Abstract:
Syfte: Syftet är att utifrån ett omvårdnadsperspektiv undersöka hur utvecklingen av syndromet sarkopeni på ett effektivt sätt kan förebyggas.   Metod: Systematisk litteraturstudie där originalartiklar söktes i databaserna PubMed och Cinahl varav 15 originalartiklar granskades och sammanställdes.   Resultat: Omvårdnadsåtgärder som förebygger sarkopeni indelas i nutrition, styrketräning, vibrationsträning, och övrig fysisk aktivitet. Styrketräning kan med medelhög intensitet och proteinrik kost minska fettmassan, öka muskelmassan och muskelstyrkan och öka den fysiska funktionen. Fysisk aktivitet och ett stort proteinintag kan inte förhindra men bromsa utvecklingen av sarkopeni. Ett högt proteinintag i samband med träning är ett effektivt sätt att förebygga sarkopeni. Vibrationsträning kan stärka muskelgrupper och i kombination med styrketräning även öka muskelstyrka, rörlighet och muskelmassa.   Slutsats: Fysisk aktivitet och då framför allt styrketräning kan motverka sarkopeni. Träningen måste utföras regelbundet minst två gånger i veckan med en medelhög intensitet för att den ska ha effekt på kroppssammansättningen. Ett högt proteinintag är en förutsättning för muskeltillväxt och för att förebygga sarkopeni. Vidare visar denna studie att sarkopeni kan förebyggas upp i hög ålder.
Objective: From a nursing perspective, the aim is to study how the development of sarcopenia can be prevented in an efficient way.   Method: Systematic literature review where original articles where searched in PubMed and Cinahl, where the result of 15 articles where reviewed and compiled in different themes.    Result: The nursing interventions which prevent sarcopenia can be divided into nutrition resistance training, vibration training, and other physical activity. Resistance training can with medium-high intensity combined with a protein-rich diet reduce fat-mass on the body, increase muscle-mass and muscle-strength and increase physical function. Physical activity and a high protein-intake cannot prevent sarcopenia but slow the degenerative process. A high protein-intake combined with exercise is an effective way to prevent sarcopenia. Vibration, training can strengthen muscle-groups and can together with resistance training also increase muscle-strength, mobility and muscle-mass.   Conclusion: Physical activity and resistance training in particular, can prevent sarcopenia. The exercise must be performed with a medium high intensity for it to have an effect on the body composition and should be performed regularly at least twice weekly continuously. A high protein intake is a prerequisite for muscle-growth in order to prevent sarcopenia. Furthermore, this study shows that sarcopenia can be prevented even in a high age.

Abstract:
Bakgrund   I samhällen där andelen äldre människor ökar i förhållande till andra åldersgrupper, blir det åldersrelaterade sjukdomstillståndet sarkopeni mer och mer aktuellt. Definitionen av sarkopeni innehåller oftast en ökad förlust av muskelmassa och tillståndet har ett tydligt samband med fysiska handikapp, skörhet, längre vårdtider, ökad fallrisk, frakturer, lägre livskvalitet, dödlighet och därmed lidande. Behandling och prevention av sarkopeni innehåller nutritionsrelaterade åtgärder, ofta i form av kosttillskott. För att kunna utföra omvårdnadsåtgärder på ett säkert sätt tillsammans med patienten, är det viktigt att vårdpersonalen har god kunskap som är evidensbaserad. Syfte Syftet var att beskriva effekter av kosttillskott på äldre patienter med sarkopeni. Metod Designen som valdes var en litteraturöversikt. Sexton artiklar hittades genom databassökning och analyserades för att hitta likheter och olikheter mellan dem. Ett resultat sammanställdes genom en integrerad analys. Resultat  Effekterna av kosttillskott på äldre patienter med sarkopeni som hittades kunde delas in i nio olika kategorier: förändring i muskelmassa, kroppsvikt, fysisk kapacitet, fettmassa, fettfri massa, nutritionsstatus, biokemiska parametrar, förekomsten av sarkopeni, samt aktiviteter i det dagliga livet (ADL) och livskvalitet.  Slutsats Effektparametrarna som studerades visade sig ha samband med fenomen som livskvalitet, ADL, fallrisk, vårdtider och dödlighet. Kosttillskottens förmåga att positivt påverka dessa fenomen, kan på det viset spela en betydande roll i lindrande och prevention av lidande bland äldre människor med sarkopeni.
