, the overall satisfaction rate of using DEPRA was 79% indicating that the participants had a high rate of user satisfaction and engagement.

The issue of psychiatric co-morbidity is a serious public health problem occurring more and more frequently in the treatment of addiction and bringing with it various complications and often premature termination of treatment. The scoring system with a fixed set regime is still being used as the main instrument for treatment of addicted clients on the most specialized departments of psychiatric hospitals, although the effectiveness of the scoring system has not yet been clinically verified. The aim of this pilot study is to demonstrate and verify of the individual cases of clients with different categories of dual diagnosis, how they perceive the balance of the sanctions and rewards and fair setting in the context of the scoring system. Other goals are to find out and verify what is the influence of the scoring system on the motivation to change the behavior and success of treatment, what weaknesses clients with dual diagnosis perceive in this system and how they represent the treatment system, which would make them more fit and motivated to change behavior. Further, this study seeks to establish whether difficulties and obstacles on the way to stand up and go through the scoring system are different for individual dual diagnoses. A qualitative approach is used in the research part of the thesis....

The narrow therapeutic index associated with most analgesic opioids creates a high degree of risk, obliging caution in patient selection; this chapter describes screening practices. More attention is due to the use of opioid therapy for chronic non-cancer pain as episodes of respiratory arrest increase. Universal precautions are recommended for all patients. Before and throughout treatment, selected tools assessing risk and misuse should be employed. Increased access to the opioid antagonist naloxone has followed the increase in opioid poisoning deaths; the Risk Index for Overdose or Serious Opioid-induced Respiratory Depression (RIOSORD) helps determine the likelihood of an opioid overdose, serving to select candidates for at-home naloxone access. Urine drug testing aids treatment plan adherence. Tables include: a comparison of screening tools for risk stratification; a comparison of tools for assessment of opioid misuse; the scoring procedure for the RIOSORD; and a list of risk classes with predicted probabilities.

The 6th edition of the textbook Ellestad’s Stress Testing: Principles and Practice was written for the new and veteran clinician alike performing stress testing. Thoroughly updated, referenced and interspersed with case examples, the book reviews how to get the most out exercise testing, without and with ancillary imaging. In addition to evaluation of ST segment depression, other powerful tools to detect ischemia and forecast the future are reviewed to increase the diagnostic accuracy and prognostic ability of exercise testing. The recognition and significance of exercise induced arrhythmias and conduction defects are examined. When to convert to pharmacologic stress or add ancillary imaging, including myocardial perfusion imaging, echocardiography, coronary calcium scoring, and magnetic reference imaging are reviewed. The use of stress testing in the management of obstructive and non-obstructive coronary artery disease (CAD), heart failure, cardiac rehabilitation, peripheral vascular disease, congenital heart and other cardiovascular diseases (CVD) is examined. Options to optimize the diagnostic capabilities of exercise and other diagnostic testing for women are highlighted. Strategic use of exercise testing in the face of a decreasing burden of CAD in the developed world, as well as the opportunity to rely on exercise testing as the first test to evaluate CVD in the developing world, are reviewed. The fundamentals of exercise physiology and myocardial ischemia that serve as the foundation for exercise testing in health and disease are explained.

By 1932, RKO was on the verge of collapse, due to a lack of quality films and industry-wide contraction caused by the Depression. But a new studio production chief—29-year-old David O. Selznick—would lead a dramatic course correction. Selznick commissioned better movies at RKO and encouraged Steiner to provide full dramatic scoring for as many of them as possible. This was the true beginning of film music in the sound era; and this chapter details Steiner and Selznick’s industry-changing innovations. Symphony of Six Million was the first Steiner score to employ separate leitmotivs (à la Wagner) for characters and extensive scoring under dialogue. Other important titles followed: Bird of Paradise, The Most Dangerous Game, and Katharine Hepburn’s screen debut, A Bill of Divorcement. By year’s end, Steiner’s scores were changing the way Hollywood approached music, paving the way for his first screen masterwork.

Coronary calcium scoring (CAC) by noncontrast computed tomography is a powerful predictor of coronary artery disease (CAD) prognosis. Exercise testing is a powerful predictor of CAD severe enough to cause ischemia. Both tests predict CAD in a graded incremental fashion. The higher the CAC score, the greater than chance of a coronary event or all cause mortality. The greater the ST depression and the shorter the exercise duration of an exercise test, the greater the cardiac event rate. Both tests are inexpensive and provide incremental information. In 2013, Alan Rozanski and colleagues suggested combining the tests, calling it a “Treadmill Calcium Test.” Such a test can inform the patient and physician with a measure of atherosclerosis with CAC and a measure of ischemia and fitness with an exercise test. CAC can also provide incremental information when combined with an exercise imaging study such as myocardial perfusion imaging or stress echocardiography.

This chapter discusses working-class realism, which is a culture instead of a reality. The culture encourages people to be realistic and scorns them when they are thought not to be. In terms of middle-class perspective, the matter is an act of lowering and differing expectations and crushing aspirations which would result in lower levels of accomplishment. The act of deference, ceding control, and taking it are aspects of working-class realism as well. The chapter also notes the two forms of upward mobility: relative and absolute. It also includes how the desperate conditions of the Great Depression is regarded as the primary circumstance that motivated the most effective working-class upsurge in the 1930s.

Aim: The general aim of this study is to screen for the prevalence of mental disorders(PTSD, anxiety and depression) amongst refugees residing in Baqaa refugee camp. Design: cross sectional quantitative study using close-ended surveys Methods: Data were collected from 124 refugees collected at baqaa refugee camp. The participants were recruited based on convenience sampling. The questionnaire included the GAD-7, PHQ-9, and part IV of the HTQ questionnaires used to screen for anxiety, depression, and symptoms of PTSD respectively. Each one of these questionnaires has its own valid and reliable scoring system that will assess how severe these mental illnesses are in the participants. For all three questionnaires, a valid Arabic translation was used, as most of the participants are fluent only in Arabic. Results: The results of this study indicated that a large portion of refugees showed signs that are indicative of suffering from mental disorders Conclusion: Our data indicate a challenging and persisting disease burden in refugees due to anxiety, depression and PTSD. Knowing this is relevant for the development of public health policies of host countries. Scalable interventions, tailored for refugees, should become more readily available. This study delved into the presence of anxiety, post-traumatic stress and depression amongst Palestinian refugees and determined that health screenings should be performed. Summary Statement What is already known about this topic? • Previous studies indicated that the quality of life in baqaa refugee camps was significantly worse than other areas in the country. • This study is one of the first one of its kind in Jordan to specifically focus on baqaa refugee camp and screen for anxiety, depression and PTSD What this paper adds? • Improving the awareness of people responsible for refugee camps about the devastating effects of mental health on the quality of life of refugees • It helps understand the population there, what kind of mental issues they are facing, which ones are most common so that governing authorities can know how to help them more efficiently. • Encourages talks about mental health and normalize seeking help especially in a group of people where mental health problems are really concentrated such as refugee camps The implications of this paper: • The researchers strongly recommend more detailed and thorough screening as well as providing refugees with options to seek therapy. Key words: Anxiety, depression, Post-traumatic stress disorder, Refugees, Mental disorders
