AbstractThe way somatization is expressed—including the actual somatoform symptoms experienced—varies in different persons and in different cultures. Traumatic experiences are intertwined with cultural and social values in shaping the resulting psychopathological phenomena, including bodily experiences. Four ideal-typical cases are presented to show the different levels involved. The effects of trauma, culture and values may be pathofacilitating (creating a social context which is necessary for the experience to take place), pathogenetic (taking a causal role in the onset of the psychopathological reaction), pathoplastic (shaping the form such a psychopathological reaction takes) or pathointerpretive (different interpretation of the same symptoms depending on the patient’s beliefs). While the roles of trauma and culture were already well recognized in previous accounts, this chapter adds an exploration of the importance of values, including cultural values, in the aetiology, presentation and management of somatization disorders. As a consequence, the therapeutic approach has to be adjusted depending on the way these factors intervene in the patient’s construction of mental distress.

Abstract Proactively addressing religious and spiritual (r/s) issues has a strong intervention effect on patients, which is generally more important than the detailed content of spiritual screenings and assessments. When asked about r/s needs or problems, patients may feel bothered, surprised, annoyed, or, conversely, satisfied, supported, acknowledged in their coping efforts. Consequently, documentation should first and foremost reflect the patient’s reaction towards the clinician’s r/s intervention and to what extent the patient wishes this interaction to be shared within the healthcare team. In psychiatry and psychotherapy, patients’ spirituality is less pathologized than in former times and more and more accepted as a universal dimension of human experience, transcending individual religions. In mental health and in other medical fields, r/s may be part of the problem or part of the solution (K. Pargament), or both. Consequently, spiritual charting should not only differentiate pathological/negative and resilient/positive coping but consists of the patient’s r/s healthcare preferences and goals as well as the role he or she attributes to the health professional.

Patient engagement ensures that the design of healthcare services are aligned with the values, the preferences, and needs of the patient community and integrates the real-life experience and the skills of the people to enhance patient safety in the patient journey.The utmost priority to implement patient engagement is the training of patients. Therefore, training for both patients/families/advocates and health professionals is the foundation on which to build active engagement of patients and consequently an effective and efficient patient journey.The chapter offers examples of successful training courses designed to foster strategic alliances among healthcare professionals and researchers with patients and their organizations. Training of patients constitutes the first step to develop shared knowledge, co-produced projects, and the achievement of active multilevel participation of patients for the implementation of patient safety in the patient journey.

AbstractThe trigeminal nerve constitutes the largest sensory cortex representation in the brain compared to any other sensory nerve. This is likely due to the fact that the trigeminal nerve underpins our very existence, as it protects, sensorially, our senses including the organs that provide sight, smell, taste, hearing, speech and meninges protecting our brain.Neurophysiologically, our affective and limbic systems in our brains are alerted before we even set foot in the dental surgery and the patient’s brain is anticipating and aligned for pain experience. Thus, when trigeminal nerve injuries occur, which in the main are preventable, the majority of patients experience mixed symptoms including ongoing and elicited neuropathic pain, numbness and altered sensation. These neuropathic features cause significant impact on the patients’ ability to function, for example, cold allodynia prevents the patient enjoying cold foods and drinks and undertaking outdoor activities or elicited pain on touch frequently interferes with sleep. The resultant chronic symptoms and functional impedance often result in significant psychological morbidity.There is no magic bullet to resolve these sensory nerve injuries, and our specialty is beholden to prevent nerve injuries where possible. The patient must have the appropriate consent, and their expectation is managed with understanding the potential benefits and risks for their chosen interventions.Prevention and management of nerve injuries related to local anaesthesia, implants and third molar surgery are outlined in this chapter. There is insufficient capacity to go in-depth for each area, but the author has provided up to date evidence base where it exists and some strategies to minimize and manage optimally these unfortunate complications.