Background Age-related conditions such as sarcopenia are becoming more common in societies where age-distribution is increasingly tilting towards older generations. Sarcopenia, involving increased loss of muscle mass, is significantly associated with physical handicap, frailty, need of medical care, falls, fractures, decreasing quality of life, mortality and suffering. Amongst the methods for treating and preventing sarcopenia are nutrition related interventions such as nutritional supplementation. For nursing and health care to be safe and satisfactory, medical personnel needs to have evidence based knowledge on the applied methods and interventions. Aim The aim was to describe effects of nutritional supplements on elderly patients with sarcopenia. Method The design chosen for this study was a literature review of scientific articles. Sixteen articles were found using databases and analysed with the aim to find similarities and differences. Results The found effects of nutritional supplements on elderly with sarcopenia could be categorized as change in muscle mass, physical capacity, body weight, fat mass, lean body mass, nutritional status, biochemical parameters, the prevalence of sarcopenia as well as activities of daily living (ADL) and quality of life.  Conclusions  The effect parameters defined in the result of this study were shown to be associated with quality of life, ADL, risk for falls, extended need of medical care and mortality. The positive effects of nutritional supplements on these phenomena can therefore play an important role in easing and preventing suffering amongst elderly patients with sarcopenia.

Syfte: Syftet med denna studie var att undersöka den fysiska aktivitetens betydelse för att motverka sarkopeni hos äldre individer, samt om protein- och D-vitamin-intag kunde ha betydelse tillsammans med fysisk aktivitet för att motverka sarkopeni utvecklingen hos äldre individer (>65år). Metod: Deskriptiv litteraturstudie med litteratursökning i databaserna Pubmed och Discovery. Tio kvalitetsgranskade artiklar valdes ut som var publicerade från år 2004 och framåt. Resultat: Fysisk aktivitet i kombination med aerob träning och styrketräning, 150 min per vecka har en betydelse för att motverka sarkopeni utvecklingen hos äldre personer men även i ett förebyggande syfte för ungdomar och vuxna individer. Kosten: Proteinintaget för de äldre personerna verkar inte ha någon större betydelse för motverkan av redan konstaterad sarkopeni i de industrialiserade länderna. Däremot ser man en positiv effekt på motverkan av sarkopeni vid intag av D-vitamin hos äldre. Slutsats: Denna studie ger stöd åt uppfattningen att regelbunden riktad fysisk aktivitet för äldre har en motverkande effekt (som är mätbar och därför faktabaserad) på utvecklingen av sarkopeni. Äldre personer med redan konstaterad sarkopeni kan genom riktad fysisk aktivitet återhämta sin fysiska kapacitet som de hade innan sarkopeni utvecklingen. Det visade sig också att D-vitamin nivåerna hos de äldre deltagarna har betydelse för bentäthet och muskelstyrka som motverkar sarkopeni utveckling.

Abstract:
The body composition is changing with age: the lean body mass – first of all the skeletal muscle mass and later the bone mineral content – reduces and the fat mass increases, also the regional re-distribution of fat mass occurs. The combination of low muscle mass and low muscle strength or physical performance is called sarcopenia. The aim of this study was to investigate and evaluate the body composition in elderly and the associations of body components in age-related sarcopenia. We examined 151 male and 203 female aged 60 years and older. Body composition was investigated using dual energy x-ray absorptiometry. In men with age-related sarcopenia, microbiopsy of skeletal muscle was performed. It was found that in men, the lowest lean mass and bone mineral content (BMC) were at the age of 80 years and older. In women, there were no differences in lean mass between age groups, and the lowest bone mineral content was found in the age group 80 years and older. Bone mineral content was associated with lean mass in both genders, and in women BMC also was associated with fat mass. In men with age-related sarcopenia, appendicular skeletal muscle mass was the most consistently associated with the total body, total hip and femoral neck bone mineral density (BMD). In women, the fat mass was the most significant determinant of bone mineral density. Mūsų tyrimas parodė, kad vyrų liesoji masė bei vyrų ir moterų kaulų mineralų masė yra mažiausios 80 metų bei vyresnio amžiaus grupėje. Moterų liesoji masė amžiaus grupėse nesiskiria. Vyrų kaulų mineralų masė teigiamai koreliuoja su liesąja mase, o moterų kaulų mineralų masė teigiamai koreliuoja su liesąja mase ir riebalų mase. Esant senatvinei sarkopenijai vyrams galūnių raumenų masė yra reikšmingiausias veiksnys, nulemiantis viso kūno, bendrą šlaunikaulio ir šlaunikaulio kaklo KMT. As the age increases, the physical fitness of seniors decreases. This shows the fragility of their body. Managing everyday activities becomes increasingly more difficult for them. Result of this is reduced self-sufficiency of the seniors, which leads to reduced mobility and to the greater loss of muscle mass and higher dependence on professional care. Aging is accompanied by loss of muscle mass and muscle strength - sarcopenia. This is one of the main causes of geriatric fragility. Sarcopenia presents a serious health problem with both social and economic consequences. The term sarcopenia (from Greek words sarx - meaning flesh referring to muscle and penia - loss) was first used in 1989 by Irwin Rosenberg to describe the loss of muscle mass accompanying aging. Exactly defining the term sarcopenia has helped explain this gradual loss of muscle mass. Three objectives were set in the thesis. The first objective was to find out whether sarcopenia reduces the quality of life in the elderly. The second goal was to determine, which quality of life tests are suitable for testing sarcopenia and the last one was whether the SARC - F questionnaire predicts sarcopenia.Quantitative research was used for the empirical part of this work. Data collection was performed using a method of a questionnaire. These were standardized questionnaires aimed at assessing sarcopenia and quality of life. The research was carried out with a total of 77 respondents with sarcopenia and respondents without sarcopenia. The quantitative part of the research was statistically processed using the MS Excel computer program.

