Abstract:
The head and neck region is a marvel of anatomic complexity and it encompasses a broad array of subsites, each with its own architectural nuances. This histologic richness begets a commensurate diversity of disease. Advances in our understanding of the genetic and epigenetic alterations that underlie many of these tumors have not only refined their classification, but also reshaped our approaches to diagnosis, prognosis, and therapy. This Fascicle reflects these scientific and diagnostic evolutions. Departing from prior editions this volume is organized by anatomic subsite. Each chapter delves

Abstract:
Essential tremor is defined as long-standing bilateral hand/arm tremor that is visible and may occur persistently during posture-holding, simple movements, and action. The tremor may be slightly asymmetrical. Other areas of the body that may be affected are head and neck (most frequently), the voice, and legs. Head tremor without limb tremor is accepted as essential tremor, although this definition remains controversial. Essential tremor is a progressive disease and manifests at any age. Tremor in Parkinson’s disease mostly starts unilaterally. A no–no or yes–yes tremor of the head indicates e

Abstract:
Reflex studies are an important part of clinical neurophysiology assessment in health and disease. They are essential to get information on conduction in proximal segments of peripheral nerves, spinal and supraspinal integration of sensory inputs on the motor pathway, and excitability of motor structures. They do not require special equipment, except for a sweep-triggering hammer that is essential, for instance, to elicit monosynaptic reflexes, such as the jaw jerk. For consensual reflexes, it is also recommended to use two recording channels, which facilitate recognition of potential disturba

Hyperactive deep tendon reflexes area sign of upper motor neuron lesion. They are also commonly seen in normal but tense people. Proper technique of reflexes examination and experience play a major role in eliciting and categorizing deep tendon reflexes. Clonus is the highest degree of hyperreflexia. The most important neuromuscular disease associated with hyperreflexia is ALS due to degeneration of the cortical motor neurons. Diagnostic difficulty occurs when hyperreflexia and spasticity are the only findings. In these cases, PLS, HSP, and other causes of myelopathies should be entertained. J

Abstract:
Amyotrophic lateral sclerosis (ALS) is the protypical motor neuron disease, which is characterized by the simultaneous presence of upper motor neuron (UMN) and lower motor neuron (LMN) signs in the same extremity or in the cranial-bulbar region. UMN signs at spinal levels include spasticity, slowness of motor activation, hyperactive deep tendon reflexes and extensor plantar responses, whereas UMN signs at the cranial level include spastic dysarthia (slow, labored, nasal); slowness of tongue movements, and hyperactive jaw, gag, and facial reflexes. LMN signs at the spinal level include muscle a
