Postoperative pain is of serious concern to patients and anesthesia providers alike. Management of a patients’ pain is a central component of anesthesia care. Ketamine as an anesthetic agent has been available for 50 years. It has been utilized as a general anesthetic and selectively as an anesthetic agent for high-risk patients. Due to dysphoric side effects associated with the dosage required to render general anesthesia, anesthesia providers may be reluctant to utilize this medication to its full potential. Recently there has been a resurgence of interest in ketamine as an analgesic agent.

Patient-controlled delivery systems deliver drugs at a rate which is controlled by the patient in order to achieve plasma concentrations consistent with acceptable efficacy and minimal side-effects. They can be used therapeutically to provide pain relief after surgery and as a research tool to measure the efficacy of other analgesic techniques. The number of surgical procedures is increasing worldwide. The Audit Commission set a target that only 5% of patients should experience severe post-operative pain. Major abdominal surgery is a cause of severe post-operative pain. Epidural analgesia is regarded as the "gold standard" for treating this category of post-operative pain.
