It is well known that acute destructive pancreatitis, pancreonecrosis is accompanied by severe endogenous intoxication, often leading to the development of multiple organ failure syndrome, liver failure is one of the important links in this process. The mortality rate of patients with pancreonecrosis related to liver failure is as high as 83%, and approximately 5% of patients with pancreonecrosis have fulminant liver failure. Proven: liver function is closely related to the progression of purulent-necrotic complications in patients with acute destructive pancreonecrosis. The authors present th

There is a case report of two years medical history from the moment of acute pancreonecrosis to reconstructive operation (pancreatodigestive anastomosis) of the patient 56 years old. We showed the ability to use criteria about the depth and configuration of pancreatic necrosis in acute pancreatitis to prognose late complications. Underestimation of deep parenchymal necrosis in the pancreatic neck and body, high -amylase activity for diagnosis of disconnected pancreatic duct syndrome, unjustified drainage removal in external pancreatic fistula led to pseudocyst formation. We analyzed the results of surgical treatment of 128 patients (66 men and 62 women) with various forms of pancreatic necrotic process not associated with biliary tract pathology. There were 76 patients aged from 29 to 60 years old and 52 patients over 60 years old. Past medical history showed that 27 patients had alcoholism, 15 patients - gastric and duodenal ulcer, 16 - heart and vascular diseases; 23 patients had acute or chronic pancreatitis.
