U.S. longevity is placing a demand on long-term care services for the impaired and elderly. Medicaid is the primary insurance program in funding costly long-term care for the aged poor. As a major health reform law, the 2010 Patient Protection and Affordable Care Act, Public Law 111-148, gives financial incentive for states to expand Medicaid, transitioning long-term care services from costly facilities toward home and community-based care. Not all states choose to expand their Medicaid long-term care program despite the financial incentive, but instead they continue spending on nursing facili

AbstractSuppose that advanced civilizations, separated by a cosmological distance and time, wish to maximize their access to cosmic resources by rapidly expanding into the universe. How does the presence of one limit the expansionistic ambitions of another, and what sort of boundary forms between their expanding domains? We describe a general scenario for any expansion speed, separation distance and time. We then specialize to a question of particular interest: What are the future prospects for a young and ambitious civilization if they can observe the presence of another at a cosmological dis

In this paper, various types of memory are considered.Development of the nervous system and the brain of organisms ensures expansion of species with more developed brain. Different characteristics of the brain zones have created different conditions for forming traces of events. The various traces of trace events formed in different parts of the brain provided a wider range of functions and brain responses, which contributed to the survival and expansion of species. The work describes the appearance of memory varieties and the causes of these varieties.

Recently, the concern on environmental emissions has increased, which in turn put pressure on power systems as one of the main sectors in contributing to environmental emissions. Accordingly, different countries have started installing renewable energies to alleviate these concerns and mitigate environmental emissions. Medicaid’s intergovernmental design with a generous federal matching rate and substantial state discretion has fostered gradual but steady expansions of the program over time. Gradual growth creates a favorable political environment in which each expansion creates a political constituency for Medicaid—among provider groups and enrollees—who then fight against retrenchment. Yet, as program expenditures continue to increase, especially during fiscally distressed times, the partisan divide over the future direction of Medicaid becomes more stark. Whenever war did have a significant impact on the core welfare state programmes (i.e. the big social insurance schemes), it was through indirect and long-term rather than direct, short-term dynamics. Longer-term rates, they argued, reflected decentralized knowledge and should be determined by market forces. By embracing forward guidance and quantitative easing (QE) to target long-term rates, central banks have crossed that line. While consistent with the post-1980s ex

Abstract:
The East Asian crisis is not all bad news for the United States and Canada (North America). Net debtors in North America – be they individual families refinancing their mortgages, businesses financing their expansion, or the U.S. government financing its debt – should benefit from the crisis, as it continues to put downward pressure on interest rates. While the crisis hurts North American farm exports, it presents opportunities for expansion in North American exports of processed foods. The study estimates that the benefits accruing to North American food producers far outweigh the losses to f

Abstract:
Long-term development finance provided by Multilateral Development Banks (MDBs) is key to advancing the United Nations 2015 Sustainable Development Goals. However, MDBs are constrained in their lending by the availability of capital. This paper argues that Risk Transfer, as a complement to equity injections, could permit higher MDB lending by attracting a broader class of investors. We describe selected examples of actual Risk Transfer transactions and provide estimates of the potential expansion in lending these techniques could yield. But we also identify obstacles that limit investors willi

Abstract:
Expanding irrigation is frequently promoted as a solution to undernutrition and food insecurity by increasing agricultural output, creating jobs, and raising incomes. However, its effects on children's diets and water supplies have not been thoroughly examined on a global scale. A study of 26 low- and middle-income countries reveals a clear, positive correlation between irrigation expansion and increased dietary diversity in children. However, in water-rich regions, irrigation expansion tends to be directed towards producing export-oriented cash crops, resulting in only minor local nutritional

Abstract:
Financial inclusion is strikingly low in emerging economies. In only a few years, financial technologies (fintech) have led to a dramatic expansion in the number of non-traditional credit intermediaries, but the macroeconomic and credit-market implications of this rapid growth of fintech are not known. We build a model with a traditional banking system and endogenous fintech intermediary creation and find that greater fintech entry delivers positive long-term effects on aggregate output and consumption. However, greater entry bolsters aggregate firm financial inclusion only if it stems from lo

The objective of the evaluation is to measure the sustainability of the Bank's interventions in the supply of water and sanitation services financed by the Water Supply and Sanitation Program for Small Communities (PR-0118). The evaluation starts by considering that, in order to achieve the development goals associated with the expansion of water and sanitation services, the sustainability of these services-understood as the long-term maintenance of the quality of the service provided in the interventions-must be guaranteed at the technical, financial, and operational level.