Abstract:
The major focus of our study is use of the presence of sarcopenia as a marker for identification of Parkinson's disease in patients with RBD (REM sleep behaviour disorder). This intention is based on higher incidence of sarcopenia in patients with Parkinson's disease. We also evaluated connection of severity of Parkinson's disease and sarcopenia. In theoretical part of our thesis we present separately sarcopenia, Parkinson's disease and RBD for better understanding of these diseases and their connection. Our measurements of sarcopenia were based on literature review. We followed common approach of three component model of diagnosis of sarcopenia. Physical performance was evaluated based on gait characteristics. Low muscle strength was measured with handheld dynamometer. For estimation of body composition, we used two methods which were bioelectrical impedance analysis and anthropometry. In our research we measured these components in total on 72 males. This cohort was divided into three groups based on diagnosis. There were 32 patients with Parkinson's disease, 28 patients with RBD and 12 probands in control group. Based on our findings we defined suitable parameters and cut-off points for identification of sarcopenia as a marker of Parkinson's disease. In group of patients with Parkinson's disease...

Abstract:
Introduction: Sarcopenia is a disease with a high prevalence in the elderly population and a significant negative impact on quality of life. Physical activity along with adequate nutritional intervention are key to mitigating these effects. Methods: The research was conducted using a questionnaire survey in which 113 nursing homes participated. The quality of nutritional care in these facilities was determined using a survey. Results: A total of 113 nursing homes were included in the research. Of these, 57% of facilities had a registered dietitian available. Nursing homes assess the presence of malnutrition at least 4 times a year in 74% of cases, of which they most often use changes in weight (80 %) and monitoring of food intake (70 %). Only 50 % of the facilities use validated nutritional screening tool. 83 % of the facilities apply nutritional support, most often in the form of diet modification and sipping (98 %). They use food fortifications less often, in a total of 57 % of facilities. The presence of a registered dietitian is affected by the size of the facility, with the larger ones being available to the registered dietitian at a higher frequency. The presence of a registered dietitian in the facility significantly positively affects the frequency of detection of malnutrition (P = 0.039)....

