The body of work presented in this dissertation has demonstrated that the interactions between donor cells and host cells are critical for bone repair and regeneration. The donor cells secrete VEGF which activates the downstream PI3K/Akt signaling pathway, ultimately leading to host cell recruitment and robust bone regeneration. Abstract Genetic factors and their resulting phenotypes were evaluated in three different bone disorders: osteogenesis imperfecta (OI), juvenile idiopathic osteoporosis (JIO), and stress fractures. The spectrum of the OI phenotypes caused by mutations in the COL1A1 and COL1A2 genes is well defined, but the mechanisms by which the variations affect the hearing phenotype are not well-known. This thesis takes a stepwise approach to address some of these challenges. The effects of the rate of distraction and of local infusion of IGF-1 upon bone deposition during mandibular distraction osteogenesis was studied in a rabbit model. Five groups of rabbits were studied. All rabbits, except sham operated controls, underwent distraction to 15 mm. The variables studied were the rate of distraction (0.5 mm twice a day versus 1.5 mm twice a day) and the effects of local IGF-1 infusion via osmotic infusion pumps. Analysis by DEXA scanning and three point bending 28 days after the end of distraction demonstrated no difference in density or strength of bone between the