In the field of hepatology, the prognosis of a patient with liver cirrhosis is extraordinarily important. As in other cases, it helps to make a better decision about the right patient management. In cases where liver transplantation is indicated, it helps to place the patient correctly on the waiting list according to the urgency of the disease. Stürze im Alter stellen ein ernstzunehmendes und häufiges Geschehen im Alter dar. Die Gründe sind multifaktoriell bedingt, wobei die Sarkopenie einen wichtigen Stellenwert einnimmt. Eine an der Universität Würzburg entwickelte Bodenreaktionskraftmessplatte ermöglicht die Erfassung von Muskelkraft und Muskelleistung während natürlicher Bewegungsabläufe wie Kniebeugen oder Aufstehen von einem Stuhl. In dieser Pilotstudie wurde untersucht, ob dieses Messverfahren als Screeningmethode zur Erkennung von Muskelkraft und -leistungsdefiziten geeignet ist und ob ein Zusammenhang mit einem erhöhten Sturzvorkommen besteht. Bei 459 zu Hause lebenden mobilen Senioren zwischen dem 50. und 80. Lebensjahr wurde die Muskelkraft und Muskelleistung der unteren Extremitäten erfasst. Zudem wurden in einem Fragebogen Sturzrisikofaktoren und Stürze der letzten 12 Monate ermittelt. Die Befunde zum Abbau von Muskelkraft und -leistung decken sich mit der gegenwärtigen Studienlage. Im Zusammenhang mit den angegegebenen Stürzen könnte anhand der Ergebnisse insbesondere die Muskelleistung eine Screeningmethode zur Einschätzung des Sturzrisikos darstellen. Falls in elderly are a serious and frequent event. The reasons are multi-attributed, in which the sarcopenia plays an important role. The University Würzburg developed a ground force reaction platform which allows for measuring muscle force and muscle power during natural movements. This pilot study examines if this technique is a practicable screening method for deficits in muscle force and muscle power, as well as their relation with an increased occurrence of falls. We measured the muscle force and muscle power in 459 mobile community dwelling elderly between the age of 50 to 80 performing knee bends and chair-rise. We recorded risk factors of falling and also falls in the past 12 months using a questionnaire. The results concerning the loss of muscle force and muscle power in age coincide with the current data. Regarding the reported falls, in particular muscle power could be a suitable method for calculating the risk of falls. In closing, further studies seem to be justified

Vitamin D and calcium are essential for the health of the elderly population. Due to the changes that occur in the body during aging, it is necessary to monitor the intake of these micronutrients and, if necessary, to supplement or substitute them appropriately. This thesis focuses on prevention of vitamin D and calcium deficiency in old age. The theoretical part explains the function of vitamin D and calcium in the human body, their metabolism, sources and problems of deficiency or excess. There is information about diseases related to the deficiency, especially osteoporosis and sarcopenia. The practical part has two sections. The first analyses the data from the SPRINTT trial. Data related to vitamin D, calcium, their supplementation and sarcopenia, as a disease associated with vitamin D deficiency, were used. The second section analyzes the data from a questionnaire survey in nursing homes, which obtained information about the health of seniors, the frequency of consumption of selected foods and vitamin D and calcium supplementation. Using data from the SPRINTT, it was found that 55 % of men and 40 % of women had the values of sarcopenia. 11 % of the participants had vitamin D deficiency. Huntington's disease is a dominantly-inherited autosomal neurodegenerative disease manifested by disorders of motility, cognitive function, behaviour, and weight loss, which is conditioned multifactorially. The aim of the study was to determine whether there are eating disorders in Huntington's disease, as well as its etiology and severity. Neurological scaling, anthropometric examinations, evaluation of three-day diet records, measurements with a manual dynamometer, bioimpedance analyses, indirect colorimetry and predictions of energy expenditure were performed on 10 patients. Algorithms were applied for the diagnosis of sarcopenia and malnutrition. Unwanted weight loss was observed in all patients and 4 out of 10 showed malnutrition. No difference was found between the values of measured resting metabolism and calculated according to the predictive equation. However, it has been shown that strict nutritional recommendations based on this data can be misleading for some patients with HN, as real energy consumption can be significantly higher. All our patients had a positive energy balance. A new diagnostic algorithm for the early diagnosis of sarcopenia has proven its worth. Patients with Turner syndrome (TS) have increased fracture risk and decreased bone density, patients with severe hemophilia have low bone density. The etiology of these changes hasn't been completely elucidated in any of the two diseases. Our aimes were to assess bone density and geometry at the radius using a new method peripheral quantitative CT densitometry (pQCT) and to describe associations between densitometry parameters and estrogen treatment in TS and laboratory as well as clinical markers of disease severity in haemophilia. Sixty-seven girls with TS (median age 14.3 years, range 6.0-19.4) and 42 boys with haemophilia (median age 12.7 years, range 6.6-19.2) have been measured using peripheral quantitative CT at the radius. The results have been compared to published reference data. Girls with TS had decreased cortical bone density and thinner cortex. These parameters were positively correlated to the length of estrogen treatment. Trabecular bone density was normal in prepubertal girls but it was decreased after puberty. There was no association between trabecular bone density and the estrogen treatment. Boys with haemophilia had decreased trabecular density and low muscle area at the forearm. Im Rahmen des demographischen Wandels kommt es zu einem kontinuierlichen Anstieg des Anteils postmenopausaler Frauen an der Weltbevölkerung, so dass die Therapie postmenopausaler Erkrankungen zu einem wichtigen Thema der ärztlichen Behandlung geworden ist. Seit Beginn des 21. Jahrhunderts werden hinsichtlich der klinischen Wirksamkeit und der Sicherheit der Hormonersatztherapie vermehrt Bedenken im Hinblick auf das erhöhte Risiko für das kardiovaskuläre System und Brustkrebs geäußert. Aus diesem Grund rücken alternative Therapieansätze verstärkt in den Mittelpunkt der Forschung. In unterschiedlichen Projekten hat sich hierbei CR als effektives, in der klinischen Praxis anwendbares Medikament gezeigt. Die vorliegende Dissertation setzt sich mit der Wirkung von Subfraktionen der Testsubstanz CR auf die klimakterisch bedingten Veränderungen bzw. Erkrankungen der Skelett- und der Herzmuskulatur auseinander. Vor diesem Hintergrund wurden 100 weibliche, zuvor ovariektomierte Sprague-Dawley-Ratten in 10 Gruppen unterteilt und über einen Zeitraum von 4 Wochen die verschiedenen Substanzen von Cimicifuga racemosa (CR) (Saponin-, S1-, S2-, S3-Fraktion und Rest-, R1-, R2- und R3-Fraktion) oder Östradiolbenzoat (EB) verabreicht. Über denselben Zeitraum wurde eine Kontrollgruppe mit sojafreiem Futter versorgt. Mittels histologischer Untersuchung der Herz- und Skelettmuskulatur wurden die Auswirkungen von EB und den verschiedenen Fraktionen und Subfraktionen aus CR ermittelt und untereinander und mit den Ergebnissen der Kontrollgruppe verglichen. Die Skelettmuskelfaserfläche hat unter dem Einfluss der Saponinfraktion, der S1-Subfraktion, der Restfraktion, der R2-Subfraktion und der EB-Gruppe signifikant im Vergleich zur Kontrollgruppe zugenommen. Eine signifikante Zunahme des elastischen Faseranteils bezogen auf die Skelettmuskelfaserfläche bestand unter dem Einfluss der Saponinfraktion, ihrer S1- und S2-Subfraktion und der R1-Subfraktion als auch der EB-Gruppe im Vergleich zur Kontrollgruppe. Die Kardiomyozytenfläche zeigte einen signifikanten Größenzuwachs im Vergleich zur Kontrollgruppe unter dem Einfluss der Saponinfraktion und all ihrer 3 Subfraktionen als auch der Restfraktion und ihrer 3 Subfraktionen und der EB-Gruppe. Eine signifikante Zunahme des elastischen Faseranteils bezogen auf die Kardiomyozytenfläche bestand unter dem Einfluss der Saponinfraktion, ihrer S1-Subfraktion und der Restfraktion und ihrer R3-Subfraktion als auch der EB-Gruppe im Vergleich mit der Kontrollgruppe. Ableiten lässt sich, dass die Subfraktionen der Saponin- und Restfraktion keine gleichmäßige Wirkung auf die strukturellen Bestandteile von Herz- und Skelettmuskulatur zeigen. Die Ergebnisse sprechen dafür, dass bestimmte Subfraktionen von CR anabol auf die Skelettmuskelfaser- und Kardiomyozytenfläche und die EZM beider Gewebe wirken. Nahezu regelmäßig erweist sich die vor allem Actein und Deoxyactein enthaltende S1-Fraktion als wirksamste Substanzgruppe. Die Wirkungen von CR sind noch Gegenstand der aktuellen Forschung. Eines der Ziele weiterer Untersuchungen sollte die Ermittlung der Kombination der jeweiligen Fraktionen und Subfraktionen oder deren Subsubstanzen sein, um eine bestmögliche osteoprotektive und anabole Wirkung auf Herz- und Skelettmuskulatur und den Knochen zur Linderung oder Beseitigung klimakterischer Beschwerden zu entfalten.

Objective: To verify whether biological age of tested seniors is in line with their calendar age. The differnce towards calendar age will be evaluated via ECM/BCM ration based on body content analysis. I will investigate the influence of physical activity on slower ageing physiology and the effect of physical activity on status of net muscle mass and on frequency of civilazation diseases. The goal is to figure out if active lifestyle is available also for seniors. Methods: The core of the work is qualitative and quantitative survey mapping antropometric parameters by bioimpedancy method. The functional values were collected by motorical step est and by handgrip. Further the subjective quality life judgment interview with yes/no options was used for the target senior group. Results: The relationship was found for the bioimpedancy results and the the volume of physical weekly activity (in hours) and level of BMI and pecentage of body fat. Further the realtionship was found for physical activity volume and for the dynamometric and motorical test results. Thjere is a difference between men and women.
